Objective: To investigate the association of daily dose-dependent Vitamin-D supplement on clinical outcomes including mortality in veterans with multiple sclerosis (MS) with low serum Vitamin-D (25[OH]D) levels. This Methods: observational, longitudinal study reports on data collected from 1/1/2000 to 12/31/2020 at the MS Regional Program. We reviewed the electronic records of 163 veterans with MS who were followed every four months in our MS clinic. The study sample was divided into three groups based on the daily Vitamin-D supplement doses: low (1000 IU/day), medium (2000 IU/day) and high (4000 IU/day). The clinical outcomes were changes in physical disability (Expanded Disability Severity Scale (EDSS)), function (Total Function Independence Measures (TFIM)), time-to-event EDSS score of 6 (progressive MS) and mortality. Results: There were 32 patients in the low, 46 in the medium, and 85 in the high-dose daily Vitamin-D supplement groups. The mean age of the entire study sample was 51 12 SD years (range 23 - 79 years). The mean duration of MS was 21.7 13 years. The mean initial EDSS and TFIM scores were 3.4 2.8 and 111 17 SD. At entry, mean serum 25[OH]D and calcium levels were 30 16 nmol/l and 9.3 0.4. The risk of MS-related progressive disability (time-to-event EDSS score 6) was lower and there were fewer deaths in the high- (compared to low- and medium-) dose groups. Conclusion: This study suggests that among veterans with MS and low 25[OH]D levels, those who received high-doses daily of supplemental Vitamin-D supplement were associated with a decreased risk of MS-related progressive disability and death than those who received a lower dose. No adverse clinical or biochemical events were noted.

Background: Acute low back pain is one of the most common reasons for adults to see a physician. However, vertebral osteomyelitis (VO) is a rare condition, which mostly affects the lumbar spine. Lumbar spine MRI imaging is the diagnostic method of choice. If left untreated can lead to vertebral destruction or spinal abscess formation. Case: An 86-year-old man presented with a stroke to our facility, while undergoing therapy complained of excruciating low back pain that prevented him from undergoing therapy specially standing and walking. His vital signs were normal, and he was afebrile. Plain X-ray of the lower spine showed osteoarthritic changes. Patient was initially started on non-steroidal anti-inflammatory (NSAID) with no change in the severity of his low back pain. Lumbar vertebral spine magnetic resonance imaging (MRI) showed lumbar vertebral osteomyelitis. Initiation of IV antibiotics dramatically decreased the pain severity. Conclusion: Low back pain is common in the elderly population and responds effectively to NSAID. Persistence of low back pain in the presence of pathognomonic changes in the lumbar spine MRI indicate the presence of VO. Prompt treatment with empirical antibiotics is warranted.

Prior research has identified abnormal platelet procoagulant responses in COVID-19. Coated-platelets, a form of procoagulant platelets, support thrombin formation and are elevated in ischemic stroke patients with increased risk for recurrent infarction. Our goal was to examine changes in coated-platelet levels over the course of COVID-19 infection and determine their association with disease severity, thrombosis, and death. Coated-platelet levels were assayed after admission and repeated weekly in COVID-19 patients, and in COVID-19 negative controls. Receiver operator characteristic (ROC) analysis was used to calculate area under the curve (AUC) values for a model including baseline coated-platelets to predict death. Kaplan–Meier and Cox proportional hazards analysis was used to predict risk for death at 90 days. We enrolled 33 patients (22 with moderate and 11 with severe infection) and 20 controls. The incidence rates of pressure ulcers (PUs) in patients with SCI in the United States varies by clinical setting, ranging from 0.4%-38% in acute care, 2.2%-23.9% in long-term care, and 0%-17% in home care [ 1 , 2 ]. Unrelieved pressure is the most important factor in the development of PUs. Other factors associated with PUs in patients with SCI include age at the time of injury, men, blacks, completeness of the injury, functional dependence, behavioral protective factors such as frequent pressure relief, self-positioning, daily skin monitoring, nutritional state, cigarette smoking, alcohol (ab)use, and being depressed [3] . Presence of PUs affects functional physical outcomes; thus, prevention of PUs is the key [4] . Infection is a common complication of PUs which can be local such as cellulitis or osteomyelitis or systemic such as septicemia with a greater than 50% mortality. We present a case of a 58-year-old paraplegic man with pressure ulcer who presented with fever in the presence of an osteomyelitis and had a pelvic abscess on magnetic resonance imaging which needed surgical drainage.

I present here a case of psoas (retroperitoneal) hematoma; which is a rare, but potentially life-threatening acute condition, with a 30-day mortality of 10% from bleeding in the retroperitoneal space without associated trauma or iatrogenic intervention. A 59-year-old man who was on long-term anticoagulation for his atrial fibrillation presented to our facility with worsening pain in the left groin and being unable to move the left leg and walk. Computed tomography showed a large left psoas hematoma. He was treated conservatively which included reversal of his anticoagulation state. This case highlights the need for early diagnosis of psoas hematoma by computed tomography scan of the abdomen and pelvis to promptly initiate treatment to improve functional outcome.

Coated-platelets are a subset of highly procoagulant platelets observed after dual agonist stimulation with collagen and thrombin. Coated-platelet levels are increased in acute stroke compared to controls, and higher levels are associated with stroke recurrence. We examined whether coated-platelet levels measured at the time of the stroke correlate with cognitive scores at 3 months following the brain infarction.Coated-platelets were assayed in consecutive patients with nonlacunar stroke. Cognitive screening was performed using the Mini-Mental State Examination (MMSE) at 3 months after discharge. Linear regression, with adjustment for individual covariates, was used to model the association between coated-platelet levels and MMSE scores.One hundred and twenty-eight patients with a mean MMSE score of 26 points (range 14-30, standard deviation [SD] 3.1) and mean coated-platelet levels of 40.9% (range 5.2-76.2, SD 13.3), completed cognitive screening. An inverse linear association was found between coated-platelet levels and MMSE score, with higher levels seen in patients with lower MMSE scores (r = -.34, RThese findings support a link between increased platelet procoagulant potential at the time of the stroke and development of cognitive impairment following cerebral infarction.

