Summary: In this paper, an ultra‐low‐power and area‐efficient demodulator circuit for wireless downlink SCM/CWM modulation schemes is proposed. The proposed demodulator offers high performances and allows simultaneous power and downlink data transmission over a single 13.56‐MHz inductive link. The proposed method requires only two carrier cycles per data bit period, and also, a power index of 75% is obtained. Although the proposed demodulator can be applicable to other wireless systems, here, it is intended for implantable medical devices. To prove the performance of the proposed demodulator circuit, it has been designed and post‐layout simulations are provided using a standard 65‐nm CMOS technology and occupies 27 × 32 μm2 of active area. Post‐layout simulation results indicate that with a 1‐V power supply, the proposed circuit achieves an ultra‐low‐power consumption of 8.51 μW at a data rate of 6.78 Mb/s corresponding to an energy efficiency of 1.25 pJ/bit. Lattice structures are a prime candidate for applications in the medical implant industry due to their versatile mechanical behaviour, which can be tailored to meet specific patient needs and reduce stress shielding, while enabling the natural flow of body fluids. In this work, the mechanical properties of metallic lattices made of five different unit cell types, Cubic (C), Truncated Octahedron (TO), Truncated Cubic (TC), Rhombicuboctahedron (RCO), and Rhombitruncated Cuboctahedron (RTCO), were evaluated under uniaxial compression at three different relative densities, 5%, 15%, and 45%. The evaluation was experimental, and it was compared with previous and new finite element simulations. Specimens for the experimental tests were fabricated in stainless steel 316L by laser powder bed fusion, and stress–strain curves were obtained for the different lattices. The combination of the test results with a critical interpretation of the deformation mechanisms allowed us to confirm that two unit cell types, TO and RTCO, are stable for the whole range of relative densities evaluated. The other three unit cells exhibit more unpredictable behaviour, either due to manufacturing defects or limitations, or because their unstable compression behaviour leads to bucking. With the rising demand for medical implants and the dominance of implant-associated failures including infections, extensive research has been prompted into the development of novel biomaterials that can offer desirable characteristics. This study develops and evaluates new titanium-based alloys containing gallium additions with the aim of offering beneficial antibacterial properties while having a reduced stiffness level to minimise the effect of stress shielding when in contact with bone. The focus is on the microstructure, mechanical properties, antimicrobial activity, and cytocompatibility to inform the suitability of the designed alloys as biometals. Novel Ti-33Nb-xGa alloys (x = 3, 5 wt%) were produced via casting followed by homogenisation treatment, where all results were compared to the currently employed alloy Ti-6Al-4V. Optical microscopy, scanning electron microscopy (SEM), and energy dispersive spectroscopy (EDS) results depicted a single beta (β) phase microstructure in both Ga-containing alloys, where Ti-33Nb-5Ga was also dominated by dendritic alpha (α) phase grains in a β-phase matrix. EDS analysis indicated that the α-phase dendrites in Ti-33Nb-5Ga were enriched with titanium, while the β-phase was richer in niobium and gallium elements. Mechanical properties were measured using nanoindentation and microhardness methods, where the Young's modulus for Ti-33Nb-3Ga and Ti-33Nb-5Ga was found to be 75.4 ± 2.4 and 67.2 ± 1.6 GPa, respectively, a significant reduction of 37% and 44% with respect to Ti-6Al-4V. This reduction helps address the disproportionate Young's modulus between titanium implant components and cortical bone. Importantly, both alloys successfully achieved superior antimicrobial properties against Gram-negative P. aeruginosa and Gram-positive S. aureus bacteria. Antibacterial efficacy was noted at up to 90 ± 5% for the 3 wt% alloy and 95 ± 3% for the 5 wt% alloy. These findings signify a substantial enhancement of the antimicrobial performance when compared to Ti-6Al-4V which exhibited very small rates (up to 6.3 ± 1.5%). No cytotoxicity was observed in hGF cell lines over 24 h. Cell morphology and cytoskeleton distribution appeared to depict typical morphology with a prominent nucleus, elongated fibroblastic spindle-shaped morphology, and F-actin filamentous stress fibres in a well-defined structure of parallel bundles along the cellular axis. The study of structural design optimization (SDO) involves techniques such as Topology Optimization (TO), Shape Optimization (SHO), and Size Optimization (SO) that determine specific parameters to achieve the best measurable performance in a defined design space under a given set of loads and constraints. Integration of SDO techniques with AM leads to utmost benefits in designing and fabricating optimized implantable medical devices with enhanced functional performance. Research and development of various lattice structures represents a powerful method for unleashing the full potential of additive manufacturing (AM) technologies in creating medical implants with improved surface roughness, biocompatibility, and mechanical properties. Furthermore, the integration of artificial intelligence (AI) and machine learning (ML) in structural optimization has expanded opportunities to improve device performance, adaptability, and durability. The review is meticulously divided into two main sections, reflecting the predictability of the implant's internal structure: (a) unpredictable interior topology, which explores topology-based optimization techniques, and (b) predictable inner topology, concentrating on lattice