Both teeth and the digestive tract show adaptations that are commonly interpreted in the context of trophic guilds—faunivory, herbivory and omnivory. Teeth prepare food for the digestive tract, and dental evolution focuses on increasing durability and functionality; in particular, size reduction of plant particles is an important preparation for microbial fermentative digestion. In narratives of digestive adaptations, microbes are typically considered as service providers, facilitating digestion. That the majority of 'herbivorous' (and possibly 'omnivorous') mammals display adaptations to maximize microbes' use as prey—by harvesting the microbes multiplying in their guts—is less emphasized and not reflected in trophic labels. Harvesting of microbes occurs either via coprophagy after separation from indigestible material by a separation mechanism in the hindgut, or from a forestomach by a 'washing mechanism' that selectively removes fines, including microbes, to the lower digestive tract. The evolution of this washing mechanism as part of the microbe farming niche opened the opportunity for the evolution of another mechanism that links teeth and guts in an innovative way—the sorting and cleaning of not-yet-sufficiently-size-reduced food that is then re-submitted to repeated mastication (rumination), leading to unprecedented chewing and digestive efficiency. Aim. The most widely reported problems among people who have been hospitalized for COVID-19 are those associated with mental health. The aim of the study was to determine the incidence of PTSD in patients hospitalized due to COVID-19 and to determine predictors of the risk of its occurrence. Material and methods. The study group included 250 people after hospitalization due to COVID-19. Several standardized measurement tools were used: the PCL-5, PSS-10, ERRI, SPP-15, and PANAS. Results. Overall, 55.6% of respondents qualified for a diagnosis of PTSD. These were mostly people hospitalized for more than two weeks, with a more severe course of the disease, examined within three and six months after hospitalization. The regression model includes factors related to the course of treatment and psychological variables: some increase the risk of PTSD (i.e., perceived stress, intrusive ruminations and negative emotions), while others are protective factors (i.e., resilience, deliberate ruminations and positive emotions). Overall, the regression model explains 72% of the total variance. Intrusive ruminations, explaining 59% of the variance, and perceived stress (12%) appeared the most significant for predicting PTSD. Conclusions. As patients diagnosed with PTSD require therapeutic assistance, it is essential to identify both risk and protective factors as these may be helpful in improving the therapy process and facilitating preventive activities. Summary: This study sought to further explain the association between general anxiety symptoms and impaired problem‐solving by testing whether this occurs, in part, through a reduced ability to retrieve event‐level, specific autobiographical memory (AM). Participants (N = 301; M age = 28.2 SD = 7.7, 55.8% female) completed assessments of the retrieval of specific AM, anxiety symptoms, depressive symptoms, and rumination. They then completed the Means‐End Problem Solving Task, which assessed their ability to produce relevant problem‐solving steps. Participants who were higher in anxiety reported a lesser number of relevant problem‐solving steps, and this association was, in part, related to anxiety being associated with reduced AM specificity (after controlling for depressive symptoms). Rumination did not mediate anxiety and problem‐solving, nor anxiety and AM specificity. These findings provide further evidence that elevated anxiety is associated with reduced ability to retrieve specific AM, and a specific cognitive pathway through which anxiety may affect problem‐solving performance. Neuroticism is strongly implicated in the development of sleep impairment and sleep disorders such as insomnia. However, it is unclear if neuroticism is a direct predictor of sleep, and/or if it moderates associations between related psychological processes (e.g., rumination and negative affect, or NA) and sleep in daily life. We investigated: 1) neuroticism as a between-person predictor of reported sleep, 2) daily fluctuations in rumination and NA as predictors of reported sleep, and 3) neuroticism as a between-person moderator of the relationships in aim 2. A sample of racially and ethnically diverse adults ( N = 242; 62% African-American, 24% Hispanic/Latina/o) completed a baseline assessment of neuroticism and then completed two weeks of daily reports, including bedtime assessments of daily rumination and NA and waking assessments of prior night's sleep. Individuals with higher neuroticism reported more impaired average sleep, but this relationship became non-significant after accounting for daily rumination and NA. Days with greater NA than one's individual average predicted more impaired sleep, regardless of average levels of NA, daily rumination, and neuroticism. Results suggest the importance of investigating both between- and within-person associations linking neuroticism, rumination, NA, and sleep and may help inform more targeted intervention efforts for improving sleep. Abstract: Rumination is a process of uncontrolled, narrowly focused negative thinking that is often self‐referential, and that is a hallmark of depression. Despite its importance, little is known about its cognitive mechanisms. Rumination can be thought of as a specific, constrained form of mind‐wandering. Here, we introduce a cognitive model of rumination that we developed on the basis of our existing model of mind‐wandering. The rumination model implements the hypothesis that rumination is caused by maladaptive habits of thought. These habits of thought are modeled by adjusting the number of memory chunks and their associative structure, which changes the sequence of memories that are retrieved during mind‐wandering, such that during rumination the same set of negative memories is retrieved repeatedly. The implementation of habits of thought was guided by empirical data from an experience sampling study in healthy and depressed participants. On the basis of this empirically derived memory structure, our model naturally predicts the declines in cognitive task performance that are typically observed in depressed patients. Kangaroos regurgitate and re-chew digesta