Despite to its importance for retaining biodiversity and human health, urban wetlands have received much less attention than other wetland types in northern Iran. This study deals with the floristic characteristics of one of the largest urban wetlands in Central Mazandaran, Roshanabad wetland in Babol. All vascular plants were collected during two growing seasons of 2014 and 2015 and water sampling was performed seasonally (autumn 2014 to summer 2015). We encountered 102 plant species belonging to 80 genera and 39 families. The largest families in the studied area were Poaceae with (11.7%) followed by Cyperaceae and Asteraceae (9.8%) and Fabaceae and Polygonaceae (5.9%). Genera represented by the greatest number of species were Cyperus (7 sp.), Polygonum (4 sp.), Ranunculus (3 sp.) and Typha (3 sp.). Classification based on life form, indicated that the therophytes (47%) comprised the largest proportion of the plants in the studied area. From chorological point of view, the largest proportion of the flora belonged to the pluriregional elements (62.3%). Various habitats of the wetland are discussed. Moreover, 63 genera of fresh water algae, belonging to eight phyla were identified in the study area. Cholorophyta with 28 genera was the most abundant phylum followed by Bacillariophyta (19 genera), Cyanophyta (6 genera), Euglenophyta (4 genera), Chrysophyta, Dinophyta (2 genera), and Charophyta, Xanthophyta (each with one genus). Moreover, a comparison between the data as well as ratios of species/genera and genera/families collected from this wetland and from the other wetlands in north Iran has been provided. Roshanabad wetland had fewer aquatic species compared to some other wetlands in north of Iran, because of anthropogenic effects such as penetration of agricultural and urban sewage which has large quantities of nitrate and phosphate, and distribution of exotic aquatic plant, Azolla filiculoides. Moreover, Palmer Index of pollution shows that the wetland has high ratio of pollution in all seasons. This urban wetland site may be considered as a pilot site for the interaction of human effects and biodiversity pool. This is among the first attempts for restoration of such an important and sensitive ecosystem in north of Iran.

Based on the capacity of stem cells to migrate extensively and target invading glioma cells, the transplantation of stem cells as a cell-based delivery system may provide additional tools for the treatment of gliomas. In addition to the use of modified stem cells for the delivery of therapeutic agents, unmodified stem cells have been shown to have growth-suppressing effects on tumors in vitro and in vivo. This review outlines the probable factors involved in tumor tropism and tumor growth suppression, with a specific focus on the use of unmodified stem cells in the treatment of gliomas. Purpose: The aim of this study was to assess the capabilities of ChatGPT-4.0 and ChatGPT-3.5 for diagnosing corneal eye diseases based on case reports and compare with human experts., Methods: We randomly selected 20 cases of corneal diseases including corneal infections, dystrophies, and degenerations from a publicly accessible online database from the University of Iowa. We then input the text of each case description into ChatGPT-4.0 and ChatGPT-3.5 and asked for a provisional diagnosis. We finally evaluated the responses based on the correct diagnoses, compared them with the diagnoses made by 3 corneal specialists (human experts), and evaluated interobserver agreements., Results: The provisional diagnosis accuracy based on ChatGPT-4.0 was 85% (17 correct of 20 cases), whereas the accuracy of ChatGPT-3.5 was 60% (12 correct cases of 20). The accuracy of 3 corneal specialists compared with ChatGPT-4.0 and ChatGPT-3.5 was 100% (20 cases, P = 0.23, P = 0.0033), 90% (18 cases, P = 0.99, P = 0.6), and 90% (18 cases, P = 0.99, P = 0.6), respectively. The interobserver agreement between ChatGPT-4.0 and ChatGPT-3.5 was 65% (13 cases), whereas the interobserver agreement between ChatGPT-4.0 and 3 corneal specialists was 85% (17 cases), 80% (16 cases), and 75% (15 cases), respectively. However, the interobserver agreement between ChatGPT-3.5 and each of 3 corneal specialists was 60% (12 cases)., Conclusions: The accuracy of ChatGPT-4.0 in diagnosing patients with various corneal conditions was markedly improved than ChatGPT-3.5 and promising for potential clinical integration. A balanced approach that combines artificial intelligence-generated insights with clinical expertise holds a key role for unveiling its full potential in eye care., Competing Interests: W. M. Community-based participatory research (CBPR) is an effective approach for addressing health disparities by integrating diverse knowledge and expertise from both academic and community partners throughout the research process. However, more is needed to invest in the foundational infrastructure and resources that are necessary for building and maintaining lasting trusting research partnerships and supporting them to generate impactful CBPR-based research knowledge and solutions. Small CBPR Grants Program is a CBPR-seed-funding program that may be particularly helpful to minority-serving institutions' and universities' goal to invest in genuine community-engaged participatory research. Between 2016 and 2019, the Morgan State University Prevention Sciences Research Center, in collaboration with other community and academic organizations, provided 14 small CBPR awards to new partnerships, and evaluated the success and challenges of the program over a period of three years. To achieve our goal, technical support and training were provided to these partnerships to help with their growth and success. The expected outcomes included trusting relationships and equitable partnerships, as well as publications, presentations, and new proposals and awards to work on mutually identified issues. The program's resulted in continued partnerships beyond the program (in most cases), a founded CBPR Center namely ASCEND, and several secured additional fundings. Keys to the program's success were supporting the formation of research partnerships through networking opportunities and information sessions, as well as providing small grants to incentivize the development of innovative concepts and projects. A learning network and local support group were also created to enhance productivity and the overall impact of each project.