Patients who adhere to their disease-modifying therapies (DMTs) have a lower rate of MS-related relapses and disability. We sought to determine the adherence rate to DMTs and the association between adherence to DMTs and employment in patients with multiple sclerosis (MS).One hundred and forty-two patients with MS who were periodically followed in our clinic between January 2000 and January 2020. We compared three groups of patients, defined according to their adherence to DMTs (non-adherent; poorly adherent; adherent), on their obtaining paid employment.Forty-seven MS patients (33.1%) were non-adherent to any DMT medication, while 88 MS patients (62.0%) demonstrated good and 7 (4.9%) poor adherences. More patients in the good-adherence group were in paid employment 42.0% compared to 23.4% in the non-adherent group (In this study veterans who adhered to their DMTs were 2.4 times more likely to be in paid-employment compared to non- or poorly-adherent patients. The clinical significance of this study is to encourage MS patients to adhere to their DMTs as a motivating factor of being in paid-employment.Implications for rehabilitationWhat is known about this subject? In the setting of a randomized controlled trial adherence rates reported for different injectable DMTs vary from 79-85% for once a week interferon beta-1a (IM IFNb-1a) to 49-78% for other injectable DMTs.•What are the new findings and/or impact on clinical practice?•Paid employment was more common among veterans whose adherence to DMTs was good (42.0% employed) or poor (42.9%) than among those who were non-adherent (23.4%). Veterans who adhered to DMTs were also younger, had less severe MS, and displayed less cognitive and physically impairment than those who did not adhere.•Being employed can act as a motivating factor to encourage adherence to treatment aimed at preventing accumulation of mental and physical disabilities.

The aim of this article was to determine whether cathodal transcranial direct current stimulation (c-tDCS) to unaffected primary motor cortex (PMC) plus conventional occupational therapy (OT) improves functional motor recovery of the affected arm hand in patients after an acute ischemic stroke compared with sham transcranial direct current stimulation plus conventional OT.In this prospective, randomized, double-blinded, sham-controlled trial of 16 severe, acute ischemic stroke patients with severe arm-hand weakness were randomly assigned to either experimental (c-tDCS plus OT; n = 8) or control (sham transcranial direct current stimulation plus OT; n = 8) groups. All patients received a standard 3-hr in-patient rehabilitation therapy, plus an additional ten 30-min sessions of tDCS. During each session, 1 mA of cathodal stimulation to the unaffected PMC is performed followed by the patient's scheduled OT. The primary outcome measure was change in Action Research Arm Test (ARAT) total and subscores on discharge.Application of c-tDCS to unaffected PMC resulted in a clinically relevant 10-point improvement in the affected arm-hand function based on ARAT total score compared with a 2-point improvement in the control group.Application of 30-min of c-tDCS to the unaffected PMC showed a 10-point improvement in the ARAT score. This corresponds to a large effect size in improvement of affected arm-hand function in patients with severe, acute ischemic stroke. Although not statistically significant, this suggests that larger studies, enrolling at least 25 patients in each group, and with a longer follow-up are warranted.

BACKGROUND The goal of this observational study was to examine the effect of common chronic medical conditions (CMCs) on long-term disability (activity limitation) in veterans already diagnosed with multiple sclerosis (MS). MATERIAL AND METHODS We retrospectively reviewed the electronic charts of 124 veterans with MS who have been regularly followed in our MS clinic for 10 or more years. General linear model analysis examined whether MS-related severity as measured by the Expanded Disability Status Scale (EDSS) and the presence of CMCs affected long-term disability as measured by the total score on the Functional Independence Measure (TFIM). RESULTS Commonly encountered CMCs were increased BMI (61%), hyperlipidemia (78%), hypertension (65%), current smokers (47%), and arthritis/arthralgia (24%). Results suggest that the number of CMCs was not predictive of final TFIM scores; of the variables examined, only initial EDSS score was predictive of final TFIM scores. CONCLUSIONS The presence of CMCs did not affect the long-term disability in veterans diagnosed with MS, this was due mainly to CMCs being closely monitored and co-treated with other medical specialties.

The purpose of this review was to discuss the prevalence, impact, pathophysiology, and treatment of headaches (H/As) in patients with multiple sclerosis (MS). Headaches and multiple sclerosis are more common in women than in men with the ratio of female to male being 3:1. It is not entirely clear if there is a correlation or an incidental comorbidity of two neurological conditions. A review of the literature shows a variable prevalence of H/As in MS patients. Using the International Classification of Headache Disorders (ICHD) criteria, the primary type of H/As, especially migraine, is the most common type seen in patients with MS. One of the theories of the pathophysiologic mechanisms of migraine in MS patients is inflammation leading to demyelinating lesions in the pain-producing centers in the midbrain. Secondary H/As due to MS medications such as interferons are also frequently present. H/As can be a cause for significant comorbidity in patients with MS. The treatment of H/As in patients with MS should be addressed in the same fashion as in the non-MS population, which is a combination of pharmacological and non-pharmacological methods. Preventive medicines for the H/As should be carefully selected because of their side effect profiles. Acute attacks of migraines can be treated with medications such as triptans. Patients with MS who have migraine H/As should be educated about the phenomenon of overuse H/As, keeping headache journals, avoiding stress, and monitoring sleeping habits. The presence of depression in patients with MS and migraine affects quality of life (QOL) and should also be addressed for better outcomes.

