- Abstract
Zusammenfassung: Die Behandlung der stabilen Angina pectoris umfasst medikamentöse wie revaskularisierende Maßnahmen. Eine Revaskularisation kann mittels perkutaner Koronarintervention (PCI) oder Bypassoperation erfolgen. Bei allen Therapiemaßnahmen wird sowohl eine Behandlung der Beschwerden als auch eine Verbesserung der Prognose angestrebt. Während die Effektivität einer PCI zur Behandlung der Beschwerden unbestritten ist, wurde die Effektivität einer PCI zur Verbesserung der Prognose der Patienten kontrovers diskutiert. Allerdings war keine der bisher randomisiert durchgeführten Einzelstudien aufgrund der fehlenden „Power“ geeignet, den Effekt einer PCI auf das Überleben der Patienten mit stabiler Angina pectoris zu evaluieren. Die hier vorgestellte Metaanalyse fasst nun alle randomisiert durchgeführten Studien zusammen und berechnet die Effektivität einer PCI auf das Langzeitüberleben der Patienten im Vergleich zur medikamentösen Therapie. Dabei konnte gezeigt werden, dass bei einer mittleren Beobachtungsdauer von 51 Monaten in allen Studien zusammengenommen die Gesamtsterblichkeit im Vergleich zur rein medikamentösen Therapie signifikant um 20% gesenkt werden konnte.

- Abstract
Die neue NSTE-ACS (akutes Koronarsyndrom ohne ST-Hebungen)-Leitlinie der Europäischen Gesellschaft für Kardiologie (ESC) inkorporiert wichtige aktuelle Erkenntnisse insbesondere hinsichtlich der Früherkennung des NSTE-ACS und erleichtert über einen 0‑h/1-h-Algorithmus die Triage in den „chest pain units“. Bezüglich der Dauer der antithrombotischen Therapie nach perkutaner Koronarintervention bei NSTE-ACS bleiben 12 Monate der Standard, kürzere wie auch längere Therapiephasen sind jedoch entsprechend aktueller Studienergebnisse nach individueller Risiko-Nutzen-Abwägung in Ausnahmefällen möglich. Bei ACS-Patienten mit zusätzlicher Indikation zur oralen Antikoagulation wird weiterhin eine Triple-Therapie empfohlen, deren Dauer sich nach dem individuellen Blutungs- und Ischämierisiko richten muss. Bei Patienten mit nichtvalvulärem Vorhofflimmern sind aufgrund ihres günstigeren Risikoprofils hinsichtlich Blutung nun auch neue orale Antikoagulanzien (NOAKs) als Alternative zu Vitamin-K-Antagonisten (VKA) in den Leitlinien verankert. Randomisierte Studien prüfen derzeit den Stellenwert von NOAKs, aber auch dualer Behandlungskonzepte (orale Antikoagulation plus antithrombozytäre Monotherapie) in der akuten und chronischen Phase. The new European Society of Cardiology (ESC) guidelines on non-ST elevation acute coronary syndrome (NSTE-ACS) reflect important current scientific evidence regarding early diagnosis of NSTE-ACS and facilitate better patient triage in chest pain units using a 0 h/1 h rule in/rule out algorithm. Furthermore, 12 months remain the standard duration of antithrombotic therapy after percutaneous coronary intervention (PCI) for NSTE-ACS, while based on current evidence shorter or longer treatment duration is possible in exceptional cases after careful consideration of the individual patient risks and benefits. For ACS patients with additional indications for oral anticoagulation, triple therapy is still recommended and the duration strongly depends on individual risks of bleeding and ischemia. In addition, due to a favorable safety profile, non-vitamin K oral anticoagulants (NOAC) might be considered in patients with non-valvular atrial fibrillation as an alternative to vitamin K antagonists (VKA). There are several randomized trials currently investigating both NOAC and different dual antithrombotic treatment strategies (e.g. OAC plus antiplatelet monotherapy) in the acute and chronic phases of ACS.

Myocardial revascularisation represents the most frequently performed therapeutic intervention worldwide. Current percutaneous and surgical revascularisation techniques provide excellent short- and long-term clinical outcomes. However, despite the technological and procedural advances with the widespread use of drug-eluting stents and arterial bypass grafts in contemporary practice, a considerable proportion of patients require repeat revascularisation procedures during long-term follow-up. The need for repeat revascularisation has a major impact on patients' quality of life and is associated with a significant economic burden. This consensus document summarises the views on the management of myocardial revascularisation failure of an expert panel of the European Association of Percutaneous Cardiovascular Interventions (EAPCI). The present document provides a broad and pragmatic overview of the clinical management of myocardial revascularisation failure with a focus on the three key underlying mechanisms leading to repeat revascularisation: 1) failure of percutaneous coronary interventions, 2) failure of coronary artery bypass grafting, and 3) progression of coronary artery disease in native coronary segments previously untreated. The aim of the present position document is to provide a patient-oriented approach for the management of myocardial revascularisation failure.

