Abstract

BackgroundThis research prioritization aimed to identify major research gaps in maternal, newborn, child and adolescent health (MNCAH) to help mitigate the direct and indirect effects of the COVID-19 pandemic.MethodsWe adapted the Child Health and Nutrition Research Initiative methodology. We defined scope, domains, themes and scoring criteria. We approached diverse global experts via email to submit their research ideas in MNCAH and MNCAH-related cross-cutting/health systems area. We curated the research ideas as research questions (RQs) and sent them to the consenting experts for scoring via the online link. For each RQ, the research priority score (RPS) was calculated as an average of individual criterion scores and ranked based on RPS in each area.ResultsWe identified top-ranked 10 RQs in each maternal, newborn, and child and adolescent health and 5 in the cross-cutting/health systems area. In maternal health, indirect effects on care, measures to improve care, health risks and outcomes, and preventing and managing SARS-CoV-2 infection/COVID-19 disease were priority RQs. In newborn health, clinical characterization and managing SARS-CoV-2 infection/COVID-19 disease, mode of transmission and interventions to prevent transmission were the focus. For child and adolescent health, top-ranked RQs were indirect effects on care, clinical status and outcomes, interventions to protect against SARS-CoV-2 infection/COVID-19 disease, and educational institute-related RQs. The cross-cutting RQs were the effects of the pandemic on availability, access, care-seeking and utilization of MNCAH services and potential solutions.ConclusionsWe call on partners, including governments, non-governmental organizations, research institutes, and donors, to address this urgent research agenda.

Abstract

BACKGROUND: Diagnostic autopsy has been in use for long to determine the cause of death. Since recently however ‘Minimally Invasive Tissue Sampling’ (MITS) is introduced to determine definitive cause of neonatal death. This study describes locally established facilitators to introduce MITS procedures to determine cause of neonatal death in Ethiopia. METHODS: Exploratory study was conducted in Butajira community where twenty-two key informants representing community opinion leaders’ health care workers, five in depth interviews with parents who recently lost neonates and eight FGDs with community members were completed to generate evidences in line with the research question. Interviews and discussions were audio recorded, transcribed verbatim and analysed facilitated by open-code software. Thematic analysis was applied to identify and interpret patterns of the evidences RESULTS: In Butajira, ANC and delivery in health facilities was found to have improved over the years. Yet, child death remains an outstanding problem. While different factors were identified to cause the death of a child, relatively few participants choose to accept newborn death as a natural occurrence or will of the creator. Majority of the study participants expressed interest to know definitive cause of death using MITS. Yet, awareness about MITS and how it works was unanimously desired. It was found that husbands and wives are key to authorize MITS procedure while community opinion leaders including religious leaders were identified as key to influence parental decisions for the procedure CONCLUSION: Building awareness of the community members and engagement of opinion leaders is critical to introduce MITS. Abstract

Background and objectivesThis article describes the development of the first training program in family medicine in Ethiopia that was launched on February 4, 2013, at Addis Ababa University (AAU). The postgraduate program will prepare highly trained doctors for all parts of the country who choose generalism for their lifelong career. The paper describes a series of strategies that were used from 2008 to 2013 to take the Ethiopian family medicine program from vision to reality. There is no single model for the development of family medicine in a country where it does not yet exist. In this case the strategies included Continuing Medical Education events, discussions with stakeholders, international collaboration, needs assessment, curriculum design, and faculty development. The article also reviews both the potential for a new program in family medicine to contribute to the country's health system plus the challenges that are expected in the early stages of establishing a new specialty. The challenges include the ambiguous roles of the family physician in the Ethiopian health care system, uncertainty about career opportunities, adaptation of the curriculum to address local needs, expansion of the training programs to produce larger numbers of family physicians, development of Ethiopian faculty who will be teachers of family medicine, and internal and external brain drain. Family physicians will need to maintain a respectful relationship with other specialist physicians as well as nonphysician primary care providers. The development of this AAU family medicine residency is an example of a successful inter- institutional relationship between local and international partners to create a sustainable, Ethiopian-led training program. Insights from this article may guide development of similar training programs.