Objective: This observational study: (a) compared serum creatinine (estimated glomerular filtration rate (EGFR)) to renal isotope 99mTc-DTPA (GFR) determined glomerular filtration rate, and evaluated whether either method (b) better determined the state of renal function, and (c) predict urinary tract infection (UTI), renal and urological structural lesions or mortality in veterans with traumatic spinal cord injury (SCI) and neurogenic bladder (NGB).Design: Observational study.Setting: VA Medical Center affiliated with Oklahoma University.Participants: Veterans with SCI and regularly followed in SCI clinic. Background The goal of this observational study was to determine factors predictive of the type of powered mobility prescribed to veterans with disability. Material/methods A retrospective chart review was conducted for all veterans (n=170) who received powered mobility from a designated power mobility clinic. Logistic regression analysis was used to determined factors predictive of the type of powered mobility provided. Results Sixty-four (38%) veterans were provided powered wheelchairs and 106 (62%) were provided powered scooters. Of the variables examined, only primary medical conditions for referral and disability severity (as measured by the 2-minute timed walk test; 2-MWT) were predictive of the types of powered mobility prescribed. Veterans who were able to walk longer distances were more likely to be prescribed powered scooters. Age, gender, race, level of education, marital and employment status, number of chronic medical conditions, and upper and lower limb muscle strength were not significant predictors. Conclusions This study suggests that the primary medical conditions for referral and 2-MWT can assist clinicians in the determination of the type of powered mobility to prescribe to veterans with disability.

Multiple sclerosis (MS) is a disease which has a variable clinical presentation followed by a variable clinical course. Therefore, accuracy of clinical rating scales to measure disability at initial clinical presentation and during follow-up visits is essential to accurately capture the variability inherent in this disease. This is particularly vital when attempting to identify the efficacy of interventions.This observational study in veterans with MS compared the Kurtkze Expanded Disability Status Scale (EDSS) and the Total Functional Independence Measure (TFIM) scale as measures of MS-related disability.We retrospectively reviewed the electronic charts of 76 veterans with MS who are regularly followed in our VA MS clinic. Local Institutional Review Board approval was obtained for the protocol. Data were analyzed using SAS (SAS System for Windows, version 9.2, SAS Institute Inc., Cary, NC, USA). Chi-square and Fisher exact tests were used to assess categorical variables. Kruskal-Wallis tests evaluated the relationships between MS types [relapsing-remitting MS (RR), secondary progressive MS (SP), primary progressive MS (PP) and clinical isolated syndrome (CIS)] and the initial TFIM, EDSS and Impairment Index (II) scores.The EDSS score accurately measured MS-related impairment at initial evaluation and follow-up relative to an II. However, the EDSS score did not change over time, compared to the TFIM suggesting reduced sensitivity of the EDSS for detecting change in MS-related disability over time.This suggests TFIM scale is a more sensitive measure of MS-related disability than EDSS for use in future MS clinical trials.This study highlights that given the paucity of scales to which EDSS has been compared, TFIM is a valuable adjunct to EDSS in measuring MS-related disability. TFIM is able to accurately measure the severity of MS-related disability and help provide for services patients with MS-related disability would need. TFIM is an easy to administer and a sensitive scale to measure the change in MS-related disability following interventions.

Objective: Musculoskeletal pain commonly occurs in the elderly, many of whom are also prone to suffer from strokes. We studied whether short-term use (≤ 4 weeks) of cyclooxygenase-2 (COX-2) inhibitors for musculoskeletal pain in stroke patients helped them to participate in their therapies and was safe and efficacious. Materials and Methods: Three hundred and three patients admitted consecutively with first ischemic stroke were studied. Two cohorts were defined, based on whether patients with acute stroke had sufficient musculoskeletal pain that warranted oral COX-2 inhibitors (COX-2 group) or not (case-matched controls). Primary efficacy measures were change in Fugl-Meyer (F-M) pain score and change in total functional independence measure (TFIM) scores on discharge from hospital. Safety was judged by the incidence of vascular episodes during the study period. Results: From the original 303 patients, 64 patients in the COX-2 group were matched with 64 patients in the non-COX-2 group. The groups were matched for age (±5 years), gender, and admission TFIM score (± 5 points). Baseline characteristics between the 2 groups were similar. The primary and secondary outcome measures were similar between the 2 groups, except for ambulation endurance, which favored the non-COX-2 group (P < 0.03). Greater change in the pain score (less pain) was found in the COX-2 group; this effect was strongest in patients who were independent prior to their stroke (on post hoc analysis). There were too few adverse events in either group of any significance. Conclusions: The short-term use of COX-2 inhibitors reduced musculoskeletal pain in acute stroke patients, improved functional motor outcome, and were found to be safe.

This observational study aimed to determine the prevalence of neurogenic bladder (NGB), and its impact (frequency of urinary tract infection [UTI], autonomic dysreflexia (AD) pressure ulcers, spasticity, and hospitalization rates) on veterans with traumatic spinal cord injury (SCI).NGB (detrusor muscle and urethral sphincter dysfunction with loss of bladder sensation to void), secondary to SCI, is commonly encountered in daily practice; however, its impact on veterans' overall health has been less well studied.We retrospectively reviewed the electronic charts of veterans with SCI enrolled in our program and regularly followed in our SCI clinic. Demographic data collected included: age, sex, race/ethnicity, and age, level, severity and cause of spinal injury. Also noted was presence of NGB, episodes of UTI, presence of pressure ulcers, AD, spasticity, and hospitalization rate. Differences between those with and without NGB were evaluated using Generalized Linear Models.Of 161 veterans with SCI, symptoms of NGB was present in 133 (83%). Presence of NGB was associated with severe spinal cord injury. Veterans with NGB had more frequent UTI and presence of pressure ulcers (P 0.05). They also were more likely to need hospitalization and were at an increased risk of dying.Incidence of NGB in veterans with SCI is high, is mainly associated with severe spinal cord injury, and severely impacts veterans' health by frequently causing UTIs, increasing hospitalization rate, and increases risk of death.

