Background and Aims: Dental environment is a stressful place for children. In many cases, parents transmit their anxiety to their children in the first dental visit. Mothers as the closest person to the children may have important influence on their children’s behavior in dentistry. The purpose of this study was to evaluate the effectiveness of an educational pamphlet for mothers on children’s behavior in the first dental visit. Materials and Methods: This study was conducted on 238 mothers who had 3-7 years old children undergoing dental treatment for the first time. The participants were divided into interventional and control group randomly. Behavior of children was assessed by questionnaire according to 4 point scale of Frankle. The questionnaire contained 5 parts: separation of child from the mother, entering the dental room, opening the mouth, communicating with dentist, getting the prize. Data were analyzed using Chi-square test. Results: The educational pamphlet was effective on the separation of child from the mother, the cooperation of children in interventional group was better than the control group (P=0.006). In interventional group 96.2% of and in the control group 90.8% of children had good and very good behavior but the difference was not significant (P=0.11). Conclusion: Although the educational pamphlet was effective on the separation of child from mother. No significant difference in behavior of 3-7 year-old children between interventional and control group was found.

Objectives: Pulpal inflammation is often associated with odontogenic pain. Dental pulp is abundantly innervated with sensory fibers encompassing neuropeptides. Neurokinin A (NKA) and substance P (SP) are important neuropeptides in the dental pulp that can cause neurogenic inflammation. Since no previous study has assessed dental pulp neuropeptides in children, this study aimed to compare the level of NKA and SP in gingival crevicular fluid (GCF) of sound and symptomatic carious primary teeth. Materials and Methods: Samples of GCF were obtained of 20 sound and 20 painful carious primary teeth. Enzyme-linked immunosorbent assay (ELISA) was used to quantify neuropeptides in GCF. Data were analyzed using paired t-test, ANOVA, Kolmogorov-Smirnov test and correlation coefficient test. Results: A significant difference was noted in the level of NKA in GCF of painful and sound teeth (2.23 pg/ml in painful, and 1.84 pg/ml in sound teeth, P0.05). Conclusions: The results showed that the level of NKA and SP was higher in GCF of painful teeth compared to that of sound teeth. This difference was statistically significant with regard to NKA. Thus, these neuropeptides can serve as indicators for pathological activities in teeth with symptomatic irreversible pulpitis.

Objectives: It is not clear what type of pre-crimped crown is more successful in achieving greater marginal adaptation following re-crimping. This study aimed to assess the changes in the circumference of 3M ESPE and MIB pre-crimped stainless steel crowns (SSCs) for the primary maxillary and mandibular first and second molars following re-crimping. Materials and Methods: This was an in-vitro, experimental study. Initial photographs were obtained from the margins of 3M and MIB SSCs for the upper and lower primary molars using a digital camera. Crown margins were crimped by applying 0.2N force using 114 and 137 pliers. Post-crimping photographs were also obtained and the changes in crown circumference after crimping were calculated using AutoCad software. The percentage of reduction in the circumference of crowns for each tooth was statistically analyzed based on the type of crown using student t-test. The effect of crown design and the associated teeth on the decreased circumference percentage was statistically analyzed by two-sided ANOVA. Results: The percentage of reduction in lower E SSC circumference was 3.71±0.39% in MIB and 6.29±0.62% in 3M crowns. These values were 3.55±0.55% and 7.15±1.13% for the lower Ds, and 3.95±0.43 and 6.24±0.85% for the upper Ds, respectively. For the upper Es, these values were found to be 3.12±0.65% and 5.14±0.94%, respectively. For each tooth, a significant difference was found between MIB and 3M SSCs in terms of the percentage of reduction in crown circumference following crimping. Homogeneously staining regions (HSR) or double minute chromosomes (dmin) are autonomously replicating extra-chromosomal elements that are frequently associated with gene amplification in a variety of cancers. The diagnosis of leukemia patients was based on characterization of the leukemic cells obtained from bone marrow cytogenetics. This study report two cases, one with Acute Myeloblastic Leukemia without maturation (AML-M1), aged 23-year-old female, and the other with chronic myelogenous leukemia (CML)-blast crisis, a 28-year-old female associated with double minute chromosomes. Most cases of acute myeloid leukemia with dmin in the literature (including our cases) have been diagnosed as having acute myeloid leukemia.