Abstract

The National Immunization Program (NIP) was introduced in Ethiopia in 1980. The NIP has expanded the number of vaccines from six to more than 14 in 2023. However, decisions on new vaccine introduction and other vaccine-related matters were not systematically deliberated nationally. Thus, the need to establish a national body to deliberate on vaccine and vaccination matters, in addition to the global immunization advisory groups, has been emphasized in the last decade. This article presents the establishment and achievements of the Ethiopian NITAG. The ENITAG was established in 2016 and maintained its active role in providing recommendations for new vaccine introduction and improving the delivery of routine vaccines. The external assessment indicated the E-NITAG was highly functional and played a critical role in enhancing the vaccination practice in Ethiopia, especially during the COVID-19 pandemic. The absence of a dedicated secretariat staff was the major bottleneck to expanding the role of the E-NITAG beyond responding to MOH requests. The E-NITAG must be strengthened by establishing a secretariat that can eventually grow as an independent institution to address complex vaccine-related issues the NIP needs to address. This disproportionately significant difference might be due to delays in diagnosis, treatment initiation, lack of adequate supportive care, and treatment abandonment. We aimed to determine the impact of overall treatment delay on induction mortality of children with acute lymphoblastic leukemia treated at Tikur Anbessa specialized hospital (TASH).Methods: A cross-sectional study was conducted among children who were treated from 2016 to 2019. Children with Down syndrome and relapsed leukemia were excluded from this study.Results: A total of 166 children were included; most patients were males (71.7%). The mean age at diagnosis was 5.9 years. The median time interval from the onset of symptoms to the first TASH visit was 30 days and the median period from TASH’s first clinic visit to diagnosis was 11 days. The median time to initiate chemotherapy after diagnosis was 8 days. The total median time from the first onset of symptoms to chemotherapy initiation was 53.5 days. Induction mortality was 31.3%. High-risk ALL and patients with an overall delay between 30 and 90 days were more likely to experience induction mortality.Discussion: Patient and healthcare system delay is high compared to most studies done and a significant association has been noted with induction mortality. 42. Oral Health Knowledge, Practices and Attitude among Parents/Caretakers of Children with Heart Disease on Follow up at Tikur Anbessa Hospital

Abstract

Background: Infective endocarditis is one of the most dreaded complications of structural heart disease. Poor oral hygiene has been implicated in the predisposition to infective endocarditis. The objective of this study is to assess the knowledge and practice of oral hygiene among parents of children with heart disease.Method: A cross sectional study using examiner administered questionnaire was used to obtain information on the oral health knowledge and practice of parents/caregivers of children with heart disease attending the pediatric cardiology clinic at Tikur Anbessa specialized Hospital. The study was conducted from April 1 to August 15, 2014.Results: A total of 384 caretakers of children participated in the study. About two-third (255) of the caretakers were informed on the importance of oral hygiene. Even though majority of the participants 317(82.6%) were aware about the benefits of tooth brushing, only 108(28.1%) reported using tooth brush and paste. The reason for not brushing teeth among the majority of caretakers (198, 71.7%) was that they did not think it was necessary. Lack of availability was mentioned by 53(19.2%) caretakers and unaffordability was mentioned by15 (5.4%).Caretakers’ knowledge on dental carries was also unsatisfactory. The role of the caretakers in the supervision of their children’s oral hygiene was also poor with only 19(4.9%) caretakers reportedly brushing their children’s teeth and 126(32.8%) watching and advising while the rest reportedly never cared, 89 (23.2%), or only gave advice but never watched, 150 (39.1%).Conclusion: This study has shown that the knowledge, attitude and practice of caretakers of children with heart disease on dental care to be poor. Many participants were not informed on the importance of oral hygiene on the prevention of infective endocarditis. Thus educating caretakers on the importance of regular tooth brushing habit, oral hygiene and regular preventive dental visit to reduce the risk of complication of the cardiac condition is recommended.