Objectives: To compare the repeatability of Cone Location and Magnitude Index expanded (CLMI.X) parameters of 2 mm diameter zone of greatest corneal curvature (Cspot-Axi) and 1 mm diameter zone of thinnest pachmymetry (Spot-Pach) with the maximum single point keratometry (Kmax), 3 mm Zonal Kmax (Z-Kmax3), and thinnest single point pachymetry (TP) in keratoconus (KC)., Methods: In this Comparative repeatability study, data from 36 eyes of 36 normal individuals and 72 eyes of 72 KC patients (28 eyes with ≤50.0D and 44 eyes with >50.0D Z-Kmax3) were analyzed. For each enrolled eye, imaging was done 6 times (3 consecutive acquisitions with a half hour break). For each parameter, the within-subject standard deviation (Sw) was calculated from the data of the six exams., Results: In the normal group, Cspot-Axi-Sw was different from Kmax-Sw (p = 0.0004). Also, Z-Kmax3-Sw was different from Kmax-Sw (p = 0.0297). The difference between Cspot-Axi-Sw and Z-Kmax3-Sw was statistically significant (p = 0.0482). In the KC group, Sw were significantly different between Cspot-Axi and Kmax (p < 0.0001), and between Z-Kmax3 and Kmax (p < 0.0001). In the Z-Kmax3 ≤ 50.0D subgroup, Sw were different between Cspot-Axi and Kmax (p = 0.0002). In the Z-Kmax3 > 50.0D subgroup, Sw were different between Cspot-Axi and Kmax (p < 0.0001), and between Z-Kmax3 and Kmax (p < 0.0001). Sw differences between Spot-Pach and TP were not significant in any of the study groups (all P > 0.05)., Conclusions: In the diagnosis and follow up of KC, the zonal averages of Cspot-Axi and Z-Kmax3 are more reliable than the single point Kmax. Purpose: To evaluate the agreement between non-cycloplegic autorefraction (NCAR) and cycloplegic autorefraction (CAR) in an ethnically diverse population of preschool-aged children and the validity of the screening criteria used to refer for further evaluation., Methods: This study included data from 7,073 preschoolers who underwent NCAR and CAR, which enabled refractive error classification based on the American Association for Pediatric Ophthalmology and Strabismus (AAPOS) 2013 criteria. Right eye data of sphere and cylinder were used to compare NCAR to CAR via paired t-testing and vector analyses, and left eye data for an analysis on anisometropia. The sensitivity and specificity of screening referral criteria for refractive error were calculated., Results: Mean values of sphere differed between NCAR and CAR by 1.95 ± 1.45 D (p < .05) with 95% limits of agreement (LoA) of -0.94 to 4.85 D, with less discrepancy found in myopic eyes. The mean values of cylinder differed by -0.08 ± 0.43 D (p < .05) with 95% LoA of -0.93 to 0.77 D. Power vector results reflected a similar lack of agreement. The sensitivity and specificity of our screening referral criteria were, respectively, 66% and 84% for myopia, 66% and 98% for hyperopia, and 98% and 58% for astigmatism., Conclusion: NCAR is insufficient in preschoolers for spherical refractive error referrals. Level of agreements was lower for spherical (15.5% within 0.5D) and higher for cylindrical refractive errors (89.6%) compared to CAR. In the absence of cycloplegic examination, screening programs using NCAR should utilize low referral thresholds for spherical refractive error.

Purpose: To evaluate the change in corneal stiffness after small incision lenticule extraction (SMILE), femtosecond laser-assisted in situ keratomileusis (FS-LASIK), and photorefractive keratectomy (PRK)., Methods: Age, gender, spherical equivalent, and central corneal thickness (CCT)-matched cases undergoing SMILE with a 120-µ cap, FS-LASIK with a 110-µ flap, and PRK were enrolled. One-year change in the stress-strain index, stiffness parameter at first applanation, integrated inverse radius, deformation amplitude ratio at 2 mm, and deformation amplitude ratio at 1 mm were compared between the surgical groups by linear mixed-effect models., Results: Within each surgical group, 120 eyes completed 1 year of follow-up. The residual stromal bed (RSB) thickness and (RSB/CCTpostop) were 348.1 ± 35.0 (0.74), 375.4 ± 31.0 (0.77) and 426.7 ± 2 µm (0.88) after SMILE, FS-LASIK, and PRK, respectively. The 1-year change in all biomechanical indices was significant, except the stress-strain index with PRK (P = 0.884). The change in all indices with SMILE were significantly greater than with FS-LASIK and with PRK (all P < 0.01), except the deformation amplitude ratio at 1 mm change between SMILE and FS-LASIK (P = 0.075). The changes in all indices with FS-LASIK were significantly greater than with PRK (all P < 0.05)., Conclusions: Although SMILE preserves the greatest amount of anterior cornea with a cap thickness of 120 µ, this also produces the smallest RSB and the greatest decrease in stiffness. Thus, the RSB is shown to be the predominant determinant of stiffness decreases, rather than the preserved anterior cornea. We recommend using a thinner cap to achieve a thicker RSB and a lesser decrease in the corneal stiffness in the SMILE procedure., Translational Relevance: After refractive surgery, RSB is predominant determinant of stiffness decreases, rather than the preserved anterior cornea.

Increasing the participation of students of African descent and other minoritized populations in the scientific workforce is imperative in generating a more equitable biomedical research infrastructure and increasing national research creativity and productivity. Undergraduate research training programs have shown to be essential tools in retaining underrepresented minority (URM) students in the sciences and attracting them into STEM and biomedical careers. This paper describes an innovative approach to harness students' entrepreneurial desire for autonomy and creativity in a Summer Research Institute (SRI) that has served as an entry point into a multiyear, National Institutes of Health Building Infrastructure Leading to Diversity (NIH BUILD)-funded research training program. The SRI was designed as an 8-week, student-centered and course-based research model in which students select their own research topics. We test here the effects of SRI training on students' science self-efficacy and science identity, along with several other constructs often associated with academic outcomes in the sciences. The data shown here comprise analysis of four different training cohorts throughout four subsequent summers. We show significant gains in students' science self-efficacy and science identity at the conclusion of SRI training, as well as academic adjustment and sense of belonging. SRI participants also displayed substantially improved retention in their science majors and graduation rates.

