Background: Higher life expectancy has produced a higher older people porpulation, not necessarily with a consistent quality of life, showing a high rate of vulnerability and dependence. The current social and health crisis situation has highlighted the need to create new integrated models of care that could be translated into social and health policies. Objective: The present study aims to develop, test, and validate an innovative integrated care model for older people with dependence and at risk of social exclusion and their caregivers. Methods: The TEC-MED project participants are nine project partners and six associated partners, with geographic coverage from six countries in the Mediterranean basin, Spain, Tunisia, Italy, Lebanon, Egypt, and Greece. Project coordination will take place at three different levels, macro, meso and micro, through six work packages. The pilot phase uses qualitative-quantitative method to assess the impact of the TEC-MED project, it is expected to train 36 Training Agents, six per country, who apply to a total of 28,200 people (mainly elders and their caregivers). Conclusion: The study proposes a new social integrated care organizational model focused on the integration of social and health care, comprising the governance model; the organization structure and the skills profiles for the caring personnel. Abstracta Antecedentes: La mayor esperanza de vida ha generado un incremento en la población de personas mayores, sin embargo, no necesariamente se ha traducido en una mejora constante de la calidad de vida, evidenciándose una alta tasa de vulnerabilidad y dependencia. La actual crisis social y sanitaria ha resaltado la necesidad de crear nuevos modelos integrados de atención que puedan traducirse en políticas sociales y de salud. Objetivo: El presente estudio tiene como objetivo desarrollar, probar y validar un modelo innovador de atención integrada para personas mayores con dependencia y/o en riesgo de exclusión social, así como para sus personas cuidadoras. Métodos: Los participantes del proyecto TEC-MED son nueve socios del proyecto y seis asociados, con cobertura geográfica en seis países de la cuenca del Mediterráneo: España, Túnez, Italia, Líbano, Egipto y Grecia. La coordinación del proyecto se llevará a cabo en tres niveles diferentes, macro, meso y micro, y a través de seis paquetes de trabajo. La fase piloto utiliza un método cualitativo-cuantitativo para evaluar el impacto del proyecto TEC-MED. Se espera capacitar a 36 Agentes de Formación, seis por país, quienes aplicarán el proyecto a un total de 28,200 personas (principalmente personas mayores y sus personas cuidadoras.). Conclusión: El estudio propone un nuevo modelo organizativo de atención social integrada, centrado en la integración de la atención social y sanitaria, que comprende el modelo de gobernanza, la estructura organizativa y los perfiles de competencias del personal de cuidado. Abstract Background Midregional-proadrenomedullin (MR-proADM) is a useful prognostic peptide in severe infectious pathologies in the adult population. However, there are no studies that analyze its utility in febrile urinary tract infection (fUTI) in children. An accurate biomarker would provide an early detection of patients with kidney damage, avoiding other invasive tests like renal scintigraphy scans. Our objective is to study the usefulness of MR-proADM as a biomarker of acute and chronic renal parenchymal damage in fUTI within the pediatric population. Methods A prospective cohort study was conducted in pediatric patients with fUTI between January 2015 and December 2018. Plasma and urine MR-proADM levels were measured at admission in addition to other laboratory parameters. After confirmation of fUTI, renal scintigraphy scans were performed during the acute and follow-up stages. A descriptive study has been carried out and sensitivity, specificity and ROC curves for MR-proADM, C-reactive protein, and procalcitonin were calculated. Results 62 pediatric patients (34 female) were enrolled. Scintigraphy showed acute pyelonephritis in 35 patients (56.5%). Of those patients, the median of plasmatic MR-proADM (P-MR-proADM) showed no differences compared to patients without pyelonephritis. 7 patients (11.3%) developed renal scars (RS). 7. Why Does the Automation Say One Thing but Does Something Else? Effect of the Feedback Consistency and the Timing of Error on Trust in Automated Driving