structures. The analysis of the reviewed literature highlights a common focus on addressing issues such as stress shielding, osseointegration enhancement, customization to individual needs, programmable functionalities, and weight reduction in implant designs. It emphasizes significant advances in reducing stress shielding effects, promoting osseointegration, and facilitating personalized implant creation. The review provides a detailed classification of optimization methods, with each approach scrutinized for its unique contribution to overcoming specific challenges in medical implant design, thus leading to more advanced, effective, and patient-oriented implantable devices. Chitin and chitosan‐based bioink for 3D‐printed flexible electronics have tremendous potential for innovation in healthcare, agriculture, the environment, and industry. This biomaterial is suitable for 3D printing because it is highly stretchable, super‐flexible, affordable, ultrathin, and lightweight. Owing to its ease of use, on‐demand manufacturing, accurate and regulated deposition, and versatility with flexible and soft functional materials, 3D printing has revolutionized free‐form construction and end‐user customization. This study examined the potential of employing chitin and chitosan‐based bioinks to build 3D‐printed flexible electronic devices and optimize bioink formulation, printing parameters, and postprocessing processes to improve mechanical and electrical properties. Wireless power and data transmission (WPDT) solutions for medical implants are highly desired. However, achieving a high-power transmission efficiency and data rate simultaneously over an inductive link remains a significant challenge. This paper presents an innovative WPDT circuit that incorporates additional MOSFETs with an inductor in a Class-E power amplifier (PA), achieving amplitude-shift keying (ASK) modulation to address this issue. Firstly, the efficiency of the inductive power transmission link and Class-E PA was analyzed, providing design insights. Then, leveraging the insights, the proposed circuit was designed in such a way that it could effectively switch between two load networks to maintain high transfer efficiency for ASK modulation. Based on the load networks, the relationship between introducing the inductor's value and the data modulation index (MI) was derived to help achieve the desired high-power transmission efficiency. Additionally, the design and calculation of the proposed circuit are also presented. Finally, the proposed circuit was validated through simulations and experiments, demonstrating a power delivery to a load of 84.1 mW with a power transmission efficiency of 70.8% at a data rate and carrier frequency of 3 Mbps and 16 MHz, respectively. This study investigates the in vitro biocompatibility, corrosion resistance, and adhesion strength of a gas abrasive-treated Ti6Al4V alloy, alongside microplasma-sprayed titanium and tantalum coatings. Employing a novel approach in selecting microplasma spray parameters, this study successfully engineers coatings with tailored porosity, roughness, and over 20% porosity with pore sizes up to 200 μm, aiming to enhance bone in-growth and implant integration. This study introduces an innovative methodology for quantifying surface roughness using laser electron microscopy and scanning electron microscopy, facilitating detailed morphological analysis of both the substrate and coatings. Extensive evaluations, including tests for in vitro biocompatibility, corrosion resistance, and adhesive strength, revealed that all three materials are biocompatible, with tantalum coatings exhibiting superior cell proliferation and osteogenic differentiation, as well as the highest corrosion resistance. Titanium coatings followed closely, demonstrating favorable osteogenic properties and enhanced roughness, which is crucial for cell behavior and attachment. These coatings also displayed superior tensile adhesive strengths (27.6 ± 0.9 MPa for Ti and 28.0 ± 4.9 MPa for Ta), surpassing the ISO 13179-1 standard and indicating a robust bond with the substrate. Our findings offer significant advancements in biomaterials for medical implants, introducing microplasma spraying as a versatile tool for customizing implant coatings, particularly emphasizing the superior performance of tantalum coatings in terms of biocompatibility, osteogenic potential, and corrosion resistance. This suggests that tantalum coatings are a promising alternative for enhancing the performance of metal implants, especially in applications demanding high biocompatibility and corrosion resistance. Medical implant-associated infections (IAI) is a growing threat to patients undergoing implantation surgery. IAI prevention typically relies on medical implants endowed with bactericidal properties achieved through surface modifications with antibiotics. However, the clinical efficacy of this traditional paradigm remains suboptimal, often necessitating revision surgery and posing potentially lethal consequences for patients. To bolster the existing anti-IAI arsenal, we propose herein a chitosan-based bioactive coating, i.e., ChitoAntibac, which exerts bacteria-inhibitory effects either through immune modulation or phage-directed microbial clearance, without relying on conventional antibiotics. The immuno-stimulating effects and phage-induced bactericidal properties can be tailored by engineering the loading dynamic of macrophage migration inhibitory factor (MIF), which polarizes macrophages towards the proinflammatory subtype (M1) with enhanced bacterial phagocytosis, and Staphylococcal Phage K, resulting in rapid and targeted pathogenic clearance (>99.99%) in less than 8 h. Our innovative antibacterial coating opens a new avenue in the pursuit of effective IAI prevention through immuno-stimulation and phage therapeutics.

