Purpose: The dual goals of this tutorial are (a) to increase awareness and use of mediation and moderation models in cognitive-communication rehabilitation research by describing options, benefits, and attainable analytic approaches for researchers with limited resources and sample sizes and (b) to describe how these findings may be interpreted for clinicians consuming research to inform clinical care. Method: We highlight key insights from the social sciences literature pointing to the risks of common approaches to linear modeling, which may slow progress in clinical–translational research and reduce the clinical utility of our work. We discuss the potential of mediation and moderation analyses to reduce the research-to-practice gap and describe how researchers may begin to implement these models, even in smaller sample sizes. We discuss how these preliminary analyses can help focus resources for larger trials to fully encapsulate the heterogeneity of individuals with cognitive-communication disorders. Results: In rehabilitation research, we study groups, but we use the findings from those studies to treat individuals. The most functional clinical research is about more than establishing only whether a given effect exists for an “average person” in the group of interest. It is critical to understand the active ingredients and mechanisms of action by which a given treatment works (mediation) and to know which circumstances, contexts, or individual characteristics might make that treatment most beneficial (moderation). Conclusions: Increased adoption of mediation and moderation approaches, executed in appropriate steps, could accelerate progress in cognitive-communication rehabilitation research and lead to the development of targeted treatments that work for more clients. In a field that has made limited progress in developing successful interventions for the last several decades, it is critical that we harness new approaches to advance clinical–translational research results for complex, heterogeneous groups with cognitive-communication disorders.

Background and aim: Deficits in decision-making are a common consequence of moderate-severe traumatic brain injury (TBI). Less is known, however, about how individuals with TBI perform on moral decision-making tasks. To address this gap in the literature, the current study probed moral decision-making in a sample of individuals with TBI using a widely employed experimental measure. Methods/hypothesis: We administered a set of 50 trolley-type dilemmas to 31 individuals with TBI and 31 demographically matched, neurotypical comparison participants. We hypothesized that individuals with TBI would be more likely to offer utilitarian responses to personal dilemmas than neurotypical peers. Results: In contrast to our hypothesis, we observed that individuals with TBI were not more likely to offer utilitarian responses for personal dilemmas. Conclusion: Our results suggest that moral decision-making ability is not uniformly impaired following TBI. Rather, neuroanatomical (lesion location) and demographic (age at injury) characteristics may be more predictive of a disruption in moral decision-making than TBI diagnosis or injury severity alone. These results inform the neurobiology of moral decision-making and have implications for characterizing patterns of spared and impaired cognitive abilities in TBI.

Purpose: The purpose of this viewpoint is to advocate for increased study of word learning abilities and word learning interventions in traumatic brain injury (TBI). Method: We describe the word learning process and the unique opportunities afforded by studying each component and stage. Building on discussions at the 2022 International Cognitive-Communication Disorders Conference, we describe how word learning may underlie a variety of functional outcomes after TBI, making it a promising target for rehabilitation. Finally, we discuss principles that may guide study in this critical area to advance outcomes after TBI for children and adults. Results: Word learning is a dynamic and iterative process taking place over time and in multiple stages. Thus, studying word learning affords the opportunity to parse the relative contributions of multiple memory systems to different phases and components of the process. However, single–timepoint designs are insufficient to capture the full word learning process, which occurs over time and across contexts. Word learning also presents an opportunity to assess the contributions of behavioral and lifestyle factors (e.g., sleep and exercise) to different memory phases. Understanding these interactions could drive clinical interventions aimed at improving memory through manipulable external behaviors. Conclusions: Word learning is key to success in functional spheres across the life span. The importance of words to daily life remains after TBI, even as the memory systems that support word learning are disrupted. The empirical study of word learning and rehabilitation of word learning deficits in TBI presents a promising new direction in understanding the breadth of neurogenic cognitive-communication disorders and an opportunity to explore a potential driver of functional outcome and impactful rehabilitation target.

