Abstract

Purpose This paper aims to estimate the delay or timely effects of the national vaccination strategy for COVID-19 on Italian gross domestic product (GDP). By adopting a knowledge management lens, the study highlights the importance of “time” for Italian recovery. Indeed, recovering an adequate growth rate is crucial for the future of employment, well-being and management of Italian public debt. Design/methodology/approach This study applies an epidemiological model of a universal access vaccination programme against COVID-19. The economic model is based on the time-shift of available quarterly projections deriving from the expected delay or acceleration of the national vaccination plan against COVID-19. Findings The basic concept underlying the scenario analysis is that the sustainability of the expected recovery of the Italian economy due to the COVID-19 shock, and consequently the growth of the GDP, is time-dependent on the rollout of the national vaccination plan. Research limitations/implications A delay in the vaccination campaign could have a twofold negative impact on the growth of the Italian gross product: it reduces the quarterly growth over the previous year in the short term and it delays the quarterly upwards trend over the next two years. Policymakers and practitioners are called to promptly face new dynamic scenarios due to public and economic policies to fight the COVID-19 crisis. Originality/value To the best of the authors’ knowledge, this is the first attempt of research that focuses attention on the synchrony between the economic time necessary for recovery and the real-time necessary to achieve vaccination coverage for the restart of production activities.

Abstract

L’Health Policy Forum 20191 , nell’analizzare l’Hta nella sua prospettiva europea, ha dedicato attenzione al contributo che i principi e i metodi dell’Hta possono dare allo sviluppo di approcci innovativi per la programmazione sanitaria. Lo scenario del Sistema sanitario nazionale (Ssn), già prima dell’emergenza legata alla diffusione del Sars-CoV-2, si caratterizzava per una debole crescita del finanziamento e una strenua ricerca di spazi di efficientamento, sempre più difficili da trovare. Anche per effetto della lunga stagione dei piani di rientro, il Ssn appare sempre più diversificato ed ineguale. Le differenze in termini di dotazioni infrastrutturali, tecnologiche e professionali segnano sempre più un solco tra le varie Regioni italiane. Questo aspetto sembra riverberarsi non solo sulla diversa percezione della qualità dei servizi da parte dei cittadini, ma anche in alcuni fondamentali indicatori di salute. Una potenziale risposta a questi problemi risiede in una crescente capacità di programmazione e allocazione delle risorse che vede nel livello nazionale uno snodo fondamentale.

Abstract

Introduction: Urinary Incontinence (UI) is a chronic condition affecting mainly elderly people, thus the budget to deal with is foreseen to grow significantly, especially in Italy, where there is a combination of private and public buying. In 2015, the Italian National Health System (NHS) spent 355 million Euros to provide pads to UI, while the out-of-pocket expenditure for the patients was 233 million Euros. The study aims to evaluate the impact of two different UI buying pathway by performing a Budget Impact Analysis (BIA) vs current model. Methods: Three different scenarios were compared over a period of 3 years. Scenario 1 (“As is”), based on current model (2016-2017 expenditure), was compared to the: Scenario 2 (“Voucher”), implying the introduction of vouchers to entitle patients to acquire pads in the private channel without NHS tenders; Scenario 3 (“Tender management Optimization”) is based on an optimization of provided from NHS pads, according to patients UI severity. Results: Scenario 2 implies increase of NHS expenditure of 262 million Euros in the 2020, while Scenario 3 generates 21 million of savings vs scenario 1. Abstract

Background Hepatitis C virus (HCV) is a leading cause of liver-related morbidity and mortality worldwide. In the European Union (EU), treatment and cure of HCV with direct-acting antiviral therapies began in 2014. WHO targets are to achieve a 65% reduction in liver-related deaths, a 90% reduction of new viral hepatitis infections, and 90% of patients with viral hepatitis infections being diagnosed by 2030. This study assessed the prevalence of HCV in the EU and the level of intervention required to achieve WHO targets for HCV elimination. Methods We populated country Markov models for the 28 EU countries through a literature search of PubMed and Embase between Jan 1, 2000, and March 31, 2016, and a Delphi process to gain expert consensus and validate inputs. We aggregated country models to create a regional EU model. We used the EU model to forecast HCV disease progression (considering the effect of immigration) and developed a strategy to acehive WHO targets. We used weighted average sustained viral response rates and fibrosis restrictions to model the effect of current therapeutic guidelines. We used the EU model to forecast HCV disease progression (considering the effect of immigration) under current screening and therapeutic guidelines. Additionally, we back-calculated the total number of patients needing to be screened and treated to achieve WHO targets. Findings We estimated the number of viraemic HCV infections in 2015 to be 3 238 000 (95% uncertainty interval [UI] 2 106 000–3 795 000) of a total population of 509 868 000 in the EU, equating to a prevalence of viraemic HCV of 0·64% (95% UI 0·41–0·74). We estimated that 1 180 000 (95% UI 1 003 000–1 357 000) people were diagnosed with viraemia (36·4%), 150 000 (12 000–180 000) were treated (4·6% of the total infected population or 12·7% of the diagnosed population), 133 000 (106 000–160 000) were cured (4·1%), and 57 900 (43 900–67 300) were newly infected (1·8%) in 2015. Additionally, 30 400 (26 600–42 500) HCV-positive immigrants entered the EU. To achieve WHO targets, unrestricted treatment needs to increase from 150 000 patients in 2015 to 187 000 patients in 2025 and diagnosis needs to increase from 88 800 new cases annually in 2015 to 180 000 in 2025. Interpretation Given its advanced health-care infrastructure, the EU is uniquely poised to eliminate HCV; however, expansion of screening programmes is essential to increase treatment to achieve the WHO targets. A united effort, grounded in sound epidemiological evidence, will also be necessary. Funding Gilead Sciences. © 2017 Elsevier Ltd
