Abstract

Background Survival is a key metric of the effectiveness of a health system in managing cancer. We set out to provide a comprehensive examination of worldwide variation and trends in survival from brain tumors in adults, by histology. Methods We analyzed individual data for adults (15–99 years) diagnosed with a brain tumor (ICD-O-3 topography code C71) during 2000–2014, regardless of tumor behavior. Data underwent a 3-phase quality control as part of CONCORD-3. We estimated net survival for 11 histology groups, using the unbiased nonparametric Pohar Perme estimator. Results The study included 556,237 adults. In 2010–2014, the global range in age-standardized 5-year net survival for the most common sub-types was broad: in the range 20%–38% for diffuse and anaplastic astrocytoma, from 4% to 17% for glioblastoma, and between 32% and 69% for oligodendroglioma. For patients with glioblastoma, the largest gains in survival occurred between 2000–2004 and 2005–2009. These improvements were more noticeable among adults diagnosed aged 40–70 years than among younger adults. Conclusions To the best of our knowledge, this study provides the largest account to date of global trends in population-based survival for brain tumors by histology in adults. We have highlighted remarkable gains in 5-year survival from glioblastoma since 2005, providing large-scale empirical evidence on the uptake of chemoradiation at population level. Worldwide, survival improvements have been extensive, but some countries still lag behind. Our findings may help clinicians involved in national and international tumor pathway boards to promote initiatives aimed at more extensive implementation of clinical guidelines.

Abstract

Introduction Tumors of the central nervous system are among the leading causes of cancer-related death in children. Population-based cancer survival reflects the overall effectiveness of a health care system in managing cancer. Inequity in access to care world-wide may result in survival disparities. Methods We considered children (0–14 years) diagnosed with a brain tumor during 2000–2014, regardless of tumor behavior. Data underwent a rigorous, three-phase quality control as part of CONCORD-3. We implemented a revised version of the International Classification of Childhood Cancer (third edition) to control for under-registration of non-malignant astrocytic tumors. We estimated net survival using the unbiased nonparametric Pohar Perme estimator. Results The study included 67,776 children. We estimated survival for 12 histology groups, each based on relevant ICD-O-3 codes. Age-standardized 5-year net survival for low-grade astrocytoma ranged between 84% and 100% world-wide during 2000–2014. In most countries, 5-year survival was 90% or more during 2000–2004, 2005–2009, and 2010–2014. Global variation in survival for medulloblastoma was much wider, with age-standardized 5-year net survival between 47% and 86% for children diagnosed during 2010–2014. Conclusions To the best of our knowledge, this study provides the largest account to date of global trends in population-based survival for brain tumors in children, by histology. We devised an enhanced version of ICCC-3 to account for differences in cancer registration practices world-wide. Our findings may have public health implications, because low-grade glioma is 1 of the 6 index childhood cancers included by WHO in the Global Initiative for Childhood Cancer.

12. PD-computed torque control for an autonomous airship

Abstract

For the trajectory following problem of an airship, the standard computed torque control law is shown to be robust with respect to unknown dynamics by judiciously choosing the feedback gains and the estimates of the nonlinear dynamics. In the first part of this paper, kinematics and dynamics modeling of the airships is presented. Euler angles and parameters are used in the formulation of this model and the technique of Computed Torque control is introduced. In the second part of the paper, we develop a methodology of control that allows the airship to accomplish a prospecting mission of an environment, as the follow-up of a trajectory by the simulation who results show that Computed Torque control method is suitable for airships.