Objective Race has been shown to affect stroke-related morbidity and mortality. The purpose of this study was to determine the influence of race on rehabilitation functional outcomes in patients with an acute stroke admitted to a designated stroke rehabilitation unit. Design A 2-yr prospective study was conducted of 670 patients admitted to an acute stroke rehabilitation unit within 30 days after an acute stroke. Demographic data along with admission and discharge Functional Independence Measure scores were recorded in a computerized stroke database. Patients participated in a standardized rehabilitation program 5 days a week for a length of stay dependent upon their individual needs. Results The distribution of patients by race was 504 whites, 115 blacks, 38 Hispanics, and 13 Asians. The mean age of the study population was 70 yrs. Whites had a higher prevalence of atrial fibrillation, and blacks, Hispanics, and Asians had a higher prevalence of hypertension and diabetes mellitus. The primary and secondary functional rehabilitation outcomes were similar for all four groups after similar intensity of therapy (3.5 hrs daily). Conclusions This study suggests that race does not influence rehabilitation functional outcomes in patients with acute stroke undergoing rehabilitation for the same intensity of therapy if admitted soon after their acute stroke.

Summary Background Recent review of the available evidence on interventions for motor recovery after stroke, showed that improvements in recovery of arm function were seen for constraint-induced movement therapy, electromyographic biofeedback, mental practice with motor imagery, and robotics. Similar improvement in transfer ability or balance were seen with repetitive task training, biofeedback, and training with a moving platform. Walking speed was improved by physical fitness training, high-intensity physiotherapy and repetitive task training. However, most of these trials were small and had design limitations. Material/Methods In this article, randomized control trials (RCT’s) published in 2009 of rehabilitation therapies for acute (≤2 weeks), sub-acute (2 to 12 weeks) and chronic (≥12 weeks) stroke was reviewed. A Medline search was performed to identify all RCT’s in stroke rehabilitation in the year 2009. The search strategy that was used for PubMed is presented in the Appendix 1. The objective was to examine the effectiveness of these treatment modalities in stroke rehabilitation. Results This generated 35 RCT’s under 5 categories which were found and analyzed. The methodological quality was assessed by using the PEDro scale for external and internal validity. Conclusions These trials were primarily efficacy studies. Most of these studies enrolled small numbers of patient which precluded their clinical applicability (limited external validity). However, the constraint induced movement therapy (CIT), regularly used in chronic stroke patients did not improve affected arm-hand function when used in acute stroke patients at ≤4 weeks. Intensive CIT did not lead to motor improvement in arm-hand function. Robotic arm treatment helped decrease motor impairment and improved function in chronic stroke patients only. Therapist provided exercise programs (when self-administered by patients during their off-therapy time in a rehabilitation setting) did improve arm-hand function. Tai Chi exercises helped improve balance and weight bearing. Exercise programs for community dwelling stroke patient helped maintain and even improve their functional state.

The overall goal of this observational study was to determine whether modifiable vascular risk factors contribute to the prevalence of pressure ulcers (PrU) in veterans with traumatic spinal cord injury (SCI).Given the increasingly limited financial resources in hospitals and clinics, identifying risk factors associated with the development of PrU in persons with SCI will be a major step in reducing the cost of care for these individuals, and may improve their quality of life.We retrospectively reviewed the electronic charts of 87 veterans with SCI who are being followed regularly in our SCI clinic and are enrolled in the SCI registry. The data collected included the basic demographics, presence of modifiable vascular risk factors such as hypertension, diabetes mellitus, hyperlipidemia, and current smoking; presence of incontinence and depression; and results from blood drawn for hemoglobin level, blood urea nitrogen, creatinine, and albumin levels and lipid profile on veteran's initial enrollment. Local Institution Review Board approval was obtained for the protocol.Of the 87 veterans with SCI, 27 had PrU. Comparisons between those with and without PrU found no significant differences for the demographic variables of age, gender, age of SCI onset, or SCI duration, but there was a trend for the groups to differ in ethnicity (P = 0.05). Similarly, the presence of modifiable vascular risk factors including hypertension, diabetes mellitus, hyperlipidemia, and current smoking did not differ between those with and without PrU. There were 36 pressure ulcer sites observed in 27 people. The proportion of pressure ulcer sites (of the 36) significantly differed by SCI severity based on the American Spinal Injury Association (ASIA) score (P0.0001).This study suggests that the presence of PrU was influenced by the severity of the SCl without any contribution from modifiable vascular risk factors.