on an occasional basis; the behaviour has been termed ‘merycism’. Merycism is distinct from rumination, as it is non-essential for digestive activity, significantly varies in rhythm and frequency, and causes seeming distress. Triggering mechanisms and potential benefits of this behaviour remain unknown. Although often reported, detailed descriptions of macropod merycism are lacking. We performed a systematic evaluation of a series of anecdotal observations of five western grey ( Macropus fuliginosus ) and one red kangaroos ( Macropus rufus ) in a feeding situation on two different diets in captivity. We recorded 14 videos with lucerne pellets and 3 videos with chopped lucerne hay feeding, covering a total of 4 h 23 min of footage with 23 regurgitation events. A characteristic merycism sequence consisted of retreat from the feeding trough, pumping abdominal contractions, slurping, re-chewing and re-swallowing. Individual kangaroos performed different behavioural features, apparently to support the abdominal contractions including throwing back their head (64 ± 40 (SD) %) or, rarely, pumping movements of the forelimbs. Additionally, instead of keeping digesta within the oral cavity and re-swallowing it, it was sometimes spilled (35 ± 40% of the cases). We confirmed characteristic features of merycism previously reported, including the clear distinction to rumination. We hypothesize that the purpose of merycism is the grinding of not sufficiently chewed food particles that trigger specific contractions of the forestomach beyond voluntary control. The numerically higher frequency of this behaviour during pellet compared to hay feeding could support this hypothesis, as pellets were often gobbled hastily. In accord with observations in the literature, we hypothesize that merycism happens more frequently in captivity, as the type of food, fixed feeding times and limited feeding spots make rapid ingestion of food, possibly without proper mastication (i.e. gobbling), more likely. Genome amplification (DNA synthesis) is one of the most demanding cellular processes in all proliferative cells. The DNA replication machinery (also known as the replisome) orchestrates genome amplification during S-phase of the cell cycle. Genetic material is particularly vulnerable to various events that can challenge the replisome during its assembly, activation (firing), progression (elongation) and disassembly from chromatin (termination). Any disturbance of the replisome leads to stalling of the DNA replication fork and firing of dormant replication origins, a process known as DNA replication stress. DNA replication stress is considered to be one of the main causes of sporadic cancers and other pathologies related to tissue degeneration and ageing. The mechanisms of replisome assembly and elongation during DNA synthesis are well understood. However, once DNA synthesis is complete, the process of replisome disassembly, and its removal from chromatin, remains unclear. In recent years, a growing body of evidence has alluded to a central role in replisome regulation for the ubiquitin-dependent protein segregase p97, also known as valosin-containing protein (VCP) in metazoans and Cdc48 in lower eukaryotes. By orchestrating the spatiotemporal turnover of the replisome, p97 plays an essential role in DNA replication. In this review, we will summarise our current knowledge about how p97 controls the replisome from replication initiation, to elongation and finally termination. We will also further examine the more recent findings concerning the role of p97 and how mutations in p97 cofactors, also known as adaptors, cause DNA replication stress induced genomic instability that leads to cancer and accelerated ageing. Co-rumination is associated with positive friendship quality (thought to buffer against anxiety and depression) but paradoxically higher levels of anxiety and depression. With the increasing use of technology for communication among adults, there is little known about co-rumination effects across different modalities of communication. In the current study, we examined co-rumination through four methods (i.e. in person, phone calls, text messaging, and social media) in two separate samples-college students and participants from the community. Classic co-rumination effects were found for in-person communications, and we found that co-rumination by telephone as well as by texting, for a college student sample only, mirrors some of these findings for in-person co-rumination. Aims Post-traumatic growth (PTG) refers to positive changes that result from coping with a major life crisis or a traumatic event. Factors influencing PTG in youth have not been well characterized but could have a great impact on overall maturation. Methods Fourth to ninth graders ( n = 3337) attending elementary or junior high schools located in a district severely damaged by the Great East Japan Earthquake completed a questionnaire, including the revised Posttraumatic Growth Inventory for Children (PTGI-C-R) along with the Post Traumatic Stress Symptoms for Children 15 items (PTSSC-15), 31 months after the event. Experience of the disaster, prior traumatic experiences, and attitudes toward memories of the disaster (rumination) were scored by their caregivers and teachers, and effects of these factors on PTG were evaluated. Results There were no significant associations between PTGI-C-R and sex or experiences of the disaster. However, PTGI-C-R was negatively correlated with age ( r = −0.132, P < 0.001). There was significant but very weak negative correlation between PTGI-C-R and PTSSC-15 among the children ( r = −0.096, P < 0.001). PTGI-C-R was significantly associated with positive attitudes to memorial services ( P < 0.001) and media coverage ( P < 0.05). The present longitudinal study tested hypotheses about the relationship of subjective well-being and neuroticism with rumination, anxiety, and post-traumatic stress in university students after a large scale disaster. Measures of subjective well-being and personality were obtained two months before the 2013 Santa Maria's fire. Measures of rumination, PTSD and anxiety were collected five months after the disaster with the same students. The results provide evidence that life satisfaction correlated negatively with rumination, anxiety, and post-traumatic stress. Positive affect presented similar but slightly smaller negative correlations with these variables, while negative affect presented higher correlations