Driving automation deeply modifies the role of the human operator behind the steering wheel. Trust is required for drivers to engage in such automation, and this trust also seems to be a determinant of drivers’ behaviors during automated drives. On the one hand, first experiences with automation, either positive or not, are essential for drivers to calibrate their level of trust. On the other hand, an automation that provides feedback about its own level of capability to handle a specific driving situation may also help drivers to calibrate their level of trust. The reported experiment was undertaken to examine how the combination of these two effects will impact the driver trust calibration process. Four groups of drivers were randomly created. Each experienced either an early (i.e., directly after the beginning of the drive) or a late (i.e., directly before the end of it) critical situation that was poorly handled by the automation. In addition, they experienced either a consistent continuous feedback (i.e., that always correctly informed them about the situation), or an inconsistent one (i.e., that sometimes indicated dangers when there were none) during an automated drive in a driving simulator. Results showed the early- and poorly-handled critical situation had an enduring negative effect on drivers’ trust development compared to drivers who did not experience it. While being correctly understood, inconsistent feedback did not have an effect on trust during properly managed situations. These results suggest that the performance of the automation has the most severe influence on trust, and the automation’s feedback does not necessarily have the ability to influence drivers’ trust calibration during automated driving.

Introduction: Cluster randomized crossover trials are often faced with a dilemma when selecting an optimal model of consent, as the traditional model of obtaining informed consent from participant's before initiating any trial related activities may not be suitable. We describe our experience of engaging patient advisors to identify an optimal model of consent for the PREP-IT trials. This paper also examines surrogate measures of success for the selected model of consent. Methods: The PREP-IT program consists of two multi-center cluster randomized crossover trials that engaged patient advisors to determine an optimal model of consent. Patient advisors and stakeholders met regularly and reached consensus on decisions related to the trial design including the model for consent. Patient advisors provided valuable insight on how key decisions on trial design and conduct would be received by participants and the impact these decisions will have. Results: Patient advisors, together with stakeholders, reviewed the pros and cons and the requirements for the traditional model of consent, deferred consent, and waiver of consent. Collectively, they agreed upon a deferred consent model, in which patients may be approached for consent after their fracture surgery and prior to data collection. The consent rate in PREP-IT is 80.7%, and 0.67% of participants have withdrawn consent for participation. Discussion: Involvement of patient advisors in the development of an optimal model of consent has been successful. Engagement of patient advisors is recommended for other large trials where the traditional model of consent may not be optimal.

Introduction: Pragmatic trials in comparative effectiveness research assess the effects of different treatment, therapeutic, or healthcare options in clinical practice. They are characterized by broad eligibility criteria and large sample sizes, which can lead to an unmanageable number of participants, increasing the risk of bias and affecting the integrity of the trial. We describe the development of a sampling strategy tool and its use in the PREPARE trial to circumvent the challenge of unmanageable work flow. Methods: Given the broad eligibility criteria and high fracture volume at participating clinical sites in the PREPARE trial, a pragmatic sampling strategy was needed. Using data from PREPARE, descriptive statistics were used to describe the use of the sampling strategy across clinical sites. A Chi-square test was performed to explore whether use of the sampling strategy was associated with a reduction in the number of missed eligible patients. Results: 7 of 20 clinical sites (35%) elected to adopt a sampling strategy. There were 1539 patients excluded due to the use of the sampling strategy, which represents 30% of all excluded patients and 20% of all patients screened for participation. 22. Allocation of Drivers’ Visual Attention During Preliminary Uses of Automated Driving: A Wizard-of-Oz Study

In this experiment, we analyzed drivers’ visual attention during their first experiences with automated driving (AD). At the beginning of the study, participants ( n = 45) received one of three AD training modules: paper, video, or practice. They then drove two sessions on a public road with a Wizard-Of-Oz vehicle, each ending with a request to intervene (RTI). The first session (forward path) included 10 min of AD with a mandatory non-driving-related task (NDRT). The second session (return path) included 10 min of AD with a nonmandatory NDRT. Control checks (number and duration of gazes) toward the road and rear-view mirrors and three self-assessment questionnaires (trust, acceptability, and technophilia) were measured during both sessions. The results indicated a decrease in road and rear-view mirror gazes over time, mainly for the practice group. Drivers with a low level of trust glanced at the road and mirrors more often than participants with a high level of trust. In addition, participants with a high level of technophilia spent less time controlling the road traffic but only during the forward path. Visual attention on the road decreased rapidly during the first minutes of AD, mainly among drivers who followed the practice training and those who had a high level of trust and were tech-savvy. We observed that participants looked at the rear-view mirrors after 10 and 14 s on average after the RTI. These results raise questions about driver situation awareness in critical situations, including the capacity to intervene in an efficient and safe way during takeover requests.