Introduction: To investigate the long-term results of laser in situ keratomileusis (LASIK) in patients with one or more topographic indices outside the suggested range preoperatively., Methods: Patients who had conventional or femtosecond laser-assisted LASIK for myopia correction between 2011 and 2015, and had at least one preoperative corneal topographic index outside the suggested range were contacted for a follow-up examination. Ranges were based on the cutoffs suggested for subclinical keratoconus: thinnest pachymetry (TP) < 497.50 µm, maximum keratometry (Kmax) > 47.20 D, maximum 8 mm best-fit-sphere anterior elevation (MaxAE) > 9.50 µm, maximum 8 mm best-fit-sphere posterior elevation (MaxPE) > 20.50 µm, and Belin/Ambrósio enhanced ectasia display-total deviation (BAD-D) > 1.60., Results: Two hundred thirty patients (377 eyes) were enrolled; their mean age at baseline and at follow-up was 30.78 ± 8.16 and 9.06 ± 1.91 years, respectively. Mean ± SD was 484.36 ± 11.49 for TP (n = 133), 48.17 ± 0.83 for Kmax (n = 133), 16.33 ± 5.39 for MaxAE (n = 35), 27.87 ± 7.86 for MaxPE (n = 34), and 1.97 ± 0.36 for BAD-D (n = 162). Post-LASIK and suspected ectasia was found in one (0.26%) and two (0.53%) eyes, respectively. In the remaining 374, the efficacy and safety indices were 0.96 ± 0.14 and 1.01 ± 0.08, respectively. Uncorrected distance visual acuity (UDVA) was not reduced in any eyes, and corrected distance visual acuity (CDVA) was reduced one line in 1.9% of the cases; 79.9% of the cases were emmetropic. The long-term changes in UDVA, CDVA, and spherical equivalent were not different between groups with one, two, or three out-of-range topographic indices (all P > 0.05)., Conclusion: A single out-of-range topographic index is not a strong predictor for postoperative complications, and one should consider the combination of topographic and clinical findings, or the pattern they create in tandem. Developing a scoring system that would take a combination of indices and topographic patterns may help improve the predictive accuracy of these indices., (© 2022. The Author(s).)

Much is told regarding the need for greater diversity in the biomedical research workforce in terms of race, ethnicity, and socioeconomic status. However, there are few evidence-based models that are tested and can have significant effects in this regard. Thus, there is a need for development and evaluation of innovative models that may help train a more diverse biomedical research workforce. In this study, we provided the rationale, conceptual model, and preliminary evaluation of a program called "A Student-Centered Entrepreneurship Development (ASCEND)". This training program was designed, implemented, and evaluated between 2017 and 2020 at Morgan State University, Baltimore, Maryland, United States. The program's conceptual model is based on four stages: Attraction and Inspiration, Ideation and Innovation, Research Implementation, and Career Growth. Results of the comparative survey between 50 students who participated in ASCEND and 86 non-member controls showed an increase in science identity, academic self-concept, science self-efficacy, and peer support. The only domain that did not show a larger increase in participants in our program compared to controls was social self-concept. In addition, a total of 59 students submitted 48 research concepts, and 16 undergraduate student projects were funded. Of participants in the Health Research Concepts Competition, 39 students graduated, and 13 were pursuing graduate programs in STEM fields at the time of evaluation. The number of research projects and trainees who started a graduate degree were also reported. The ASCEND training model fosters an entrepreneurial mindset among undergraduate students. Such a program might be effective in diversifying the biomedical research workforce. While this preliminary evaluation indicates the efficacy of the ASCEND model, there is a need for further long-term and multi-center evaluations with the trainees' research productivity and receipt of independent funding as outcomes.

Objective: To investigate the incidence of coronavirus disease 2019 (COVID-19) cases, hospitalizations and deaths in Iranians vaccinated with either AZD1222 Vaxzevria, CovIran® vaccine, SARS-CoV-2 Vaccine (Vero Cell), Inactivated (lnCoV) or Sputnik V., Methods: We enrolled individuals 18 years or older receiving their first COVID-19 vaccine dose between April 2021 and January 2022 in seven Iranian cities. Participants completed weekly follow-up surveys for 17 weeks (25 weeks for AZD1222) to report their COVID-19 status and hospitalization. We used Cox regression models to assess risk factors for contracting COVID-19, hospitalization and death., Findings: Of 89 783 participants enrolled, incidence rates per 1 000 000 person-days were: 528.2 (95% confidence interval, CI: 514.0-542.7) for contracting COVID-19; 55.8 (95% CI: 51.4-60.5) for hospitalization; and 4.1 (95% CI: 3.0-5.5) for death. Compared with SARS-CoV-2 Vaccine (Vero Cell), hazard ratios (HR) for contracting COVID-19 were: 0.70 (95% CI: 0.61-0.80) with AZD1222; 0.73 (95% CI: 0.62-0.86) with Sputnik V; and 0.73 (95% CI: 0.63-0.86) with CovIran®. For hospitalization and death, all vaccines provided similar protection 14 days after the second dose. History of COVID-19 protected against contracting COVID-19 again (HR: 0.76; 95% CI: 0.69-0.84). Diabetes and respiratory, cardiac and renal disease were associated with higher risks of contracting COVID-19 after vaccination., Conclusion: The rates of contracting COVID-19 after vaccination were relatively high. SARS-CoV-2 Vaccine (Vero Cell) provided lower protection against COVID-19 than other vaccines. People with comorbidities had higher risks of contracting COVID-19 and hospitalization and should be prioritized for preventive interventions., ((c) 2022 The authors; licensee World Health Organization.)

Purpose: To compare the long-term results of accelerated corneal cross-linking (CXL) (9 mW/cm 2 , 10 min) with standard CXL (3 mW/cm 2 , 30 min) in patients with Down syndrome who had keratoconus., Methods: In this contralateral randomized clinical trial, 27 patients with Down syndrome aged 15.78 ± 2.46 years (range: 10 to 19 years) were enrolled. CXL was performed using the KXL System (Avedro, Inc) under general anesthesia, and patients were followed up for 3 years. The main outcome measure was a change in average keratometry in the 3-mm zone around the steepest point (zonal Kmax-3mm). Secondary outcomes were changes in Corvis ST (Oculus Optikgeräte GmbH) biomechanical parameters and vision, refraction, and corneal tomography measurements., Results: Mean 3-year changes in zonal Kmax-3mm were not significantly different between the accelerated and standard groups (-0.06 ± 0.75 and -0.35 ± 0.94 diopters [D], respectively, P = .727). Despite the contralateral design of the study, based on most baseline biomechanical indices, corneas in the standard group were weaker before treatment. The standard group also showed significantly fewer 3-year changes in the stress-strain index (-0.11 ± 0.21 vs -0.30 ± 0.32), integrated radius (+0.99 ± 3.48 vs +3.14 ± 2.84), and deformation amplitude ratio-2mm (-1.38 ± 1.33 vs +0.30 ± 1.75) (all P < .0167). Corneal stiffness in the accelerated group was stable for 2 years, and the decline mainly occurred during the third year., Conclusions: In young patients with Down syndrome who had keratoconus, accelerated and standard CXL showed a similar flattening effect. Standard CXL is better able to maintain corneal stiffness in weaker corneas. With accelerated CXL, despite stable results for 2 years, there was decreased corneal stiffness in the third year. Longer follow-up periods are warranted to study the decreased efficacy on keratoconus progression. [ J Refract Surg . 2022;38(6):381-388.] .

