9. Interaoperative use of epidural methylprednisolone or bupivacaine for postsurgical lumbar discectomy pain relief : A randomized, placebo-controlled trial

Background: Many patients with lumbar disc surgery experience postoperative back and radicular pain, delaying hospital discharge and resumption of normal activity. Some surgeons have used intraoperative epidural corticosteroids and local anesthetics to decrease pain following surgery for a herniated lumbar disc. Controversies still exist regarding the benefits of these drugs. The present study was meant to compare the effects of the intraoperative administration of epidural methylprednisolone and bupivacaine with that of normal saline (placebo) in lumbar disc surgery for postoperative pain control. Patients and Methods: One hundred fifty patients with single level herniated nucleus pulposus (L4-L5 or L5-S1), which was refractory to 6 weeks of conservative management, were divided randomly in three groups. A standard hemipartial lamimectomy and discectomy was performed on all patients. At the end of the surgery, before the closure of fascia, 40 mg methylprednisolone with 3 mL normal saline for group 1, 2 mL bupivacaine 5% with 2 mL normal saline for group 2 and 4 mL normal saline for group 3 were instilled onto the epidural and exposed nerve root. Postoperative back and radicular pain intensity was assessed by a visual analogue scale (VAS) before and at 24, 48, 72, and 96 hours after surgery. Results: There was no significant difference in back and radicular pain intensity between the three groups. Conclusion: Intraoperative administration of epidural methylprednisolone or bupivacaine does not relieve postoperative back and radicular pain.

Abstract

Objective: To evaluate the effects of estrogen on the prognosis of patients with diffuse axonal injury due to severe traumatic brain injury. Design: Triple-blind placebo-controlled randomized clinical trial study. Setting: The study was conducted at the Neurosurgery Department of Imam Reza Hospital of Tabriz University of Medical Sciences. Patients: A total of 200 patients were enrolled in the study. Methods: Two hundred patients were randomly allocated into two cases, with a control group of 100 each. The case group daily received 8mg/kg of estrogen by nasogastric for three days and the control group only received a placebo. The Glasgow Coma Scale (GCS) at discharge, Glasgow Outcome Score (GOS) at discharge, and the GOS three months later were investigated to evaluate the effect of estrogen on patients with diffuse axonal injury (DAI) following traumatic brain injury (TBI) in the case and control group. Results: It was shown that there is a significant difference between the case and control groups based on GCS at discharge (p-value = 0.007), GOS at discharge (p-value = 0.02), and GOS at a 3-months follow-up period (p-value = 0.025). Ten patients (5 patients in each group) died during the study. Conclusion: This study indicated the efficacy of estrogen in patients with Diffuse Axonal Injury (DAI) following TBI. Abstract

Objectives Cognitive abilities might be impaired due to brain lesions in children and adolescents. This study aimed to investigate neuropsychiatric indicators in children and adolescents with primary brain tumors and other brain space-occupying lesions (SOLs) before and after the surgical procedure. Materials & Methods The current pre-post study was conducted on 81 patients with brain SOLs aged under 18 years hospitalized in the Neurosurgery Ward of Imam Reza university hospital, Tabriz, Iran, within 20 December 2016 to 20 December 2017. The patients with metastatic brain tumors were excluded. Before and after the surgical procedure, Digit Span forward and backward task (to assess working memory), Stroop Task and Trail Making Task A and B (to assess attention), and Rey-Osterrieth Complex Figure Test (ROCF) (to assess visuospatial memory) were carried out. Then, the scores of the tests were compared to standard values and postsurgical scores. Results The most prevalent type of brain SOLs was medulloblastoma, and the most prevalent region of involvement was the posterior fossa. The scores of all tests after the surgery were significantly improved, compared to those before the surgery (P<0.05). In the assessment of Digit Span forward and backward task scores, there was no significant difference between the scores of patients before the surgery and standard values (P>0.05). Regarding the scores of various stages of the ROCF, the scores of the immediate recall stage were significantly low (P<0.05). Regarding Trail Making Task A and B and Stroop Task before the surgery, only Trail Making Task A and B scores were significantly increased (P<0.05). The scores of Trail Making Task A were significantly higher in patients with medulloblastoma and anatomically in left temporal tumors, which indicated greater damage to the attention field (P<0.05). In addition, in cerebellar tumors, the scores of the immediate recall stage of the ROCF were significantly lower than in other brain tumors or SOLs (P<0.05). Conclusion The visuospatial memory and attention of preoperative assessments were significantly impaired, compared to those of the healthy population (P<0.05). Abstract