25. Urgent and non-urgent takeovers during conditional automated driving on public roads: The impact of different training programmes

Supplying training to drivers that teaches them about automated driving and requests to intervene may help them to build and maintain a mental representation of how automation works and thereby improve takeover performance. We aimed to investigate the effect of different types of training programmes about the functioning of automated driving on drivers’ takeover performance during real driving. Fifty-two participants were split into three groups for training sessions: paper (short notice), video (3-minute tutorial) and practice (short drive). After the training, participants experienced automated driving and both urgent and non-urgent requests to intervene in a Wizard-of-Oz vehicle on public roads. Participants’ takeover time, visual behaviour, mental workload, and flow levels during the requests to intervene were assessed. Our results indicated that in urgent circumstances, participants’ takeover response times were faster in the practice training condition compared to the other training conditions. Nevertheless, the practice training session did not present any other positive effect on drivers’ visual behaviour. This could indicate that prior training, particularly when reinforcing drivers' motor skills, improved their takeover response time at the latest motor stages rather than in the early sensory states. In addition, the analysis of in-vehicle videos revealed that participants’ attention was captured in the first place by the in-vehicle human-machine interface during the urgent request to intervene. This highlights the importance for designers to display information on the HMI in an appropriate way to optimise drivers’ situation awareness in critical situations.

Chlorhexidine skin antisepsis is frequently recommended for most surgical procedures; however, it is unclear if these recommendations should apply to surgery involving traumatic contaminated wounds where povidone-iodine has previously been preferred. We aimed to compare the effect of aqueous 10% povidone-iodine versus aqueous 4% chlorhexidine gluconate on the risk of surgical site infection in patients who required surgery for an open fracture.We conducted a multiple-period, cluster-randomised, crossover trial (Aqueous-PREP) at 14 hospitals in Canada, Spain, and the USA. Eligible patients were adults aged 18 years or older with an open extremity fracture treated with a surgical fixation implant. For inclusion, the open fracture required formal surgical debridement within 72 h of the injury. Participating sites were randomly assigned (1:1) to use either aqueous 10% povidone-iodine or aqueous 4% chlorhexidine gluconate immediately before surgical incision; sites then alternated between the study interventions every 2 months. Participants, health-care providers, and study personnel were aware of the treatment assignment due to the colour of the solutions. The outcome adjudicators and data analysts were masked to treatment allocation. The primary outcome was surgical site infection, guided by the 2017 US Centers for Disease Control and Prevention National Healthcare Safety Network reporting criteria, which included superficial incisional infection within 30 days or deep incisional or organ space infection within 90 days of surgery. The primary analyses followed the intention-to-treat principle and included all participants in the groups to which they were randomly assigned. This study is registered with ClinicalTrials.gov, NCT03385304.Between April 8, 2018, and June 8, 2021, 3619 patients were assessed for eligibility and 1683 were enrolled and randomly assigned to povidone-iodine (n=847) or chlorhexidine gluconate (n=836). The trial's adjudication committee determined that 45 participants were ineligible, leaving 1638 participants in the primary analysis, with 828 in the povidone-iodine group and 810 in the chlorhexidine gluconate group (mean age 44·9 years [SD 18·0]; 629 [38%] were female and 1009 [62%] were male). Among 1571 participants in whom the primary outcome was known, a surgical site infection occurred in 59 (7%) of 787 participants in the povidone-iodine group and 58 (7%) of 784 in the chlorhexidine gluconate group (odds ratio 1·11, 95% CI 0·74 to 1·65; p=0·61; risk difference 0·6%, 95% CI -1·4 to 3·4).For patients who require surgical fixation of an open fracture, either aqueous 10% povidone-iodine or aqueous 4% chlorhexidine gluconate can be selected for skin antisepsis on the basis of solution availability, patient contraindications, or product cost. Resumen La crisis sanitaria provocada por la pandemia del virus Covid-19 ha vuelto a destacar el papel de los profesionales sanitarios como elemento clave para su contención que sufren una alta tensión marcada por la presión asistencial y la falta de medios de protección. Ante esta tensión, resulta relevante analizar el impacto emocional en los profesionales sanitarios de la pandemia de coronavirus y los recursos de afrontamiento para disminuir o atenuar este impacto. Destacan el estrés, los trastornos del sueño y los síntomas depresivos. Se ofrecen algunas estrategias que han sido utilizados por profesionales que anteriormente han sufrido la presión del COVID-19 y les han sido de utilidad. Por último, se señalan algunas recomendaciones cuya eficacia es conocida para el manejo del impacto emocional.

