Abstract

Haplo-insufficiency of the TGF beta-activated kinase 1 binding protein 2 (TAB2) gene is associated with short stature, facial dysmorphisms, connective tissue abnormalities, hearing loss, and cardiac disease. Skeletal dysplasia and sacral dimples are also found in a minority of patients. Here, we describe a 3-generation family with caudal appendage, other sacral anomalies, and skeletal abnormalities including hypoplasia of the iliac wings and scapulae, fusion of the carpal bones and stenosis of the spinal canal, as well as a remarkable course of prenatally-detected cardiomyopathy with characteristics changing over time. Genetic analysis showed a heterozygous nonsense variant in the TAB2 gene.

Abstract

Background and Purpose: Observational studies have suggested a link between fibromuscular dysplasia and spontaneous cervical artery dissection (sCeAD). However, whether patients with coexistence of the two conditions have distinctive clinical characteristics has not been extensively investigated. Methods: In a cohort of consecutive patients with first-ever sCeAD, enrolled in the setting of the multicenter IPSYS CeAD study (Italian Project on Stroke in Young Adults Cervical Artery Dissection) between January 2000 and June 2019, we compared demographic and clinical characteristics, risk factor profile, vascular pathology, and midterm outcome of patients with coexistent cerebrovascular fibromuscular dysplasia (cFMD; cFMD+) with those of patients without cFMD (cFMD-). Results: A total of 1283 sCeAD patients (mean age, 47.8±11.4 years; women, 545 [42.5%]) qualified for the analysis, of whom 103 (8.0%) were diagnosed with cFMD+. In multivariable analysis, history of migraine (odds ratio, 1.78 [95% CI, 1.13-2.79]), the presence of intracranial aneurysms (odds ratio, 8.71 [95% CI, 4.06-18.68]), and the occurrence of minor traumas before the event (odds ratio, 0.48 [95% CI, 0.26-0.89]) were associated with cFMD. After a median follow-up of 34.0 months (25th to 75th percentile, 60.0), 39 (3.3%) patients had recurrent sCeAD events. cFMD+ and history of migraine predicted independently the risk of recurrent sCeAD (hazard ratio, 3.40 [95% CI, 1.58-7.31] and 2.07 [95% CI, 1.06-4.03], respectively) in multivariable Cox proportional hazards analysis. Conclusions: Risk factor profile of sCeAD patients with cFMD differs from that of patients without cFMD. cFMD and migraine are independent predictors of midterm risk of sCeAD recurrence.

13. Is oral antibiotic therapy as effective as intravenous treatment in bacterial osteomyelitis? A real-life experience.

Abstract

OBJECTIVE: Osteomyelitis is a relatively understudied disease with no standardized and evidence-based approach to treatment. We aimed to evaluate a cohort of patients with osteomyelitis, comparing the outcomes between intravenous (IV) and oral treatment. PATIENTS AND METHODS: We performed an observational retrospective study on osteomyelitis cases in adult patients seen for care between 2017 and 2019. We collected information on patient characteristics, microbiological etiology, infection treatment, and outcome. In addition, we divided osteomyelitis cases by antibiotic regimens [oral (OTG) vs. intravenous±oral (ITG)] and therapy durations to evaluate outcomes differences. RESULTS: A total of 235 episodes of osteomyelitis were evaluated, with a higher prevalence in male gender. Staphylococci, especially S. aureus, were the most common strains. Out of the 235 evaluated episodes, we selected 142 cases. Of these, 75 were treated with OTG and 67 with ITG. Gram-positive bacteria were the most frequent aetiological agents, with 81 isolates (61.8%). Full recovery was observed in 79 (55.6%) cases; of these, 36 (53.7%) were in the ITG and 43 (57.3%) in the OTG (p = 0.666). At the logistic regression, a polymicrobial infection [OR 4.16 (95%CI 1.28-13.4), p = 0.017] and a less than six weeks treatment duration [OR 4.24 (95%CI 1.38-5.43) p = 0.004] were significantly associated with a higher risk of treatment failure. CONCLUSIONS: Our study suggests that oral treatment efficacy is comparable to ITG therapy for osteomyelitis, confirming the most recent evidence suggesting that oral therapy is non-inferior to intravenous therapy to treat osteomyelitis. Abstract

The present investigation is an experimental study of the mixing of two phases of strongly different viscosity in a co-rotating twin-screw extruder (TSE). Two cases were considered: a miscible (copolymer of ethylene and vinyl acetate/bis(2-ethylhexyl)phtalate [EVA/DOP]) and an immiscible (EVA/ethylene-glycol [EVA/EG]) binary system. The residence time distribution (RTD) has been determined for each phase of both systems for different screw profiles and different operating conditions. The behavior of the first system is simple: the type of flow is basically the same. However, in the case of nonmiscibility, when the flow rate is high, the shape of the RTD of both phases may be different. We interpret this result with the appearance of a lubrication phenomenon. Abstract

A new process for elaborating continuously organic-inorganic hybrid materials by reactive extrusion is presented. This synthesis consists in formulating a copolymer of ethylene and vinyl acetate (EVA)/tetrapropoxysilane (TPOS or Si(OC3H7)4)/catalyst blend by a corotating twin screw extruder followed by two successive reaction steps: on line microwave crosslinking reaction and hydrolysis-condensation reactions leading to a silica network cografted onto the organic matrix.

