Abstract

BackgroundThe impact of the 2017 American College of Cardiology (ACC)/American Heart Association (AHA) guidelines for diagnosis and management of hypertension on the prevalence of hypertension in India is unknown.MethodsWe analyzed data from the Cardiac Prevent 2015 survey to estimate the change in the prevalence of hypertension. The JNC8 guidelines defined hypertension as a systolic blood pressure of ≥140 ​mmHg or diastolic blood pressure of ≥90 ​mmHg. The 2017 ACC/AHA guidelines define hypertension as a systolic blood pressure of ≥130 ​mmHg or diastolic blood pressure of ≥80 ​mmHg. We standardized the prevalence as per the 2011 census population of India. We also calculated the prevalence as per the World Health Organization (WHO) World Standard Population (2000-2025).ResultsAmong 180,335 participants (33.2% women), the mean age was 40.6 ​± ​14.9 years (41.1 ​± ​15.0 and 39.7 ​± ​14.7 years in men and women, respectively). Among them, 8,898 (4.9%), 99,791 (55.3%), 35,694 (11.9%), 23,084 (12.8%), 9,989 (5.5%) and 2,878 (1.6%) participants belonged to age group 18-19, 20-44, 45-54, 55-64, 65-74 and ​≥ ​75 years respectively. The prevalence of hypertension according to the JNC8 and 2017 ACC/AHA guidelines was 29.7% and 63.8%, respectively- an increase of 115%. With the 2011 census population of India, this suggests that currently, 486 million Indian adults have hypertension according to the 2017 ACC/AHA guidelines, an addition of 260 million as compared to the JNC8 guidelines.ConclusionAccording to the 2017 ACC/AHA guidelines, 3 in every 5 Indian adults have hypertension.

Abstract

Snakebite envenoming (SBE) is a neglected tropical disease that disproportionately affects underprivileged communities in the Tropics. India is particularly affected by SBE, with an estimated 58,000 deaths each year. The Indian Council of Medical Research (ICMR) has designated the Amrita Institute of Medical Sciences (AIMS) as a Collaborating Centre of Excellence (CCoE) for snakebite. The CCoE aims to generate evidence, coordinate research, and serve as a knowledge partner for the prevention and control of SBE. The CCoE will address various aspects of SBE, including anti-snake venom, venom research, healthcare system and delivery, policy advocacy, research, preventive measures, and a one-health approach. The challenges ahead include bringing stakeholders together and contextualizing information for diverse needs. The funding for the CCoE will be project-based, and the CCoE will align its activities with the WHO's goal of reducing SBE mortality by 50% by 2030. [Extracted from the article]

Abstract

There is ongoing debate around rate versus rhythm control strategies for managing atrial fibrillation (AF), however, much of the data comes from Western cohorts. Kerala‐AF represents the largest prospective AF cohort study from the Indian subcontinent. To compare 12‐month outcomes between rate and rhythm control strategies. Patients aged ≥18 years with non‐transient AF were recruited from 53 hospitals across Kerala. Patients were stratified by rate or rhythm control. The primary outcome was a composite of all‐cause mortality, arterial thromboembolism, acute coronary syndrome or hospitalization due to heart failure or arrhythmia at 12 months. Secondary outcomes included bleeding events and individual components of the primary. Predictors of the composite outcome were analysed by logistic regression. A total of 2901 patients (mean age 64.6 years, 51% female) were included (2464 rate control, 437 rhythm control). Rates of the primary composite outcome did not differ between groups (29.7% vs 30.0%; p= .955), nor did any component of the primary. Bleeding outcomes were also similar (1.6% vs 1.9%; p= .848). Independent predictors of the primary composite outcome were older age (aOR 1.01; p= .013), BMI <18 (aOR 1.51; p= .025), permanent AF (aOR 0.78; p= .010), HFpEF (aOR 1.40; p= .023), HFrEF (aOR 1.39; p= .004), chronic kidney disease (aOR 1.36; p< .001), and prior thromboembolism (aOR 1.31; p= .014). In the Kerala‐AF registry, 12‐month outcomes did not differ between rate and rhythm control cohorts. No significant difference was found between rate and rhythm control approaches to atrial fibrillation management in the 1‐year follow‐up of the Kerala‐AF cohort. Independent predictors of adverse outcomes included age, underweight BMI, heart failure, and chronic renal failure.

Abstract

Despite notable scientific and medical advances, broader political, socioeconomic and behavioural factors continue to undercut the response to the COVID-19 pandemic . Here we convened, as part of this Delphi study, a diverse, multidisciplinary panel of 386 academic, health, non-governmental organization, government and other experts in COVID-19 response from 112 countries and territories to recommend specific actions to end this persistent global threat to public health. The panel developed a set of 41 consensus statements and 57 recommendations to governments, health systems, industry and other key stakeholders across six domains: communication; health systems; vaccination; prevention; treatment and care; and inequities. In the wake of nearly three years of fragmented global and national responses, it is instructive to note that three of the highest-ranked recommendations call for the adoption of whole-of-society and whole-of-government approaches , while maintaining proven prevention measures using a vaccines-plus approach that employs a range of public health and financial support measures to complement vaccination. Other recommendations with at least 99% combined agreement advise governments and other stakeholders to improve communication, rebuild public trust and engage communities in the management of pandemic responses. The findings of the study, which have been further endorsed by 184 organizations globally, include points of unanimous agreement, as well as six recommendations with >5% disagreement, that provide health and social policy actions to address inadequacies in the pandemic response and help to bring this public health threat to an end.

Abstract

The state of Kerala stands out as having the highest prevalence of diabetes and coronary artery disease in comparison to other states within India. This observation positions India as the global epicenter for noncommunicable diseases (NCDs), specifically diabetes, and cardiovascular ailments. Kerala is in a highly advanced stage of epidemiological transition, surpassing other states in India. This transition is characterized by factors such as an aging population, a high incidence of NCDs, and elevated levels of morbidity. Kerala boasts a robust health-care system encompassing a strong public sector, further reinforced by a network of private hospitals spread across the state. Moreover, the population of Kerala possesses a high literacy rate. It is well-informed about their rights, while local self-governance and nongovernmental organizations (NGOs) actively collaborate to promote community health. These factors have collectively contributed to the state's proactive approach toward addressing NCDs. In the domains of surveillance, monitoring, and prevention of NCDs, as well as the prevention of complications arising from these diseases, Kerala has implemented various schemes across the public, private, and NGO sectors. These initiatives encompass activities to raise awareness about NCDs, provide treatment options, and focus on preventive measures. By documenting and highlighting these schemes, Kerala's health-care system and its achievements can serve as a roadmap for the rest of the country, outlining strategies for prevention, monitoring, and therapeutic interventions. It is imperative that other states within India, as well as low- and middle-income countries (LMICs) at large, adopt and adapt the processes and practices established by Kerala to effectively combat the NCDs pandemic. By following this path, the nation and other LMICs can effectively confront the challenges posed by NCDs and work toward improving their populations' overall health and well-being.