Jan Y-K, Jones MA, Rabadi MH, Foreman RD, Thiessen A. Effect of wheelchair tilt-in-space and recline angles on skin perfusion over the ischial tuberosity in people with spinal cord injury. Objective To investigate the efficacy of wheelchair tilt-in-space and recline on enhancing skin perfusion over the ischial tuberosity in wheelchair users with spinal cord injury (SCI). Design Repeated-measures, intervention, and outcomes-measure design. Setting A university research laboratory. Participants Wheelchair users with SCI (N=11; 9 men, 2 women; mean ± SD age, 37.7±14.2y; body mass index, 24.7±2.6kg/m 2 ; duration of injury, 8.1±7.5y). Interventions Protocols (N=6) of various wheelchair tilt-in-space and recline angles were randomly assigned to participants. Each protocol consisted of a 5-minute sitting-induced ischemic period and a 5-minute wheelchair tilt-in-space and recline pressure-relieving period. Participants sat in a position without tilt or recline for 5 minutes and then sat in 1 of 6 wheelchair tilted and reclined positions, including (1) 15° tilt-in-space and 100° recline, (2) 25° tilt-in-space and 100° recline, (3) 35° tilt-in-space and 100° recline, (4) 15° tilt-in-space and 120° recline, (5) 25° tilt-in-space and 120° recline, and (6) 35° tilt-in-space and 120° recline. A 5-minute washout period (at 35° tilt-in-space and 120° recline) was allowed between protocols. Main Outcome Measures Laser Doppler flowmetry was used to measure skin perfusion over the ischial tuberosity in response to changes in body positions caused by performing wheelchair tilt-in-space and recline. Skin perfusion response to wheelchair tilt-in-space and recline was normalized to skin perfusion of the upright seated position (no tilt/recline). Results Combined with 100° recline, wheelchair tilt-in-space at 35° resulted in a significant increase in skin perfusion compared with the upright seated position (no tilt/recline; P P Conclusions Our results indicate that wheelchair tilt-in-space should be at least 35° for enhancing skin perfusion over the ischial tuberosity when combined with recline at 100° and should be at least 25° when combined with recline at 120°. Although smaller angles of wheelchair tilt-in-space and recline are preferred by wheelchair users for functional purposes, wheelchair tilt-in-space less than 25° and recline less than 100° may not be sufficient for effective pressure reduction for enhancing skin perfusion over the ischial tuberosity in people with SCI.

Problem statement: Stroke ranks as the leading cause of serious, long-term disability and death world-wide. It is estimated direct and indirect cost of stroke for 2009 was $68.9 billion. Approach: The objective of this review was to examine the effectiveness of emerging pharmacotherapy’s in patients with acute (≤2 weeks), sub-acute (2-12 weeks) and chronic (≥12 weeks) stroke studied in Randomized Control Trials (RCT’s) published in 2009. Medline search was performed to identify all RCT’s in acute, sub-acute and chronic stroke treatment in the year 2008. The search strategy used for Pub Med included key words such as Randomized Controlled Trials (RCT), Stroke OR cerebrovascular disorders OR CVA, Ischemic stroke OR ischemia, Hemorrhage OR intraparenchymal hemorrhage OR subarachnoid hemorrhage, Thrombolytics OR tissue plasminogen activator OR alteplase OR t-PA, Diabetes mellitus OR hyperglycemia, Hypertension OR raised blood pressure, Aspirin OR anti-platelets, Warfare OR anticoagulant, Antidepressants , Neuroprotectants and Coiling OR stents OR endovascular. Search limits included Human, Adult (age>19 years), English language and Publication date: 1/1/2009-12/31/2009. Results: Eleven categories of 27 RCT’s were found and analyzed. Conclusion: There was sufficient evidence to suggest that: (1) extending the time-window for administration of desmoteplase between 3-9 h after ischemic stroke onset did not improve clinical outcome; (2) at present neither lidocaine, erythropoietin or ropinotan had been effective neuroprotective agents in ischemic stroke; (3) treatment with dabigatran was associated with lower rate of stroke and systemic embolism in patients with non-valvular aerial fibrillation; (4) memantine and constraint-induced aphasia therapy were able to reduce the severity of aphasia in chronic post-stroke aphasia and (5) percutaneous closure of the left aerial appendage can be an effective alternative to chronic warfare treatment.

Objective: To determine the efficacy of activity-based therapies using arm ergometer or robotic or group occupational therapy for motor recovery of the paretic arm in patients with an acute stroke (≤4 weeks) admitted to an inpatient rehabilitation facility, and to obtain information to plan a large randomized controlled trial. Design: Prospective, randomized controlled study. Setting: Stroke unit in a rehabilitation hospital. Subjects: Thirty patients with an acute stroke (≤4 weeks) who had arm weakness (Medical Research Council grade 2 or less at the shoulder joint). Intervention: Occupational therapy (OT) group (control) (n = 10), arm ergometer (n = 10) or robotic (n = 10) therapy group. All patients received standard, inpatient, post-stroke rehabilitation training for 3 hours a day, plus 12 additional 40-minute sessions of the activity-based therapy. Main measures: The primary outcome measures were discharge scores in the Fugl-Meyer Assessment Scale for upper limb impairment, Motor Status Scale, total Functional Independence Measure (FIM) and FIM-motor and FIM-cognition subscores. Results: The three groups (OT group versus arm ergometer versus robotic) were comparable on clinical demographic measures except the robotic group was significantly older and there were more haemorrhagic stroke patients in the arm ergometer group. After adjusting for age, stroke type and outcome measures at baseline, a similar degree of improvement in the discharge scores was found in all of the primary outcome measures. Conclusion: This study suggests that activity-based therapies using an arm ergometer or robot when used over shortened training periods have the same effect as OT group therapy in decreasing impairment and improving disability in the paretic arm of severely affected stroke patients in the subacute phase.

This article reviews the randomized control trials (RCT's) that were published in 2007 of emerging pharmacotherapies in patients with acute (/= 2 weeks), sub-acute (2 to 12 weeks) and chronic (/= 12 weeks) stroke. A Medline search generated 22 RCT's in stroke in the year 2007 in the English language. These trials were primarily efficacy studies. These included the role of statins (an anti-lipid agent) in reducing post-stroke morbidity and mortality, and decreasing the carotid atherosclerotic plaque in middle aged patients at increased risk of cardiovascular disease; glucose-potassium-insulin infusion in hyperglyceamic acute stroke patients; pioglitazone (an anti-diabetic medication) to reduce recurrence of stroke in Type 2 diabetic patients; administration of intra-arterial urokinase (a thrombolytic agent) and the role of laser therapy in clot dissolution given that at present there is only one FDA approved thrombolytic agent (r TPA); benefit of warfarin (an anticoagulant) in elderly patients with atrial fibrillation in the community; NXY (a free radical trapping agent) and minocycline both tested as neuroprotectants; and zoledronate (an intravenous bisphosphonate) to prevent loss of bone mineral density of the affected extremity, and finally the role of nicardipine (a Calcium channel blocker) in the prevention of vasospasm, and hydrocortisone to prevent hyponatraemia after sub-arachnoid hemorrhage. Finally the role of non-pharmacotherapy like stents for patient's with internal carotid artery dissection with tandem internal carotid and middle cerebral artery occlusion and in vertebral artery stenosis.

