A new mode of reactivity of N‐sulfonylhydrazones towards alkynes is described: ketone‐derived N‐tosylhydrazones (NTHs) bearing α‐H atoms behave as [NN] synthons in their reactions with alkoxy alkynyl Fischer carbene complexes (FCCs) and only the two N atoms are incorporated into the newly formed pyrazole ring. The scope of the reaction, leading to 5‐methoxy‐N‐alkenyl pyrazoles, proved to be wide regarding both partners; thus, a variety of aryl and primary, secondary, and tertiary alkyl substituents are tolerated within the FCC, while aryl alkyl NTHs, symmetric and asymmetric dialkyl NTHs and NTHs derived of cyclic ketones can be also employed. In addition, the reaction can be performed at large scale and applied for late‐stage diversification of natural products. A reasonable reaction mechanism is also proposed, which is supported by deuteration experiments and DFT calculations. [ABSTRACT FROM AUTHOR]

We studied the influence of the superconducting state on the magnetic properties in a thick IrMn(100 nm)/NiFe(100 nm) bilayer deposited by magnetron sputtering onto an yttrium–barium–copper–oxide (YBCO) substrate that was previously synthesized by the acetate method. The results from magnetization experiments showed that the sign of the effective exchange fields switched from positive, in the as-prepared heterostructure, to negative values when the sample goes below the superconducting temperature of the YBCO substrate. We, thus, demonstrated that the YBCO substrate, in its superconducting state, strongly influences the magnetic anisotropy of the thick ferromagnetic NiFe layer due to the proximity effect that occurs at the YBCO–NiFe interface, where a non-collinear magnetic spin structure is formed during the in-field sample deposition. This study was framed in the research: Characterization of professional competency-based model in medical education developed in twelve clinical and nine surgical specializations at the Faculty of Medicine, University of Antioquia. Its aim was to inquire about the state of the art in medical postgraduate education. The guiding question was: Where is present-day research headed in medical postgraduate education. For this descriptive, nonexperimental work, 12 bibliographic databases were reviewed and 28 research articles related to graduate medical formation were selected. The findings were compared, analyzed and interpreted. The tendency in research on graduate medical education points to the need of having multi-inter-trans-disciplinary and humanistic proposals based on constructivism; to consider evaluation as a process emphasizing on learning and the participation of students, and to build systems of pedagogical formation of tutors and interactive and flexible curricula. The lack of studies that promote competencies-based training in postgraduate medical education is notorious.

The article is derived from the research Narratives of students, parents, teachers and directors about the processes of educational inclusion-exclusion in extracurricular students at the Monseñor Francisco Cristóbal Toro Educational Institution in the city of Medellín, Colombia during the COVID-19 pandemic. In this sense, this text intends to problematize the discourse of inclusion in the educational context from the sociological theory of reproduction proposed by authors such as Bourdeau and Francoi Dubet and from some philosophical postulates of Kantian theory. It is argued as a thesis that the processes of inclusion and exclusion within the school are not unrelated to the processes of inequality that are woven in society, hence the difficulty of putting into practice the legal discourse of inclusion and the need to look at it as an ethical bet of the teacher. Research on m6A-associated SNPs (m6A-SNPs) has emerged recently due to their possible critical roles in many key biological processes. In this sense, several investigations have identified m6A-SNPs in different diseases. In order to gain a more complete understanding of the role that m6A-SNPs can play in breast cancer, we performed an in silico analysis to identify the m6A-SNPs associated with breast cancer and to evaluate their possible effects. For this purpose, we downloaded SNPs related to breast cancer and a list of m6A-SNPs from public databases in order to identify which ones appear in both. Subsequently, we assessed the identified m6A-SNPs in silico by expression quantitative trait loci (eQTL) analysis and differential gene expression analysis. We genotyped the m6A-SNPs found in the in silico analysis in 35 patients with breast cancer, and we carried out a gene expression analysis experimentally on those that showed differences. Our results identified 981 m6A-SNPs related to breast cancer. Four m6A-SNPs showed an eQTL effect and only three were in genes that presented an altered gene expression. When the three m6A-SNPs were evaluated in the tissue sample of our breast cancer patients, only the m6A-SNP rs76563149 located in ZNF354A gene presented differences in allele frequencies and a low gene expression in breast cancer tissues, especially in luminal B HER2+ subtype. Future investigations of these m6A-SNPs should expand the study in different ethnic groups and increase the sample sizes to test their association with breast cancer and elucidate their molecular function. Background Non-adherence to medications continues to be a burden worldwide, with significant negative consequences. Community pharmacist interventions seem to be effective at improving medication adherence. However, more evidence is needed regarding their impact on disease-specific outcomes. The aim was to evaluate the impact of a community pharmacist-led adherence management intervention on adherence and clinical outcomes in patients with hypertension, asthma and chronic obstructive pulmonary disease (COPD). Methods A 6-month cluster randomised controlled trial was conducted in Spanish community pharmacies. Patients suffering from hypertension, asthma and COPD were recruited. Patients in the intervention group received a medication adherence management intervention and the control group received usual care. The intervention was based on theoretical frameworks for changing patient behaviour. Medication adherence, disease-specific outcomes (Asthma Control Questionnaire (ACQ) scores, Clinical COPD Questionnaire (CCQ) scores and blood pressure levels) and disease control were evaluated. A multilevel regression model was used to analyse the data. Results Ninety-eight pharmacies and 1186 patients were recruited, with 1038 patients completing the study. Patients receiving the intervention had an OR of 5.12 (95% CI 3.20 to 8.20, p<0.05) of being adherent after the 6 months. At the end of the study, patients in the intervention group had lower diastolic blood pressure levels (mean difference (MD) -2.88, 95% CI -5.33 to -0.43, p=0.02), lower CCQ scores (MD -0.50, 95% CI -0.82 to -0.18, p<0.05) and lower ACQ scores (MD -0.28, 95% CI -0.56 to 0.00, p<0.05) when compared with the control group. Conclusions A community pharmacist-led medication adherence intervention was effective at improving medication adherence and clinical outcomes in patients suffering from hypertension, asthma and COPD. To The Editor - We read with great interest the article by Van Praet et al [[1]] investigating the decline in humoral and cellular immune response after administration of 2 doses of the Pfizer-BioNTech 162b2 vaccine in coronavirus disease 2019 (COVID-19)-naive nursing home residents, a population particularly vulnerable to the disease. Meanwhile, it may be worthwhile to offer a booster vaccine dose, not only to elderly or immunocompromised patients, but also to healthcare staff, especially to COVID-19-naive individuals, because previous SARS-CoV-2 infection could make this unnecessary.