with rumination, anxiety, and post-traumatic stress. These findings provide evidence that subjective well-being components may constitute important predictors of psychopathological symptomatology after a disaster and may be helpful to plan clinical interventions. This quasi-experimental longitudinal study assessed the effect of a one-day Cognitive Behaviour Therapy (CBT)-based workshop on work-related rumination, chronic fatigue, and sleep quality. We hypothesized that participants who attended the workshop would report lower levels of affective work-related rumination, chronic fatigue, and improved sleep quality, at follow-up, 6 months after workshop completion. Two hundred and twenty-seven participants took part in the study, with 102 participants attending a one-day workshop delivered in their workplace. Participants completed an online questionnaire at two time-points, with follow-up occurring 6 months after initial survey completion. Results showed that participants who took part in the CBT workshop reported significantly lower levels of affective rumination (p = .03) and chronic fatigue (p = .003), at follow-up in comparison with individuals who did not attend the workshop; however, there were no significant differences between the groups in self-reported sleep quality (p = .06). A combination of more effective recovery both at work and outside of work may explain the reductions in both affective rumination and fatigue over time. Behavioral observations and small fecal particles compared to other primates indicate that free-ranging proboscis monkeys ( Nasalis larvatus ) have a strategy of facultative merycism (rumination). In functional ruminants (ruminant and camelids), rumination is facilitated by a particle sorting mechanism in the forestomach that selectively retains larger particles and subjects them to repeated mastication. Using a set of a solute and three particle markers of different sizes (< 2, 5 and 8 mm), we displayed digesta passage kinetics and measured mean retention times (MRTs) in four captive proboscis monkeys (6–18 kg) and compared the marker excretion patterns to those in domestic cattle. In addition, we evaluated various methods of calculating and displaying passage characteristics. The mean ± SD dry matter intake was 98 ± 22 g kg − 0. 75 d − 1 , 68 ± 7% of which was browse. Accounting for sampling intervals in MRT calculation yielded results that were not affected by the sampling frequency. Displaying marker excretion patterns using fecal marker concentrations (rather than amounts) facilitated comparisons with reactor theory outputs and indicated that both proboscis and cattle digestive tracts represent a series of very few tank reactors. However, the separation of the solute and particle marker and the different-sized particle markers, evident in cattle, did not occur in proboscis monkeys, in which all markers moved together, at MRTs of approximately 40 h. The results indicate that the digestive physiology of proboscis monkeys does not show typical characteristics of ruminants, which may explain why merycism is only a facultative strategy in this species. Objective There is growing interest in the role of transdiagnostic processes in the onset, maintenance, and treatment of mental disorders (Nolen-Hoeksema & Watkins, 2011). Two such transdiagnostic processes-rumination and reappraisal-are the focus of the present study. The main objective was to examine the roles of rumination (thought to be harmful) and reappraisal (thought to be helpful) in adults with social anxiety disorder (SAD). Method We conducted a randomized controlled trial of cognitive-behavioral therapy (CBT) with 75 adults with SAD and examined pre- to post-CBT changes as well as weekly fluctuations in rumination, reappraisal, and social anxiety symptoms. Results Socially anxious individuals' baseline rumination (brooding) scores predicted weekly levels of social anxiety, rumination, and reappraisal, whereas baseline reappraisal scores did not. Greater weekly rumination was associated with greater weekly social anxiety, but reappraisal was not related to social anxiety. Conclusion These findings suggest that rumination may have a more significant role than reappraisal in understanding fluctuations in social anxiety during CBT for SAD. [ABSTRACT FROM AUTHOR]

Major Depressive Disorder (MDD) is characterized by rumination. Prior research suggests that resting-state brain activation reflects rumination when depressed individuals are not task engaged. However, no study has directly tested this. Here we investigated whether resting-state epochs differ from induced ruminative states for healthy and depressed individuals. Most previous research on resting-state networks comes from seed-based analyses with the posterior cingulate cortex (PCC). By contrast, we examined resting state connectivity by using the complete multivariate connectivity profile (i.e., connections across all brain nodes) and by comparing these results to seeded analyses. We find that unconstrained resting-state intervals differ from active rumination states in strength of connectivity and that overall connectivity was higher for healthy vs. depressed individuals. Relationships between connectivity and subjective mood (i.e., behavior) were strongly observed during induced rumination epochs. Furthermore, connectivity patterns that related to subjective mood were strikingly different for MDD and healthy control (HC) groups suggesting different mood regulation mechanisms. Excessive belching is a commonly observed complaint in clinical practice that can occur not only as an isolated symptom but also as a concomitant symptom in patients with gastroesophageal reflux disease (GERD) or functional dyspepsia. Impedance monitoring has revealed that there are two mechanisms through which belching can occur: the gastric belch and the supragastric belch. The gastric belch is the result of a vagally mediated reflex leading to relaxation of the lower esophageal sphincter and venting of gastric air. The supragastric belch is a behavioral peculiarity. During this type of belch, pharyngeal air is sucked or injected into the esophagus, after which it is immediately expulsed before it has reached the stomach. Patients who belch excessively invariably exhibit an increased incidence of supragastric, not of gastric belches. Moreover, supragastric belches can elicit regurgitation episodes in patients with the rumination syndrome and sometimes appear to induce reflux episodes as