Falls are a common occurrence in stroke patients admitted to a rehabilitation unit. This study evaluates the effect of current fall risk screening and prevention strategies on the number of falls in stroke patients admitted to an acute rehabilitation facility. All stroke patients admitted to a designated acute rehabilitation unit were considered at risk for falls by virtue of their diagnosis. These patients were studied retrospectively during a 24-month period. Wheelchair lap belts and bedrails were provided to all patients admitted to the unit. Further measures consisting of bed and chair alarms, enclosed beds, and placement in rooms close to the nursing station were implemented with high-fall risk patients. One hundred seventeen (15.5%) of the 754 patients in this study fell. The fall index rate was 8.2 falls, based on patient care days during the study period. No injury was observed in 143 of 159 fall cases (90%). In 13 cases (8%) there were only minor injuries, and 3 falls (2%) resulted in serious injuries. Current preventive strategies decrease the number of falls and the severity of fall-related injuries. Patients who do fall are cognitively impaired on admission and have lower ambulation speed.

Rabadi MH, Rabadi FM, Edelstein L, Peterson M. Cognitively impaired stroke patients do benefit from admission to an acute rehabilitation unit. Objective To determine whether cognitively impaired stroke patients benefit (defined as having an improved level of functional independence and capable of being discharged home) from admission to an acute rehabilitation unit. Design Retrospective analysis of data from a historical cohort of patients with acute stroke within the last 4 weeks or less. Setting Acute stroke rehabilitation unit. Participants The study sample was divided into 4 distinct groups based on admission Mini-Mental State Examination (MMSE) scores: cognitively intact (MMSE score range, ≥25 points), mild cognitive impairment (MMSE score range, 21–24), moderate cognitive impairment (MMSE score range, 10–20), and severe cognitive impairment (MMSE score range, ≤9 points). Interventions Not applicable. Main Outcome Measures Primary outcome measures were: change in total FIM instrument score, cognitive FIM subscore, length of stay (LOS), FIM efficiency, and discharge disposition (home vs not-to-home). Results Based on the MMSE cut scores, there were 233 cognitively intact patients and 435 cognitively impaired (mild, n=139; moderate, n=165; severe, n=131) patients. The cognitively intact and the 3 cognitively impaired groups were similar in age, sex, and ethnicity. The data show that the 3 cognitively impaired groups of patients had delayed onset to acute rehabilitation admission and greater stroke severity and disability. The change in FIM total score and FIM efficiency was similar between the cognitively intact and the 3 cognitively impaired groups ( P =.058). There were, however, statistically significant changes in the FIM cognitive subscore favoring the cognitively impaired groups ( P P P Conclusions Our results suggest that despite severe neurologic impairment(s) and disability, cognitively impaired stroke patients make significant functional gains while undergoing rehabilitation and many can be discharged home. Based on these results, stroke patients with cognitive impairments benefit from rehabilitation and should be given the same access to acute rehabilitation services as stroke patients who are cognitively intact.

The effect of vitamin C supplementation on functional recovery after stroke has not been studied. Method: In this retrospective, case-control study of 23 patients with ischemic stroke taking vitamin C were identified and matched for age, sex, onset to admission, and admission total functional independence measure (TFIM) with 23 patients with ischemic stroke not taking Vitamin C supplementation. Vitamin C 1000 mg daily was prescribed on admission to our unit mainly to patients who were undernourished (defined as significant weight loss and/or 90% or less ideal body weight for age and sex) and those with pressure sores. The outcome measures were: change in the TFIM, FIM-Cognition (FIM-Cog), and FIM-Motor sub-scores, discharge disposition, and length of stay (LOS).Results: The change in TFIM (20 ± 13 standard deviation [SD] vs. 26 ± 6, p = 0.20), FIM-Cog (3 ± 3 SD vs. 4 ± 5, p = 0.41), FIM-Motor (15 ± 11 SD vs. 20 ± 13, p = 0.21) sub-scores were less in the vitamin C treated group, but these differences did not reach statistical significance. Similarly, no significant differences were found in LOS (21 ± 9 SD vs. 23 ± 9, p = 0.59), and discharge disposition (home/institution) (9/10 vs. 13/9, p = 0.60) between the vitamin C and the control groups.Conclusion: This study suggests vitamin C supplementation did not enhance functional recovery in undernourished ischemic stroke patients.Keywords: vitamin C; ischemic stroke; functional recovery

This observational study: (a) compared serum creatinine (estimated glomerular filtration rate (EGFR)) to renal isotopeObservational study.VA Medical Center affiliated with Oklahoma University.Veterans with SCI and regularly followed in SCI clinic. Demographic and clinical data, as well as, EGFR, GFR, blood urea nitrogen (BUN) and serum creatinine levels, and presence of UTI, renal and urinary bladder lesions on renal nuclear scan, renal ultrasound, and cystoscopy studies were recorded.None.Urological lesions, UTI, and Mortality.For 161 patients with SCI and NGB the mean ± SD for EGFR was 104 ± 36 and 84 ± 32 for GFR. EGFR and GFR were positively correlated (r = 0.34, P = 0.015). GFR was significantly (P 0.05) more sensitive and specific in determining renal functional state. Neither measures were significant indicator for UTI, renal or urological lesions; GFR was a significant predictor of risk of death (1.2 times increase in risk per 10 unit drop in GFR) even after adjusting for age (P = 0.040).While GFR and EGFR are comparable measures of glomerular filtration, GFR was a more informative measure of renal functional state and risk of mortality than EGFR. Neither method predicted the presence of UTI related renal or urological lesions.