Purpose: The purpose of this study was to investigate 3-year changes in keratoconus (KC) indices to determine the indicators of KC progression compared with age-related changes in children aged 6 to 12 years., Methods: In this report of the Shahroud Schoolchildren Eye Cohort Study, KC was diagnosed based on vision, refraction, tomography, and slitlamp examination findings. KC progression was defined as changes in refractive astigmatism or zonal maximum keratometry (Zonal Kmax-3 mm). Then, 3-year changes (Δ) in vision, refraction, and tomographic indices were compared between progressive KC and normal eyes. The best set of ∆parameters for distinguishing KC progression from age-related changes were determined using the area under curve (AUC)., Results: Eighteen KC eyes and 10,422 normal eyes were analyzed. All KC cases showed progression after 3 years. Δindices were statistically different between the 2 groups except corrected distance visual acuity, anterior radius of curvature, irregularity index, and KC percentage index. The best Δindices, in descending order of AUC value, were index of height decentration, Zonal Kmax-3 mm, refractive astigmatism, single point Kmax, and index of vertical asymmetry (all AUC > 0.9), followed by inferior-superior asymmetry, index of surface variance, minimum corneal thickness, index of height asymmetry, KC index, posterior radius of curvature, and Belin/Ambrósio total Deviation (AUC = 0.8-0.9)., Conclusions: According to our findings, changes in index of height decentration, Zonal Kmax-3 mm, refractive astigmatism, single point Kmax, and index of vertical asymmetry are the best indices for detecting KC progression in children younger than 12 years., Competing Interests: The authors have no conflicts of interest to disclose., (Copyright © 2021 Wolters Kluwer Health, Inc. Unilateral corneal indices and topography maps are routinely used in practice, however, although there is consensus that fellow-eye asymmetry can be clinically significant, symmetry studies are limited to local curvature and single-point thickness or elevation measures. To improve our current practices, there is a need to devise algorithms for generating symmetry colormaps, study and categorize their patterns, and develop reference ranges for new global discriminative indices for identifying abnormal corneas. In this work, we test the feasibility of using the fellow eye as the reference surface for studying elevation symmetry throughout the entire corneal surface using 9230 raw Pentacam files from a population-based cohort of 4613 middle-aged adults. The 140 × 140 matrix of anterior elevation data in these files were handled with Python to subtract matrices, create color-coded maps, and engineer features for machine learning. The most common pattern was a monochrome circle ("flat") denoting excellent mirror symmetry. Other discernible patterns were named "tilt", "cone", and "four-leaf". Clustering was done with different combinations of features and various algorithms using Waikato Environment for Knowledge Analysis (WEKA). Our proposed approach can identify cases that may appear normal in each eye individually but need further testing. This work will be enhanced by including data of posterior elevation, thickness, and common diagnostic indices.

In this study, corneal findings regarding keratoconus (KC) and early KC among mothers with Down syndrome children (MDS) and a group of age-at-delivery-matched mothers with normal children (MNC) were compared. KC was diagnosed based on the presence of a clinical sign and at least one abnormal tomographic or biomechanical criterion. Early KC was defined as having no clinical sign in the presence of at least one abnormal tomographic or biomechanical criterion. The normal subgroups in each group were compared in terms tomographic and biomechanical parameters. In MDS and MNC, the prevalence rates were 6.5% and 1.6% for KC (P = 0.047), and 30.9% and 14.3% for early KC (P = 0.014), respectively. Comparison between the two normal subgroups showed significant differences in mean index of height asymmetry, irregularity index, anterior asphericity, pentacam random forest index, corneal stiffness parameters at first applanation, deformation amplitude ratios, integrated radius-1 mm, highest concavity deflection amplitude, biomechanical corrected IOP, peak distance, and radius (all P < 0.05). This study showed that MDS are more likely to have KC and also to have thinner, steeper and softer corneas compared to MNC. In pediatric cohort studies, disclosing findings and providing treatment counseling to parents, legal guardians, or other authorized representatives is an ethical imperative. Keratoconus (KC) is a corneal degenerative disease that can progress to severe vision impairment and need for corneal transplantation. This report concerns the disclosure of results to the parents of children who were diagnosed with KC in the Shahroud Schoolchildren Eye Cohort Study and received essential treatment in a timely manner., (Copyright 2021 The Journal of Clinical Ethics. Objectives: To determine the prevalence of refractive errors and visual impairment in Down syndrome (DS) patients compared to normal controls., Materials and Methods: Cycloplegic refraction was tested in 213 DS patients and 184 normal age- and gender-matched controls using autorefraction followed by retinoscopy. Data from the worse eye of each case were used in the analyses., Results: In the DS and control groups, respectively, mean age was 17.2±4.8 and 17.2±4.4 years (p=0.993) and 53.0% and 49.5% were male (p=0.473). In the DS and control groups, respectively, mean spherical equivalent (SE) was -5.13±4.47 and -4.15±3.04 diopters (D) in myopics (p=0.050) and 2.47±1.64 and 2.36±2.04 D in hyperopics (p=0.482), mean cylinder error was -2.17±1.39 and -2.05±1.57 D (p=0.451), mean J0 was -0.03±0.89 and 0.12±0.76 D (p=0.086), and mean J45 was 0.11±1.02 and -0.13±1.03 D (p=0.024). The prevalence of oblique astigmatism was higher in the DS group (20.4% vs. 6.1%) while against-the-rule astigmatism was more prevalent in the control group (84.0% vs. 71.6%) (p<0.001). The prevalence of anisometropia was not significantly different between the groups (19.4% vs. 13.8%). Visual impairment was detected in 11.7% of the DS and 0.5% of the control group (p<0.001). The prevalence of amblyopia was 36.3% and 3.8% in the DS and control groups, respectively (p<0.001). Based on the multiple model, only absolute SE inversely correlated with age and differed between males and females (all p<0.05)., Conclusion: In DS patients, the prevalence rates of refractive errors, amblyopia, and visual impairment are higher than those in non-DS individuals, and emmetropization appears to be either defective or slow. Cylinder error is stable in this age range, but the rotation of astigmatism axis is different from normal samples.

