Abstract

Limited English proficiency (LEP) is defined as individuals in whom English is not the primary language and who have limited ability to read, speak, write, or understand the English language. Cardiovascular (CV) team members routinely encounter language barriers in their practice. These barriers have a significant impact on the quality of CV care that patients with LEP receive. Despite evidence demonstrating the negative association between language barriers and health disparities, the impact on CV care is insufficiently known. In addition, older adults with CV disease and LEP are facing increasing risk of adverse events when complex medical information is not optimally delivered. Overcoming language barriers in CV care will need a thoughtful approach. Although well recognized, the initial step will be to continue to highlight the importance of language needs identification and appropriate use of professional interpreter services. In parallel, a health system-level approach is essential that describes initiatives and key policies to ensure a high-level quality of care for a growing LEP population. This review aims to present the topic of LEP during the CV care of older adults, for continued awareness along with practical considerations for clinical use and directions for future research.

26. Bioimpedance Spectroscopy-Based Edema Supervision Wearable System for Noninvasive Monitoring of Heart Failure

Abstract

Heart failure (HF) is a complex syndrome in which the heart is unable to pump enough blood containing oxygen and nutrients to meet the body’s demands. HF is the leading cause of hospitalization for patients over 65 years of age. After a patient is diagnosed with HF, the mortality rate is 50% within the first five years. Presently, there are no unanimous diagnostic criteria for HF. Bioimpedance (BI) analysis has been proposed in recent years as a technique to detect one of the main symptoms: changes in body volume due to edema. This research presents a portable device (Volum), capable of performing real-time BI measurements in a low-cost and noninvasive way. The goal is to improve patient monitoring at home to ensure rapid intervention in cases of worsening conditions, either with timely hospitalizations or adjustments to a patient’s usual treatment. Volum is a small, wearable, wireless, lightweight, low-power clinical pilot prototype that takes measurements through four electrodes and sends the data via Bluetooth to an Android device.

Abstract

A Pneumocystis jirovecii is one of the most important microorganisms that cause pneumonia in immunosupressed individuals. The guideline for treatment and prophylaxis of Pneumocystis pneumonia (PcP) is the use of a combination of sulfa drug-containing trimethroprim and sulfamethoxazole. In the absence of a reliable method to culture Pneumocystis, molecular techniques have been developed to detect mutations in the dihydropteroate synthase gene, the target of sulfa drugs, where mutations are related to sulfa resistance in other microorganisms. The presence of dihydropteroate synthase (DHPS) mutations has been described at codon 55 and 57 and found almost around the world. In the current work, we analyzed the most common methods to identify these mutations, their geographical distribution around the world, and their clinical implications. In addition, we describe new emerging DHPS mutations. Other aspects, such as the possibility of transmitting Pneumocystis mutated organisms between susceptible patients is also described, as well as a brief summary of approaches to study these mutations in a heterologous expression system. Abstract

Synthesis, characterization, and biological activity of a new water-soluble Pd(II)-deoxyalliin (S-allyl-L-cysteine) complex are described in this article. Elemental and thermal analysis for the complex are consistent with the formula [Pd([[C.sub.6][H.sub.10]N[O.sub.2]S).sub.2]]. [sup.13]C NMR, [sup.1]H NMR, and IR spectroscopy show coordination of the ligand to Pd(II) through S and N atoms in a square planar geometry. Final residue of the thermal treatment was identified as a mixture of PdO and metallic Pd. Antiproliferative assays using aqueous solutions of the complex against HeLa and TM5 tumor cells showed a pronounced activity of the complex even at low concentrations. After incubation for 24 h, the complex induced cytotoxic effect over HeLa cells when used at concentrations higher than 0.40 mmol/L. At lower concentrations, the complex was nontoxic, indicating its action is probably due to cell cycle arrest, rather than cell death. In agreement with these results, the flow cytometric analysis indicated that after incubation for 24 h at low concentrations of the complex cells are arrested in G0/G1. Abstract

[Purpose] Heart failure (HF) is a chronic, frequent and disabling condition but with a modifiable course and a large potential for improving. The aim of this study was to validate the two available clinical prediction rules for mortality at one year in patients with primo-hospitalization for decompensated HF: PREDICE and AHEAD. The secondary aim was to evaluate in our setting the changes in the clinical pattern of HF in the last decade in patients hospitalized for a first episode of the disease., [Patients and methods] A prospective multicenter cohort study, which included 180 patients hospitalized with “de novo” HF was conducted to validate the PREDICE score. Calibration and discrimination measurements were calculated for the PREDICE model and the PREDICE score (using the validation cohort of the PREDICE) and the AHEAD score (using both the development and the validation cohort of the PREDICE)., [Results] For the PREDICE models, the area under the curve (AUC) was 0.68 (95% confidence interval [CI]: 0.57–0.79) and the calibration slope 0.65 (95% CI: 0.21–1.20). For the PREDICE score AUC was 0.59 (95% CI: 0.47–0.71) and slope 0.42 (95% CI: −0.20–1.17). For the AHEAD score the AUC was 0.68 (95% CI: 0.62–0.73) and slope 1.38 (95% CI: 0.62–0.73) when used the development cohort of PREDICE and the AUC was 0.58 (95% CI: 0.49–0.67), and slope 0.68 (95% CI: −0.06 to 1.47) when used its validation cohort., [Conclusion] The present study shows that the two risk scores available for patients with primo-hospitalization for decompensated HF (PREDICE and AHEAD) are not currently valid for predicting mortality at one-year. In our setting the clinical spectrum of hospitalized patients with new-onset HF has been modified over time. Abstract

A molecular recognition approach to specific immobilization of proteins onto surfaces is introduced. Its application is demonstrated via the preparation of choline- and hydroxyl-functionalized self-assembled monolayers of thiol chains onto thiocarboxylic acid on gold electrodes. The choline-functionalized monolayers exhibited binding affinity towards a chimera protein obtained via the fusion of Beta-galactosidase from Escherichia coli and the choline-binding domain of the (acetylmuramoyl)-L-alanine amidase from Streptococcus pneumonia.

Abstract

A small, wearable, low-weight, and low-power-consumption device for plethysmography capacitive sensing is proposed herein. The device allows carrying out real-time monitoring of leg volume changes in patients suffering from heart failure (HF) conditions. The dynamic of fluid overload in patients with acute HF serves as a prognosis marker for this type of severe disease and, consequently, these patients can benefit from a wearable monitoring system to measure their body volume evolution during and after hospitalization. Our approach is based on contactless capacitive wearable structures implemented by two different sensor realizations located in the ankle: 180°-parallel capacitor plates (two modes of operations are compared, with the patient’s body connected to ground and to the average voltage between plates) and planar-parallel capacitor plates whose overlapped surface varies with the volume of the patient’s leg. Both realizations exhibit good sensitivity to leg volume changes. The acquisition of capacitance values is performed via a simple circuit that achieves notable performance in simulated volume analysis. A preliminary pilot clinical prototype is described as well.