Purpose When we speak, we gesture, and indeed, persons with aphasia gesture more frequently. The reason(s) for this is still being investigated, spurring an increase in the number of studies of gesture in persons with aphasia. As the number of studies increases, so too does the need for a shared set of best practices for gesture research in aphasia. After briefly reviewing the importance and use of gesture in persons with aphasia, this viewpoint puts forth methodological and design considerations when evaluating gesture in persons with aphasia. Method & Results We explore several different design and methodological considerations for gesture research specific to persons with aphasia, such as video angle specifications, data collection techniques, and analysis considerations. The goal of these suggestions is to develop transparent and reproducible methods for evaluating gesture in aphasia to build a solid foundation for continued work in this area. Conclusions We have proposed that it is critical to evaluate multimodal communication in a methodologically robust way to facilitate increased knowledge about the relationship of gesture to spoken language, cognition, and to other aspects of living with aphasia and recovery from aphasia. We conclude by postulating future directions for gesture research in aphasia.

Purpose The purpose of this article is to highlight the need for increased focus on cognitive communication in North American speech-language pathology graduate education models. Method We describe key findings from a recent survey of acute care speech-language pathologists (SLPs) in the United States and expand upon the ensuing discussion at the 2020 International Cognitive-Communication Disorders Conference to consider some of the specific challenges of training for cognitive communication and make suggestions for rethinking how to prepare future clinicians to manage cognitive-communication disorders. Results Results from the survey of acute care SLPs indicated inconsistent confidence and training in managing cognitive-communication disorders. We discuss the pros and cons of several avenues for improving the consistency of cognitive-communication training, including a standalone cognitive-communication course, integrating cognitive communication in all courses across the speech-language pathology undergraduate and graduate curriculum, and using problem-based learning frameworks to better prepare students as independent thinkers in the area of cognitive communication and beyond. Conclusions Cognitive-communication disorders cut across clinical diagnoses and settings and are one of the largest and fastest growing parts of the SLP's scope of practice. Yet, surveys, including the one discussed here, have repeatedly indicated that SLPs do not feel prepared or confident to work with individuals with cognitive-communication disorders. We propose several avenues for increasing educational emphasis on cognitive communication. We hope these ideas will generate discussion and guide decision making to empower SLPs to think critically and step confidently into their roles as leaders in managing the heterogeneous and ever-growing populations of individuals with cognitive-communication disorders.

Objective The aim of this study was to determine the health utility states of the most commonly used traumatic brain injury (TBI) clinical trial endpoint, the Extended Glasgow Outcome Scale (GOSE). Summary background data Health utilities represent the strength of one's preferences under conditions of uncertainty. There are insufficient data to indicate how an individual would value levels of disability after a TBI. Methods This was a cross-sectional web-based online convenience sampling adaptive survey. Using a standard gamble approach, participants evaluated their preferences for GOSE health states 1 year after a hypothetical TBI. The categorical GOSE was studied from vegetative state (GOSE2) to upper good recovery (GOSE8). Median (25th percentile, 75th percentile) health utility values for different GOSE states after TBI, ranging from -1 (worse than death) to 1 (full health), with 0 as reference (death). Results Of 3508 eligible participants, 3235 (92.22%) completed the survey. Participants rated lower GOSE states as having lower utility, with some states rated as worse than death, though the relationship was nonlinear and intervals were unequal between health states. Over 75% of participants rated a vegetative state (GOSE2, absence of awareness and bedridden) and about 50% rated lower severe disability (GOSE3, housebound needing all-day assistance) as conditions worse than death. Conclusions In the largest investigation of public perceptions about post-TBI disability, we demonstrate unequally rated health states, with some states perceived as worse than death. Although limited by selection bias, these results may guide future comparative-effectiveness research and shared medical decision-making after neurologic injury.

Referring to things in the world - that woman, her idea, she - is a central component of language. Understanding reference requires the listener to keep track of the unfolding discourse history while integrating multiple sources of information to interpret the speech stream as it unfolds in time. Pronouns are a common way to establish reference. But due to their impoverished form, to understand them listeners must relate features of the pronoun (e.g., gender, animacy) with existing representations of potential discourse referents. Successful referential processing seems to place demands on memory. In a previous study, patients with hippocampal amnesia and healthy participants listened to short stories as their eye movements were monitored. When interpreting ambiguous pronouns, healthy participants demonstrated order-of-mention effects, whereby ambiguous pronouns are interpreted as referring to the first-mentioned referent in the story. By contrast, memory-impaired patients exhibited significant disruptions in their ability to use information about which character had been mentioned first to interpret pronouns. Repetition of the most salient information is a common clinical recommendation for improving pronoun resolution and communication in individuals with memory disorders (e.g., Alzheimer's disease) but this recommendation lacks an evidentiary basis. The present study seeks to determine whether the pronoun resolution performance of hippocampal patients can be improved, by repetition of the target referent, increasing its salience. Results indicate that patients with hippocampal damage demonstrate improved processing of pronouns following repetition of the target referent, but benefit from this repetition to a significantly smaller degree compared to healthy participants. These results provide further evidence for the role of the hippocampal-dependent memory system in language processing and point to the need for empirically tested communication interventions.