To assess the relative responsiveness of 2 commonly used upper-extremity motor scales, the Action Research Arm Test (ARAT) and the Fugl-Meyer Assessment (FMA), in evaluating recovery of upper-extremity function after an acute stroke in patients undergoing inpatient rehabilitation.Prospective.An acute stroke rehabilitation unit.One hundred four consecutive admissions (43 men, 61 women; mean age +/- standard deviation, 72+/-13y) to a rehabilitation unit 16+/-9 days after acute stroke.Not applicable.The following assessments were completed within 72 hours of admission and 24 hours before discharge: ARAT, upper-extremity motor domain of the FMA, National Institutes of Health Stroke Scale, FIM instrument total score, and FIM activities of daily living (FIM-ADL) subscore.The Spearman rank correlation statistic indicated that the 2 upper-limb motor scales (ARAT, FMA) correlated highly with one another, both on admission (rho = .77, P.001) and on discharge (rho = .87, P.001). The mean change in score from admission to discharge was 10+/-15 for the ARAT and 10+/-13 for the FMA motor score. The responsiveness to change as measured by the standard response mean was .68 for the ARAT and .74 for the FMA motor score. The Spearman rank correlation of each upper-limb motor scale with the FIM-ADL at the time of admission was as follows: ARAT, rho equal to .32 (P.001) and FMA motor score, rho equal to .54 (P.001).Both the FMA motor score and the ARAT were equally sensitive to change during inpatient acute rehabilitation and could be routinely used to measure recovery of upper-extremity motor function.

Randomized clinical stroke trials published during 2004 dealt primarily with prevention of strokes by reducing risk factors. The usefulness of innovative versions of widely known treatment modalities was documented. These included angiotensin-converting enzyme inhibitors against hypertension, acarabose against diabetes, and the antiplatelet agent triflusal instead of aspirin. A large British study confirmed the value of treatment with simvastatin. Appropriately powered studies found no benefit for stroke prevention of either vitamin treatment to lower homocysteine or hormonal replacement in post-menopausal women. The circumstances under which antithrombotic, anticoagulant and surgical treatments of acute ischemic stroke are appropriate were further specified.

Brainstem and cerebellar lesions are commonly associated with disorders of extraocular movements and these have localising value. The dorsal midbrain syndrome is characterised by (1) impaired voluntary vertical eye movements, (2) light-near dissociation of the pupillary response (pseudo-Argyll Robertson pupils), (3) convergence nystagmus on attempted upward gaze, (4) lid retraction (Collier sign) and (5) skew deviation.1 ,2 This syndrome is usually produced by pressure on the midbrain tectum.2 In this syndrome, the mesencephalic reticular formation that includes the rostral interstitial nucleus of medial longitudinal (riMLF) fasciculus and its connection with the interstitial N of Cajal and Darkschewitsch and the posterior commisure are involved. A 57-year-old woman presented with a rapid onset of headache, vomiting, and confusion. She had a temperature of 103° Fahrenheit and was delirious. Brain magnetic resonance imaging (MRI) showed high signal intensity in the right temporal, bilateral orbito-frontal lobes, and left temporal pole on T2-weighted imaging (Figs. 1 and ​and2).2). Cerebral spinal fluid (CSF) demonstrated lymphocytic pleocytosis with a red blood cell count of 45 cells/mm3. Intravenous acyclovir was started, and CSF polymerase chain reaction (PCR) subsequently returned positive for herpes simplex virus (HSV). Figure 1. Brain MRI is the diagnostic test of choice in herpes simplex encephalitis (HSE) especially in the early course of the disease, with a sensitivity of > 90 %, 1–3 compared to a sensitivity of 50 % with computed tomography.4 HSV PCR of the CSF is often negative in the initial 72 hours of illness.5 Characteristic MRI findings involve the medial temporal lobes, cingulate gyrus, and insular cortex, with gyral edema.6 The International Herpes Management Forum recommends that all patients with HSE receive intravenous acyclovir 10 mg/kg every 8 hours for 14–21 days. Due to the high morbidity and mortality of untreated HSE, acyclovir should not be withheld if diagnostic test results are delayed.7,8

BACKGROUND There is paucity of data concerning the self-perceptions of health status and health-related quality of life (HRQoL) of veterans with multiple chronic medical conditions. MATERIAL AND METHODS Veterans who attended an out-patient power wheelchair clinic at a tertiary VA Medical Center were assessed. Health status and HRQoL were measured by using the EuroQol (EQ-5D) questionnaire. The EuroQol (ED-5D) visual analogue scale (EQ-5DVAS) measured their health state, and average values of the (EQ-5D) questionnaire for mobility, self-care, usual activities, pain or discomfort, and anxiety or depression (EQ-5Dprofile), and the EQ-5Dutility measured their HRQoL. RESULTS Of the 170 veterans who attended the out-patient clinic, the mean (±SD) age was 69.6±10.7 years, male/female ratio was 163/7, and 88% were non-Hispanic whites. Fifty-four percent were retired, 39% had a registered disabled, and only 3% were employed. Thirty-three percent were current smokers. More than 64% of the veterans had 4 or more co-morbid conditions for which they were receiving treatment. The mean (±SD) initial EQ-5Dprofile, EQ-5Dutility, and EQ-5DVAS scores were 10.3±1.5, 0.75±0.05 and 45.3±18.9, respectively. The social-demographic variables studied (age, gender, education, marital status, employment, co-morbid conditions, and current smoking history) were only able to predict the mobility and anxiety/depression domains of the EQ-5D. CONCLUSIONS Veterans who considered themselves disabled had multiple chronic medical conditions. Age, employment state, and number of chronic medical conditions were associated with poor health state and HRQoL. No relationship was found between health state and HRQoL in this sample of veterans.

