Fundamentos: El cáncer de mama es el tumor más frecuente en mujeres. El objetivo de este estudio es describir el comportamiento de la mortalidad por cáncer de mama en España y en las Comunidades Autónomas (CCAA) desde 1980 a 2005 y estudiar la relación con la implantación de los programas de cribado poblacionales. Métodos: Se ha realizado un análisis de tendencia de la mortalidad por cáncer de mama a partir de los datos de mortalidad del Ministerio de Sanidad y Política Social. Se calcularon las tasas de mortalidad cruda, estandarizada, específica y truncada por 100.000 mujeres. El análisis de tendencia se realizó mediante un modelo de regresión joinpoint y se obtuvo el porcentaje de cambio anual (PCA). Resultados: Entre 1980 y 2005 en España se produjeron 131.746 defunciones por cáncer de mama. En España las tasas bruta, estandarizada, y truncada, presentaron un comportamiento creciente hasta 1992, año en el que se produjo un cambio de tendencia (PCA= -0,1; -0,9; -0,8 respectivamente). Conclusión: La mortalidad por cáncer de mama en las mujeres en España presenta un cambio de tendencia descendente a partir de 1992 que persistió hasta 2005 y que no fue homogénea en todos los grupos de edad.Esta reducción en la tasa de mortalidad también se observó en las CCAA con cierta variabilidad en el año donde se produce el cambio.

We hare analysed the influence of patient and hospital characteristics and region, on the use of breast-conserving surgery (BCS) in Catalonia (Spain). Data for this study was obtained from the Catalan Hospital Discharge Data Base. The study period was 1995–1998. The Mantel-Haenszel test was used to examine overall trends in the use of BCS. A regression analysis was performed to assess the effect of period adjusted for patient, hospital characteristics, and area of residence on use of BCS, and a multilevel analysis was performed to consider possible associations between individual and aggregate level variables. BCS was carried out in 43% of women in the period 1995–98 and there was an increasing significant trend in its use. Multilevel analysis showed that age and hospital volume were significant predictors of the use of BCS, and hospitals with higher volumes of activity having higher rates of BCS relative to mastectomy. Despite the evidence demonstrating the effectiveness of breast conserving surgery, there are still considerable variations in its use, which may be in part due to physician attitudes. Breast cancer is the most common cancer in women. The aim of this study is to describe the development of breast cancer mortality and its relation with the implementation of population screening programs in Spain and its Autonomous Communities (AC) from 1980 to 2005.A breast cancer mortality trends analysis was done. We used the data from mortality of Ministry of Health and Social Policies. We calculated crude, standardized, specific and truncated mortality rates per 100,000 women. Trend analysis was done using a Joinpoint Regression Model to compute the Annual Percentage Change (APC).131746 people died from breast cancer in Spain between 1980 and 2005. In Spain, mortality rates increased until 1992, when there was a change in the trend (APC = -0.1, -0.9, -0.8 for standard, specific and truncated rates respectively),Mortality from breast cancer in women in Spain indicates a general downward trend from 1992 to 2005 that was different among the different age groups. This downward trend was also observed in all the AC with some variability in the year that change occurs.

BRCA1 and BRCA2 mutations underlie a substantial proportion of all hereditary breast cancer. The mutational spectrum in these genes is very broad, with hundreds of different BRCA mutations reported worldwide. However, high frequency founder mutations make up a substantial fraction of all mutations in some ethnic groups. We directly sequenced BRCA1 and BRCA2 in 35 Spanish breast/ovarian cancer families and found 13 mutations of which 3 had been reported previously in Spain. The ten novel mutations are: IVS5+1 GA, 1491delA, Leu1086Ter, and Gln895Ter in BRCA1; Glu49Ter, 5373delGTAT, 5947delCTCT, 6672delTA, 8281insA, and Pro3039Leu (which also involves a splice site) in BRCA2. Our data, in combination with previous reports, indicate that 14 mutations have been seen recurrently in Spanish families. Analyzing these 14 mutations in 42 previously untested breast/ovarian cancer families revealed only two families testing positive, one for BRCA1 185delAG and one for BRCA2 9254delATCAT. While several mutations have been found recurrently in Spain, none appear to be high frequency founder mutations based on studies of breast and ovarian cancer families.
