Abstract

Background: Young adult cancer survivors frequently suffer from fatigue, depressive symptoms, and impaired quality of life. Ocean-sailing with its specific challenges could have beneficial effects on the physical and mental well-being of this patient group.Methods: 28 cancer patients (18 to 40 years) embarked on an 8 to 13-day ocean sailing trip requiring physical exertion. Quality of life (EORTC QLQ-C30), depressive symptoms (PHQ-9), fatigue (FSS), social support (ESSI), and physical activity (BSA) were assessed before the sailing trip and 1, 3 and 6 months thereafter.Results: Depressive symptoms improved from baseline (median 8, IQR [4.3; 10]) to 1 month after sailing (median 5.5, IQR [3.3; 7.5]; p < 0.01), as did the EORTC QLQ-C30 subscales global health status (from 62.5 [35.4; 66.7] to 75 [52.1; 83.3]; p < 0.01), emotional function (from 50 [25; 66.7] to 75 [50; 87.5]; p < 0.001), and appetite loss (from 0 [0; 33.3] to 0 [0; 0]; p < 0.05). Global health status was still improved after 3 months (66.7 [58.3; 83.3]; p < 0.05). Leisure time physical activity (min/week) decreased from baseline (346.3 [202.4; 653.1]) to 6 months (220.6 [130.6; 421.9]; p < 0.05).Conclusion: Ocean sailing was associated with short-term improvements in quality of life, emotional function, and depressive symptoms, while leisure time physical activity decreased. Abstract

The paper presents the low temperature expansion of the 2D Ising model in the presence of the magnetic field in powers of $x=\exp(-J/(kT))$ and $z=\exp(B/(kT))$ with full polynomials in $z$ up to $x^{88}$ and full polynomials in $x^4$ up to $z^{-60}$, in the latter case the polynomials are explicitly given. The new result presented in the paper is an expansion not in inverse powers of $z$ but in $(z^2+x^8)^{-k}$ where the subsequent coefficients (polynomials in $x^4$) turn out to be divisible by increasing powers of $(1-x^4)$. The paper describes both the analytic expansions of the partition function and the efficient combinatorial methods to get the coefficients of the expansion.

Abstract

Kidney response to acute and mechanically induced variation in ventilation associated with different levels of PEEP has not been investigated. We aimed to quantify the effect of ventilatory settings on renal acid–base compensation. Forty‐one pigs undergoing hypo‐ (<0.2 Lkg−1 min−1, PEEP 25 cmH2O), intermediate (0.2–0.4 Lkg−1 min−1 with either PEEP 5 or 25 cmH2O), or hyper‐ventilation (>0.4 Lkg−1 min−1, PEEP 5 cmH2O) for 48 h were retrospectively included. The decrease in pH paralleled the decrease in plasma strong ion difference (SID) in hyper‐ and intermediately ventilated groups with lower PEEP. In contrast, the plasma SID remained nearly constant in hypo‐ and intermediately ventilated groups with higher PEEP. Changes in plasma chloride concentration accounted for the changes in plasma SID (conditional R2 = 0.86). The plasma SID changes were paralleled by mirror changes in urinary SID. Higher PEEP (25 cmH2O), compared to lower PEEP (5 cmH2O) dampened or abolished the renal compensation through its effect on hemodynamics (higher central venous and mean pulmonary pressures), irrespective of minute ventilation. During mechanical ventilation, the compensatory renal response to respiratory derangement is immediate and progressive but can be dampened by high PEEP levels. [ABSTRACT FROM AUTHOR]

Abstract

We argue that if particularly powerful electromagnetic afterglows of the gravitational waves bursts will be observed in the future, this could be used as a strong observational support for some suggested quantum alternatives for black holes (e.g., firewalls and gravastars). A universal absence of powerful afterglows should be taken as a suggestive argument against such hypothetical quantum-gravity objects.