Introduction: In 2014 the Consensus Document produced by the Spanish Paediatric Societies (SEIP-SERPE-SEOP) was published to help in the diagnosis and treatment of osteoarticular infections (OAI). In 2015 the RIOPed was considered as a multidisciplinary national network for the investigation into OAI. The aim of this study was to assess the level of adaption to the recommendations established in the Consensus during one year of follow-up. Material and methods: A prospective, national multicentre study was carried out in 37 hospitals between September 2015 and September 2016. The study included patients >16 years-old with a diagnosis of OAI, confirmed by microbiological isolation, or probable: septic arthritis (SA) with >40,000 white cells in synovial fluid, or osteomyelitis (OM)/spondylodiscitis (SD) with a compatible imaging test. The results were compared with those obtained in a retrospective study conducted between 2008 and 2012. Results: A total of 235 cases were included, of which 131 were OM, 79 SA, 30 OA, and 15 SD. As regards the complementary tests that the Consensus considered mandatory to perform, radiography was carried out on 87.8% of the cases, a blood culture on 91.6%, and culture of the synovial fluid in 99% of SA. A magnetic resonance (MR) was performed on 71% of the OM cases. The choice of intravenous empirical antibiotic treatment was adapted to the recommendations in 65.1% of cases, and in 62.3% for the oral treatment. Surgery was performed in 36.8% of SA cases (85.7% arthrotomy), with a significant decrease compared to the retrospective study (P = .014). Only 58.5% of cases followed the recommendations on the duration of the treatment; however, a lower duration of intravenous treatment was observed. Conclusions: In general, the level of adaptation to the recommendations that were set by the Expert Group, is good for the complementary tests, and acceptable as regards the choice of antibiotic treatment, although inadequate in almost 40% of cases. A decrease in hospital stay was achieved. Resumen: Introducción: En 2014 se publicó el Documento de Consenso desarrollado por SEIP-SERPE-SEOP para el diagnóstico y el tratamiento de las infecciones osteoarticulares (IOA). En 2015 se constituyó RIOPed como red nacional multidisciplinar para la investigación en IOA. El objetivo del estudio ha sido valorar el grado de adecuación a las recomendaciones establecidas en el consenso durante un año de seguimiento. Material y métodos: Estudio prospectivo multicéntrico nacional realizado entre septiembre de 2015 y septiembre de 2016 en 37 hospitales con inclusión de pacientes menores de 16 años diagnosticados de IOA, confirmada mediante aislamiento microbiológico, o probable: artritis séptica (AS) con >40.000 leucocitos en líquido sinovial u osteomielitis (OM)/osteoartritis (OA)/espondilodiscitis (ED) con prueba de imagen compatible. Los resultados se compararon con los obtenidos en el estudio retrospectivo realizado entre 2008 y 2012. Resultados: Se incluyeron 255 casos: 131 OM, 79 AS, 30 OA y 15 ED. Respecto a las pruebas complementarias que el consenso consideró de obligada realización, la radiografía se llevó a cabo en el 87,8% de los casos, el hemocultivo en el 91,6% y el cultivo de líquido sinovial en el 99% de AS. Se realizó RM en el 71% de las OM. La elección del tratamiento antibiótico intravenoso empírico se adecuó a las recomendaciones en el 65,1% de los casos, y en el 62,3% para el tratamiento oral. Se llevó a cabo cirugía en el 36,8% de las AS (85,7% artrotomía), con un descenso significativo respecto al estudio retrospectivo (P = ,014). Solo el 58,5% de casos se ajustaron a las recomendaciones de duración del tratamiento; sin embargo, se comprobó una menor duración del tratamiento intravenoso. Conclusiones: En general, el grado de adecuación a las recomendaciones que marcaron el grupo de expertos es bueno para las pruebas complementarias y aceptable respecto a la elección del tratamiento antibiótico, aun detectándose casi un 40% de inadecuación. Se ha conseguido un descenso de la estancia hospitalaria.