A 60-year-old man with hypertension, hyperlipidemia, and a prior abdominal aortic aneurysm repair in 1995 presented with sudden weakness of both legs. His blood pressure was 91/56 mmHg, his pulse was 107 beats/min, and he had flaccid paraplegia with a spinal sensory level at T6. Magnetic resonance imaging (MRI) of the thoracic spinal cord showed no lesion. Chest and abdominal computed tomography (CT) with contrast showed a large Type B dissecting aortic aneurysm (Figs. 1 and ​and2).2). The following day, he developed acute renal failure requiring hemodialysis, as the dissection extended into his right renal artery. The patient was medically managed and died one week later. Acute aortic dissection (AD) usually presents in hypertensive males over the age of 60 years, as a sudden tearing chest pain radiating to the back.1 Paraplegia due to spinal cord ischemia and infarction as a presenting manifestation of AD has been found in 1.9 % cases.2 However, painless paraplegia, as in this case, is exceedingly rare and limited to six case reports.3–8 Paralysis in AD is due to anterior spinal artery syndrome, which usually includes loss of pain and temperature sensation, with preservation of vibration and proprioception sensation. This pattern typically manifests after spinal shock resolves over weeks.

The overall goal of this observational study was to determine the type and prevalence of colonoscopic lesions encountered in veterans with traumatic spinal cord injury (SCI) and to examine their relationship to lesion level, completeness, and duration postinjury. We retrospectively reviewed the electronic charts of veterans with SCI who are regularly followed in our SCI clinic. Colonoscopy in veterans with SCI was undertaken for their gastrointestinal (GI) complaints. Of the 87 veterans with SCI, 71 who were 50 years of age or older were included in this study. Of these 71, 28 underwent colonoscopies (39.4%). Demographic variables were matched between patients who underwent colonoscopies and those who did not for age, sex, race/ethnicity, age of onset of SCI, and duration of SCI (p > 0.05). The colonoscopic lesions seen in 26 (93%) of the 28 veterans with SCI who underwent colonoscopies included diverticulae, internal hemorrhoids, and polyps. No relationship was found between colonoscopic lesion type and SCI lesion location or severity. A relationship was found between total colonoscopic lesions and duration of SCI (p < 0.001). Age-, sex-, and race/ethnicity-matched veterans without SCI (controls) who underwent colonoscopy for their GI complaints had more colonoscopic lesions than the veterans with SCI who underwent colonoscopies (p < 0.001).

Objective Neurologic manifestations of copper deficiency in adults are increasingly recognized. We sought to determine if copper supplementation over a period of 12 months would improve functional activities of daily living (ADLs) in patients with copper deficiency. Methods We studied 15 consecutively diagnosed patients with copper deficiency that received 12 months of copper supplementation. Functional ADLs were evaluated by the Barthel Index (BI), administered at the time of diagnosis and repeated after 12 months of copper supplementation. Results BI scores were significantly higher after 12 months of continued supplementation when compared with initial scores [(mean ± SD), 74 ± 11.7 versus 83 ± 13.2, P = 0.007, paired t-test]. A significant inverse linear correlation between the duration of symptoms before treatment and the change in the BI scores was noted (P = 0.005). Conclusions Early initiation and sustained supplementation with copper for at least 12 months may improve functional ADLs in patients with copper deficiency.

Background and Purpose— Current guidelines for commercial motor vehicle (CMV) drivers to return to driving after a TIA or stroke was last reviewed in 1988. Updated recommendations are presented based on recent evidence. Methods— A 3-member medical expert panel chosen by the U.S. Department of Transportation’s Federal Motor Carrier Safety Administration used findings of systematic literature review up to January 2008 to provide updated recommendations for return to driving for CMV drivers after TIA or stroke. Results— Evidence from the systematic review showed that TIA patients are at increased risk for stroke, and stroke patients are at increased risk for motor vehicle crash. Although no studies provide direct evidence of predictability of crash risk of CMV drivers using outcomes of neuropsychological assessments, there is evidence that certain neuropsychological tests can identify on-road driving performance after stroke. Conclusions— The medical expert panel recommended driving cessation for 1 year after a TIA or stroke, and return to driving a CMV should be allowed only after successful completion of a comprehensive neurological evaluation, neuropsychological assessments, and on-road testing.

Poor nutrition is a common complication of strokes severe enough to require inpatient rehabilitation. We therefore tested whether intensive nutritional supplements given to undernourished patients from the time of their admission to a specialized stroke rehabilitation service would improve patient outcomes.Randomized, prospective, double-blind, single center study comparing intensive nutritional supplementation to routine nutritional supplementation in 116 undernourished patients admitted to a stroke service. The analysis included the 90% of patients who were not lost to follow-up due to acute or subacute hospitalization (n = 102; 51 in each group). The nutritional supplements are commercially available and Food and Drug Administration approved. The primary outcome variable was change in total score on the Functional Independence Measure (FIM). The secondary outcome measurements included the FIM motor and cognitive subscores, length of stay (taken from day of admission), 2-minute and 6-minute timed walk tests measured at admission and on discharge, and discharge disposition (home/not home).Patients receiving intensive nutritional supplementation improved more than those on standard nutritional supplements on measures of motor function (total FIM, FIM motor subscore, 2-minute and 6-minute timed walk tests, all significant at p0.002). They did not, however, improve on measures of cognition (FIM cognition score). A higher proportion of patients who received the intensive nutritional supplementation went home compared to those on standard supplementation (p = 0.05).Intensive nutritional supplementation, using readily available commercial preparations, improves motor recovery in previously undernourished patients receiving intensive in-patient rehabilitation after stroke.