Abstract

Monitoring programs at sub-national and national scales lack coordination, harmonization, and systematic review and analysis at continental and global scales, and thus fail to adequately assess and evaluate drivers of biodiversity and ecosystem degradation and loss at large spatial scales. Here we review the state of the art, gaps and challenges in the freshwater assessment programs for both the biological condition (bioassessment) and biodiversity monitoring of freshwater ecosystems using the benthic macroinvertebrate community. To assess the existence of nationally- and regionally- (sub-nationally-) accepted freshwater benthic macroinvertebrate protocols that are put in practice/used in each country, we conducted a survey from November 2022 to May 2023. Responses from 110 respondents based in 67 countries were received. Although the responses varied in their consistency, the responses clearly demonstrated a lack of biodiversity monitoring being done at both national and sub-national levels for lakes, rivers and artificial waterbodies. Programs for bioassessment were more widespread, and in some cases even harmonized among several countries. We identified 20 gaps and challenges, which we classed into five major categories, these being (a) field sampling, (b) sample processing and identification, (c) metrics and indices, (d) assessment, and (e) other gaps and challenges. Above all, we identify the lack of harmonization as one of the most important gaps, hindering efficient collaboration and communication. We identify the IUCN SSC Global Freshwater Macroinvertebrate Sampling Protocols Task Force (GLOSAM) as a means to address the lack of globally-harmonized biodiversity monitoring and biological assessment protocols.

Abstract

Physics beyond the Standard Model predicts the possible existence of new particles that can be searched at the low energy frontier in the sub-eV range. The OSQAR photon regeneration experiment looks for "Light Shining through a Wall" from the quantum oscillation of optical photons into "Weakly Interacting Sub-eV Particles", such as axion or Axion-Like Particles (ALPs), in a 9 T transverse magnetic field over the unprecedented length of $2 \times 14.3$ m. In 2014, this experiment has been run with an outstanding sensitivity, using an 18.5 W continuous wave laser emitting in the green at the single wavelength of 532 nm. No regenerated photons have been detected after the wall, pushing the limits for the existence of axions and ALPs down to an unprecedented level for such a type of laboratory experiment. The di-photon couplings of possible pseudo-scalar and scalar ALPs can be constrained in the nearly massless limit to be less than $3.5\cdot 10^{-8}$ GeV$^{-1}$ and $3.2\cdot 10^{-8}$ GeV$^{-1}$, respectively, at 95% Confidence Level., Comment: 6 pages, 6 figures

Abstract

The OSQAR photon regeneration experiment searches for pseudoscalar and scalar axion-like particles by the method of "Light Shining Through a Wall", based on the assumption that these weakly interacting sub-eV particles couple to two photons to give rise to quantum oscillations with optical photons in strong magnetic field. No excess of events has been observed, which constrains the di-photon coupling strength of both pseudoscalar and scalar particles down to $5.7 \cdot 10^{-8}$ GeV$^{-1}$ in the massless limit. Abstract

Recent theoretical and experimental studies highlight the possibility of new fundamental particle physics beyond the Standard Model that can be probed by sub-eV energy experiments. The OSQAR photon regeneration experiment looks for "Light Shining through a Wall" (LSW) from the quantum oscillation of optical photons into "Weakly Interacting Sub-eV Particles" (WISPs), like axion or axion-like particles (ALPs), in a 9 T transverse magnetic field over the unprecedented length of $2 \times 14.3$ m. No excess of events has been detected over the background. The di-photon couplings of possible new light scalar and pseudo-scalar particles can be constrained in the massless limit to be less than $8.0\times10^{-8}$ GeV$^{-1}$. These results are very close to the most stringent laboratory constraints obtained for the coupling of ALPs to two photons. We combine old ideas about exact renormalization-group-flow (RGF) equations with the Vilkovisky-De Witt (VDW) approach to reparametrization invariant effective actions and arrive at a new, exact, gauge-invariant RGF equation. The price to be paid for such a result is that both the action and the RGF equation depend explicitly upon the base point (in field space) needed for the VDW construction. Abstract

We consider the higher order gravity with dilaton and with the leading string theory corrections taken into account. The domain wall type solutions are investigated for arbitrary number of space-time dimensions. The explicit formulae for the fixed points and asymptotic behavior of generic solutions are given. We analyze and classify solutions with finite effective gravitational constant. There is a class of such solutions which have no singularities. We discuss in detail the relation between fine tuning and self tuning and clarify in which sense our solutions are fine-tuning free. Abstract

Within two specific string cosmology scenarios --differing in the way the pre- and post-big bang phases are joined-- we compute the size and spectral slope of various types of cosmologically amplified quantum fluctuations that arise in generic compactifications of heterotic string theory. By further imposing that these perturbations become the dominant source of energy at the onset of the radiation era, we obtain physical bounds on the background's moduli, and discuss the conditions under which both a (quasi-) scale-invariant spectrum of axionic perturbations and sufficiently large seeds for the galactic magnetic fields are generated. Abstract

