12. Acceptance Factors for In-Hospital Pharmacist Interventions in Daily Practice: A Retrospective Study

Abstract

Introduction: Performing pharmacist interventions (PIs) during the medication review helps to improve the quality of care. The acceptance by the physician of these PIs is a good indicator of the quality of this clinical pharmacy activity. The objective of this study was to determine, in the Amiens-Picardie teaching hospital (France), factors of acceptance in a variable environment of activity (central pharmacy, in the care units, computer assisted).Methods: All PIs transcribed by pharmacists on the Act-IP© site between November 2018 and April 2019 were analyzed using a complementary search in patient records. The environment, type, and clinical impact on patient health of each PI was collected. Linear mixed-effects models with a random pharmacist intercept were used to investigate the relationship between PI modalities and their chance of being accepted.Results: A total of 3,100 PIs were traced, of which 2,930 had been followed over time. Of these, 2,930 PIs, 1,504 (51.3%) were performed by a postgraduate pharmacist and 1,426 (48.7%) by a pharmacy resident, 1,623 (55.4%) were performed by verbal exchange, 455 (15.5%) by telephone, 846 (28.9%) by computer software, and 6 (0.2%) by paper. The clinical impact on patient health was major for 976 PIs (33.3%) and vital for 26 PIs (0.9%). According to the Anatomical Therapeutic Chemical Classification (ATC), they were mainly related to anti-infectives (30.3%), the nervous system (18.7%), and blood and blood-forming organs (17.3%). In total, 2,415 PIs (82.4%) were accepted. According to the multivariate model, a PI was more often accepted when it was transmitted orally rather than by software (+27.7%, 95% CI: +23.2 to +32.1%) and when it was transmitted to a medical resident rather than a postgraduate physician (+4.4%, 95% CI: 1.2–7.6%). In these cases, there was a major rather than a moderate clinical impact on patient health (+4.3%, 95% CI: +1.1–+7.6%).Conclusion: This study highlights the importance of the quality of the exchange with the prescriber and the prioritization of high-risk interventions as key points of medication review to improve rate of pharmacist interventions accepted by physician.

Abstract

Anarchism is a political current that is distinguished by its diversity and heterogeneity. Yet, the libertarian circles have often been comprehended by historiography in a homogeneous way, contributing to reify their stories. In the face of these pitfalls and also against hagiography activists, the purpose of this paper is to introduce a synthesis of new interpretations of “anarchism” during the Second Republic and the Spanish Civil War. We cannot understand the history of the thirties in Spain without looking at the discourses and practices of libertarian circles.

Abstract

The objective of this article is to demonstrate how the Loire inhabitants managed to keep informed about the Spanish war events. Indeed, despite being far from Spain, they felt concerned by any information related to those incidents. According to the newspapers political orientations (communist, radical, socio-democrat, right wing), the analysis fed different types of fear. The fear of being surrounded by dictatorship, the fear of Bolshevism destroying places of cult, the fear produced by the pictures of people suffering, victims of bombings. This fear produced various reactions and actions: people organised collects, called for donations, some convoys were sent from Saint-Etienne to Spain, in 1937 children from the Basque country were fostered… Other newspapers used the tensions between republicans to depreciate the French left wing.

Abstract

International economic advising is built on the complex interaction between different actors on many levels. Multiple entities are positioned between international lending institutions and local governments in the coordination of development programs. Our article focuses on the interaction between the International Bank for Reconstruction and Development, commonly called “The World Bank”, and David Lilienthal who collaborated with the international missions led by the Bank during the 50s. We analyze their first projects together, shedding light on the way in which their policies emerged and on the reception of these policies by host countries, through the development of a “know-how” in the interaction with the World Bank. The three cases taken in exam illustrate how the first steps of the collaboration between David Lilienthal and the World Bank contributed to the development of a transnational network of economic advisors that shaped developmental policies and projects as a set of institutions continually readapted by actors’ strategies.