Lapatinib (L) plus trastuzumab (T), with endocrine therapy for estrogen receptor (ER)+ tumors, but without chemotherapy, yielded meaningful response in HER2+ breast cancer (BC) neoadjuvant trials. The irreversible/pan-HER inhibitor neratinib (N) has proven more potent than L. However, the efficacy of N+T in comparison to pertuzumab (P) + T or L + T (without chemotherapy) remains less studied. To address this, mice bearing HER2+ BT474-AZ (ER+) cell and BCM-3963 patient-derived BC xenografts were randomized to vehicle, N, T, P, N+T, or P+T, with simultaneous estrogen deprivation for BT474-AZ. Time to tumor regression/progression and incidence/time to complete response (CR) were determined. Changes in key HER pathway and proliferative markers were assessed by immunohistochemistry and western blot of short-term-treated tumors. In the BT474-AZ model, while all N, P, T, N + T, and P + T treated tumors regressed, N + T-treated tumors regressed faster than P, T, and P + T. Further, N + T was superior to N and T alone in accelerating CR. In the BCM-3963 model, which was refractory to T, P, and P + T, while N and N + T yielded 100% CR, N + T accelerated the CR compared to N. Ki67, phosphorylated (p) AKT, pS6, and pERK levels were largely inhibited by N and N + T, but not by T, P, or P + T. Phosphorylated HER receptor levels were also markedly inhibited by N and N + T, but not by P + T or L + T. Our findings establish the efficacy of combining N with T and support clinical testing to investigate the efficacy of N + T with or without chemotherapy in the neoadjuvant setting for HER2+ BC.

Background: Despite studies about anxiety in the older adult, the prevalence of anxiety in this age group is not exactly clear, which may be due to the use of tools and criteria that were not born for this age group. One of the instruments designed to assess anxiety in the elderly is the Geriatric Anxiety Inventory (GAI). The aim of this research was to analyze the psychometric properties of the Persian version of the Geriatric Anxiety Inventory (GAI-PV) and its short form (GAI-PV-SF) in a sample of older adults in Iran. Methods: In this cross-sectional study, a sample of 150 community-dwelling and a psychogeriatric sample of 48 adults older than 60 years completed the GAI-PV and GAI-PV-SF, the anxiety sub-scale of the General Health Questionnaire (GHQ-28), the Geriatric Depression Scale (GDS-15), and the Structured Clinical Interview for DSM-IV (SCID-I). Different types of validity and reliability were evaluated for GAI-PV and GAI-PV-SF using SPSS and the LISREL software. Results: Both the GAI-PV and GAI-PV-SF exhibited excellent internal consistency (over 80 %) and desirable concurrent validity against GHQ-28 and GDS-15. The optimal cutpoint score to detect current generalized anxiety disorder (GAD) was 10/11 and 13/14 for GAI-PV in the community-dwelling and psychogeriatric samples, respectively, and 3/4 for GAI-PV-SF in both study samples. Good test-retest reliability (correlation coefficient: 0.96 and 0.88 for GAI-PV and GAI-PV-SF, respectively) and a single-factor structure were also demonstrated. Patients with chronic lymphocytic leukemia (CLL), a B-cell malignancy characterized by impaired humoral and cellular immunity, are at increased risk of developing cutaneous squamous cell carcinoma (cSCC). Human papilloma virus (HPV) is the most common sexually transmitted infection worldwide and it has been associated with various malignancies, including cSCC. Impaired cell-mediated immunity is considered a primary risk factor in HPV-induced cSCC. We examined cSCC lesions from CLL patients with consensus review and HPV genetic analysis to further characterize the relationship between HPV and prevalence of cutaneous malignancy in this population. Eleven patients with CLL contributed 35 cSCCs. Treatment with chemotherapy shortened the latency time to first cSCC. HPV was detected in 54% of the lesions. Among the HPV-positive cSCC lesions, 84% of the lesions contained alpha-genus HPV, 42% contained beta-genus HPV, and 26% of the lesions contained both genera. There was a significant association between HPV-containing lesions and peritumoral lymphocytic inflammation, suggesting this as a future area for further characterization. The majority of the lesions, including those with alpha-genus HPV, occurred in sun-exposed areas, such as the scalp and face. Purpose: Using the screening data collected through the University of California, Los Angeles Preschool Vision Program (UPVP) that provides free vision screening to children 2.5 to 6 years of age attending public preschools in five Supervisory Districts in Los Angeles County, this paper explores whether geographic location of where children attended preschool affects the vision screening results., Methods: 62,936 children were screened using a handheld device (Nikon Retinomax) between academic years 2012 and 2015. The criteria for failing the screening was set to sphere of ≤ -3.25 diopters (D), sphere ≥ +1.75 D, or cylinder of ≥1.50 D in either eye, or inter-ocular difference of >2.00 D in spherical error (>1.50D in case of antimetropia) or ≥1.00 D in cylinder error., Results: Higher fail rates were found among girls, Latino race, and those who primarily spoke Spanish at home. In addition, there was a geographical variance in screening outcomes among Supervisory Districts with District 1 having the highest fail rate and District 4 having the lowest fail rate., Conclusion: This paper provides preliminary evidence of geographic factors contributing to the development of refractive errors among preschoolers which may be used in future studies to address the unmet needs of children with vision impairments in Los Angeles County.

Purpose: To determine the total corneal refractive power in 1-8 mm corneal zones and the 8 mm Q-value in non-keratoconic patients with Down syndrome and normal subjects aged 10-30 years., Methods: Right eye data from 203 Down syndrome patients (mean ± standard deviation age: 17.0 ± 4.7 years) and 189 age- and gender-matched normal subjects (17.1 ± 4.5 years) were compared. Main extracted Pentacam indices were total corneal refractive power in steep and flat axes, and mean and difference (corneal astigmatism) total corneal refractive power in 1-8 mm zones., Results: Mean total corneal refractive power in 1-8 mm zones was 45.17-45.74 D and 42.91-43.52 D in Down and normal group, respectively (all p < 0.001). The coefficients of variation of total corneal refractive power from the center to the periphery were similar in the two groups (p = 0.855). None of the mean total corneal refractive powers significantly correlated with age, and all of them were significantly higher in females (p < 0.001). Mean total corneal refractive power-based corneal astigmatism in these zones changed from 1.46 to 1.66 D in Down syndrome patients and 1.64 to 1.99 D in normal group. All corneal astigmatism indices were similar between two groups (all p > 0.05). The prevalence of against the rule and oblique astigmatism in all zones were higher in the Down syndrome group (all p < 0.05)., Conclusion: Adolescent and young non-keratoconic patients with Down syndrome have a more prolate cornea and a homogeneous keratometry distribution. In this population, females have a steeper cornea.