Background Individuals with a history of traumatic brain injury (TBI) report fewer social contacts, less social participation, and more social isolation than noninjured peers. Cognitive-communication disabilities may prevent individuals with TBI from accessing the opportunities for social connection afforded by computer-mediated communication, as individuals with TBI report lower overall usage of social media than noninjured peers and substantial challenges with accessibility and usability. Although adaptations for individuals with motor and sensory impairments exist to support social media use, there have been no parallel advances to support individuals with cognitive disabilities, such as those exhibited by some people with TBI. In this study, we take a preliminary step in the development process by learning more about patterns of social media use in individuals with TBI as well as their input and priorities for developing social media adaptations. Objective This study aims to characterize how and why adults with TBI use social media and computer-mediated communication platforms, to evaluate changes in computer-mediated communication after brain injury, and to elicit suggestions from individuals with TBI to improve access to social media after injury. Methods We conducted a web-based survey of 53 individuals with a chronic history of moderate-to-severe TBI and a demographically matched group of 51 noninjured comparison peers. Results More than 90% of participants in both groups had an account on at least one computer-mediated communication platform, with Facebook and Facebook Messenger being the most popular platforms in both groups. Participants with and without a history of TBI reported that they use Facebook more passively than actively and reported that they most frequently maintain web-based relationships with close friends and family members. However, participants with TBI reported less frequently than noninjured comparison participants that they use synchronous videoconferencing platforms, are connected with acquaintances on the web, or use social media as a gateway for offline social connection (eg, to find events). Of the participants with TBI, 23% (12/53) reported a change in their patterns of social media use caused by brain injury and listed concerns about accessibility, safety, and usability as major barriers. Conclusions Although individuals with TBI maintain social media accounts to the same extent as healthy comparisons, some may not use them in a way that promotes social connection. Thus, it is important to design social media adaptations that address the needs and priorities of individuals with TBI, so they can also reap the benefits of social connectedness offered by these platforms. By considering computer-mediated communication as part of individuals’ broader social health, we may be able to increase web-based participation in a way that is meaningful, positive, and beneficial to broader social life.

Objectives To identify the consequences of the coronavirus 2019 (COVID-19) pandemic for individuals with traumatic brain injury (TBI), with particular attention to unique effects for individuals with chronic disability. Design Individuals with and without a history of TBI completed a web-based survey. Setting Participants were recruited from the Vanderbilt Brain Injury Patient Registry in Nashville, TN, and completed the survey from their homes between May and June 2020, during social distancing related to the COVID-19 pandemic. Participants Participants (N=47) in the chronic phase of moderate-severe TBI (>6mo postinjury) and 51 noninjured comparison (NC) peers completed the survey. Interventions Not applicable. Main Outcome Measures Participants, or respondents, answered a mix of multiple choice and free text questions about how the COVID-19 pandemic has affected their work, education, medical care, social communication, sources of information and decision making, and mental and physical well-being. Individuals with TBI also answered questions about how TBI has affected their experiences of the pandemic. Results As a group, respondents with TBI reported less pandemic-related behavior change (eg, daily habits, virtual social visits, and masking) than NC peers. Both NCs and respondents with TBI identified health care providers as trusted sources of public health information. One-third of individuals with TBI indicated that brain injury has made coping with the pandemic more difficult, and respondents identified mental health challenges and social isolation as key barriers. Conclusions These results suggest that health care providers should look for ways to provide tailored education and reduce social isolation for individuals with disability during the ongoing COVID-19 pandemic. We discuss several direct suggestions from participant responses.