Abstract

The spectral variability of reflectance properties of pictorial layers can be caused by chemical parameters, such as the nature of pigments and binders, or by morphological parameters, such as the pigment concentration, the thickness of the layer or the size distribution of pigments. This work proposes a study of the influence of morphological parameters on reflectance spectra using both hyperspectral measurements on physical samples and reflectance spectra calculated using an optical model based on the four-flux model. The spectral variability is evaluated by comparing the spectra to a reference spectrum, thanks to the Kullback–Leibler Pseudo-Divergence.

Abstract

Background and importance In our psychiatric hospital, it is common to prescribe psychotropic medications such as neuroleptics. One side effect of this drug class is to induce QT prolongation. The more torsadogen drugs we prescribe, the more rhythmic cardiac disease, such as torsade de pointe, may occur. Our hospital drug formulary contains three highly torsadogen drugs: escitalopram, hydroxyzine and domperidone. They must be avoided in situations that may increase the risk of torsades de pointes, such as hypokalaemia, bradycardia and other drugs that induce QT prolongation. Aim and objectives To analyse prescriptions containing at least one of three highly torsadogen drugs and detect torsadogen risk situations. Material and methods We retrospectively analysed all prescriptions over 3 months containing at least one of our three highly torsadogen drugs. We also had access to biological results and bradycardia was mentioned in the patient medical file. For each prescription of one of these three drugs, we checked that no other torsadogen drugs was prescribed, and that there was no bradycardia or hypokalaemia. Results During our study period, among all 584 prescriptions, we found 28 containing at least one of our three highly torsadogen drugs, including 8 contraindications (CI) due to co-prescription with other torsadogen drugs: 13 prescriptions containing escitalopram with 2 CI, 10 prescriptions containing hydroxyzine with 2 CI and 5 prescriptions containing domperidone with 4 CI. For each of those 8 contraindications, a pharmacist intervention was redacted to stop the highly torsadogen drug prescription. Seven were accepted and followed and one was partially accepted. Of the total prescriptions, 11% contained drugs that might induce bradycardia and 53% contained drugs that might induce hypokalaemia. However, among 28 prescriptions containing our three highly torsadogen drugs, bradycardia and hypokalaemia were not found. Conclusion and relevance Prescribers may not know enough about the torsadogen risks of escitalopram, hydroxyzine and domperidone, with 80% of these prescriptions containing CI. The pharmacy intervention helped to avoid those 8 CI. This work reminds us to be vigilant about torsadogen drugs, providing many interactions. We regularly inform prescribers about drugs inducing QT prolongation, hypokalaemia or bradycardia to improve prescriptions. References and/or acknowledgements No conflict of interest.

A l’issue de ses auditions, le groupe de travail de l’Academie nationale de medecine formule des pistes de reflexion et une serie de propositions pour permettre a la France de retrouver son attractivite dans un domaine capital de l’innovation medicale pour le bien des patients.

Abstract

Background To identify the mechanisms of the hypercoagulability associated with antiphospholipid antibodies, we investigated antibody-mediated platelet activation and interference of antibodies with phospholipid-dependent reactions.Design and Methods We used two murine monoclonal antibodies, one against β2-glycoprotein I (7F6G), the other against prothrombin (28F4). Platelet activation was assessed by phospholipid-related platelet procoagulant activity. Endogenous thrombin potential without activated protein C (ETP0) and the activated protein C concentration that reduced the ETP0 by 50% (IC50-APC) were determined by calibrated automated thrombography.Results Both monoclonal antibodies mimicked the effect of IgG in 11 out of a series of 40 patients with antiphospholipid antibodies in thrombography. In the presence of their target, 7F6G and 28F4 at 200 μg/mL exhibited comparatively low and high binding to platelets and elicited low and high levels of procoagulant phospholipids on platelet surface, respectively. In platelet-poor plasma, these antibodies induced a 1.6 and >12-fold increase in IC50-APC, respectively, thus providing evidence for a procoagulant effect independent of platelet activation. The 84% decrease in ETP0 indicated that 28F4 also displayed an anticoagulant effect. In platelet-rich plasma, this anticoagulant effect was significantly less (23% decrease in ETP0), demonstrating that a high increase in procoagulant surfaces by platelet activation significantly antagonizes the anticoagulant effect of antiphospholipid antibodies. In both types of plasma, the inhibition of thrombin generation (reduced ETP0) was less than the inhibition of activated protein C activity (increased IC50-APC).Conclusions Our findings show that platelet activation reinforces the hypercoagulability induced by competition between antiphospholipid antibodies/target complexes and pro- and anticoagulant complexes for phospholipid surfaces.