We generalize previous work on inhomogeneous pre-big bang cosmology by including the effect of non-trivial moduli and antisymmetric-tensor/axion fields. The general quasi-homogeneous asymptotic solution---as one approaches the big bang singularity from perturbative initial data---is given and its range of validity is discussed, allowing us to give a general quantitative estimate of the amount of inflation obtained during the perturbative pre-big bang era. The question of determining early-time ``attractors'' for generic pre-big bang cosmologies is also addressed, and a motivated conjecture is advanced. We discuss the temporal variation of the equation of state of a classical string network, evolving in a background in which the Hubble radius 1/H shrinks to a minimum and then re-expands to infinity. Matter is coupled to three-dimensional gravity such that the topological phase is allowed and the (anti-) de Sitter or Poincar\'e symmetry remains intact. Spontaneous symmetry breaking to the Lorentz group occurs if a scalar field is included. This Higgs field can then be used to couple matter so that the familiar form of the matter coupling is established in the broken phase. Abstract

Background: Ventilator-induced lung injury (VILI) via respiratory mechanics is deeply interwoven with hemodynamic, kidney and fluid/electrolyte changes. We aimed to assess the role of positive fluid balance in the framework of ventilation-induced lung injury. Methods: Post-hoc analysis of seventy-eight pigs invasively ventilated for 48 h with mechanical power ranging from 18 to 137 J/min and divided into two groups: high vs. low pleural pressure (10.0 ± 2.8 vs. 4.4 ± 1.5 cmH2O; p < 0.01). Respiratory mechanics, hemodynamics, fluid, sodium and osmotic balances, were assessed at 0, 6, 12, 24, 48 h. Sodium distribution between intracellular, extracellular and non-osmotic sodium storage compartments was estimated assuming osmotic equilibrium. Lung weight, wet-to-dry ratios of lung, kidney, liver, bowel and muscle were measured at the end of the experiment. Results: High pleural pressure group had significant higher cardiac output (2.96 ± 0.92 vs. 3.41 ± 1.68 L/min; p < 0.01), use of norepinephrine/epinephrine (1.76 ± 3.31 vs. 5.79 ± 9.69 mcg/kg; p < 0.01) and total fluid infusions (3.06 ± 2.32 vs. 4.04 ± 3.04 L; p < 0.01). This hemodynamic status was associated with significantly increased sodium and fluid retention (at 48 h, respectively, 601.3 ± 334.7 vs. 1073.2 ± 525.9 mmol, p < 0.01; and 2.99 ± 2.54 vs. 6.66 ± 3.87 L, p < 0.01). Ten percent of the infused sodium was stored in an osmotically inactive compartment. Increasing fluid and sodium retention was positively associated with lung-weight (R2 = 0.43, p < 0.01; R2 = 0.48, p < 0.01) and with wet-to-dry ratio of the lungs (R2 = 0.14, p < 0.01; R2 = 0.18, p < 0.01) and kidneys (R2 = 0.11, p = 0.02; R2 = 0.12, p = 0.01). Conclusion: Increased mechanical power and pleural pressures dictated an increase in hemodynamic support resulting in proportionally increased sodium and fluid retention and pulmonary edema.

Abstract

The Australian Community Climate and Earth System Simulator (ACCESS) has contributed to the World Climate Research Programme’s Coupled Model Intercomparison Project Phase 6 (CMIP6) using two fully coupled model versions (ACCESS-CM2 and ACCESS-ESM1.5) and two ocean–sea-ice model versions (1° and 0.25° resolution versions of ACCESS-OM2). The fully coupled models differ primarily in the configuration and version of their atmosphere components (including the aerosol scheme), with smaller differences in their sea-ice and land model versions. Additionally, ACCESS-ESM1.5 includes biogeochemistry in the land and ocean components and can be run with an interactive carbon cycle. CMIP6 comprises core experiments and associated thematic Model Intercomparison Projects (MIPs). This paper provides an overview of the CMIP6 submission, including the methods used for the preparation of input forcing datasets and the post-processing of model output, along with a comprehensive list of experiments performed, detailing their initialisation, duration, ensemble number and computational cost. A small selection of model output is presented, focusing on idealised experiments and their variants at global scale. Differences in the climate simulation of the two coupled models are highlighted. ACCESS-CM2 produces a larger equilibrium climate sensitivity (4.7°C) than ACCESS-ESM1.5 (3.9°C), likely a result of updated atmospheric parameterisation in recent versions of the atmospheric component of ACCESS-CM2. The idealised experiments run with ACCESS-ESM1.5 show that land and ocean carbon fluxes respond to both changing atmospheric CO2 and to changing temperature. ACCESS data submitted to CMIP6 are available from the Earth System Grid Federation (https://doi.org/10.22033/ESGF/CMIP6.2281 and https://doi.org/10.22033/ESGF/CMIP6.2288). Abstract

