Background: Lymph nodes are the most important prognostic factors in lung cancer. Controversy still continues on lymph nodes prognosis. This study aims to evaluate the effects of lymph node dissection techniques on survival in geriatric patients.Methods: The study was conducted retrospectively in patients who were operated on for non-small cell lung cancer (NSCLC) between 2007 and 2016. Lobe-specific lymph node dissection (LsLND) was performed in 77 patients, while complete (systematic) lymph node dissection (SLND) was performed in 206 patients.Results: A total of 283 patients were included in the study, of which 258 were male (91.2%) and 25 were female (8.8%). The median age of the patients was 69 years (IQR: 65-84). The mean survival time of the patients was 46 months, and the 5-year survival rate was 38.9%. The survival rate in pN2 disease was 15.6%, while the survival rate in pN0 disease was 46%. While the 5-year survival rate was 34.4% in patients who underwent lobe-specific lymph node sampling, it was 40.5% in patients who underwent systematic lymph node dissection (p=0.147).Conclusions: As a result of our study, no difference was found in terms of survival between lobe-specific lymph node dissection or systematic lymph node dissection in the geriatric age group. Especially pN2 disease, histopathological, and resection width were observed among the most important prognostic factors affecting survival. Therefore, we think lobe-specific lymph node sampling may be preferred instead of performing complete-systematic lymph node dissection in advanced ages because of less morbidity.

Giriş:Soliter fibröz tümör (SFT), tüm yumuşak doku tümörlerinin %2'sinden daha azını oluşturan mezenkimal kökenli nadir bir tümördür. Çalışmamız, soliter fibröz tümör nedeniyle opere ettiğimiz hastaların erken ve uzun dönem sonuçlarını literatür eşliğinde değerlendirmektedir. Gereç ve Yöntemler:Ocak 2011 ve Aralık 2021 tarihleri arasında SFT nedeniyle opere edilen 21 hasta retrospektif olarak değerlendirildi. Hastaların demografik özellikleri, morbidite ve mortaliteleri , sağkalım sonuçları çalışmada değerlendirildi. Bulgular: Çalışmadaki hastaların 16 (%76.2)’si benign, 5’i (%23.8)ise malign karakterde idi. On sekiz hastanın (%85,7) plevral soliter fibröz tümörü mevcut iken, 3 hastanın (%14,3) ekstraplevral soliter fibröz tümörü vardı. Benign hastalarda, 10 cm ve üzerinde tümör çapı olan 7 hasta (%43.8) varken; malign grupta 3 hasta (%60) yer almaktaydı (p=0.635). Hastaların ortalama takip süresi 58,7 ay; SFT’li hastaların ortalama sağkalım süresi 102 aydır (%95 CI: 85-120). İki yıllık sağkalım süresi %100, 5 yıllık sağkalım süresi ise %79 olarak izlenmiştir. Grup 1 hastalarda 2 yıllık sağkalım oranı %100 iken 5 yıllık sağkalım %90 saptanmış olup; grup 2 de bu oranlar sırasıyla %53,3 ve %53.3’tür. Tümör çapı 10 cm ve altında olan hastalarda 5 yıllık sağkalım %85,7 iken, 10 cm ve altında %72,9 olarak saptanmıştır (p=0.494). Sonuç: Benign soliter fibröz tümörlerde komplet cerrahi tedavi hastalığın nüksünü engellemektedir. Ancak malign fibröz tümörlerde hastaların takibi oldukça önemlidir., Background:A solitary fibrous tumor (SFT) is a rare tumor of mesenchymal origin, accounting for less than 2% of all soft tissue tumors. Our study evaluates the early and long-term outcomes of patients operated for solitary fibrous tumors in the light of the literature.Material and Method:Twenty-one patients who were operated on for SFT between January 2011 and December 2021 were evaluated retrospectively. Demographic characteristics, morbidity, mortality, and survival results of the patients were evaluated in the study.Results:Of the patients in the study, 16 (76.2%) were benign, and 5 (23.8%) were malignant. Eighteen patients (85.7%) had pleural solitary fibrous tumors, while 3 patients (14.3%) had extrapleural solitary fibrous tumors. In benign patients, there were 7 patients (43.8%) with a tumor diameter of 10 cm or more; There were 3 patients (60%) in the malignant group (p=0.635). The mean follow-up period of the patients was 58.7 months; The mean survival time of patients with PFT is 102 months (95% CI: 85-120). The two-year survival rate was 100%, and the 5-year survival rate was 79%. While the 2-year survival rate was 100% in Group 1 patients, the 5-year survival was 90%; In group 2, these rates are 53.3% and 53.3%, respectively. In patients with a 10 cm or less tumor diameter, 5-year survival was 85.7%, while it was 72.9% under 10 cm (p=0.494).Conclusion: Complete surgical treatment in benign solitary fibrous tumors prevents the recurrence of the disease. However, follow-up of patients in malignant fibrous tumors is crucial.

