ObjectiveAn instrument that facilitates the advancement of hearing healthcare delivery from a biomedical model to a biopsychosocial one that underpins the International Classification of Functioning, Disability, and Health framework (ICF) brief and comprehensive Core Sets for Hearing Loss (CSHL) is currently unavailable. The objective is to describe the process of developing and validating a new questionnaire named the HEAR-COMMAND Tool created by transferring the ICF CSHL into a theory-supported, practically manageable concept.DesignA team from Germany, the USA, the Netherlands, and Egypt collaborated on development. The following ICF domains were considered; “Body Functions” (BF), “Activities and Participation” (AP), and “Environmental Factors” (EF). The development yielded English, German, and Arabic versions. A pilot validation study with a total of 109 respondents across three countries, Germany, Egypt, and the USA was conducted to revise the item terminology according to the feedback provided by the respondents.ResultsThe questionnaire included a total of 120 items. Ninety items were designed to collect information on the functioning and 30 items inquiring about demographic information, hearing status, and Personal Factors. Except for the “Body Structures” (BS) domain, all the categories of the brief ICF CSHL were covered (a total of 85% of the categories). Moreover, the items covered 44% of the comprehensive ICF CSHL categories including 73% of BF, 55% of AP, and 27% of EF domains. Overall, the terminology of 24 ICF-based items was revised based on the qualitative analysis of the respondents' feedback to further clarify the items that were found tod be unclear or misleading. The tool highlighted the broad connection of HL with bodily health and contextual factors.ConclusionsThe HEAR-COMMAND Tool was developed based on the ICF CSHL and from multinational experts' and patients' perspectives with the aim to improve the execution of audiological services, treatment, and rehabilitation for adult patients with HL. Additional validation of the tool is ongoing. The next step would be to pair the tool with BS categories since it was excluded from the tool and determine its effectiveness in guiding hearing health care practitioners to holistically classify categories influencing hearing, communication, and conversation disability.

Loudness complaints are still very common among hearing-aid users. Therefore, loudness is an important issue that needs to be addressed in hearing aid research to improve and optimize hearing aid fitting. Empirical data show that only 2/3 of the hearing-aid users are satisfied with the comfort of loud sounds. The topic itself is quite challenging, as loudness is a subjective measure and it can be perceived differently depending on the context and conditions. To extend the research between laboratory and field presentation, we collected the ratings of loudness and annoyance of four different land vehicles (car, van, motorcycle and street sweeper) with different speeds and behaviours in normal-hearing and hearing-impaired listeners. In this work we present and analyse the ratings of 15 normal-hearing participants and compare their average field ratings to the calculated loudness with loudness models. Furthermore, possible future immersive audiovisual experiments for lab vs reality loudness comparisons are presented.

Diese Studie befasste sich mit der Replikation von zwei alltäglichen Kommunikationsbedingungen. Eine Stadtstraße und eine Unterhaltung in einer Cafeteria wurden im Labor als virtuelle audiovisuelle Umgebungen simuliert und für die Studie verwendet. Verglichen wurde die Erfahrung der Probanden in dieser audiovisuellen Laborumgebung mit entsprechenden realen Alltagsumgebungen. Die Erfahrungen wurden anhand von Fragebögen und Interviews bewertet, die relevante Faktoren, insbesondere die generelle Präsenz, räumliche Präsenz, Einbindung und Realität, berücksichtigten. Erhoben wurden die Daten in zwei Situationen (Straßenverkehr und Mensa) mit N = 21 Probanden im Alter von 51-72 Jahren in Bezug auf zwei Hörgerätemodi (HG) (unversorgt und omni-direktionaler HG-Mikrofonmodus) mit sieben erfahrenen HG-Nutzern (EXPU), sieben erstmaligen HG-Nutzern mit Hörminderung (FTU) und mit sieben gleichaltrigen Probanden mit normalem Hörvermögen (NH). Der Präsenzfragebogen und das Interviewmaterial zeigten, dass die virtuell akustische Replikation im Labor als plausibel im Vergleich zur realen Erfahrung bewertet wurde, während die visuelle Simulation in Bezug auf visuelle Details und Bewegung als weniger plausibel erlebt wurde. This study addressed the replication of two everyday life communication conditions: an urban street and a conversation in a cafeteria, in the laboratory using virtual audiovisual environments, and compared subject experience in this audiovisual laboratory environment with the corresponding real-life condition. Experiences were assessed using questionnaires and interviews addressing relevant factors, in particular the general presence, spatial presence, involvement and realism. Data were collected with N = 21 users (51 to 72 yrs.) in two situations (street traffic and cafeteria), using two hearing aid (HA) modes (unaided and omnidirectional HA microphone mode) in seven experienced HA users (EXPU), in seven first-time HA users with hearing impairment (FTU), and in seven age-matched subjects with normal-hearing (NH). Both the questionnaire focusing on aspects of presence and the interview responses showed that the virtual acoustic replication in the laboratory condition was evaluated as being plausible compared to the real-life experience, whereas subjects found the visual simulation to be less plausible in terms of visual detail and the presentation of movement.