The proposed 2020 Core Curriculum for Percutaneous Cardiovascular Interventions aims to provide an updated European consensus that defines the level of experience and knowledge in the field of percutaneous cardiovascular intervention (PCI). It promotes homogenous education and training programmes among countries, and is the cornerstone of the new EAPCI certification, designed to support the recognition of competencies at the European level and the free movement of certified specialists in the European Community. It is based on a thorough review of the ESC guidelines and of the EAPCI textbook on percutaneous interventional cardiovascular medicine. The structure of the current core curriculum evolved from previous EAPCI core curricula and from the "2013 core curriculum of the general cardiologist"to follow the current ESC recommendations for core curricula. In most subject areas, there was a wide - if not unanimous - consensus among the task force members on the training required for the interventional cardiologist of the future. The document recommends that acquisition of competence in interventional cardiology requires at least two years of postgraduate training, in addition to four years devoted to cardiology. The first part of the curriculum covers general aspects of training and is followed by a comprehensive description of the specific components in 54 chapters. - Abstract
Objective: To evaluate the incidence, predictors and outcomes of female patients with patient-prosthesis mismatch (PPM) following transcatheter aortic valve intervention (TAVI) for severe aortic stenosis (AS). Background: Female AS TAVI recipients have a significantly lower mortality than surgical aortic valve replacement (SAVR) recipients, which could be attributed to the potentially lower PPM rates. TAVI has been associated with lower rates of PPM compared to SAVR. PPM in females post TAVI has not been investigated to date. AIMS Our aim was to report on a survey initiated by the European Association of Percutaneous Cardiovascular Interventions (EAPCI) concerning opinion on the evidence relating to dual antiplatelet therapy (DAPT) duration after coronary stenting. METHODS AND RESULTS Results from three randomised clinical trials were scheduled to be presented at the American Heart Association Scientific Sessions 2014 (AHA 2014). A web-based survey was distributed to all individuals registered in the EuroIntervention mailing list (n=15,200) both before and after AHA 2014. A total of 1,134 physicians responded to the first (i.e., before AHA 2014) and 542 to the second (i.e., after AHA 2014) survey. The majority of respondents interpreted trial results consistent with a substantial equipoise regarding the benefits and risks of an extended versus a standard DAPT strategy. Two respondents out of ten believed extended DAPT should be implemented in selected patients. After AHA 2014, 46.1% of participants expressed uncertainty about the available evidence on DAPT duration, and 40.0% the need for clinical guidance. CONCLUSIONS This EAPCI survey highlights considerable uncertainty within the medical community with regard to the optimal duration of DAPT after coronary stenting in the light of recent reported trial results. Updated recommendations for practising physicians to guide treatment decisions in routine clinical practice should be provided by international societies.