Introduction: The implementation of interprofessional education (IPE) could be a potential approach to dealing with increasing complexity in health care. And thus, encouraging interprofessional collaborations to minimize errors in patient care. However, empirical evidence regarding the effectiveness of IPE is inconclusive. One reason for this is a lack of valid and reliable evaluation instruments. This study aims to illustrate the first steps of the development and validation of a German evaluation instrument for the measurement of interprofessional attitudes. Methods: To achieve high psychometric quality, we first selected relevant attitude dimensions and specified criteria for the wording of the items. The a priori developed factor structure was evaluated via factor analysis and the internal consistencies of the scales were analysed in a sample of medical students and nursing trainees, both participants of an IPE course (n=338). Stability was evaluated in an additional sample of nursing trainees (n=14). Results: The Factor analysis revealed three dimensions. Whereby, the two factors "Relevance of learning interprofessional communicational techniques" (German: Wichtigkeit Techniken interprofessioneller Kommunikation zu lernen) (alpha=.85) and "Doubts, dismissal and perceived barriers" (German: Zweifel, Ablehnung und wahrgenommene Barrieren) (alpha=.73) revealed good to acceptable internal consistency. Third-factor "Attitude towards another profession" (German: Einstellung zur anderen Berufsgruppe) (alpha=.62) remained below a desired internal consistency of alpha>.70. Factors "Doubts, dismissal and perceived barriers", as well as "Attitude towards another profession" showed sufficient stability for pre-/post-measurements. Discussion: The Greifswald Questionnaire for the Measurement of Interprofessional Attitudes is the first version of a three-dimensional tool to evaluate IPE in German-speaking countries. Results showed insufficient item difficulty in the tested sa, Einleitung: In einem komplexer werdenden Gesundheitssystem sind Maßnahmen nötig, um Kompetenzen der interprofessionellen Zusammenarbeit zu fördern und somit das Fehlerpotential bei der Versorgung von Patienten zu minimieren. Eine Maßnahme, um dies zu erreichen, könnte die Integration von interprofessionellem Lernen (IPL) in die Ausbildung von Gesundheitsberufen sein. Es fehlen bisher allerdings belastbare Befunde, die die Wirksamkeit von IPL belegen. Eine Ursache hierfür ist die geringe Validität und Reliabilität verfügbarer Evaluationsinstrumente.Die vorliegende Publikation beschreibt erste Schritte der Validierung eines deutschsprachigen Evaluationsinstrumentes zur Messung interprofessioneller Einstellungen. Methoden: Um eine möglichste hohe Validität und Reliabilität zu erreichen, wurden zunächst geeignete Fragebogendimensionen ausgewählt sowie methodische Kriterien für die Itemkonstruktion festgelegt. An einer Stichprobe von Studierenden der Humanmedizin sowie Auszubildenden der Gesundheits- und Krankenpflege, die gemeinsam an einem IPL-Kurs teilnahmen, wurden die a priori festgelegten Skalen faktorenanalytisch überprüft und hinsichtlich ihrer internen Konsistenz analysiert (N=338). Die Stabilität der Dimensionen wurde an einer weiteren Stichprobe von 14 Auszubildenden der Gesundheits- und Krankenpflege untersucht. Ergebnisse: Es wurden drei Einstellungsdimensionen extrahiert. Die Dimensionen Wichtigkeit Techniken interprofessioneller Kommunikation zu lernen (alpha=.85) sowie Zweifel, Ablehnung und wahrgenommene Barrieren (alpha=.73) sind gut bis akzeptabel intern konsistent. Unterhalb eines angestrebten Niveaus von alpha>.7 blieb die Dimension Einstellung zur anderen Berufsgruppe (alpha=.62).