Background: The Pain is the most common complaint in various diseases. Postoperative pain is common complication and spatially in elderly patient because of exacerbation of heard and vessel was impotents. The aim of this study was evaluation of the effect of low-dose IV dexamethasone on postoperative pain in patients with lumbar discectomy. Methods: In a clinical trial that studied in neurosurgery wards of Shohada and Imam Reza hospitals affiliated to the Tabriz University of Medical Sciences on patients underwent lumbar discectomy, the effect of low-dose IV dexamethasone on postoperative pain was evaluated. 80 patients divided in 2 equal groups, we used IV morphine (present routine treatment) in group A and IV morphine in addition to 8mg IV dexamethasone in group B, for reducing post lumbar discectomy pain. Results: 21 patients in group A, & 22 in group B were male and 19 patients in group A & and 18 in group B were female (P=0.823). Mean age of patients in groups A and B was 39.32 and 39.22 years, respectively (P equal to 0.945). Mean of pain score (VAS) at 6 hours post-operation in group A and B was 6.97 and 6.75, respectively (P equal to 0.065). VAS at 12, 18 or 24 hours post-operation in both groups didn’t differ significantly (P greater than 0.05). Conclusions: We didn't observe any significant reduction in post lumbar discectomy pain after adding 8 mg dexamethasone into morphine. Regarding other studies, it seems that higher doses of dexamethasone should be used to achieve a significant pain reduction. 32. Two case series reports: 8 cases of arachnoid Temporoparietal cysts (middle fossa & sylvian fissure) and 2 cases of chronic subdural hematoma

Abstract

Background: Arachnoid cysts are common intracranial space-occupying lesions which are often found in middle fossa and temporal regions of the skull. Many of these lesions are asymptomatic but some might appear as space-occupying lesions. Almost arachnoid cyst rupture, either following a trauma or spontaneously can result in intracystic hemorrhage, subdural hematoma and hygroma. The present study presents two case series including 8 cases of arachnoid cysts in temporal region and 2 cases of subdural hemorrhage. Methods: Demographic data and clinical and neuroimaging features of 8 patients were evaluated. Results: A total of 8 patients with arachnoid cysts in temporal region were assessed: age range 3 to 27 years old, 5 male and 3 female. The most important complains of the patients during their visit were seizure (3 cases), headache (4 cases), increased head circumference (1 case), parietotemporal arachnoid cyst in right (4 cases) and left hemisphere (4 cases). The conservative treatment and follow-up were performed in 6 out of 8 patients. In the other 2 patients, for craniotomy surgery with hematoma evacuation was performed. Furthermore, in the surgery the fenestration of arachnoid cyst wall into the basal cisterns as well as low pressure cysto-peritoneal shunt was performed. Conclusions: The risk of annual hemorrhage for patients with arachnoid cyst is very low. However, when the hemorrhage occurs it is treated by hematoma evacuation in most cases, but sometimes there is a need for fenestration of the cyst into basal cisterns under endoscopy, microsurgical or cystoperitoneal shunt. Abstract

Background: S100B Protein is a biomarker that reflects post-trauma brain injury. In traumatic brain injury definite prediction of outcome of the patients is important goal. In this regard our study focuses on the S100B protein value in predicting brain death after head trauma. Methods: This was a cross-sectional study conducted on seventy-two patients (50 male and 22 female) aged 5-80 years (median 40 ± 17.72 years) with severe head trauma (GCS less than or equal to 8). For all patients, determination of GCS level and computed tomography (CT) scan were performed. Then, a single 5ml blood sample was obtained from each patient on admission, 48 hour later, and a week later or after brain death to determine the level of S100B protein. Results: Primary and last GCS of patients had a predictive value in determining brain death (p less than 0.0005). Furthermore, there was a significant correlation between GCS and level of S100B protein. Based on CT scan findings, there was a significant correlation between CT scan and S100B protein only after 48 hours post-trauma. Results of the present study demonstrate that the level of S100B protein at 48 hours after trauma can be used as a marker to predict brain death. Conclusions: In our study level of S100B protein at 48 hours after trauma was a good predictor of brain death. Based on the findings of the present study it can be concluded that a combined method incorporating the GCS test, CT scan, clinical symptoms and biomarkers can be a perfect tool for brain death prediction. Abstract