[Background] Demographic changes and the increased chronical diseases burden are global challenges that cannot go unnoticed by healthcare systems, which must be organized without losing sight of the increasing influence of social determinants., [Aim] To evaluate the results of a primary care program implemented to reduce health inequalities associated with social determinants in patients with type 2 diabetes., [Method] An exploratory pilot retrospective cross-sectional study that includes secondary data of 404 nonrandomized patients belonging to socially depressed areas and conventional areas. Descriptive, bivariate, and multivariate analyses were performed., [Results] The age of the subjects included in the study was 66.80 ± 9.7 years with a proportion of 56.7% men. Proportions of patients from socially depressed areas and adherence to the Nursing Follow-up Program were around 33% and 60%, respectively. The obesity rate was 51%, percentage of patients with HbA1c, [Conclusions] The program has proven to be efficient in equating the health outcomes related with cardiovascular risk of patients from both types of areas. Well-directed health policies could bring primary care systems closer to sustainable development goals through the reduction of health disparities that affect socially vulnerable groups., [Clinical relevance] To introduce a risk strategy and to adopt a family approach, contemplating the sociocultural and educational differences that diversely affect men and women in their health status has proven to be useful in reducing health disparities due to social determinants in patients with type 2 diabetes.

31. Why do they trust it? Determinants of the initial Trust in Automated Driving

- Abstract
Because driving automation is quickly increasing, humans’ relation to automated driving evolves accordingly. Trust in automated driving has been shown to strongly correlate with drivers’ acceptance of such systems. Nonetheless, there is currently a lack of specific questionnaires to assess drivers’ level of trust in Highly Automated Driving (HAD). Moreover, little is known about which factors determine trust before any interaction with HAD systems. Theoretical trust in automation models propose several factors to explain how trust builds before and during interactions with automation. Using 844 answers collected through an online survey, this paper aims to propose a new scale designed to evaluate initial trust in automated driving. Moreover, we also measure other factors linked to drivers’ initial level of trust in HAD by operationalizing the trust components proposed by Hoff and Bashir (2015)’s theoretical trust model. In order to better describe what determines trust in HAD, a linear model based on the collected data is proposed. The model not only weights the factors determining trust but is also able to predict the level of trust considering these factors.

Future autonomous driving users confirmed the need for the monitoring systems to improve safety as long as they are simple and not intrusive. They request specific information to understand and initiate appropriated actions in case of a critical situation. However, to avoid overloading and detri-mental effects, it is important that the system limits the amount of infor-mation at the right moment.