Die Nutzenbewertung von Hörsystemen erfolgt in der Regel durch die Bestimmung klinisch relevanter Parameter, wie z. B. des Sprachverstehens im Störgeräusch. Neben diesen Parametern zur Wirksamkeit ist die Bewertung des alltäglichen Nutzens mittels Fragebögen zur gesundheitsbezogenen Lebensqualität (HrQoL) von zunehmender Relevanz und spiegelt die Wirksamkeit im Zeitablauf wieder. Um den Nutzen von Hörsystemen im Verlauf beurteilen zu können, müssen die Fragebögen in der Lage sein, Veränderungen abzubilden. Die Änderungssensitivität wurde vergleichsweise in einer Reihe von generischen und hörspezifischen Fragebögen mit bimodal versorgten Patienten (N = 40, Durchschnittsalter = 64,7 Jahre) getestet, die von einer bilateralen Hörgeräteversorgung (HG) auf eine bimodale Versorgung (HG und Cochlea-Implantat (CI)) gewechselt sind. Die Probanden füllten die Fragebogenbatterie zu vier Messzeitpunkten aus (vor der Implantation und drei bis neun Monate nach der Implantation). Einige der getesteten Skalen zeigten sich als änderungssensitiv, sodass sie für die evidenzbasierte Evaluation im Bereich der CI und bimodalen Rehabilitation verwendet werden können. Darauf aufbauend erfolgte eine Auswahl von krankheitsspezifischen (z. B. HHIE [Sozialskala], SSQ-12) sowie generischen Skalen (z. B. GHSI [gen. Subskala]) mit mittleren bis guten Wirkungsgraden für eine reduzierte und ökonomisch zu handhabende Testbatterie. Diese Testbatterie scheint für prospektive Längsschnittstudien geeignet zu sein, um den Nutzen für die Patienten im Alltag zu bewerten und verschiedene regulatorische Vorgaben zu erfüllen. The benefit assessment of hearing systems is usually carried out by determining clinically relevant parameters, such as speech comprehension in noise. In addition to such parameters addressing efficacy, the assessment of everyday benefits by means of questionnaires on health-related quality of life (HrQoL) is of increasing relevance reflecting effectiveness over time. To assess the benefit of hearing systems over time, the questionnaires must be suitable to demonstrate sensitivity to change. Sensitivity to change was tested comparatively across a range of generic and hearing-specific questionnaires with N = 40 bimodal patients (average age M = 64.7 years), who switched from bilateral hearing aid (HA) provision to a bimodal supply (HA and cochlear implant (CI)). The subjects answered 166 items of a large questionnaire battery at four measuring time points (before implantation and three to nine months after implantation). For some of the questionnaire scales it was shown, that these were change sensitive and can be used for evidence-based evaluation in the field of CI and bimodal rehabilitation. Based on this, we selected disease-specific (e.g., HHIE [social scale], SSQ-12) as well as generic scales (e.g., GHSI [gen. subscale]) with moderate to good within effect sizes for a reduced and economic to handle test battery. This test battery seems to be suitable for prospective longitudinal studies to evaluate the patients’ benefit in everyday life to meet different regulatory recommendations.

Differences in understanding speech in noise among hearing-impaired individuals cannot be explained entirely by hearing thresholds alone, suggesting the contribution of other factors beyond standard auditory ones as derived from the audiogram. This paper reports two analyses addressing individual differences in the explanation of unaided speech-in-noise performance among n = 438 elderly hearing-impaired listeners (mean = 71.1 ± 5.8 years). The main analysis was designed to identify clinically relevant auditory and non-auditory measures for speech-in-noise prediction using auditory (audiogram, categorical loudness scaling) and cognitive tests (verbal-intelligence test, screening test of dementia), as well as questionnaires assessing various self-reported measures (health status, socio-economic status, and subjective hearing problems). Using stepwise linear regression analysis, 62% of the variance in unaided speech-in-noise performance was explained, with measures Pure-tone average (PTA), Age, and Verbal intelligence emerging as the three most important predictors. In the complementary analysis, those individuals with the same hearing loss profile were separated into hearing aid users (HAU) and non-users (NU), and were then compared regarding potential differences in the test measures and in explaining unaided speech-in-noise recognition. The groupwise comparisons revealed significant differences in auditory measures and self-reported subjective hearing problems, while no differences in the cognitive domain were found. Furthermore, groupwise regression analyses revealed that Verbal intelligence had a predictive value in both groups, whereas Age and PTA only emerged significant in the group of hearing aid NU.

This chapter provides background information for researchers who wish to become familiar with some basic medical and audiological aspects of hearing loss and the technology of hearing aids. It introduces (1) the disciplines involved in research on hearing loss, (2) the medical categories of hearing loss and their various effects on communication, (3) the different degrees of hearing loss as defined by different national and international organizations, (4) statistics on the preva- lence of hearing loss worldwide, (5) some technological aspects of hearing instruments, (6) sta- tistics on non-usage of hearing instruments worldwide, and (7) barriers to using hearing aids. Since hearing loss is a worldwide pervasive phenomenon which is likely to increase even further in the future, the authors point out that an understanding of the non-use of hearing aids is crucial.

The five western countries represented in this volume share highly developed medical and technological provisions for hearing health, yet they differ in the compliance rates for hearing aid usage. A contributing factor may lie in the diversity of the national health care systems. We examine the similarities and differences in these five countries’ health care systems in order to find features which may contribute to (non-)use of hearing instruments. After a general overview of the commonalities, we compare legal provisions, medical indication for prescription of a hearing aid, accessing health care and diagnosing hearing impairment, selection and fitting of hearing aid, costs of hearing aids, and help to learn how to live with hearing loss and using a hearing aid. Finally, we reflect on how aspects of the health care systems may contribute to hearing aid use.
