American women age 65 or over may not pursue mammography screening for breast cancer because they have to pay part of the cost. An analysis of Medicare bills in 1991 and 1992 of 4,110 women 65 and over revealed that only 36.9% took advantage of the screening mammography benefit of Medicare. Medicare pays for most of the cost of screening mammography, but people without supplemental insurance have to pay copayments, deductibles, or balance bills. Balance bills are issued by health care providers who charge above the Medicare amount. Less than 15% of women without supplemental insurance had mammography, compared to 44.7% of women with employer- supplemented insurance, 40.1% with self-purchased supplemental insurance, and 23.9% with Medicaid supplemental insurance.

In this article, the authors evaluate the cost and utilization effects of the SELECT implementations in 11 States. In particular they compare the before-and-after enrollment experiences of Medicare beneficiaries newly enrolled in SELECT plans with the experiences of those newly enrolled in traditional medigap plans. Using Medicare claims data for 1991 through 1994, the authors find that Medicare SELECT increased costs in five States, decreased costs in three States, and had no effect in three States. The Health Insurance Association of America (HIAA) has concluded that a bill before Congress on Medicare supplement insurance, or 'Medigap,' would increase Medigap premiums by at least 24% within 2 years. The bill guarantees that people who cancel Medigap coverage to enroll in a Medicare managed care plan can go back to fee-for-service within a year regardless of changes in health status. The HIAA contends that the law would, in effect, make Medigap policy holders subsidize Medicare managed care.

The Omnibus Reconciliation Act of 1989 provided strong incentives for corporations to use Medigap policies for structuring their retiree health benefits plans. Benefit managers can derive favorable financial benefits by providing employees with the incentive to purchase Medigap policies. By reimbursing employees' Medigap policy premiums, employees can pick the policy that best fits their financial needs and receive the reimbursement as nontaxable income. The firm ceases to act as an insurance company for the retirees and removes medical trends from calculations of post-retiree liabilities.