BACKGROUND: Adults with traumatic brain injury (TBI) report loss of friendship and reduced social participation after injury, but there is limited information regarding quantity of friends and methods of communication. Our objective was to characterize friendship networks, social participation, and methods of communication, including computer-mediated communication (CMC), used by adults with TBI compared to uninjured adults. METHODS: Participants were 25 adults with TBI and 26 uninjured healthy comparisons (HC) adults, who completed the Participation Assessment with Recombined Tools-Objective (PART-O) and the Social Network Questionnaire (SNQ). RESULTS: Adults with TBI had significantly fewer total friends and significantly lower levels of productivity and overall social participation. Face-to-face interaction was the preferred method of contact for both groups. Adults with TBI were significantly less likely to use texting as a primary method of communication than their uninjured peers, but used other methods of communication at similar rates. CONCLUSION: Our study supports prior findings of reduced friendships and reduced social participation after TBI and adds new information about similarities and differences in communication methods between adults with and without TBI.

Objectives:Although individuals with traumatic brain injury (TBI) often report higher levels of social isolation, little is known about the factors influencing their self-perception of loneliness. The aim of the current study was to investigate the relationship between loneliness, social network size, and personality variables (neuroticism and extraversion) after TBI, and in particular whether specific personality variables mediate the relationship between social network size and perception of loneliness.Methods:Here, we assessed self-reported loneliness, personality variables, and social network size of 24 individuals with moderate-to-severe TBI and 41 healthy comparison participants. We then carried out a mediation analysis to examine whether personality variables mediated the relationship between loneliness and social network size.Results:Our results indicate that individuals with TBI reported higher levels of loneliness and neuroticism, but there was no group difference in social network size or extraversion. The mediation analysis revealed that the association between social network size and loneliness was mediated by neuroticism, but not by extraversion.Conclusions:Our findings show that neuroticism is an intervening variable in the relationship between social network size and self-perception of loneliness in individuals with moderate-to-severe TBI, and presents a new possible target for clinicians and rehabilitators seeking to address reports of loneliness and social isolation in TBI. Purpose In both basic science and intervention research in traumatic brain injury (TBI), heterogeneity in the patient population is frequently cited as a limitation and is often interpreted as a factor reducing certainty in the generalizability of research findings and as a source of conflicting findings across studies. Historically, much of TBI research in rehabilitation and cognition has relied upon case–control studies, with small to modest sample sizes. In this context, heterogeneity is indeed a significant limitation. Here, however, we argue that heterogeneity in patient profiles is a hallmark characteristic of TBI and therefore cannot be avoided or ignored. We argue that this inherent heterogeneity must be acknowledged and accounted for prior to study design. Fortunately, advances in statistical methods and computing power allow researchers to leverage heterogeneity, rather than be constrained by it. Method In this article, we review sources of heterogeneity that contribute to challenges in TBI research, highlight methodological advances in statistical analysis and in other fields with high degrees of heterogeneity (e.g., psychiatry) that may be fruitfully applied to decomposing heterogeneity in TBI, and offer an example from our research group incorporating this approach. Conclusion Only by adopting new methodological approaches can we advance the science of rehabilitation following TBI in ways that will impact clinical practice and inform decision making, allowing us to understand and respond to the range of individual differences that are a hallmark in this population.

When damage to left hemisphere perisylvian regions unexpectedly does not result in aphasia, it is often assumed that the individual must be right lateralized for language. However, this supposition is rarely confirmed, and there are two other explanations that should be considered. First, many individuals demonstrate some degree of bilateral representation for language, raising the possibility that the contributions of the undamaged right hemisphere could be sufficient to sustain normal language function following left hemisphere damage. Second, there is considerable individual variability in the precise localization of critical language regions, raising the possibility that a localized lesion could entirely avoid a left-lateralized language network, leaving language function intact. We used functional neuroimaging to determine the reason for a lack of aphasia in a left-handed gentleman who sustained a left temporal lobe hemorrhage. We found unequivocal evidence for right lateralization of language function to explain his lack of aphasia. Although the alternative explanations were not observed in our case, we discuss their potential to underlie a lack of aphasia in other individuals. This report highlights questions that still remain about the various ways language can be organized in the brain, and the implications of damage to different regions in different scenarios.