Abstract

© 2018 Elsevier Ltd Background: In 2015, the second cycle of the CONCORD programme established global surveillance of cancer survival as a metric of the effectiveness of health systems and to inform global policy on cancer control. CONCORD-3 updates the worldwide surveillance of cancer survival to 2014. Methods: CONCORD-3 includes individual records for 37·5 million patients diagnosed with cancer during the 15-year period 2000–14. Data were provided by 322 population-based cancer registries in 71 countries and territories, 47 of which provided data with 100% population coverage. The study includes 18 cancers or groups of cancers: oesophagus, stomach, colon, rectum, liver, pancreas, lung, breast (women), cervix, ovary, prostate, and melanoma of the skin in adults, and brain tumours, leukaemias, and lymphomas in both adults and children. Standardised quality control procedures were applied; errors were rectified by the registry concerned. We estimated 5-year net survival. Estimates were age-standardised with the International Cancer Survival Standard weights. Findings: For most cancers, 5-year net survival remains among the highest in the world in the USA and Canada, in Australia and New Zealand, and in Finland, Iceland, Norway, and Sweden. For many cancers, Denmark is closing the survival gap with the other Nordic countries. Survival trends are generally increasing, even for some of the more lethal cancers: in some countries, survival has increased by up to 5% for cancers of the liver, pancreas, and lung. For women diagnosed during 2010–14, 5-year survival for breast cancer is now 89·5% in Australia and 90·2% in the USA, but international differences remain very wide, with levels as low as 66·1% in India. For gastrointestinal cancers, the highest levels of 5-year survival are seen in southeast Asia: in South Korea for cancers of the stomach (68·9%), colon (71·8%), and rectum (71·1%); in Japan for oesophageal cancer (36·0%); and in Taiwan for liver cancer (27·9%). By contrast, in the same world region, survival is generally lower than elsewhere for melanoma of the skin (59·9% in South Korea, 52·1% in Taiwan, and 49·6% in China), and for both lymphoid malignancies (52·5%, 50·5%, and 38·3%) and myeloid malignancies (45·9%, 33·4%, and 24·8%). For children diagnosed during 2010–14, 5-year survival for acute lymphoblastic leukaemia ranged from 49·8% in Ecuador to 95·2% in Finland. 5-year survival from brain tumours in children is higher than for adults but the global range is very wide (from 28·9% in Brazil to nearly 80% in Sweden and Denmark). Interpretation: The CONCORD programme enables timely comparisons of the overall effectiveness of health systems in providing care for 18 cancers that collectively represent 75% of all cancers diagnosed worldwide every year. It contributes to the evidence base for global policy on cancer control. Since 2017, the Organisation for Economic Co-operation and Development has used findings from the CONCORD programme as the official benchmark of cancer survival, among their indicators of the quality of health care in 48 countries worldwide. Governments must recognise population-based cancer registries as key policy tools that can be used to evaluate both the impact of cancer prevention strategies and the effectiveness of health systems for all patients diagnosed with cancer. Abstract

© 2018 Elsevier Ltd Background: In 2015, the second cycle of the CONCORD programme established global surveillance of cancer survival as a metric of the effectiveness of health systems and to inform global policy on cancer control. CONCORD-3 updates the worldwide surveillance of cancer survival to 2014. Methods: CONCORD-3 includes individual records for 37·5 million patients diagnosed with cancer during the 15-year period 2000–14. Data were provided by 322 population-based cancer registries in 71 countries and territories, 47 of which provided data with 100% population coverage. The study includes 18 cancers or groups of cancers: oesophagus, stomach, colon, rectum, liver, pancreas, lung, breast (women), cervix, ovary, prostate, and melanoma of the skin in adults, and brain tumours, leukaemias, and lymphomas in both adults and children. Standardised quality control procedures were applied; errors were rectified by the registry concerned. We estimated 5-year net survival. Estimates were age-standardised with the International Cancer Survival Standard weights. Findings: For most cancers, 5-year net survival remains among the highest in the world in the USA and Canada, in Australia and New Zealand, and in Finland, Iceland, Norway, and Sweden. For many cancers, Denmark is closing the survival gap with the other Nordic countries. Survival trends are generally increasing, even for some of the more lethal cancers: in some countries, survival has increased by up to 5% for cancers of the liver, pancreas, and lung. For women diagnosed during 2010–14, 5-year survival for breast cancer is now 89·5% in Australia and 90·2% in the USA, but international differences remain very wide, with levels as low as 66·1% in India. For gastrointestinal cancers, the highest levels of 5-year survival are seen in southeast Asia: in South Korea for cancers of the stomach (68·9%), colon (71·8%), and rectum (71·1%); in Japan for oesophageal cancer (36·0%); and in Taiwan for liver cancer (27·9%). Abstract

In this paper, we present the result of nonlinear control technique applied to an autonomous quad-rotor helicopter called X4-flyer. By using Backstepping procedure, we can stabilize the engine in hovering and generate its trajectory. Moreover, for rejecting external load disturbance, two controls are presented, PI-Backstepping which is the combination between PI and Backstepping into the so-called Integral Backstepping, and the second is Backstepping with an estimator. We studied the robustness of the used controllers in the presence of disturbances. We presented the case of the disturbances with wind influence. Abstract

Introduction: Clinical Reasoning Learning (CRL) is one of the most important methods of active learning in medicine. This instruction is designed for small groups of students., Objectives and Methods: We conducted a prospective descriptive study with the primary goal of adapting this instruction for large groups of students. The secondary objective was to compare two methods of adapting this instruction (fish bowl technique and whole-group participation), and the comparison was conducted using an evaluation questionnaire with a Likert scale., Results: Our sample included 130 students, divided into 2 groups: the Fish bowl CRL group (57 students) and the group with the participation of all students (73 students). Strengthening of prior knowledge, the working atmosphere, assimilation of information, and supervision were the strengths of this instruction. The negatives were related to insufficient time. The analysis found a statistically significant difference in averages between the 2 groups, and this difference pertained to the variables of participation and appreciation of the atmosphere, with a p-value of 0.01 for both., Conclusion: CRL can be adapted for large groups in the form of fish bowl learning. Currently, it is challenging to measure the actual impact of educational activities on the development of clinical reasoning due to the lack of precise measurement instruments. Evaluation questionnaires can be a first step in measuring the impact of these instructions with modified structures. It would be interesting in future studies to create assessment tools for these restructured instructions.