35. How drowsiness and distraction can interfere with take-over performance: A systematic and meta-analysis review

Drowsiness and distraction are major factors of road crashes and responsible of35% of road fatalities. Automated driving could solve or minimize their impact, yet it is also in itself a way to promote them. Previous literature reviews and meta-analysis regarding take-overs during automated driving primarily focused on distraction rather than drowsiness. We thus present a systematic and meta-analysis literature review focused on the effect of distraction and drowsiness on take-over performance. From an initial selection of 1896 articles from databases, we obtained by applying systematic review methodology a total of 58 articles with 42 articles dedicated to distraction and 17 articles related to drowsiness. According to our analysis, we demonstrated that distraction and drowsiness increased the take-over request reaction time (TOR-RT), which could also lead to a reduction of the quality of take-overs. In addition, this longer reaction time was even more important in the case of handheld non-driving related tasks, which is important to consider as phone use is among the most frequent tasks done during automated driving. On a more methodological aspect, we also demonstrated that take-over time budget had a significant effect on TOR-RT. However, it is difficult to estimate to what extend distraction and drowsiness could impact the take-over quality, even if several elements supported safety issues. We underpinned several limits of the current methodologies applied in the study of distraction and drowsiness such as (i) the lack of additional measures related to the take-over quality (e.g., accelerations, collision rate), (ii) the many different methodologies applied to the determination of the TOR-RT (e.g., deactivation by the steering wheel, pedals, button), (iii) the high frequency of take-over requests which can lead to habituation effects, (iv) the lack of control conditions, (v) the fact that the level of drowsiness was relatively low in most studies. We thus highlighted recommendations for a better estimation of the effect of distraction and drowsiness on take-over performance. Further studies should adopt more standardized measures of TOR-RT and additional take-over quality measures, try minimizing the number of take-over requests, and carefully consider the time budget available for the use case since it influences the TOR-RT. Regarding distraction, researchers should consider the impact of tasks requiring handholding items. Concerning drowsiness, further protocols should consider the non-linearity of drowsiness and presence of micro sleeps and favor take-over requests based on drowsiness level protocols rather than on fixed duration protocols.

Objectives: To evaluate electrocardiogram markers to predict coronary involvement in patients with Kawasaki disease by assessing measures of ventricular repolarization parameters on the 12-lead electrocardiogram. Study design: This cross-sectional study included 180 Spanish and Japanese patients ≤14 years of age with Kawasaki disease, with or without coronary involvement, from 2011 to 2016. We manually measured the Tp-Te/QT ratio and QTc interval (with Bazett's formula) in 12-lead electrocardiogram in the acute and recovery period and explored their potential association with coronary involvement. Results: No association was found between Tp-Te/QT ratio obtained manually in V5 and V6 leads and coronary involvement in the acute (V5:0.25 [IQR, 0.21-0.27] vs 0.25 [IQR, 0.20-0.27], P = .80; V6:0.24 [IQR, 0.21-0.27] vs 0.25 [IQR, 0.20-0.27], P = .86) or the recovery (V5: 0.23 [IQR, 0.20-0.25] vs 0.23 [IQR, 0.19-0.25], P = .68; V6: 0.23 [IQR, 0.20-0.25] vs 0.23 [IQR, 0.17-0.25], P = .50) period. Stressful working conditions are correlated with a negative impact on the well-being of nurses, job satisfaction, quality of patient care and the health of the staff. The Nursing Stress Scale (NSS) has been shown to be a valid and reliable instrument to assess occupational stressors among nurses. This study updates the psychometric properties of the &ldquo, NSS-Spanish version&rdquo, and validates a short-form version. A cross-sectional design was carried out for this study. A reliability analysis and a confirmatory factor analysis and an exploratory factor analysis were undertaken. Items were systematically identified for reduction using statistical and theoretical analysis. Correlation testing and criterion validity confirmed scale equivalence. A total of 2195 Registered Nurses and 1914 Licensed Practical Nurses were enrolled. The original 34-item scale obtained a good internal consistency but an unsatisfactory confirmatory and exploratory factor analysis. The short-form Nursing Stress Scale (11-items) obtained a good internal consistency for Registered Nurses (&alpha, = 0.83) and for Licensed Practical Nurses (&alpha, = 0.79). Both Nursing Stress Scales obtained a strong correlation for Registered Nurses (rho = 0.904) and for Licensed Practical Nurses (rho = 0.888). The 11-item version of the Nursing Stress Scale is a valid and reliable scale to assess stress perception among Registered Nurses and Licensed Practical Nurses. Its short-form nature improves the psychometric properties and the feasibility of the tool.

