Background: Out-of-hours care in the primary care setting is rapidly changing and evolving towards general practitioner 'cooperatives' (GPC). GPCs already exist in the Netherlands, the United Kingdom and Scandinavia, all countries with strong general practice, including gatekeepers' role. This intervention study reports the use and caseload of out-of-hours care before and after implementation of a GPC in a well subscribed region in a country with an open access health care system and no gatekeepers' role for general practice., Methods: We used a prospective before/after interventional study design. The intervention was the implementation of a GPC., Results: One year after the implementation of a GPC, the number of patient contacts in the intervention region significantly increased at the GPC (OR: 1.645; 95% CI: 1.439-1.880), while there were no significant changes in patient contacts at the Emergency Department (ED) or in other regions where a simultaneous registration was performed. Although home visits decreased in all general practitioner registrations, the difference was more pronounced in the intervention region (intervention region: OR: 0.515; 95% CI: 0.411-0.646, other regions: OR: 0.743; 95% CI: 0.608-0.908). At the ED we observed a decrease in the number of trauma cases (OR: 0.789; 95% CI: 0.648-0.960) and of patients who came to hospital by ambulance (OR: 0.687; 95% CI: 0.565-0.836)., Conclusions: One year after its implementation more people seek help at the GPC, while the number of contacts at the ED remains the same. The most prominent changes in caseload are found in the trauma cases. Establishing a GPC in an open health care system, might redirect some patients with particular medical problems to primary care. This could lead to a lowering of costs or a more cost-effective out of hours care, but further research should focus on effective usage to divert patient flows and on quality and outcome of care.

The influence of maternal position on the spread of local anesthetics in low concentration has not been well examined during epidural analgesia for labor. This study was designed to investigate the differences in sensory block, pain relief and incidence of supine hypotensive syndrome between parturients in the left lateral position and in a modified supine position. Sixty-seven parturients were randomly assigned to lie either in the left lateral position (n = 34) or in a modified supine position (n = 33), and received 0.125% bupivacaine 10 mL with epinephrine 1:800000 and sufentanil 7.5 microg. At 20 min parturients in the modified supine position turned to the left lateral position and a second investigator, unaware of the initial position, measured the extent of the sensory block at 20 and 30 min and just before a second epidural injection was requested. More dermatomes were blocked on the dependent side when the dose was injected in the left lateral position (at 20 and 30 min: P < 0.05; before the second epidural injection: P < 0.0005). In the modified supine position the incidence of bilaterally blocked dermatomes T10-L1 was greater at 20 and 30 min (P < 0.05) and the pain on a visual analogue scale was better at 30 min (P < 0.05). Three parturients in the modified supine position had signs and symptoms of supine hypotensive syndrome. We conclude that injecting in the modified supine position results in a more equal spread of local anesthetic and better pain relief.

An anaphylactic reaction is a rare, but severe anaesthetic complication. In this retrospective survey we report on patients with a severe suspected anaphylactic reaction during anaesthesia and the investigation with intradermal skin testing of these suspected anaphylactic reactions. In the patients with an anaphylactic reaction to neuromuscular blocking drugs, the subsequent anaesthetic history was examined. Sixty-five patients suffered a suspected anaphylactic reaction between 1976 and 2001. In 47 patients skin testing was performed and 43 of these patients had positive skin tests: neuromuscular blockings drugs and succinylcholine more specifically, were the most frequently incriminated drugs. After the anaphylactic reaction 19 patients had surgery on 26 occasions with the use of a skin-test-negative neuromuscular blocking drug; no problems occurred. Skin testing proved to be a reliable tool to investigate suspected anaphylactic reactions during anaesthesia and to guide the future use of neuromuscular blocking drugs.

Although theoretical models predict uptake of inhaled anesthetics during closed-circuit anesthesia (CCA), clinical data for most anesthetics are conflicting or non-existent. In addition, the effects of patient characteristics and mode of ventilation on anesthetic uptake are unclear. Forty-one ASA physical status I or II adult patients undergoing a variety of 1-1.5 h surgical procedures were randomly allocated to receive CCA with desflurane or isoflurane with ventilation being either spontaneous or controlled. An end-expired anesthetic concentration of 1.3 minimum alveolar anesthetic concentration (MAC) was maintained by continuous injection of the liquid anesthetic into the circuit using a syringe pump. After an initial 4-min wash-in period, uptake during the first hour of CCA was nearly constant. Uptake was the same whether ventilation was spontaneous or controlled. Patient characteristics (age, height, weight, weight3/4, and body surface area) were comparable between groups and did not correlate with uptake. The virtually constant uptake after wash-in of desflurane and isoflurane contrasts with the square root of time model of Lowe and Ernst. These findings may greatly simplify CCA.

The effects of lumbar epidural anesthesia (LEA) on a previously described test of function of the rectus abdominalis muscle (the RAM-test) were compared with the effects of LEA on the Bromage test of muscle power in the hips and legs in 20 women having elective cesarean sections under LEA using 0.5% bupivacaine with epinephrine 1:200,000. The results showed no statistically significant correlation between the two tests. We conclude that zones of differential somatic motor blockade are present during LEA and that in obstetrics the RAM-test is the more appropriate test for evaluating the effects of LEA on somatic motor function. Use of the RAM-test is suggested in situations where one is interested in motor function of the abdominal wall muscles under LEA.

Successful management of a large bronchopleural fistula in a 3-yr-old child with high frequency jet ventilation (HFJV) is described. Respiratory insufficiency in the child occurred secondary to hemophilus influenza pneumonia. After 7 days of conventional ventilatory support, a bronchopleural fistula occurred with massive lung collapse and subcutaneous and mediastinal emphysema. The child was ventilated with a high frequency jet ventilator for 37 days with resulting healing of the fistula. During ventilatory support with HFJV, no sedation or muscle relaxants were needed. Two problem areas in long-term support in children were discussed, namely, partial tube obstruction because of thick secretion and the need for proper humidification. A significant advantage of HFJV was the ability to superimpose it on spontaneous breathing with elimination of sedation or muscle relaxants.