The development of tailored additive manufacturing (AM) approaches is resulting in novel strategies for engineering the macro/microstructural details of potential medical devices made of biocompatible polymers. The aim of this work is the employment of a multifunctional AM machine for fabricating poly(methyl methacrylate) (PMMA) constructs with a predefined shape and porous architecture, by either melt‐ or solution‐extrusion AM. In particular, PMMA porous scaffolds are manufactured by employing fused filament fabrication (FFF) or computer‐aided wet‐spinning (CAWS). The comparative characterization of the two developed PMMA implant prototypes demonstrates significant differences in terms of porous structure, polymer surface topography, material composition, glass transition temperature, and mechanical properties. As a consequence, FFF‐printed PMMA samples support a faster in vitro proliferation of a murine fibroblast cell line in comparison to PMMA scaffolds by CAWS. The results achieved encourage further research on the developed processing protocols, aimed at the development of tailored PMMA implants for personalized surgery applications. With the rising demand for implantable orthopaedic medical devices and the dominance of device-associated infections, extensive research into the development of novel materials has been prompted. Among these, new-generation titanium alloys with biocompatible elements and improved stiffness levels have received much attention. Furthermore, the development of titanium-based materials that can impart antibacterial function has demonstrated promising results, where gallium has exhibited superior antimicrobial action. This has been evidenced by the addition of gallium to various biomaterials including titanium alloys. Therefore, this paper aims to review the antibacterial activity of gallium when incorporated into biomedical materials, with a focus on titanium-based alloys. First, discussion into the development of new-generation Ti alloys that possess biocompatible elements and reduced Young's moduli is presented. This includes a brief review of the influence of alloying elements, processing techniques and the resulting biocompatibilities of the materials found in the literature. The antibacterial effect of gallium added to various materials, including bioglasses, liquid metals, and bioceramics, is then reviewed and discussed. Finally, a key focus is given to the incorporation of gallium into titanium systems for which the inherent mechanical, biocompatible, and antibacterial effects are reviewed and discussed in more detail, leading to suggestions and directions for further research in this area. In recent years, antibacterial coatings have become an important approach in the global fight against bacterial pathogens. Developments in materials science, chemistry, and biochemistry have led to a plethora of materials and chemical compounds that have the potential to create antibacterial coatings. However, insufficient attention has been paid to the analysis of the techniques and technologies used to apply these coatings. Among the various inorganic coating techniques, atomic layer deposition (ALD) is worthy of note. It enables the successful synthesis of high-purity inorganic nanocoatings on surfaces of complex shape and topography, while also providing precise control over their thickness and composition. ALD has various industrial applications, but its practical application in medicine is still limited. In recent years, a considerable number of papers have been published on the proposed use of thin films and coatings produced via ALD in medicine, notably those with antibacterial properties. The aim of this paper is to carefully evaluate and analyze the relevant literature on this topic. Simple oxide coatings, including TiO2, ZnO, Fe2O3, MgO, and ZrO2, were examined, as well as coatings containing metal nanoparticles such as Ag, Cu, Pt, and Au, and mixed systems such as TiO2-ZnO, TiO2-ZrO2, ZnO-Al2O3, TiO2-Ag, and ZnO-Ag. Through comparative analysis, we have been able to draw conclusions on the effectiveness of various antibacterial coatings of different compositions, including key characteristics such as thickness, morphology, and crystal structure. The use of ALD in the development of antibacterial coatings for various applications was analyzed. Furthermore, assumptions were made about the most promising areas of development. The final section provides a comparison of different coatings, as well as the advantages, disadvantages, and prospects of using ALD for the industrial production of antibacterial coatings. The present study investigated the influence of pore size of strut-based Diamond and surface-based Gyroid structures for their