well. Behavioral therapy has been proven to decrease belching complaints in patients with isolated excessive belching, but its effect is unknown in frequently belching patients with GERD, functional dyspepsia or rumination. OBJECTIVES:The rumination syndrome is a behavioral disorder resulting in recurrent regurgitation of undigested food. The diagnosis of this syndrome is currently based on clinical features. We aimed to determine criteria for the rumination syndrome based on physiological measurements.METHODS:We studied patients with clinically confirmed rumination syndrome and gastroesophageal reflux disease (GERD) patients with predominant symptoms of regurgitation. All patients underwent combined high-resolution manometry and pH-impedance measurement after a standardized meal. All reflux events extending to the proximal esophagus were analyzed. Furthermore, ambulatory measurements were performed in the majority of patients.RESULTS:In the rumination group, the amplitude of the abdominal pressure increase during proximal reflux events and the esophageal pressure peaks were significantly higher compared with GERD patients. None of the GERD patients exhibited abdominal pressure peaks >30 mm Hg, whereas in the rumination patients 70% of the pressure peaks had an amplitude >30 mm Hg. Abdominal pressure patterns were also observed during ambulatory pH impedance-pressure monitoring in the rumination patients. pH-impedance monitoring alone could not differentiate between GERD and rumination, however, a higher percentage of reflux events reached the proximal esophagus in the rumination patients. Notably, three different mechanisms of rumination were observed: (i) primary rumination, in which the abdominal pressure increase preceded the retrograde flow, (ii) secondary rumination, consisting of an increase in abdominal pressure following the onset of a reflux event and (iii) supragastric belch-associated rumination, consisting of a supragastric belch immediately followed by a rumination event.CONCLUSIONS:The diagnosis of the rumination syndrome can be made when reflux events extending to the proximal esophagus that are closely associated with an abdominal pressure increase >30 mm Hg and an esophageal pressure increase are observed during combined pressure-impedance monitoring. Extensive studies have been conducted on the rumination behavior of domestic herbivores. However, studies on wild animals are limited, particularly wild animals with specific ruminating parameters. In this study, Père David’s deer, a previously extirpated species, was observed to analyze the effects of sex-age, feeding habitat, and rainfall on rumination behavior in the Dafeng Nature Reserve, China. Rumination behavior was investigated based on four parameters: proportion of bedding time spent chewing, bolus processing time (s/bolus), chewing frequency (chews/bolus), and chewing rate (chews/s). Results showed that all three factors affect rumination behavior. The extent of their effects varied based on the four rumination parameters. Chewing rate and frequency decreased based on sex–age levels, i.e., from fawns to juvenile female, juvenile male, adult female, stag, and harem holder. Therefore, body size played a major role in shaping rumination behavior. Deer found in grasslands could chew faster compared with deer found in woodlands. This result might be caused by the effects of dietary composition and sunlight intensity. A deer spends a longer time ruminating while bedding during rainy days compared with rainless days to maximize energy and nutrition intake and compensate for the loss of feeding time during rainy days. This article explores the relatively neglected topic of the role of Scripture in public theology. It proffers a provisional taxonomy of approaches to relating Scripture to pub-lic theology, with a view to demonstrating that there are various ways in which Scrip-ture and Scripture scholarship play a vital role in public theology, broadly construed. It then discusses in more detail three of the eight approaches, focusing especially on recent works by Gerd Theissen and Paul Hanson, and illustrating the value of inner-biblical critique for public theology with reference to the themes of violence and justice. Ruminative habits of thought about one's problems and the resulting consequences are correlated with symptoms of depression and cognitive biases (Nolen-Hoeksema, Wisco, & Lyubomirsky, 2008). In our orienting task, brooders and non-brooders concentrated on self-focusing phrases while they were also exposed to neutral target words. On each trial in the unfocused condition, participants saw and then reported the target before concentrating on the phrase; in the focused condition, the target was reported after phrase concentration. A brooding-related deficit on a subsequent unexpected test of free and forced recall was obtained in the unfocused condition only. Brooders recalled more successfully in the focused than in the unfocused condition. Like many other mental disorders, depression is characterised by psychological inflexibility. Two instances of such inflexibility are rumination: repetitive cognitions focusing on the causes and consequences of depressive symptoms; and emotional inertia: the tendency for affective states to be resistant to change. In two studies, we tested the predictions that: (1) rumination and emotional inertia are related; and (2) both independently contribute to depressive symptoms. We examined emotional inertia of subjective affective experiences in daily life among a sample of non-clinical undergraduates (Study 1), and of affective behaviours during a family interaction task in a sample of clinically depressed and non-depressed adolescents (Study 2), and related it to self-reported rumination and depression severity. In both studies, rumination (particularly the brooding facet) and emotional inertia (particularly of sad/dysphoric affect) were positively associated, and both independently predicted depression severity. The article reports on the proposed change for both pica and rumination disorder in the Diagnostic and Statistical Manual for Mental Disorders, Fifth Edition (DSM-V) to relocate from their section, titled "Disorders Usually First Diagnosed in Infancy, Childhood, or Adolescence," to the proposed section "Feeding and Eating Disorders." The recommendation suggests that consumption of products regarded as foods or beverages would not be consistent with pica. The prevalence of pica is discussed.