Abstract

Aims: The objective of this work is to evaluate the epidemiological profile of colorectal cancers, histologically proven over a 5-year period (2012-2016) in the Tlemcen region., Methods: A retrospective study of 581 cases of colorectal cancer collected at the epidemiology department of the University Hospital Center (UHC) of Tlemcen between January 2012 and December 2016 was performed. Epidemiological data were processed using SPSS version 25 and Microsoft Excel 2010., Results: The epidemiological profile has shown that colorectal cancer in our region ranks 3 rd in both sexes. There were 322 men (55.4%) affected compared to 259 women (44.6%) with a sex ratio of 1.2. A predominance of males is noted in 50-60 age group, while for the female sex, the dominance is between 60-70 years old. The mean age of CRC occurrence was 60±13 years, with an extremity ranging from16 to 90. A significantly higher rate was recorded for rectal cancer (43.7%) followed by sigmoid colon (5.7%). Variable rates were recorded during the 5 years with a peak in 2014 (27.9%)., Conclusion: Regular analysis of these data, if supplemented with additional data on diagnostic modalities like circular RNA diagnostic, will contribute to the assessment of the impact over time of public policies on nationally organized CRC screening., Competing Interests: The authors declare that they have no conflict of interest., (© 2023 Dahmani B et al.)

Abstract

Introduction: adherence to drugs is critical for achieving the best clinical results in the treatment of chronic diseases. Adherence to chronic drugs might be influenced by beliefs about medications and other variables. The goal of this study was to assess relevant determinants of medication adherence in Algerian population with insulin-dependent diabetes., Methods: from July 1 st 2019 to February 29 th 2020, a cross-sectional study was conducted. Participants who had been on insulin for at least 6 months were recruited from Tlemcen (Algeria) clinics in secondary care settings. Patients were invited to a face-to-face interview, in order to complete out the Morisky Medication Adherence Scale-8 (MMAS) tools to report their attitudes towards medication adherence and views about their insulin. The recruited patients' socio-demographic data was also collected. The related determinants of chronic drug non-adherence in the tested population were identified using a stepwise binary logistical regression model., Results: in this study, 147 patients out of 400 were not adhering to their insulin therapy (36.5%). Insulin non-adherence was linked to single status (AOR=2.088, CI=1.180-3.694), non-insurance (AOR=2.949, CI=1.323-6.572), number of daily insulin injections (AOR=1.269, CI=1.033-1.559), unawareness of the insulin regimen (AOR=3.528, CI=1.453-8.565), hypertension (AOR= 3.497, CI=1.98-6.154) and the non-practice of self-monitoring of blood glucose (SMBG) (AOR=2.635, CI=1.472-4.718)., Conclusion: insulin adherence in Algerians is still well below international standards. Abstract

Purpose: Stomach cancer is a major global health problem; it is one of the ten most common cancers with poor survival, and its incidence is characterized by wide variation. The aim of this work is to carry out a retrospective epidemiological study on gastric cancer in the wilaya of Tlemcen (West Algeria) over a period of 5 years (2011-2015)., Methods: The data set was provided by the cancer registry of the Tlemcen wilaya. The statistical analysis was performed using software SPSS.21., Results: During this period, 394 cases of gastric cancers were collected, including 199 patients with gastric adenocarcinoma (50.50%); gastric cancer was ranked in 5th rank of the ten most answered cancers in the wilaya. The average age of the patients was 60.5 ±  14.208 years (60.618 ±  13.556 men, 56.654 ±  14.8761 women) with an extremity ranging from 18 to 91 years. A predominance of men has been observed with 60.4% against 39.6% of women, with a sex ratio of 1.5 and a significant difference between the two sexes (p = 0.08). Patients over 50 years of age represented the predominant age group (73.4%). The tumor topography was antropyloric in 6.09% of the cases, and the vast majority of the diagnosed cases was in the local stage (17.01%)., Conclusion: Stomach cancer remains one of the top ten cancers in the Tlemcen willaya, so extensive research on the risk factors for gastric cancer remains important to plan effective preventive and curative strategies to reduce this burden.