Amelogenesis Imperfecta (AI) is a disorder of tooth development where there is an abnormal formation of enamel or the external layer of teeth. The aim of this study was to screen mutations in the four most important candidate genes, ENAM, KLK4, MMP20 and FAM83H responsible for amelogenesis imperfect.Geneomic DNA was isolated from five Iranian families with 22 members affected with enamel malformations. The PCR amplifications were typically carried out for amplification the coding regions for AI patients and unaffected family members. The PCR products were subjected to direct sequencing. The pedigree analysis was performed using Cyrillic software.One family had four affected members with autosomal dominant hypocalcified amelogenesis imperfecta (ADHPCAI); pedigree analysis revealed four consanguineous families with 18 patients with autosomal recessive hypoplastic amelogenesis imperfecta (ARHPAI). One non-synonymous single-nucleotide substitution, c.1150T>A, p. Ser 342Thr was identified in the FAM83H, which resulted in ADHCAI. Furthermore, different polymorphisms or unclassified variants were detected in MMP20, ENAM and KLK4.Our results are consistent with other studies and provide further evidence for pathogenic mutations of FAM83H gene. These findings suggest different loci and genes could be implicated in the pathogenesis of AI.

Central anticholinergic syndrome (CAS) following general anesthesia (GA) is a well known syndrome in children and adults. Many cases of CAS have been previously reported in the literature. However, there are only two reports of post resuscitation CAS after administration of small doses of atropine. Hereby, we report a case of CAS in a child undergoing complete dental restoration under GA after receiving a small dose of atropine to reverse hypoxia induced bradycardia. Intraoperative events such as hypoxia or cardiac arrest may play a role as triggers for CAS. However, we cannot establish a causal relationship between the occurrence of CAS and such critical events.

Background and Aim: Oropharyngeal cancer is a significant cause of death and disability worldwide. In Iran it is the 13th and 17th most common cancer found in men and women, respectively. The aim of this study was to investigate etiological factors and clinical features of lesions related to oropharyngeal cancers over a 5-year period in Tehran. Materials and Methods: In this retrospective cross-sectional descriptive study, 256 cases were selected from 5 clinical and educational cancer centres. Study forms were completed by collecting information from medical records; incomplete files were filled in by contacting patients by phone. The International Classification of Disease for Oncology coding system (ICD-O) was used to determine the type of malignant lesion, and SPSS software was used for data analysis. Results: Squamous cell carcinoma (SCC) was the most commonly identified lesion (70.3%). The most common site of malignancy in the mouth was the tongue (25.8%). The ratio of disease incidence in men compared w th women was almost 2:1. The mean age of patients was 60 years. Alcohol, drug and tobacco consumption were the most important factors in the development of oropharyngeal cancer. Conclusion: The results of this study demonstrated that the most common malignancy in the mouth and pharynx was SCC. As with previous studies, we found that the ratio of disease occurrence was almost 2:1 for men compared with women. Referral of patients to specialized centres will make considerable progress towards controlling and managing these cancers.

Primary antibody deficiencies (PAD) are a group of immune system disorders, associated with decreased levels of secretory and protective immunoglobulins. Because of the important role of immunoglobulins in the protection of oral cavity, patients with PADs are more susceptible to dental caries or oral manifestations. This study was performed to investigate the oral and dental manifestations of PADs patients. In this study, 33 patients with PADs (21 common variable immunodeficiency, 8 X- linked agammaglobulinemia and 4 hyper IgM syndrome) and 66 controls were examined; the number of decayed, missed and filled teeth (DMFT) were investigated. Aphthous was the most frequent manifestation in PADs patients (38.7%), which wassignificantly16.7% higher than the controls (p=0.03). The patients with PADs showed significantly higher presentation of other oral and dental manifestations, including herpes sores, candidiasis tonsillitis, gingivitis, calculus, enamel hypoplasia and other ulcerations.