Introduction: To compare the results of an accelerated corneal cross-linking (CXL) protocol (9 mW/cm 2 , 10 min) with the standard CXL protocol (3 mW/cm 2 , 30 min) in patients with Down syndrome (DS) who have keratoconus (KC)., Methods: Twenty-seven 10- to 20-year-old patients with DS who had bilateral progressive KC were enrolled in a contralateral randomized trial and completed 2 years of follow-up examinations. Fellow eyes were randomly allocated to the accelerated CXL group or the standard CXL group. The main outcome measure was change in maximum keratometry (K max ) centered on the steepest point (zonal K max - 3 mm) with a non-inferiority margin of 1.0 diopter (D). Vision and refraction tests, ophthalmic examinations, and corneal tomography were performed at baseline and at 6, 12, and 24 months after CXL. Failure was defined as an increase of ≥ 1.0 D in zonal K max - 3 mm within a 12-month period., Results: The mean age (± standard deviation) of the patients was 15.71 ± 2.40 years. The within-group change in zonal K max - 3 mm was not significant after 2 years in either group, and within-group zonal K max - 3 mm remained stable. At 2 years after CXL, the mean change in the zonal K max - 3 mm was - 0.02 ± 0.81 D and - 0.31 ± 0.86 D in the accelerated CXL and standard CXL groups, respectively (P = 0.088). At 1 year of follow-up, three patients in the accelerated CXL group showed treatment failure (mean change in zonal K max - 3 mm + 2.12 ± 0.11 D); no patients in the standard CXL group showed treatment failure. At 2 years of follow-up, these three patients showed a decrease of - 0.43 ± 0.18 D in zonal K max - 3 mm from a baseline value of 55.11 ± 0.32 D. The 2-year trends of the inferior-superior asymmetry and vertical coma were statistically significantly different between the two groups, with the accelerated CXL protocol showing superiority in patients with higher baseline values., Conclusion: In young patients with Down syndrome, the accelerated CXL protocol was able to halt disease progression and may be an alternative for the standard CXL protocol. In advanced KC, the efficacy of the accelerated approach was delayed and appeared later in the follow-up. The NIH-funded Research Centers in Minority Institutions (RCMI) program is currently funding 18 academic institutions to strengthen the research environment and contribution to health disparities research. The purpose of this multiphase mixed-methods study was to establish a uniform evaluation framework for demonstrating the collective success of this research consortium. Methods included discussions of aims and logic models at the RCMI Evaluators' Workshop, a literature review to inform an evaluation conceptual framework, and a case study survey to obtain evaluation-related information and metrics. Ten RCMIs participated in the workshop and 14 submitted responses to the survey. The resultant RCMI Evaluation Conceptual Model presents a practical ongoing approach to document RCMIs' impacts on health disparities. Survey results identified 37 common metrics under four primary categories. Evaluation challenges were issues related to limited human resources, data collection, decision-making, defining metrics, cost-sharing, and revenue-generation. There is a need for further collaborative efforts across RCMI sites to engage program leadership and community stakeholders in addressing the identified evaluation challenges and measurement. Program leadership should be engaged to apply the Evaluation Conceptual Framework and common metrics to allow for valid inter-institutional comparisons and consortium-wide evaluations. Stakeholders could ensure evaluation metrics are used to facilitate community impacts.

Background: To assess the diagnostic ability of Pentacam HR (Oculus Optikgeräte, GmbH, Wetzlar, Germany) tomographic indices in discriminating keratoconus (KC) and KC suspect (KCS) in 10- to 30-year-old patients with Down syndrome (DS)., Methods: In this study, DS patients were enrolled through special needs schools, the National Down Syndrome Society, and relevant non-profit organizations. Diagnoses were made independently by two experienced specialists. Forty Pentacam indices related to corneal thickness, volume, density, keratometry, power, shape, aberration, and elevation were extracted. For each index, the accuracy for KC and KCS diagnosis was evaluated using discriminant analysis and the area under receiver operating characteristic curve (AUROC). From each enrolled case, data from only one eye was entered in the analyses., Results: Analyses were performed on data from 25 KC, 46 KCS, and 154 non-ectatic DS eyes. The best discriminants for KC were anterior higher order aberrations (HOA) (cutoff > 0.643, AUROC = 0.879), posterior vertical coma (cutoff > 0.0702 μm, AUROC = 0.875), anterior vertical coma (cutoff > 0.4124 μm, AUROC = 0.868), and total HOA (cutoff > 0.608, AUROC = 0.867). The difference between AUROCs were not statistically significant (all P  > 0.05). For KCS, the best discriminants were minimum corneal thickness (cutoff ≤ 480.0 μm, AUROC = 0.775), corneal volume (cutoff ≤ 55.3 μm, AUROC = 0.727) and Belin Ambrosio display-total deviation (BAD-D) (cutoff > 2.23, AUROC = 0.718) with no significant difference between AUROCs (all P  > 0.05)., Conclusions: In this sample of DS patients, best KC discriminators were HOA and coma which showed good diagnostic ability. For KCS, best predictors were minimum corneal thickness, corneal volume, and BAD-D with relatively good diagnostic ability. Objective: The biomedical/behavioral sciences lag in the recruitment and advancement of students from historically underrepresented backgrounds. In 2014 the NIH created the Diversity Program Consortium (DPC), a prospective, multi-site study comprising 10 Building Infrastructure Leading to Diversity (BUILD) institutional grantees, the National Research Mentoring Network (NRMN) and a Coordination and Evaluation Center (CEC). This article describes baseline characteristics of four incoming, first-year student cohorts at the primary BUILD institutions who completed the Higher Education Research Institute, The Freshmen Survey between 2015-2019. These freshmen are the primary student cohorts for longitudinal analyses comparing outcomes of BUILD program participants and non-participants., Design: Baseline description of first-year students entering college at BUILD institutions during 2015-2019., Setting: Ten colleges/universities that each received <$7.5mil/yr in NIH Research Project Grants and have high proportions of low-income students., Participants: First-year undergraduate students who participated in BUILD-sponsored activities and a sample of non-BUILD students at the same BUILD institutions. A total of 32,963 first-year students were enrolled in the project; 64% were female, 18% Hispanic/Latinx, 19% African American/Black, 2% American Indian/Alaska Native and Native Hawaiian/Pacific Islander, 17% Asian, and 29% White. Keratoconus (KCN) and Down syndrome affect the corneal density and volume. In this study included Down syndrome patients with and without KCN (24 Down-KCN and 204 Down-nonKCN eyes) and normal age- and gender-matched individuals (184 eyes). Studied parameters were the corneal density measured with Pentacam HR in 5 concentric zones and annuli (0-2 mm, 2-6 mm, 6-10 mm, 10-12 mm, and 0-12 mm) in 4 different depth layers (anterior 120 µm, posterior 60 µm, middle layer, and the full thickness of the cornea), and the 10 mm zone corneal volume. In Down-KCN, Down-nonKCN, and control groups, respectively, mean full thickness density in the 0-12 mm zone was 19.35 ± 2.92, 17.85 ± 2.55, and 15.78 ± 2.67 GSU, and mean corneal volume was 57.45 ± 4.37, 56.99 ± 3.46, and 61.43 ± 3.42mm 3 . All density readings were significantly different between the three studied groups (all P < 0.01) except full thickness density in 0-2 mm and 2-6 mm (P > 0.05) and corneal volume (P = 0.519) between Down-KCN and Down-nonKCN groups; these inter-group densitometry differences within the 6 mm zone were only in the middle layer, and not the anterior or posterior thickness layers (all P > 0.05). Corneal density increased with age and corneal thickness, but there was no significant relationship with gender. Overall, Down syndrome is associated with increased density and light scatter in all corneal layers up to the 12 mm diameter. In Down patients with KCN, the increased light scatter and density in the 6 mm zone is only in the middle thickness layer. Corneal volume is reduced in Down syndrome irrespective of the presence or absence of KCN.