Item does not contain fulltext OBJECTIVES: This study sought to examine the safety and performance of contemporary transcatheter aortic valve replacement (TAVR) in an exclusive all-women TAVR population, and to further investigate the potential impact of female sex-specific characteristics on composite 1-year clinical outcomes. BACKGROUND: Women comprise >/=50% patients undergoing TAVR. Several data have shown the noninferiority of TAVR compared with surgical aortic valve replacement for symptomatic significant aortic stenosis, but no study so far has been specifically powered to detect differences by sex. METHODS: The WIN-TAVI (Women's INternational Transcatheter Aortic Valve Implantation) registry is a multinational, prospective, observational registry of women undergoing TAVR for significant aortic stenosis, across 18 sites in Europe and 1 site in the United States, between January 2013 and December 2015. The primary Valve Academic Research Consortium (VARC)-2 efficacy endpoint was a composite of mortality, stroke, myocardial infarction, hospitalization for valve-related symptoms or heart failure or valve-related dysfunction beyond 30 days. Secondary endpoints included composite 1-year death or stroke. Predictors of 1-year outcomes were determined using Cox regression methods. RESULTS: A total of 1,019 intermediate to high-risk women, with mean age 82.5 +/- 6.3 years, mean European System for Cardiac Operative Risk Evaluation (EuroSCORE) I 17.8 +/- 11.7% and mean Society of Thoracic Surgeons score 8.3 +/- 7.4% were enrolled. TAVR was performed via transfemoral access in 90.6% and new-generation devices were used in 42.1%. The primary VARC-2 efficacy composite endpoint occurred in 111 (10.9%) patients beyond 30 days and in 167 (16.5%) patients at 1 year. The incidence of 1-year death or stroke was 13.9% (n = 141). Death occurred in 127 (12.5%) patients and stroke in 22 (2.2%) patients. Prior coronary revascularization (hazard ratio [HR]: 1.72; 95% confidence interval [CI]: 1.17 to 2.52; p = 0.006) and EuroSCORE I (HR: 1.02; 95% CI: 1.00 to 1.04; p = 0.027) were independent predictors of the VARC-2 efficacy endpoint. Similarly, EuroSCORE I (HR: 1.02; 95% CI: 1.00 to 1.04; p = 0.013), baseline atrial fibrillation (HR: 1.58; 95% CI: 1.07 to 2.33; p = 0.022), and prior percutaneous coronary intervention (HR: 1.50; 95% CI: 1.03 to 2.19; p = 0.035) were independent predictors of 1-year death or stroke. After adjustment, no significant association was observed between history of pregnancy or any sex-specific factors and 1-year TAVR outcomes. CONCLUSIONS: Intermediate to high-risk women enrolled in this first ever all-women contemporary TAVR registry experienced a 1-year VARC-2 composite efficacy endpoint of 16.5%, with a low incidence of 1-year mortality and stroke. Prior revascularization and EuroSCORE I were independent predictors of the VARC-2 efficacy endpoint, whereas EuroSCORE I, baseline atrial fibrillation, and prior percutaneous coronary intervention were independent predictors of the 1-year death or stroke.

- Abstract
Aims: Life-threatening complications occur during transcatheter aortic valve implantation (TAVI) which can require emergent cardiac surgery (ECS). Risks and outcomes of patients needing ECS during or immediately after TAVI are still unclear. Methods and results: Incidence, risk factors, management, and outcomes of patients requiring ECS during transfemoral (TF)-TAVI were analysed from a contemporary real-world multicentre registry. Between 2013 and 2016, 27 760 patients underwent TF-TAVI in 79 centres. Of these, 212 (0.76%) patients required ECS (age 82.4 ± 6.3 years, 67.5% females, logistic EuroSCORE: 17.1%, STS-score 5.8%). The risk of ECS declined from 2013 (1.07%) to 2014 (0.70%) but remained stable since. Annual TF-TAVI numbers have more than doubled from 2013 to 2016. Leading causes for ECS were left ventricular perforation by the guidewire (28.3%) and annular rupture (21.2%). Immediate procedural mortality (<72 h) of TF-TAVI patients requiring ECS was 34.6%. Overall in-hospital mortality was 46.0%, and highest in case of annular rupture (62%). Independent predictors of in-hospital mortality following ECS were age > 85 years [odds ratio (OR) 1.87, 95% confidence interval (95% CI) (1.02–3.45), P = 0.044], annular rupture [OR 1.96, 95% CI (0.94–4.10), P = 0.060], and immediate ECS [OR 3.12, 95% CI (1.07–9.11), P = 0.037]. One year of survival of the 114 patients surviving the in-hospital period was only 40.4%. Conclusion: Between 2014 and 2016, the need for ECS remained stable around 0.7%. Left ventricular guidewire perforation and annular rupture were the most frequent causes, accounting for almost half of ECS cases. Half of the patients could be salvaged by ECS—nevertheless, 1 year of all-cause mortality was high even in those ECS patients surviving the in-hospital period.

OBJECTIVES To evaluate the long-term prognostic value of risk scores in the setting of drug-eluting stent (DES) implantation for uLMCA. BACKGROUND Data on the prognostic value of novel risk scores developed to select the most appropriate revascularization strategy in patients undergoing DES implantation for uLMCA disease are relatively limited. METHODS The study represents a patient-level pooled analysis of the ISAR-LEFT-MAIN (607 patients randomized to paclitaxel-eluting or sirolimus-eluting stents) and the ISAR-LEFT-MAIN-2 (650 patients randomized to everolimus-eluting or zotarolimus-eluting stents) randomized trials. The Syntax Score (SxScore) as well the Syntax Score II (SS-II), the EuroSCORE and the Global Risk Classification (GRC) were calculated. The primary outcome was all-cause mortality. RESULTS At a mean follow-up of 3 years there were 160 deaths (12.7%).