Background: Previous studies have shown that magnesium sulfate (MgSO4) administered in a patient with a diffuse axonal injury (DAI) can serve as a useful neuroprotective agent. The present study was conducted to examine whether magnesium sulfate has a therapeutic efficacy and safety in patients with a severe diffuse axonal injury. Methods: Adult patients admitted within 1 hour of a closed Traumatic Brain Injury (TBI) with a severe diffuse axonal injury that met eligibility criteria were randomly selected and divided into two groups. Our treatment guidelines consisted of an initial loading dose of 50 mg/kg magnesium sulfate and then 50 mg/kg QID (quarter in die) up to 24 hours after the trauma. The outcome measures were mortality, Glasgow Coma Scale (GCS) score, and motor function scores which were assessed up to 2 months post-trauma. Results: Magnesium showed a significant positive effect on GCS score at 2 months post-trauma (P=0.03). Motor functioning scores of patients in the MgSO4 group were higher than those in the control group but this was not statistically significant (P equal to 0.51). Conclusions: Findings of the present study demonstrated that administration of magnesium sulfate following severe DAI can have neuroprotective effect. 35. Diagnostic value of lasik test for detection of lumbar discopathy in patients attending emergency department with acute low back pain

Abstract

Background: Low back pain is one of the most important health problems. Disk herniation is one of the common causes of low back pain. Accurate examination is the best and simplest diagnostic methods. Physical examination, Straight leg raise (SLR) and neurologic tests as well as other tests are usually used to locate the exact place of herniation. Paraclinical studies such as magnetic resonance imaging (MRI) should be used because of the low diagnostic efficiency of physical examination. Methods: 100 patients attended the emergency unit because of back pain or radicular pain, were enrolled in the study. SLR, Reverse- SLR (R-SLR) and Crossed-SLR (C-SLR) tests were performed for all patients. Then, the patients were referred to the neurosurgery unit for treatment. Lumbar MRI was taken for all patients. Then, all patients were followed up and these data were collected. Results: Sensitivity and specificity of SLR test, C-SLR, and R-SLR tests was 77.52% and 36.36%, 29.21% and 90.9%, and 88.89% and 10.94%, respectively. There was no significant relationship between MRI and positive discopathy in the physical examination and SLR (P=0.11). Conclusions: SLR test has high sensitivity and low specificity. If the SLR test was accompanied by with physical examination the resulting sensitivity and specificity will increase. Findings of this study showed that SLR test has more sensitivity, while C-SLR has more specificity in low back pain diagnosis. Keywords: SLR test, Low Back Pain, Discopathy, Straight leg raise

Abstract

Background: Traumatic Brain Injury (TBI) is the leading cause of mortality and morbidity especially in young ages. Despite over 30 years of using Neuroprotective agents for TBI management, there is no absolute recommended agent for the condition yet. Methods: This study is a part of a scoping review thesis on "Neuroprotective agents using for Traumatic Brain Injury: a systematic review & meta-analyses", which had a wide proposal keywords and ran in "Cochrane CENTRAL", "MedLine/PubMed", "SCOPUS", "Thomson Reuters Web of Science", "SID.ir", "Barket Foundation", and "clinicaltrials.gov" databases up to September 06, 2015. This study limits the retrieved search results only to those which used citicoline for TBI management. The included Randomized Clinical Trials' (RCTs) were assessed for their quality of reporting by adapting CONSORT-checklist prior to extracting their data into meta-analysis. Meta-analyses of this review were conducted by Glasgow Outcome Scale (GOS) in acute TBI patients and total neuropsychological assessments in both acute and chronic TBI management, mortalities and adverse-effects. Results: Four RCTs were retrieved and included in this review with 1196 participants (10 were chronic TBI impaired patients); the analysis of 1128 patients for their favorable GOS outcomes in two studies showed no significant difference between the study groups; however, neuropsychological outcomes were significantly better in placebo/control group of 971 patients of three studies. Mortality rates and adverse-effects analysis based on two studies with 1429 patients showed no significant difference between the study groups. Conclusions: Citicoline use for acute TBI seems to have no field of support anymore, whereas it may have some benefits in improving the neuro-cognitive state in chronic TBI patients. Abstract

The article presents a study conducted by the authors related to the use of diffusion-weighted (DW) magnetic resonance imaging (MRI) to differentiate acute infectious spondylitis from Modic type 1 and assessing discriminative accuracy of apparent diffusion coefficient (ADC), amorphous increased signal and claw sign based on b-values.