Purpose: To measure the central-to-peripheral corneal thickness and its volume according to age and gender in 10-30-year-old patients with Down syndrome (DS) and in matched individuals without DS., Methods: In the report, 202 normal pattern right eyes of patients with Down syndrome and 190 right eyes of individuals without Down syndrome and compared averages using independent sample t-tests and multiple linear regression models. The measured variables included the apical corneal thickness; the minimum corneal thickness; the average thickness on rings at 2 mm (R2), 3 mm (R3), and 4 mm (R4); the corneal volume in the central zones at 2-, 3-, 4-, and 10-mm diameters; Ambrosio's relational thickness; and the pachymetric progression indices., Results: The mean age of the participants was 16.99 ± 4.70 and 17.22 ± 4.54 years (p=0.636). The means ± SD were 516.7 ± 33.0 and 555.7 ± 33.1 µm for apical corneal thicknesses, 508.0 ± 33.5 and 549.0 ± 40.6 µm for minimum corneal thicknesses, 543.0 ± and 588.4 ± 33.8 µm for R2s, 584.9 ± 35.6 and 637.0 ± µm for R3s, 646.9 ± 38.5 and 707.6 ± 37.1 µm for R4s, 396.4 ± 102.3 and 462.7 ± 96.2 µm for Ambrosio's relational thicknesses, 1.36 ± 0.37 and 1.22 ± 0.18 for pachymetric progression index maximums, 1.62 ± 0.11 and 1.74 ± 0.11 mm3 for corneal volume at 2 mm, 3.73 ± 0.24 and 4.01 ± 0.24 mm3 for corneal volume at 3 mm, 6.76 ± 0.44 and 7.30 ± 0.43 mm3 for corneal volume at 4 mm, and 57.03 ± 3.44 and 61.51 ± 3.40 mm3 for total corneal volume in the Down syndrome and control groups, respectively (all p<0.001). All the above indices were inversely related to age, but not to gender. Ambrosio's relational thickness maximum and the pachymetric progression index maximum were independent of age and gender., Conclusion: Non-keratoconic patients with Down syndrome had thin corneas with a homogeneous distribution. Therefore, the reference ranges of cornea thickness and volume should be re-defined for this patient population.

Purpose: To study the anterior chamber (AC) dimensions, angles and pupil diameter (PD) in patients with Down syndrome compared to normal controls., Methods: Prospective study is comparing the AC parameters in patients with Down syndrome aged 10-30 years and age-matched controls. Extracted indices included average anterior chamber depth on the 2-mm ring (ACD-2 mm), 4-mm ring (ACD-4 mm), at the corneal apex from the endothelium (endo-ACD), at the corneal apex from the epithelium (epi-ACD), anterior chamber volume (ACV), mean anterior chamber angle (ACA), and PD measured by Pentacam., Results: Data from 202 patients with Down syndrome (age 17.2 ± 4.8 years) were compared with 190 normal controls (age 17.2 ± 4.5 years). In Down and normal groups, mean ± SD were 2.51 ± 0.31 and 2.83 ± 0.34 mm for ACD-2 mm, 1.65 ± 0.30 and 1.93 ± 0.31 mm for ACD-4 mm, and 3.03 ± 0.29 and 3.24 ± 0.26 mm for endo-ACD, 3.54 ± 0.29 and 3.80 ± 0.26 mm for epi-ACD, mean 169.31 ± 30.38 and 200.17 ± 33.20 mm 3 for ACV, 40.69 ± 4.50 and 39.97 ± 4.12° for ACA, and 2.79 ± 0.62 and 3.59 ± 0.80 mm for PD, respectively (all P < 0.001). None of the studied indices significantly correlated with age, except for ACA (P = 0.011). All parameters, except for PD, were significantly higher in males compared to females (all P < 0.001). Temporal ACA was significantly wider in male subjects (44.61 ± 6.52 vs. 42.24 ± 6.52°; P < 0.001)., Conclusion: The AC in patients with Down syndrome is smaller than normal individuals. AC in females with Down syndrome is smaller than males, and the narrower ACA is attributable to the difference in the temporal angle and not the ACA in other meridians., Competing Interests: None