Abstract

Research conducted on the spoilage yeast Zygosaccharomyces rouxii growth rate as a result of pH and sugar influence is presented. Experiments include measurements of cell colonies biomass, and indicate that sugar and pH concentrations have different effect on the growth rate reduction.

Abstract

The microorganism Bacillus thuringiensis has been associated with cases of gastroenteritis. It is widely used as a biological insecticide, and could therefore pose a potential danger to human health. Research in this area has sought to determine the significance of heat treatments and environmental factors for the recovery of heat-treated B. thuringiensis spores. It was possible to identify a number of conditions to determine the heat resistance of spores suspended in various media. Heat-stressed spores should be suspended in peptone water 15 mg/L before counting.

Abstract

International audience; Native of North America, Black cherry (Prunus serotina) is one of the most invasive tree species in European temperate forests. Although the abilities of P. serotina for light competition are well documented, we know little about competition for belowground resources (water, nutrients). Our study aimed to better understand the incidence of belowground processes in the invasion abilities of P. serotina. We thus compared the root functional traits and the mycorrhizal colonisation rates between P. serotina and two European native species (European beech and Scots pine), in pure and mixed-species stands. To analyse the effects of competition, we focused on (i) fine roots of beech and pine, in pure stand and in mixture with P. serotina, and (ii) on fine roots of Prunus serotina, in pure stand and in mixture with the beech or the pine. In each case, we measured the mean root diameter, the branching intensity, the specific root length (SRL) and the abundance of arbuscular and ectomycorrhizae. Our results show that, although P. serotina seemed disadvantaged by its root morphology (lower branching intensity than pine and beech), its ability to host both arbuscular and ectomycorrhizae could confer an advantage. Finally, we did not find any global negative effect of P. serotina presence on the native species root traits, whereas pine reduced the mycorrhizal colonisation rate of P. serotina.; Originaire d’Amérique du Nord, le Cerisier tardif (Prunus serotina) est une espèce invasive des forêts tempérées européennes. Si les aptitudes à la compétition pour la lumière de P. serotina sont actuellement bien documentées, on en sait peu sur la compétition pour les ressources souterraines (eau, nutriments). Notre étude vise à mieux comprendre l’implication des processus souterrains dans les capacités d’invasion de P. serotina. Pour cela, nous comparons les traits fonctionnels racinaires et les taux de mycorhization associés à P. serotina et à deux espèces natives (Hêtre commun et Pin sylvestre), en peuplement pur et en mélange. Afin d’analyser l’effet de la compétition, nous nous sommes intéressés aux racines fines (i) de hêtre et de pin, en peuplement pur et en mélange avec P. serotina, et (ii) de Prunus serotina, en peuplement pur et en mélange avec le hêtre ou le pin. A chaque fois, nous avons mesuré le diamètre moyen, l’intensité de branchement, la longueur racinaire spécifique (SRL) et l’abondance des endo- et ectomycorhizes. Nos résultats montrent que si la morphologie racinaire de P. serotina ne semble pas lui être favorable (intensité de branchement inférieure à celle du pin et du hêtre), sa capacité à former des endo- et des ectomycorhizes pourrait lui conférer un avantage. Enfin, nous n’avons globalement pas montré d’effets négatifs de la présence de P. serotina sur les traits racinaires des espèces natives, alors que le pin semble réduire le taux de colonisation mycorhizienne de P. serotina.