The development of the high-resolution esophageal manometry (HRM) and the Chicago classification have improved the diagnosis and management of esophageal motility disorders. However, some conditions have yet to be addressed by this classification. This review describes findings in HRM which are not included in the current Chicago classification based on the experience in our center. This includes the analysis of the upper esophageal sphincter, proximal esophagus, longitudinal muscle contraction, disorders related to gastroesophageal reflux disease and respiratory symptoms. The utility of provocative tests and the use of HRM in the evaluation of rumination syndrome and post-surgical patients will also be discussed. We believe that characterization of the manometric findings in these areas will eventually lead to incorporation of new criteria into the existing classification. [ABSTRACT FROM AUTHOR]

Abstract: After a selective overview of the literature about merycism, the author proposes – with lacanian theory – to question early frustration and their relations with psychosomatic phenomena. In this way, we’ll see that we can’t understand early object relationships between the mother and his baby without this fundamental concept. We also see that the question concerning the “desire of the great Other” (Lacan) is clearly the center point about this strange alimentary behavior. Rumination to negative affect has been linked to the onset and maintenance of mood disorders in adults as well as children. Responses to positive affect have received far less attention thus far. A few recent studies in adults suggest that responses to positive affect are involved in the development of both depressive and hypomanic symptoms, but thus far no study has investigated their role in childhood mood problems. The purpose of the present study was to validate a child version of the Responses to Positive Affect questionnaire and examine the extent to which responses to positive affect prospectively predict mood symptoms over a 3-month interval. The Responses to Positive Affect questionnaire for Children was found to assess two types of responses to positive affect: Positive Rumination and Dampening. Both subscales showed sufficient internal consistency and moderate stability over a 3-month interval. Low levels of positive rumination and high levels of dampening were concurrently associated with depressive symptoms, over and above responses to negative affect. Importantly, low levels of positive rumination also predicted increases in depressive symptoms over a 3-month interval over and above baseline symptoms in children reporting high levels of stress. Both positive rumination and dampening were positively related to concurrent hypomanic symptoms and high levels of positive rumination predicted increases in hypomanic symptoms over a 3-month interval over and above baseline symptoms in children reporting high levels of stress. Background & Aims: In patients with clinically suspected rumination, esophageal impedance manometry differentiates episodes of rumination (involuntary straining with intragastric pressure increases) from aerophagia/supragastric belching. Treatment options are limited and focused on behavioral therapy. Baclofen, an agonist of the γ-aminobutyric acid B receptor, increases lower esophageal sphincter pressure and decreases swallowing rate. We investigated its effects in these patients. Methods: High-resolution manometry-impedance recordings were taken from 12 patients (8 women; mean age, 45 years; range, 18–89 years) with clinically suspected rumination or supragastric belching before and during treatment with baclofen (10 mg, 3 times daily). After 30 minutes of recordings, patients received a 1000-kcal solid meal; recordings were then continued for 1 hour. Patients were asked to register symptoms with an event marker. The number of symptoms registered and number and type of flow events were compared before and during treatment. Results: An average of 20 symptom markers (range, 14–34) were recorded at baseline (10 [range, 4–25] for belching and 9 [range, 0–11] for regurgitation). This was significantly reduced to 6 (range, 2–22) (3 [range, 1–15] for belching and 1 [range, 0–13] for regurgitation) during baclofen treatment (P = .01). The number of flow events (473 at baseline [42 reflux, 192 rumination, 188 supragastric belching, and 42 aerophagia]) was significantly reduced to 282 (32 reflux, 99 rumination, 123 supragastric belching, and 13 aerophagia) during baclofen therapy (P = .02). The reduction in flow events correlated with the increase in lower esophageal sphincter pressure (r = −0.62; P = .03) and reduction in swallowing frequency (r = 0.64; P = .02). This study examined the relationship between social anxiety (SA) and upward counterfactual thinking (U-CFT; 'if only...' thoughts imagining better outcomes to past events). Although generally considered adaptive, U-CFT has also been associated with negative affect and social anxiety (e.g., Kocovski, Endler, Rector, & Flett, 2005). Participants ( n == 88) were randomly assigned to generate U-CFT after reading a scenario depicting either controllable social-evaluative failure (e.g., lack of preparation for a presentation) or uncontrollable social-evaluative failure (e.g., technological malfunction during a presentation for which one is well-prepared). When both scenarios were analyzed together, SA was not significantly related to U-CFT. Instead, scenario controllability emerged as a significant predictor of U-CFT, qualified by a significant interaction between SA and scenario controllability. Indeed, in the uncontrollable scenario, SA was not related to U-CFT generation; in contrast, in the controllable scenario, SA was significantly correlated with U-CFT, such that higher levels of SA predicted more U-CFT. The relationship between SA and U-CFT in the controllable condition could not be accounted for by self-reported depression, perfectionism, or rumination. Background: Rumination syndrome (RS) is a functional gastrointestinal disorder (FGD) increasingly recognized in children and adolescents. The epidemiology of this condition in school aged children is poorly understood. The main objective of this study was to assess the prevalence of rumination and other related associations in a cohort of Sri Lankan children. Methods: Children aged 10-16 years were randomly selected from 8 schools in 4 provinces in Sri Lanka. RS was diagnosed using Rome III criteria. Data was collected using a self administered questionnaire distributed in an examination setting. It was translated into Sinhala, the native language and pretested before distribution. Results: A total of 2163 children were included in the study (55% boys, mean age 13.4 years, SD 1.8 years). Prevalence of RS was 5.1% (n = 110); boys 5.1% and girls 5.0%. When symptoms were analyzed, 73.6% reported re-swallowing of regurgitated food, while the rest spat it out. In 94.5% regurgitation occurred during the first hour after the meal. Only 8.2% had daily