Object naming involves accessing meaning and retrieving the associated word form from remote semantic memory. Historically, previously acquired semantic knowledge (i.e., remote semantic memory) was thought to be independent of the hippocampus via neocortical consolidation. This view is based on evidence demonstrating a dissociation in behavior in patients with hippocampal amnesia: amnesic patients are impaired in acquiring new vocabulary yet can name and define previously acquired words. More recently, the view that remote semantic memory is hippocampus-independent has been challenged by the documentation of disruptions in aspects of remote semantic memory in patients with hippocampal amnesia, particularly in language use and depth of semantic knowledge. Based on these findings, we hypothesized that the hippocampus plays a long-term role in remote semantic memory. We tested amnesic patients and demographically matched healthy comparison participants in an extensive naming task using photographic images of objects normalized for familiarity, object agreement, and visual complexity. Amnesic patients were less likely to correctly name objects than healthy comparison participants. Further, amnesic patients' performance worsened for words that were less familiar, more visually complex, and had less object agreement. These findings suggest that the hippocampus may play a long-term role in semantic memory processes, rather than a time-limited role in the initial acquisition of semantic information, and that hippocampal damage can disrupt object naming.

However, when a person gets hurt in real life, it may take a lot of patience and hard work to get better. People who are recovering from an injury may also need support from the community and care from healthcare and educational professionals. In particular, it takes time and hard work to recover after a traumatic brain injury (TBI). People with TBI work with a special group of professionals, called a rehabilitation team, to get better. In this article, we will learn about a boy named Dante, who had a TBI after falling off his bicycle. We will learn what happened in Dante’s brain after he got hurt and how his rehabilitation team helped him after his TBI. We will finish up by talking about how you can help people with TBI in your community.

To characterize current knowledge, beliefs, confidence, and practice patterns of acute care speech-language pathologists (SLPs) in assessing and managing cognitive-communication disorders following traumatic brain injury (TBI).We developed an online survey to learn more about current TBI knowledge and practice patterns of acute care SLPs, with the goal of establishing a baseline upon which changes in SLP training and practice standards may be measured.We distributed the survey to 1800 SLPs in 18 states via postal mail, in addition to posting it to relevant online groups. One hundred and eighty-two practicing acute care SLPs responded to the survey.Respondents were highly variable in their training and knowledge about TBI, their beliefs about cognitive-communication rehabilitation, and their practice patterns in assessing, managing, and communicating about TBI during the acute stage of injury.These results highlight the need for more consistent training about cognitive-communication deficits during and after graduate school, as well as the development of sensitive, specific, and standardized assessment tools, education protocols, and shared language for describing patients with TBI along the continuum of care.

PRIMARY OBJECTIVE: The current study examined how creative divergent thinking (i.e., the ability to produce varied and original solutions to a problem) is impacted by moderate-to-severe traumatic brain injury (TBI). RESEARCH DESIGN: Descriptive, observational. METHODS AND PROCEDURES: We administered two tasks of divergent thinking, the Abbreviated Torrance Test for Adults (ATTA) and the Alternative Uses Test (AUT), as well as a battery of neuropsychological tests and psychosocial variables (assessing memory and learning, processing speed, set shifting and psychological distress), to 29 individuals with TBI and 20 demographically-matched healthy comparison participants. MAIN OUTCOMES AND RESULTS: Individuals with TBI performed similarly to healthy individuals on both tests of creative thinking, although they were impaired on the neuropsychological tasks. Moreover, there was no significant correlation between performance on the ATTA and performance on neuropsychological tests, but within the TBI group AUT performance and memory were significantly and positively associated. CONCLUSIONS: Our findings reveal that divergent thinking, as measured by the ATTA and AUT, might be spared following moderate-to-severe TBI. These findings further our understanding of the higher-level cognitive sequelae of TBI and suggest that divergent thinking might be leveraged during treatment planning.

