Data related to barriers limiting uptake of DMEK surgery, self-perceived levels of competence, and difficulty with different steps of DMEK surgery were analyzed.Results: The survey response rate was 31% (22 of 72). The most common barrier to uptake of DMEK surgery identified was anxiety related to incorrect insertion of the tissue and the need to regraft (64%, 14 of 22), followed by anxiety related to tissue preparation (50%, eleven of 22). Surgeons also felt anxious regarding the possibility of rebubbling with initial DMEK (41%, nine of 22). Steps related to DMEK graft (76%) preparation, tissue insertion (41%), and graft unfolding (72%) were identified as the most difficult steps to learn by the respondents.Conclusion: The DMEK learning curve, especially for the novice surgeon, may be shortened by seeking educational resources, including wet labs and surgical videos. Eye banks may facilitate adoption of DMEK by making validated DMEK tissue more accessible to surgeons globally.Keywords: DMEK, uptake, barriers, difficulties

12. Sustainable Community Development or Voluntourism: Illustrative Case Study of Sustainable Housing in Maharashtra

Abstract

Volunteer tourism or ‘voluntourism’ packages development and poverty as culturally exotic and ethical experiences for tourists from industrialized countries. In the university sector study abroad tours network voluntourism agencies, local actors, e.g. NGOs, universities and government funding to offer students ‘life changing’ experiential and community development learning. Recent criticism of the commodification of development and poverty through such tours points to multiple pernicious effects of such travel, especially the failure to deliver community impact. Following review of current criticism of voluntourism, this illustrative case study of a purported sustainable housing project in rural Maharashtra employs multiple data sources and covert research to explore the multiple gaps between participatory community sustainable development and voluntourism. The study finds signal lack of financial transparency, incompetent assessment of material needs, and limited local participation and control, failure to deliver on objectives, and recommends that socially responsible short term international exchanges should be carefully monitored and prefer knowledge exchange.

28. Content, structure and orientations of the practice-led exegesis

Abstract

The emergent field of practice-led research is a unique research paradigm that situates creative practice as both a driver and outcome of the research process. The exegesis that accompanies the creative practice in higher research degrees remains open to experimentation and discussion around what content should be included, how it should be structured, and its orientations. This paper contributes to this discussion by reporting on a content analysis of a large, local sample of exegeses. We have observed a broad pattern in contents and structure within this sample. Besides the introduction and conclusion, it has three main parts: situating concepts (conceptual definitions and theories), practical contexts (precedents in related practices), and new creations (the creative process, the artifacts produced and their value as research). This model appears to combine earlier approaches to the exegesis, which oscillated between academic objectivity in providing a context for the practice and personal reflection or commentary upon the creative practice. We argue that this hybrid or connective model assumes both orientations and so allows the researcher to effectively frame the practice as a research contribution to a wider field while doing justice to its invested poetics.

31. Gender and discipline: design publication practices

Abstract

Design writing began to appear in scholarly journals over thirty years ago, coinciding in Australia with the transition of design into universities. Concurrently, a significant increase of women in the field actuated feminist-informed 'women and design' writing, raising important questions about gendered practices. Yet these ideas are not taken up in broader literatures, while publication and citation rates demonstrate the dominance of men in discipline-building 'practices' (Green 2009). This paper argues that the problematically gendered interactions between women, design and scholarly writing are reproduced through the operation of certain scholarly practices. The argument is supported through an empirical audit and analysis of the publication histories of two key journals, conducted in conjunction with a feminist reading of the Australian ERA Indicator Descriptors (ARC, 2008) of research output. I suggest this reading has the potential to productively disrupt and reconceptualise the gendered relations between women, men and design scholarship.

33. Credit-Based Discipline Specific English for Academic Purposes Programmes in Higher Education Revitalizing the profession

Abstract

In the UK, North America and Australia, credit-bearing discipline specific English for Academic Purposes (EAP) courses are seen as a challenge to remedial views of English as a Second Language and a key element in revitalizing a profession on the periphery of the institution. However, the EAP field has to confront not only institutional challenges to its acceptability as a discipline but also tensions within the field. In this article we examine the tensions which underpin current and future directions in the field, review the development of credit-based EAP courses in the US, UK and Australia, and illustrate our discussion with a case study from the University of Melbourne. We conclude by arguing that discipline specific credit-based EAP offers promising hope for the future of the EAP discipline in higher education, but that to achieve this end the field and practitioners need to find a position between critique of and accommodation to discipline specific content.

45. Amniotic membrane transplantation in the management of conjunctival malignant melanoma and primary acquired melanosis with atypia

Abstract

AIM: To evaluate the efficacy of amniotic membrane transplantation (AMT) for the management of conjunctival malignant melanoma and primary acquired melanosis (PAM) with atypia. METHODS: Four consecutive patients with histologically proved invasive, primary conjunctival malignant melanoma were treated with wide surgical excision and AMT. Amniotic membrane grafts were harvested and processed under sterile conditions according to a standard protocol. The grafts were sutured to the margins of the surface defect. In one case, AMT was combined with a corneoscleral graft. RESULTS: A satisfactory result and rapid postoperative recovery with few, transient side effects was noted in three patients with limbal/epibulbar melanomas. In another patient with an extensive lesion, involving the epibulbar, forniceal, and palpebral conjunctiva, AMT following wide excision was complicated by symblepharon formation and restricted ocular motility. Monitoring of local recurrence was facilitated by the transparency of the thin graft in all cases. The postoperative follow up time varied between several months and 3 years. In one case, local recurrence of PAM was observed and treated using topical mitomycin. CONCLUSIONS: AMT is a useful technique for the reconstruction of both small and large surface defects that result from the surgical excision of conjunctival malignant melanoma and PAM. This method facilitates wide conjunctivectomy, although its role in repairing larger defects involving the fornix or palpebral conjunctiva still needs to be established. The transparency of amniotic membrane allows for monitoring of tumour recurrence, which is--together with superior cosmesis--an advantage over thicker (for example, buccal) mucous membrane grafts.

Abstract

Transplantation of labial salivary glands to the eyelids for patients with dry eye appears to give excellent results clinically. To obtain more scientific evidence for this surgery, the human tear proteome was characterized and compared with salivary proteins. Not only the possible beneficial effects of the surgery in patients with dry eye disease were confirmed; the tear fluid proteome appears to contain a lot of additional information, possibly reflecting other ophthalmological and even non-ophthalmological pathologies. Top-down proteomics of tears is superior to the bottom-up approach because multiple molecular forms are detected as products of the same genes. Since structurally different protein forms are likely to have different bioactivities, the identification of these so called proteoforms, which is largely missed in bottom-up proteomics, is essential to understand the effect of a specific protein in relation to a disease. The combination of Schirmer strip based tear fluid sample collection, automated heat stabilization, a microfluidic chip-based capillary electrophoresis unit, and a benchtop high resolution mass spectrometer may make proteoform profiling of tears become applicable in the peripheral ophthalmic practice.