symptoms while 62.7% had symptoms weekly. Abdominal pain, bloating and weight loss were the commonest symptoms associated with RS (19.1%, 17.3% and 11.8% respectively). No significant association was observed between exposure to stressful events and rumination (p > 0.05). Twenty (18.2%) with RS fulfilled Rome III criteria for at least one other FGD. School absenteeism was seen in 11.8% of affected children. Conclusion: RS was reasonably common in this cohort of school-aged children and adolescents in Sri Lanka. Abstract: A process account of eating disorders (EDs) () proposes that preoccupation with ruminative themes of eating, weight and shape may be important in ED maintenance. No self-report measure exists to capture disorder-specific rumination in EDs. 275 healthy participants rated rumination items and completed self-report measures of ED symptoms, depression and anxiety. Principal component analysis revealed two factors, reflection and brooding. The final nine-item Ruminative Response Scale for Eating Disorders (RRS-ED) demonstrated good convergent and discriminant validity and test–retest reliability. The psychometric properties were replicated in an anorexia nervosa sample. The findings support the notion that rumination in EDs is distinct from rumination in depression and is not adequately captured by existing measures. [Copyright &y& Elsevier]

Background. Rumination syndrome is a disorder of unknown etiology characterized by regurgitation of recently ingested food. We aimed to improve the diagnosis of rumination syndrome by classification of separate rumination symptoms using (1) an ambulatory manometry/impedance (AMIM) measurement and (2) a single-catheter high-resolution manometry/impedance (HRIM) measurement. Methods. A total of 96 symptoms during AMIM and 37 symptoms during HRIM were analyzed in five patients with clinically diagnosed rumination syndrome. Key results. AMIM identified rumination events in 85 out of 96 reported symptoms (symptom index (SI): 89%). Of these events, 63% were non-acidic and would have been missed by pH-metry. HRIM identified 32 out of 37 reported symptoms (SI: 86%). Upper esophageal sphincter (UES) relaxation was observed during all rumination events identified by HRIM and could be an additional criterion in the definition of rumination events. Conclusions. Impedance measurement and high-resolution manometry contribute to a more detailed description of rumination events. Rumination events defined as gastric strain, common cavity phenomenon, retrograde esophageal fluid flow, and UES relaxation show a high SI when measured with AMIM or single-catheter HRIM. Abstract: We conducted a systematic analysis of studies that involved the treatment of rumination and operant vomiting in individuals with developmental disabilities. A total of 21 studies involving a combined 32 participants were identified and analyzed in terms of (a) participant characteristics, (b) dependent variables, (c) intervention procedures, (d) functional assessment procedures and results, (e) intervention outcomes, and (f) certainty of evidence. In comparison to previous reviews on rumination and operant vomiting, this review identified fewer studies involving punishment-based interventions and an increase in function-based reinforcement interventions. Preliminary guidelines for practitioners faced with assessing and treating these behaviors are offered and directions for future research are discussed. [Copyright &y& Elsevier]

The article discusses research examining regurgitation and remastication behaviours in the foregut-fermenting proboscis monkey Nasalis larvatus. Video camera event sampling was used to effectively observe behaviours of monkeys at Kinabatangan River in Malaysia. Results show that the behaviours occurred in both male and female monkeys. Additional studies are required to understand the physiological and ecological connotations of the behaviour.

Abstract: Although rumination is an important mediator of depressive symptoms, there is insufficient proof that an intervention that specifically targets rumination ameliorates the clinical condition of, depressed patients. This study investigates whether a time-limited cognitive behavioral intervention (Competitive Memory Training, or COMET for depressive rumination) is an effective treatment for depression and rumination. This intervention was tested in older adult depressed outpatients. A total of 93 patients (aged ≥65 years with major depression and suffering from rumination) were treated in small groups according to the COMET protocol in addition to their regular treatment. Patients were randomized to two treatment conditions: 7 weeks of COMET + treatment-as-usual (TAU) versus TAU only. COMET + TAU showed a significant improvement in depression and rumination compared with TAU alone. This study shows that the transdiagnostic COMET protocol for depressive rumination might also be successful in treating depression and rumination in older adults. [Copyright &y& Elsevier]

Background. Chronic forms of depression are highly prevalent especially in inpatient settings. Defined only by episode duration greater than 2 years, heterogeneous diagnoses like dysthymic disorder, double depression, major depression and recurrent major depression are included. Chronic depression is considered difficult to treat, although its distinguishing characteristics remain unclear. Aims. The purpose of the present study was empirically to approach proposed differences between acute and chronic forms of depression. Methods. 80 psychiatric inpatients positively screened for depression and diagnosed with the Composite International Diagnostic Interview (CIDI), completed a set of questionnaires regarding symptom severity (a simplified version of the Beck Depression Inventory), dysfunctional attitudes (Dysfunctional Attitudes Scale), ruminative response style (Response Styles Questionnaire, Why Ruminate Scale) and interpersonal problems (Inventory of Interpersonal Problems; IIP). Results. 