Background : Determining the age and gender distribution of ocular disorders in Down syndrome patients aged 10 to 30 years. Methods : In this study, 226 of 250 invited patients through special needs schools, the National Down Syndrome Society, and relevant nonprofit organizations were included. In Noor Eye Hospital, the patients underwent a complete eye examination by a general ophthalmologist and suspect cases were reexamined by a sub-specialist. Examinations included ocular alignment, conjunctiva, eyelid, lacrimal system, cornea, iris, and lens assessment. Results : Mean age of participants was 16.05 ± 4.82 years and 53.0% were male. The most common ocular abnormalities were blepharitis (81.9%, 95% CI:78.0 to 85.3), lens opacity (37.8%, 95% CI:33.3 to 42.3), strabismus (23.4%, 95% CI:19.5 to 27.4; 21.2% esotropia, 0.9% exotropia, and 1.8% dissociated vertical deviation), floppy eyelid (19.9%, 95% CI:16.3 to 23.9), posterior embryotoxon (17.7%, 95% CI:14.2 to 21.2) and nystagmus (11.7%, 95% CI:8.9 to 15.0). Based on independent sample t test, the prevalence of nystagmus ( P = .041) and congenital lens opacity  (P<0.001) significantly increased with age. There was no significant inter-gender difference in the prevalence of any of the studied disorders by chi-square test. Conclusion : In young patients with Down syndrome, the prevalence of ocular pathologies appears to be high and increase with aging. It can be resulted from the cumulative prevalence of undiagnosed or untreated cases. Findings of the study can be a reliable reference for health policy in terms of screening for eye disease and addressing eye care needs.

Purpose: To determine 5-year changes in keratoconus indices and corrected distance visual acuity in 40-64-year-old keratoconus compared with normal subjects., Methods: In this prospective population-based cohort study, 5-year changes in Belin grading system indices including the average radii of curvature in the 3 mm zone surrounding the thinnest point in the anterior (ARC-3 mm) and posterior (PRC-3 mm) cornea, corrected distance visual acuity, minimum corneal thickness, maximum Ambrosio's relational thickness (ART-max), and maximum anterior keratometry indices centered on steepest point in the central 3 mm (Kmax-3 mm), 4 mm (Kmax-4 mm), and 5 mm (Kmax-5 mm) zones were compared between keratoconus and normal participants. In the analysis, comparisons were made between all keratoconus eyes and the right eyes of normal participants., Results: The mean age in the keratoconus (n = 16 eyes) and normal (n = 1986 eyes) groups (48.31 ± 4.78, 49.37 ± 5.79 years, respectively) was not statistically different (P = 0.327). The two groups differed in terms of changes in PRC-3 mm (- 0.07 ± 0.15 vs. + 0.001 ± 0.14 mm, respectively, P = 0.042) and ART-max (- 6.28 ± 25.19 vs. + 15.8 ± 72.7 μm, respectively, P = 0.003). There were significant correlations between the reduction in PRC-3 mm and its baseline value (β = - 0.20, P < 0.001) and keratoconus (β = - 0.26, P < 0.001). The reduction in ART-max significantly correlated with its baseline value (β = - 0.43, P < 0.001) and keratoconus (β = - 111.74, P < 0.001)., Conclusion: According to these findings, posterior corneal steepening and thinning in keratoconus patients continue after the age of 40 years, but it is clinically negligible. The changes are independent of normal age-related changes and appear to be slower in cases with steeper and thinner corneas.

Purpose: To summarize the results of 5 years of vision screening with the University of California, Los Angeles (UCLA) Preschool Vision Program (UPVP)., Design: Retrospective evaluation of a screening program., Methods: The UPVP performed visual acuity and undilated noncycloplegic refractive screening using an autorefractor on 93,097 children between 2012 and 2017. Of these, 79,451 children, who were between 3 and 5 years old, were screened for the first time, and 14,259 were referred for full cycloplegic examination if they met specific refractive criteria for myopia, hyperopia, astigmatism, or anisometropia. UPVP performed 6779 cycloplegic examinations on this population. Data from the right eye only were included in this analysis., Results: Of the examined population, hyperopia was found in 61% (4018), myopia in 20% (1336), and astigmatism in 93% (6122) of children. Latino children had higher rates of astigmatism and worse visual acuity compared to all other races/ethnicities. An astigmatism cutoff of ≥1.50 diopters (D) in either eye correctly predicted the need for glasses 93% of the time; increasing this cutoff to ≥1.50 D in both eyes increased the positive predictive value to 96%. Refractive amblyopia was found in 780 children (1.0% of the screened population and 11.5% of the examined population), and of these, 211 (27%) were bilaterally amblyopic., Conclusions: These data represent the largest published sample of vision screening results on preschool-aged children, provide additional insight on the proportion of common refractive errors and their association with race/ethnicity, and can inform screening criteria to more accurately identify children who need intervention to prevent permanent vision loss., (Copyright © 2019 Elsevier Inc. Forkhead box A1 (FOXA1) is a pioneer factor that facilitates chromatin binding and function of lineage-specific and oncogenic transcription factors. Hyperactive FOXA1 signaling due to gene amplification or overexpression has been reported in estrogen receptor-positive (ER + ) endocrine-resistant metastatic breast cancer. However, the molecular mechanisms by which FOXA1 up-regulation promotes these processes and the key downstream targets of the FOXA1 oncogenic network remain elusive. Here, we demonstrate that FOXA1 overexpression in ER + breast cancer cells drives genome-wide enhancer reprogramming to activate prometastatic transcriptional programs. Up-regulated FOXA1 employs superenhancers (SEs) to synchronize transcriptional reprogramming in endocrine-resistant breast cancer cells, reflecting an early embryonic development process. We identify the hypoxia-inducible transcription factor hypoxia-inducible factor-2α (HIF-2α) as the top high FOXA1-induced SE target, mediating the impact of high FOXA1 in activating prometastatic gene sets and pathways associated with poor clinical outcome. Using clinical ER + /HER2 - metastatic breast cancer datasets, we show that the aberrant FOXA1/HIF-2α transcriptional axis is largely nonconcurrent with the ESR1 mutations, suggesting different mechanisms of endocrine resistance and treatment strategies. We further demonstrate the selective efficacy of an HIF-2α antagonist, currently in clinical trials for advanced kidney cancer and recurrent glioblastoma, in reducing the clonogenicity, migration, and invasion of endocrine-resistant breast cancer cells expressing high FOXA1. Our study has uncovered high FOXA1-induced enhancer reprogramming and HIF-2α-dependent transcriptional programs as vulnerable targets for treating endocrine-resistant and metastatic breast cancer.
