Patients who have both a psychiatric disorder and a substance abuse problem cause most clinicians to throw up their hands in despair. The clinical problems that these'dual diagnosis'patients present are enor­ mously complex. Diagnostically, how is one to tell if disorders of mood and thinking, for instance, are signs of a mental illness or consequences of substance abuse? How is one to obtain important historic information when the patient may be unable or unwilling to provide it and there are no readily available collateral sources of information? In any case, why bother? Treatments for dually diagnosed patients are ineffective; patients won't stay in treatment; recidivism occurs at a very high rate. To make matters even more difficult, traditional health care reim­ bursement mechanisms do not provide for the multimodality clinical programs and special services needed by the patient who is both mentally ill and a substance abuser. Since its launching in 1955, rational-emotive therapy (RET) has become one of the most influential forms of counseling and psychotherapy used by literally thousands of mental health practitioners throughout the world. From its beginnings, RET has dealt with problems of human disturbance. It presents a theory of how people primarily disturb themselves and what they can do, particularly with the help of a therapist or counselor, to reduce their disturbances (Ellis, 1957a,b, 1958a,b, 1962). Almost im­ mediately after the creation of RET, it became obvious that the meth­ odology could be used in many other fields-especially those involving human relations (Ellis & Harper, 1961a), and in love, sex, and marital relationships (Ellis, 1958a, 1960, 1963a,b; Ellis & Harper, 1961b). The evident popularity and clinical utility of RET in different cultures and its increasing application to contemporary problems of living indicate that rational-emotive therapy continues to be vital and dynamic. The growing appeal of RET may be due in part to its essentially optimistic outlook and humanistic orientation; optimistic because it pro­ vides people with the possibility and the means for change. Showing to people how their attitudes and beliefs are responsible for their emo­ tional distress and interpersonal problems (and not some out-of-con­ scious early childhood experience), awakens in them the hope that, in reality, they have some control over their destiny.

Cognitive therapy is fast becoming one of the more popular and well­ respected forms of psychotherapy. In both research and clinical practice, several advantages of cognitive therapy have been identified. Cognitive therapy is structured enough to provide a therapeutic framework for clinicians, as well as a theoretical framework for clinical researchers, yet flexible enough to address an individual's problems in a highly idio­ syncratic manner. Accompanying the popularity of cognitive therapy is the expansion of its application beyond the areas in which it was initially developed and validated (the'traditional'areas of depression and anx­ iety) to areas where validation has not yet occurred (the'nontraditional'areas). We strongly believe that such broadening of cognitive therapy should be encouraged, but that conceptual models to guide the therapist and researcher in these areas should be explicated. It is the purpose of this text to provide a conceptual framework for dealing with select, nontraditional populations. The idea and motivation for this text develops from a cognitive therapy interest group in Toronto. All of the authors contributing to this text are involved in this group. We represent a group of cognitive thera­ pists functioning in a variety of diverse settings, including clinical re­ search units, general hospital settings, private or public rehabilitation centers, and private practices. Thus, the diversity of referrals for cogni­ tive therapy within our group is great.

It is clear that child and adolescent psychiatric disorders impose a heavy burden of suffering. Recent large-scale community epidemiological studies suggest that as many as 20% of children and adolescents in the general population may have clinically important mental disorders. These disorders are accompanied by associated impairments in various domains of the child's life, resulting in lowered life quality for the child and his or her family. In addition, for some conditions, the onset of the disorder in childhood heralds a lifetime of serious psychosocial disturbance for a significant subgroup of affected children. For instance, about 40% of children seen in clinic settings with conduct disorder in late childhood and early adolescence will have serious psychiatric disorders in adult life. Finally, the heavy burden of suffering of these conditions is indicated by the large amounts of both human and financial resources devoted to their assessment and treatment. There is a pressing need in the field for effective treatments (that is, those that have been shown to do more good than harm) that will result in a significant reduction in the burden of suffering resulting from these disorders. Further, these effective interventions must be readily available and acceptable to clini­ cians in the settings in which children with mental disorders (and their families) seek care. This book addresses an important need in the treatment field.

This book could not have been conceptualized or published 20 years ago. Indeed, it is doubtful that we could have organized the material for this handbook 10 years ago. Over the last 20 years, however, the painstaking efforts of many clinical researchers working with a variety of resistive psychopathologies have resulted in specific psychotherapies and pharmacotherapies that are effective with a significant propor­ tion of patients, at least for some of the disorders. Much clinical research remains to be carried out in the forthcoming decades. But now that we are nearing the 21st century, at least some statement about efficacy can be made. In 1967, Gordon Paul succinctly stated that the ultimate goal of treatment outcome research is to determine'What treatment, by whom, is most effective for this individual with that specific problem, and under which set of circumstances'(p. 111). At that time, empirical evaluations of psychosocial and pharmacologic treatments were few and far between. Methodological strategies for determining treatment effectiveness were also in the formative stage, as exemplified by introduc­ tion of control groups that received inactive interventions (i. e., placebo) and the relatively recent practice of comparing two or more treatments in addition to placebo. In the almost three decades since Paul's oft-quoted dictum, both the quantity and the quality of treatment outcome research with adults have increased dramati­ cally.

This book is a research mono graph reporting empirical results, but we have tried to place the data in a very broad national perspective. Our intent is a volume on mental health policy in the United States, most notably our de facto policies, as indicated by empirical data. The book gives a broad perspective of mental disorders and mental disorder treatment in general hospitals in the United States. The audi­ ence that we ho pe to reach is those interested in mental health policy, planning, and treatment alternatives.