30 patients were diagnosed with chronic forms of depression; 34 patients with acute depression. Patients did not differ regarding symptom severity, ruminative response styles, all but one subscales of the IIP or presence of comorbid post-traumatic stress disorder. However, chronic depression did show higher dysfunctional attitudes than acute depression, and reported higher rates of socially avoidant behavior. Conclusions. In line with previous findings, acute and chronic forms of depression differed in dysfunctional attitudes, which might actually reflect a distinguishing pattern of chronicity. Chronic depression patients also reported higher socially avoidant behavior, which might be crucial to focus in treatment. Surprisingly, other variables that are assumed to characterize chronic depression could not be confirmed as distinguishing features. Future research should take etiological aspects into account. This study examined whether polymorphisms in the serotonin transporter (SLC6A4, 5-HTTLPR) and brain-derived neurotropic factor (BDNF Val66Met, rs6265) genes moderate the relationship between life stress and rumination. Participants were a large homogenous group of healthy, unmedicated, never depressed individuals with few current symptoms of depression ( N = 273). Results indicate that individuals with two short (S) alleles of the 5-HTTLPR polymorphism or two Met alleles of the BDNF Val66Met polymorphism ruminate more under conditions of life stress, compared to the other genotypes. Moreover, the accumulation of risk alleles (i.e. S and Met alleles) across genes is associated with significantly greater rumination in the context of life stress. These results suggest that both 5-HTTLPR and BDNF Val66Met moderate the relationship between life stress and rumination. Background: Major depressive disorder (MDD) has been associated reliably with ruminative responding; this kind of responding is composed of both maladaptive and adaptive components. Levels of activity in the default-mode network (DMN) relative to the task-positive network (TPN), as well as activity in structures that influence DMN and TPN functioning, may represent important neural substrates of maladaptive and adaptive rumination in MDD. Methods: We used a unique metric to estimate DMN dominance over TPN from blood oxygenation level-dependent data collected during eyes-closed rest in 17 currently depressed and 17 never-disordered adults. We calculated correlations between this metric of DMN dominance over TPN and the depressive, brooding, and reflective subscales of the Ruminative Responses Scale, correcting for associations between these measures both with one another and with severity of depression. Finally, we estimated and compared across groups right fronto-insular cortex (RFIC) response during initiations of ascent in DMN and in TPN activity. Results: In the MDD participants, increasing levels of DMN dominance were associated with higher levels of maladaptive, depressive rumination and lower levels of adaptive, reflective rumination. Moreover, our RFIC state-change analysis showed increased RFIC activation in the MDD participants at the onset of increases in TPN activity; conversely, healthy control participants exhibited increased RFIC response at the onset of increases in DMN activity. Conclusions: These findings support a formulation in which the DMN undergirds representation of negative, self-referential information in depression, and the RFIC, when prompted by increased levels of DMN activity, initiates an adaptive engagement of the TPN. Background: Although rumination is a key process in the onset and maintenance of depressive symptoms and a powerful predictor of persistent posttraumatic stress disorder (PTSD), little is known about the differences and similarities of rumination in these conditions. Previous research has not always differentiated between rumination and intrusive images. Aims: We sought to systematically evaluate rumination and to gather more information about the content and associated emotions in three patient groups (PTSD, and depressed with and without trauma; n = 65). Furthermore, we examined the interaction between rumination and another predominant intrusive cognition, intrusive image. Method: A multi-method assessment for rumination, including a rumination questionnaire and a rumination log (kept for one week), was employed. Results: Rumination was found to be complex and composed of subcomponents that are similar across the diagnostic groups. Rumination rarely stopped intrusive images and it made the participants feel worse. There were, however, also important differences: in PTSD, rumination always or often triggered intrusive images and the traumatized individuals (PTSD and depressed with trauma) ruminated more than non-traumatized depressed patients. Conclusions: The results corroborate the assumption of rumination being a transdiagnostic process, with similarities but also with important differences across diagnostic groups. Objective: This investigation attempted to determine whether trait and state hostile rumination functioned as risk factors for the relation between acute alcohol intoxication and aggression. Method: Participants were 516 social drinkers (252 men and 264 women). Trait hostile rumination was assessed using Caprara's Dissipation-Rumination Scale. Following the consumption of either an alcohol or a placebo beverage, participants were tested on a laboratory task in which electric shocks were received from and administered to a fictitious opponent under the guise of a competitive reaction-time task. Aggression was operationalized as the combined mean responses for shock intensity and duration across all trials. In a subset of the sample (n = 320), state hostile rumination was assessed following the aggression task using a self-report measure. Results: As expected, both trait and state measures acted as moderators. Specifically, acute alcohol intoxication was more likely to increase aggression in persons with higher trait and state hostile rumination scores compared with their equally intoxicated lower rumination counterparts. Conclusions: This was the first investigation to demonstrate that trait or state rumination significantly heighten the risk of intoxicated aggression. We believe that hostile rumination facilitates intoxicated aggression because ruminators have difficulty diverting their attention away from anger-provoking stimuli and related thoughts, thus making violent reactions more likely. Clinical and public health interventions would benefit by developing strategies to distract ruminative attention away from violence-promoting messages, especially when persons are under the influence of alcohol. Background Rumination syndrome, characterised by the effortless, often repetitive, regurgitation of recently ingested food into the mouth, was originally described in children and in the developmentally disabled. It is now well-recognised that rumination syndrome occurs in patients of all ages and cognitive abilities. Aim To review a scholarly review on our current understanding of the rumination syndrome. Methods The review was conducted on the basis of a medline search to identify relevant publications pertaining to the pathophysiology, clinical diagnosis and management of rumination syndrome. Results The Rome III consensus established diagnostic criteria for rumination syndrome in adults, children and infants. A typical history can be highly suggestive but oesophageal (high resolution) manometry / impedance with ingestion of a meal may help to distinguish rumination syndrome from other belching / regurgitation disorders. The pathophysiology is incompletely understood, but involves a rise in intra-gastric pressure, generated by a voluntary, but often unintentional, contraction of the abdominal wall musculature, at a time of low pressure in the lower oesophageal sphincter, causing retrograde movement of gastric contents into the oesophagus. To date, controlled trials in the treatment rumination syndrome are lacking. The mainstay of treatment for rumination