Introduction Hospital-wide SARS-CoV-2 seroprevalence is rarely explored and can identify areas of unexpected risk. We determined the seroprevalence against SARS-CoV-2 in all health care workers (HCW) at a hospital. Methods Cross-sectional study (14-27/04/2020). We determined SARS-CoV-2 IgG by ELISA in all HCW including external workers of a teaching hospital in Madrid. They were classified by professional category, working area, and risk for SARS-CoV-2 exposure. Results Among 2919 HCW, 2590 (88,7%) were evaluated. The mean age was 43.8 years (SD 11.1), and 73.9% were females. Globally, 818 (31.6%) workers were IgG positive with no differences for age, sex or previous diseases. Of these, 48.5% did not report previous symptoms. Seropositivity was more frequent in high- (33.1%) and medium- (33.8%) than in low-risk areas (25.8%, p = 0.007), but not for hospitalization areas attending COVID-19 and non-COVID-19 patients (35.5 vs 38.3% p > 0.05). HWC with a previous SARS-CoV2 PCR-positive test were IgG seropositive in 90.8%. By multivariate logistic regression analysis seropositivity was significantly associated with being physicians (OR 2.37, CI95% 1.61–3.49), nurses (OR 1.67, CI95% 1.14–2.46), nurse assistants (OR 1.84, CI95% 1.24–2.73), HCW working at COVID-19 hospitalization areas (OR 1.71, CI95% 1.22–2.40), non-COVID-19 hospitalization areas (OR 1.88, CI95% 1.30–2.73), and at the Emergency Room (OR 1.51, CI95% 1.01–2.27). Conclusions Seroprevalence uncovered a high rate of infection previously unnoticed among HCW. Patients not suspected of having COVID-19 as well as asymptomatic HCW may be a relevant source for nosocomial SARS-CoV-2 transmission.

They are characterized by broad eligibility criteria and large sample sizes, which can lead to an unmanageable number of participants, increasing the risk of bias and affecting the integrity of the trial. We describe the development of a sampling strategy tool and its use in the PREPARE trial to circumvent the challenge of unmanageable work flow.MethodsGiven the broad eligibility criteria and high fracture volume at participating clinical sites in the PREPARE trial, a pragmatic sampling strategy was needed. Using data from PREPARE, descriptive statistics were used to describe the use of the sampling strategy across clinical sites. A Chi-square test was performed to explore whether use of the sampling strategy was associated with a reduction in the number of missed eligible patients.Results7 of 20 clinical sites (35%) elected to adopt a sampling strategy. There were 1539 patients excluded due to the use of the sampling strategy, which represents 30% of all excluded patients and 20% of all patients screened for participation. Use of the sampling strategy was associated with lower odds of missed eligible patients (297/4545 (6.5%) versus 341/3200 (10.7%) p l 0.001).ConclusionsImplementing a sampling strategy in the PREPARE trial has helped to limit the number of missed eligible patients. This sampling strategy represents a simple, easy to use tool for managing work flow at clinical sites and maintaining the integrity of a large trial.

KAWA-RACE study group., [Background] Kawasaki disease (KD) is an acute self-limited systemic vasculitis of unknown etiology affecting mainly children less than 5 years of age. Risk factors for cardiac involvement and resistance to treatment are insufficiently studied in non-Japanese children., [Objective] This study aimed to investigate the epidemiology, clinical features and risk factors for resistance to treatment and coronary artery lesions (CAL) in KD in Spain., [Methods] Retrospective study (May 2011-June 2016) of all patients less than 16 years of age diagnosed with KD included in KAWA-RACE network (84 Spanish hospitals)., [Results] A total of 625 cases were analyzed, 63% were males, 79% under 5 year-olds and 16.8% younger than 12 months. On echocardiographic examination CAL were the most frequent findings (23%) being ectasia the most common (12%). Coronary aneurysms were diagnosed in 9.6%, reaching 20% in infants under 12 months (p 900,000 cells/mm3, maximum temperature < 39.5°C, total duration of fever > 10 days and fever before treatment ≥ 8 days as independent risk factors for developing coronary aneurysms., [Conclusions] In our population, children under 12 months develop coronary aneurysms more frequently and children with KD with anemia and leukocytosis have high risk of cardiac involvement. Adding steroids early should be considered in those patients, especially if the treatment is not started before 8 days of fever. In the past, lane departure warnings (LDWs) were demonstrated to improve driving behaviours during lane departures but little is known about the effects of unreliable warnings. This experiment focused on the influence of false warnings alone or in combination with missed warnings and warning onset on assistance effectiveness and acceptance. Two assistance unreliability levels (33 and 17%) and two warning onsets (partial and full lane departure) were manipulated in order to investigate interaction. Results showed that assistance, regardless unreliability levels and warning onsets, improved driving behaviours during lane departure episodes and outside of these episodes by favouring better lane-keeping performances. Full lane departure and highly unreliable warnings, however, reduced assistance efficiency. Drivers' assistance acceptance was better for the most reliable warnings and for the subsequent warnings. The data indicate that imperfect LDWs (false warnings or false and missed warnings) further improve driving behaviours compared to no assistance. Practitioner Summary: This study revealed that imperfect lane departure warnings are able to significantly improve driving performances and that warning onset is a key element for assistance effectiveness and acceptance. The conclusion may be of particular interest for lane departure warning designers.