Purpose This tutorial aims to draw attention to the interactions among memory, sleep, and therapy potential and to increase awareness and knowledge in the field of speech-language pathology of the potential impact of sleep as a mediating or moderating factor in promoting therapeutic outcome. Method We highlight key findings from the literature on the cognitive neuroscience of memory, the neurophysiology of sleep, how sleep supports memory, and how sleep disruption affects memory and learning abilities in populations commonly served in speech-language pathology. Results Research increasingly points to the critical importance of sleep quality and quantity to memory and learning, and sleep disruption is linked to deficits in functional cognition that may limit our clients' ability to benefit from speech pathology interventions. Conclusions As a field dedicated to promoting memory, learning, and relearning through our interventions, any systemic factors that affect these abilities demand our attention. Although speech-language pathologists do not treat sleep disturbance, we play a critical role in recognizing the signs and symptoms of sleep disturbance and making appropriate referrals, as undiagnosed and untreated sleep disturbance can have serious impacts on success in therapeutic contexts. By considering how related factors affect memory and learning, we have the opportunity to take a whole client approach to maximizing our clients' therapy potential and functional progress.

Primary objective Research studies and clinical observations of individuals with traumatic brain injury (TBI) indicate marked deficits in mentalizing-perceiving social information and integrating it into judgements about the affective and mental states of others. The current study investigates social-cognitive mechanisms that underlie mentalizing ability to advance our understanding of social consequences of TBI and inform the development of more effective clinical interventions. Research design The study followed a mixed-design experiment, manipulating the presence of a mentalizing gaze cue across trials and participant population (TBI vs. healthy comparisons). Methods and procedures Participants, 153 adults, 74 with moderate-severe TBI and 79 demographically matched healthy comparison peers, were asked to judge a humanoid robot's mental state based on precisely controlled gaze cues presented by the robot and apply those judgements to respond accurately on the experimental task. Main outcomes and results Results showed that, contrary to our hypothesis, the social cues improved task performance in the TBI group but not the healthy comparison group. Conclusions Results provide evidence that, in specific contexts, individuals with TBI can perceive, correctly recognize, and integrate dynamic gaze cues and motivate further research to understand why this ability may not translate to day-to-day social interactions.

OBJECTIVE: While deficits in several cognitive domains following moderate-to-severe traumatic brain injury (TBI) have been well documented, little is known about the impact of TBI on creativity. In the current study, our goal is to determine whether convergent problem solving, which contributes to creative thinking, is impaired following TBI. METHODS: We administered a test of convergent problem solving, the Remote Associate Task (RAT), as well as a battery of neuropsychological tests, to 29 individuals with TBI and 20 healthy comparisons. RESULTS: A mixed-effect regression analysis revealed that individuals with TBI were significantly less likely to produce a correct response, although on average they attempted to respond to the same number of items. Moreover, we found that the TBI (but not the comparison) group’s performance on the RAT was significantly and positively associated with verbal learning and memory, providing further evidence supporting the association between declarative memory and creative convergent thinking. CONCLUSION: In summary, our findings reveal that convergent thinking can be compromised by moderate-to-severe TBI, furthering our understanding of the higher-level cognitive sequelae of TBI.

Facial affect recognition deficits following traumatic brain injury (TBI) have been well documented, as has their relationship with impairment in several other cognitive domains. However, little is known about the neurobiological mechanisms underlying affect recognition deficits, in particular mechanisms underlying different aspects of facial affect recognition (e.g., perceptual and interpretive processes). In the current study, 33 adults with moderate-to-severe TBI and 24 demographically matched healthy comparison (HC) participants completed an fMRI facial affect recognition study. While in the scanner, participants were asked to match the affect of a target face to either (a) one of two faces differing in affect (perceptual condition) or (b) one of two written affect labels (interpretative condition). In both groups we found activations in regions typically involved in affect recognition. Our results revealed that in the perceptual condition individuals with TBI tended to activate the left dorsolateral prefrontal cortex less than HCs, and within the HC group individuals with higher perceptual affect recognition scores showed higher levels of activation in the same brain region. Individuals with TBI who were specifically impaired at interpretative affect recognition showed less activation than HCs in the right fusiform gyrus. Moreover, in the labeling condition individuals with TBI tended to de-activate medial prefrontal regions less than HCs. A region of interest analysis revealed that individuals with TBI showed significantly less activation than HCs in the FFA for all the contrasts of interest. Our results suggest involvement of several brain regions in facial affect recognition impairment post TBI, and provide neurobiological support for the notion that distinct aspects of facial affect recognition can be differentially impaired following TBI.