syndrome is explanation and behavioural treatment which consists of habit reversal techniques that compete with the urge to regurgitate. Chewing gum, prokinetics, baclofen and even antireflux surgery have been proposed as adjunctive therapies, but high quality studies are generally lacking. Conclusions Rumination is an under-recognised condition with incompletely understood pathophysiology. Behavioural therapy seems effective, but controlled treatment trials are lacking. Background: It is recommended that Mindfulness-Based Cognitive Therapy (MBCT) instructors should undertake MBCT themselves before teaching others. Aim: To investigate the impact of MBCT (modified for stress not depression) on trainee clinical psychologists. Method: Twenty trainees completed questionnaires pre- and post-MBCT. Results: There was a significant decrease in rumination, and increases in self-compassion and mindfulness. More frequent home practice was associated with larger decreases in stress, anxiety and rumination, and larger increases in empathic concern. Only first-year trainees showed a significant decrease in stress. Content analysis of written responses indicated that the most commonly reported effects were increased acceptance of thoughts/feelings (70%), increased understanding of what it is like to be a client (60%), greater awareness of thoughts/feelings/behaviours/bodily sensations (55%) and increased understanding of oneself and one's patterns of responding (55%). Participants reported increased metacognitive awareness and decentring in relation to negative thoughts. Eighty-five percent reported an impact on their clinical work by the end of the course. Conclusions: Trainee psychologists undergoing MBCT experienced many of the psychological processes/effects that they may eventually be helping to cultivate in clients using mindfulness interventions, and also benefits in their general clinical work. The present study investigated the relationships between two forms of ruminative thoughts, brooding, and reflective processing, in relation with irrational beliefs and their impact on distress. While significant amount of data support the role of irrational beliefs and depressive rumination as vulnerability factors in distress, no attempts have been made so far to study the relationships between these individual characteristics. Participants completed a battery of questionnaires including measures of the above constructs. Results show that irrational beliefs, brooding and reflective pondering are related to distress, and that the impact of irrational beliefs on distress is completely mediated by brooding. Abstract: Persisting negative thoughts are considered a hallmark of depression. Recent information-processing approaches have begun to uncover underlying mechanisms of depressive rumination. Despite marked advances in this area, there is a lack of integration between psychopathology and cognitive (neuro) science research. We propose the ‘impaired disengagement’ hypothesis as a unifying framework between both approaches. The core tenet of our model is that prolonged processing of self-referent material is due to impaired attentional disengagement from negative self-referent information. We discuss empirical evidence for this framework and outline future ways in which the causal predictions of this model can be tested. Overgeneral autobiographical memory (OGM) is an important cognitive phenomenon in depression, but questions remain regarding the underlying mechanisms. The CaR-FA-X model (Williams et al., 2007) proposes three mechanisms that may contribute to OGM, but little work has examined the possible additive and/or interactive effects among them. We examined two mechanisms of CaR-FA-X: capture and rumination, and impaired executive control. We analysed data from undergraduates (N=109) scoring high or low on rumination who were presented with cues of high and low self-relevance on the Autobiographical Memory Test (AMT). Executive control was operationalised as performance on both the Stroop Colour-Word Task and the Controlled Oral Word Association Test (COWAT). Hierarchical generalised linear modelling was used to predict whether participants would generate a specific memory on a trial of the AMT. Higher COWAT scores, lower rumination, and greater cue self-relevance predicted a higher probability of a specific memory. There was also a rumination×cue self-relevance interaction: Higher (vs lower) rumination was associated with a lower probability of a specific memory primarily for low self-relevant cues. We found no evidence of interactions between these mechanisms. Findings are interpreted with respect to current autobiographical memory models. Intrusive thoughts about traumatic events are an important factor in determining the development of posttraumatic growth, although research has focused on the frequency rather than the type of intrusions. Based on cognitive processing models of posttraumatic growth, the authors present two cross-sectional studies exploring different types of intrusive ruminations about trauma and their associations with posttraumatic growth. Study 1 examines brooding and reflection; Study 2 uses measures of both deliberate and intrusive rumination regarding a past trauma. The results indicate that intrusive reexperiencing and ruminative brooding are not significantly associated with posttraumatic growth, whereas deliberate rumination is significantly positively associated with posttraumatic growth (β = .44, p < .001) and reflection (partial r = .20, p = .003) is positively associated with posttraumatic growth in the context of low brooding. Abstract: Alexithymia and ruminative thinking style are two dispositions typically associated with depression. Alexithymia encompasses difficulties identifying feelings (DIF), difficulties describing feelings (DDF) and externally oriented thinking (EOT). Rumination consists of repetitive thinking about one’s own state, which has adaptive or maladaptive consequences, depending on the processing mode involved. This is maladaptive when the mode is abstract-analytic and adaptive when it is concrete-experiential (Watkins, 2008). In order to investigate the combined contribution of alexithymia and rumination in depression, the present study investigated correlations between the multiple dimensions of alexithymia and rumination before and after controlling for depressive symptoms. The aim was to see which alexithymia dimensions are associated with abstract-analytic rumination, which ones with concrete-experiential rumination, and which dimensions are not related to rumination at all. Self-report measures of depressive symptoms, alexithymia and rumination were administered to a non-clinical sample (N =174, M age =21.40). After controlling for depression, two complementary patterns emerged, and a null association: DIF positively correlated with abstract-analytic rumination, EOT negatively correlated with concrete-experiential rumination, whereas no association was found between DDF and any rumination component.