This paper presents a review of the literature to shed light on the impact of distractions on the effectiveness of warning indicating a risk of collision. The rear-end collisions account for a significant share of injury accidents. One of the major contributing factors to these collisions is drivers’ inattention. To reduce the impact of this kind of collisions, driving assistance consisting in alerts have emerged. The effectiveness of these alerts has been evaluated in different situations (simulation, simulator and on road) in non-distracted and distracted drivers. In the latter case, the majority of distraction tasks used involves a diversion of eyes off the road to a device inside the vehicle, coupled or not with manual handling. However, few studies have also examined the impact of distractive tasks having only a cognitive component. The data show that the efficiency of an alert may depend on the interaction between its sensory modality and that of the distractive task. Finally, the effectiveness of these systems can also be affected by the interaction between its reliability and being or not distracted. This review raises research questions that should be further developed to ensure the safety of a larger number of users.; Ce papier présente une revue de la littérature visant à apporter un éclairage sur l'impact des distractions sur l'efficacité d'alerte signalant un risque de collision. Les collisions par l'arrière représentent une part non négligeable des accidents corporels. Un des facteurs contributifs majeurs à ces collisions est l'inattention des conducteurs. Afin de diminuer l'impact de ces collisions, des assistances à la conduite de type alerte ont vu le jour. L'efficacité de ces alertes a été évaluée dans différentes situations (par simulation, sur simulateur et sur route) chez des conducteurs non distraits et distraits. Dans le derniers cas, la majorité des tâches distractives employées impliquent un détournement du regard de la route vers un dispositif à l'intérieur du véhicule couplé ou non avec une manipulation manuelle. Toutefois, quelques études se sont également intéressées à l'impact de tâches distractives ayant seulement une composante cognitive. Les données montrent que l'efficacité d'une alerte peut dépendre de l'interaction entre sa modalité sensorielle et celle de la tâche distractive. Enfin, l'efficacité de ces systèmes peut également être impactée par l'interaction entre sa fiabilité et le fait d'être ou non distrait. Cette revue soulève des questions de recherche qui devraient encore être approfondies en vue de garantir la sécurité du plus grand nombre d'usagers.

Objective: Respiratory tract infections are among the most common causes of morbidity and mortality worldwide. Acute bronchiolitis (AB) is the leading cause of hospital admission among infants. Clinical scores have proven to be inaccurate in predicting prognosis. Our aim was to build a score based on findings of lung ultrasound (LU) performed at admission, to stratify patients at risk of needing respiratory support (non-invasive and invasive ventilation). Study design: Prospective, multicenter study including infants 1 cm was the main factor associated to NIV (RR 4.4; [CI95%1.8-10.8]) The LU score built according to the findings on admission showed an AUC: 0.845(CI95%:0.78-0.91). A score ≥3.5 showed a sensitivity of 89.1% (CI95%:78.2-94.9%) and specificity of 56% (CI95%: 45.3-66.1%) CONCLUSIONS: Among infants below 6 months of age admitted with AB, point-of-care LU was a helpful tool to identify patients at risk of needing respiratory support.