Abstract

The discovery of the archives of Francesco di Marco Datini of Prato - a 14th century Merchant and Banker of international standing - dates back to 1870. The archives, perhaps the largest and most complete collection of mercantile documents of the Middle Ages, gave rise to research work that culminated in 1928 with an important work by Enrico Bensa, which afforded a general view of the Merchant of Prato himself and of the multifarious activities of his Companies. Today, two initiatives are being carried out for utilising the Datini Archives and making them more widely known. First is the publication of the documents edited along modern lines, in a collection of 30 volumes that will facilitate their consultation and use. Second is a special exhibition of the Archives now being arranged at Prato, which will call the attention of the public to the importance and interest of this cultural patrimony. The present article gives account of both of these undertakings, framed in a brief comprehensive account of Datini himself, showing the significance of his papers and of the studies devoted to them. Abstract

La literatura empírica ha constatado una asociación positiva entre el nivel de psicopatía y la presencia de intentos de suicidio. Esta asociación se centra en los aspectos de impulsividad y desinhibición de la psicopatía, mientras que suicidio y pobreza emocional se muestran independientes. Los datos sobre la asociación entre suicidio y psicopatía en personas con enfermedad mental no son concluyentes. Este trabajo explora la relación entre distintas medidas de la personalidad antisocial y la presencia de intentos de suicidio y episodios de autolesión deliberada en una muestra de internos de un Hospital Psiquiátrico Penitenciario. Los resultados apoyan la asociación entre aspectos desinhibidos de personalidad y suicidio también en esta población. Empirical literature has yielded a positive association between psychopathy levels and suicide attempts. This association is centred around impulsivity and disinhibitory facets of psychopathy, whereas suicide and emotional poverty remain independent. Evidence about the relation between suicide and psychopathy in mentally disordered offenders is not conclusive. The present work explores the relation between several measures of antisocial personality, suicide attempt and deliberate self mutilation in a sample of inmates from a forensic psychiatric hospital. Results support the association between disinhibitory aspects of personality and suicide in this population.

Abstract

Rationale Intravenous (i.v.) thrombolysis with recombinant tissue-Plasminogen Activator (rt-PA) (alteplase) within three hours from symptom onset is the only approved treatment of pharmacological revascularization in acute ischaemic stroke. However, the current license limits the use of rt-PA to patients aged ≤80 years due to the lack of evidence of safety and efficacy of this treatment in the elderly from randomized clinical trials. This article describes the design of the Thrombolysis in Elderly Stroke Patients in Italy (TESPI) trial planned to fill the lack of controlled data on i.v. thrombolysis in this age category of stroke patients. Aims To collect efficacy and safety data on i.v. alteplase (rt-PA) in patients aged more than 80 years, to demonstrate that the treatment of these patients within three hours of symptoms onset of an acute ischaemic stroke with i.v. rt-PA, compared to patients receiving standard treatment (according to the national guidelines), will result in an improved clinical outcome with a favourable benefit/risk ratio. Design TESPI is a prospective, multicenter, national, open-label, controlled (non-treated group as control), randomized, parallel group trial with blinded evaluation of outcome in patients older than 80 years treated with i.v. rt-PA within three hours after ischaemic stroke onset. The randomization procedure assigns patients to the treatment group with IV alteplase (0.9 mg/kg of body weight) or to standard treatment group with a 1 : 1 basis. A three-month follow-up, when applicable, is performed by a blind assessor. Six hundred patients will be enrolled (300 patients per arm) The study period has been planned to be of three years. Study Outcomes The primary efficacy end-point is the disability at day 90, dichotomized as a favourable outcome (modified Rankin Scale 0–2) or unfavourable outcome (modified Rankin Scale 3–6). The main primary safety end-point is symptomatic intracerebral haemorrhage defined as any hemorrhage at the 22–36 h post-treatment scan combined with neurological deterioration leading to an increase of one or more points at the National Institutes of Health Stroke Scale. The TESPI trial, with the protocol number FARM65KNKY, is registered in the European Union Drug Regulating Authorities Clinical Trials database with the number 2007-006177-88 and in the Stroke Trials Registry of the Washington University Internet Stroke Center.

Abstract

El siguiente estudio tiene por objetivo identificar un tipo de conflicto cognitivo, los dilemas implicativos (DI), en pacientes con Trastorno de Ansiedad (TA). El concepto de DI se refiere a las implicaciones positivas y negativas que tiene el síntoma para la persona que lo sufre. La alternativa deseada, es decir, la dirección en la que el cliente quiere ir, tiene implicaciones negativas, por lo cual se resiste al cambio; pudiendo acarrear consecuencias negativas para su sentido de identidad. Desde un enfoque constructivista y mediante la Técnica de la Rejilla de Kelly, se pretende comparar la presencia y número de dilemas en una muestra de 45 pacientes y un grupo de 39 controles. Los resultados evidencian una diferencia significativa entre la presencia de dilemas en los pacientes ansiosos (49%) y el grupo control (25%). Si los DI implican conflicto psicológico y sufrimiento, la ansiedad podría ser una expresión sintomática del conflicto y de su intensidad. Ello plantea la necesidad de tratar a los pacientes con TA tanto sintomáticamente como en sus conflictos cognitivos. Estos resultados son particularmente relevantes al planificar y desarrollaruna intervención terapéutica que debería ser focalizada, eficiente y adecuada a las necesidades del paciente y a sus estructuras cognitivas.