Amaç: Bu yazıda video yardımlı torakoskopik cerrahi (VATS) ile sleeve rezeksiyonu yaptığımız izole sağ ana bronşta gelişen glomus tümör olgusunu literatür eşliğinde değerlendirdik. Olgu: Kırk altı yaşında erkek hastaya 4 aydır devam eden öksürük şikayeti nedeniyle yapılan bilgisayarlı toraks tomografisinde sağ ana bronşta yaklaşık 10 mm boyutlarında endobronşial kitle görünümü saptanmıştır. Fiberoptik bronkoskopi ile alınan biyopsi sonucunun glomus tümör olarak raporlanması üzerine hastaya VATS ile sağ ana bronşiyal sleeve rezeksiyon yapılmıştır. Postoperatif 4. günde komplikasyonsuz olarak hasta taburcu edilmiştir. Sonuçlar: Video yardımlı torakoskopik cerrahi yaklaşımı ile bronşiyal sleeve rezeksiyonlar onkolojik prensiplere uygun olarak güvenli bir şekilde uygulanabilmektedir., BackgroundThis article evaluates the right endobronchial glomus tumor operated with sleeve resection using video-assisted thoracoscopic surgery (VATS) with the literature.In this article, we evaluated a case of glomus tumor developing in the isolated right main bronchus. We performed sleeve resection with video-assisted thoracoscopic surgery (VATS), with the literature. Case: A 46-year-old male patient with a cough going on for 4 months. Computed tomography of the thorax revealed an endobronchial mass of approximately 10 mm in the right main bronchus. After the biopsy result obtained by fiberoptic bronchoscopy was reported as a glomus tumor, the patient underwent right main bronchial sleeve resection with VATS. The patient was discharged on the fourth postoperative day without complications. Conclusion With the VATS approach, bronchial sleeve resections can be performed safely in accordance with oncological principles.

Amaç: Multipl primer akciğer kanseri (MAK), birden fazla farklı akciğer kanserinin aynı hastada bulunma durumuna verilen isimdir. Bizim bu makaledeki amacımız metakron primer akciğer kanserli olgumuzu sunmaktır. Olgu: Yetmiş iki yaşında erkek hasta öksürük şikayeti ile hastaneye başvurmuştur. Hastanın bilgisayarlı toraks tomografisinde (BT) sağ üst lobda 38 mm’lik kitle saptanmıştır. Tanı amaçlı transtorasik iğne biyopsisinde (TTİA) ise adenokarsinom tanısı olan hastanın pozitron emisyon tomografisinde (PET-BT) lezyon dışında tutulum saptanmamıştır. Hastaya sağ üst lobektomi ve sistematik lenf nodu diseksiyonu yapılmıştır. Postoperatif patolojisinde lepidik patern adenokarsinom histopatolojik tanısı alan pT2aN0M0 gelmiştir. Operasyondan 3 yıl sonra BT’de sol alt lob posterobazalde plevral tabanlı 15 mm’lik (T4), alt lob superior segment bronşlarına komşu 16 mm’lik (T3), alt lob superior segmentte buzlu cam dansitesinin eşlik ettiği 18 mm’lik (T2) ve yine alt lob superiorda plevral tabanlı 21 mm’lik 4 nodül (T1) saptanmıştır. PET-BT’de tanımlı lezyonda SUVmax tutulumu 4.5 tutulum saptanmıştır. Hastaya tanısal amaçlı yapılan TTİA sonucu adenokarsinom lehine sonuçlanmıştır. Hastaya mediastinoskopi sonrasında VATS ile sol alt lobektomi ve mediastinal lenf nodu diseksiyonu yapılmıştır. Patoloji raporu T1 lepidik adenokarsinom, T2 ve T3 musinöz lepidik adenokarsinom, T4 ise makropapiller adenokarsinom olarak sonuçlanıyor, mediastinal lenf nodları ise metastaz açısından negatif olarak sonuçlanıyor. Sonuç: Multipl primer akciğer kanserili hastalarda cerrahi tedavi altın standart tedavi yöntemidir. Gelecekte MAK hastalarına daha erken tanı konulabileceğini, cerrahi tedavinin ise bu hastaların sağkalımını artıracağını düşünüyoruz.

A Case of Diaphragmatic Plication with Surgeon-Powered Robotic Surgery

Objectives: This study aims to compare the outcomes of video-assisted thoracoscopic surgery (VATS) lobectomy with open thoracotomy lobectomy in patients with non-small cell lung cancer (NSCLC). Methods: There were 269 cases with NSCLC who underwent lobectomy between 2017-2019; these cases were retrospectively studied. VATS lobectomy (VATS Group) and open thoracotomy lobectomy (Thoracotomy Group) patients’ results were compared according to the length of hospitalizations, early postoperative complications and tumor size and stages. Results: VATS lobectomy was performed in 89 (33%) of these patients, whereas 180 (67%) patients underwent lobectomy using open thoracotomy for NSCLC.