Abstract

Background: In Algeria, few studies have been conducted to determine the prevalence of genetic and environmental risk factors of cardiovascular diseases as a function of residence. The objective of this study was therefore to determine the prevalence of cardiovascular risk factors according to sex and age in urban and rural communities in the Wilaya of Tlemcen (Algeria)., Material and Methods: A population survey was conducted on a representative sample of 864 individuals aged 20years and over, among inhabitants in urban and rural communes in the Wilaya of Tlemcen. Each subject answered the questionnaire on cardiovascular risk factors, underwent a physical examination and had a blood sample drawn., Results: The prevalence of hypercholesterolemia was 6.6%, it was found to be higher in urban areas (8.4%) than in rural areas (4.6%). The prevalence of low HDL cholesterol levels was higher in urban (28.8%) than rural (23.9%) areas. The prevalence of family history of cardiovascular disease did not differ between the two sexes and between urban (15.7%) and rural (14.0%) areas. The prevalence of hypertension was higher in urban (28.0%) than in rural (16.8%) areas, and was highest among women aged 65 years or older in urban areas (67.3%) and in rural areas (66.6%). The prevalence of diabetes was higher among women living in urban areas (21.4%) compared with rural areas (15.4%). Obesity was much more frequent among women than among men in urban areas (24.7% in women and 9.5% in men) and in rural areas (28.3% in women and 8.3% in men). The prevalence of smoking was 45.8% for men and with no significant variations between urban areas and rural areas., Conclusion: The prevalence of cardiovascular risk factors according to sex and age in the two communes are high in two communes in the Wilaya of Tlemcen. However, the prevalence of hypertension, hypercholesterolemia and diabetes were higher in women in urban than in rural areas. This finding focuses attention on the need for measures to reduce the prevalence of these cardiovascular risk factors in these communities. Abstract

Context: Support for non-Hodgkin's lymphoma (NHL) with large cells that is refractory or relapsed after first-line chemotherapy poses a greater therapeutic problem with bone marrow transplant therapy or when old age is a contra-indication for high-dose chemotherapy, especially among developing countries such as Algeria., Aim: To show that the regimen, including gemcitabine, could be more effective in treating elderly patients with diffuse large B-cell lymphoma (DLBCL) in relapse / refractory, without complete remission, when compared with the ESHAP (etoposide, cisplatine, solumedrol, aracytine) regimen., Materials and Methods: Ninety-six patients in the age group of 60-70 years were volunteers for a prospective randomized single-blind study, carried out for three years. Patients were divided into two groups by the drawing of lots. The first group (GA, n = 48, relapse; n = 27 [56.3%], refractory; n = 21 [43.7%]) received treatment with ESHAP protocol and the second one (GB, n = 48, relapse; n = 28 [58%], refractory; n = 20 [42%]) with GPD (gemcitabine, dexamethasone, cisplatine) protocol., Results: The overall response rates and mean survival at three years were significantly higher among patients subjected to GPD treatment compared with those subjected to ESHAP treatment (63% vs. 55%, P = 0.01 and 20.5% [95% CI 16.5-24.5] vs. 11.8% [8.9-14.6], respectively). Additionally, three-year progression-free and event-free survival rates were 20.5% (16.3-24) and 19.7% (15.9-23.5), respectively, for the GPD regimen and 10.9% (8.2-13.7) and 11.1% (95% CI 8.5-13.7), respectively, for the ESHAP regimen. Moreover, the GPD regimen was associated with improving overall survival (RR=2.02, 95% CI 1.59-2.56; P = 0.000), event-free survival (2.03, 1.64-2.52; P < 0.001) and progression-free survival (1.86, 1.46-2.37; P < 0.001)., Conclusion: In cases of contra-indication for high-dose chemotherapy for elderly patients with DLBCL, without complete remission, the Gemcitabine-based therapy protocol represents a more effective and less toxic than that of ESHAP.

Abstract

Diabetes mellitus has become a major international health problem in recent decades. In this study we report a prevalence of diabetes of 14.2% in a set of 7,656 subjects in urban and rural areas of Tlemcen (in western Algeria), higher among men (20.4%) than women (10.7%). The prevalence of type 2 diabetes (initially non-insulin-dependent diabetes mellitus: NIDDM) was 10.5% and of type 1 diabetes (insulin-dependent diabetes mellitus: IDDM) 3.7%. Overall prevalence was higher in urban (15.3%) than rural (12.9%) areas. More than half of all patients with diabetes had family members with the disease. Estimating the obesity rate according to body mass index (BMI), we found that 56.7% of all men and more than half of all women in urban areas were obese. Degenerative complications were found in 60% of diabetes patients. A policy for tracking, treating and preventing diabetes and obesity is strongly needed.