suitability as medical implants. Samples were made additively from laser powder bed fusion process with a relative density of 0.3 and pore sizes ranging from 300 to 1300 μm. They were subsequently examined for their manufacturability and mechanical properties. In addition, non-Newtonian computational fluid dynamics and discrete phase models were conducted to assess pressure drop and cell seeding efficiency. The results showed that both Diamond and Gyroid had higher as-built densities with smaller pore sizes. However, Gyroid demonstrated better manufacturability as its relative density was closer to the as-designed one. In addition, based on mechanical testing, the elastic modulus was largely unaffected by pore size, but post-yielding behaviors differed, especially in Diamond. High mechanical sensitivity in Diamond could be explained partly by Finite Element simulations, which revealed stress localization in Diamond and more uniform stress distribution in Gyroid. Furthermore, we defined the product of the normalized specific surface, normalized pressure drop, and cell seeding efficiency as the indicator of an optimal pore size, in which this factor identified an optimal pore size of approximately 500 μm for both Diamond and Gyroid. Besides, based on such criterion, Gyroid exhibited greater applicability as bone scaffolds. In summary, this study provides comprehensive assessment of the effect of pore size and demonstrates the efficient estimation of an in-silico framework for evaluating lattice structures as medical implants, which could be applied to other lattice architectures.

Metal-based additive manufacturing has gained significant attention in the field of medical implants over the past decade. The application of 3D-printing technology in medical implants offers several advantages over traditional manufacturing methods, including increased design flexibility for implant customization, reduced lead time for emergency cases, and the ability to create complex geometry shapes for patient-specific implants. In this review study, the working principles and conditions of metal 3D-printing technologies such as selective laser sintering, selective laser melting, and electron beam melting, as well as their applications and advantages in the medical field, are investigated in detail. The application scenarios and research status of non-degradable metals including titanium alloy, medical stainless steel, etc., and degradable metals like magnesium alloy are introduced as printing materials. We discuss the improvement of mechanical properties and biocompatibility of implants through surface modification, porous structure design, and the optimization of molding processes. Finally, the biocompatibility issues and challenges caused by the accuracy of CT imaging, fabrication, implant placement, and other aspects are summarized.

The effect of hot extrusion on the mechanical and corrosion properties of Mg–Zn–Ga alloys intended for medicine purposes is studied. The Mg4Zn4Ga alloy is shown to exhibit a favorable level of mechanical properties (σu = 343 MPa, σy = 256 MPa, δ = 14%) and a controlled corrosion rate (0.4 mm/year). Low cytotoxicity of the Mg4Zn4Ga alloy determined according to GOST R ISO 10993.5–99 makes it possible to recommend it for further in vivo clinical tests and producing medicine implants. Diamond-like carbon (DLC) coatings doped with bioactive elements of silver (Ag) and copper (Cu) have been receiving increasing attention in the last decade, particularly in the last 5 years, due to their potential to offer a combination of enhanced antimicrobial and mechanical performance. These multi-functional bioactive DLC coatings offer great potential to impart the next generation of load-bearing medical implants with improved wear resistance and strong potency against microbial infections. This review begins with an overview of the status and issues with current total joint implant materials and the state-of-the art in DLC coatings and their application to medical implants. A detailed discussion of recent advances in wear resistant bioactive DLC coatings is then presented with a focus on doping the DLC matrix with controlled quantities of Ag and Cu elements. It is shown that both Ag and Cu doping can impart strong antimicrobial potency against a range of Gram-positive and Gram-negative bacteria, but this is always accompanied so far by a reduction in mechanical performance of the DLC coating matrix. The article concludes with discussion of potential synthesis methods to accurately control bioactive element doping without jeopardising mechanical properties and gives an outlook to the potential long-term impact of developing a superior multifunctional bioactive DLC coating on implant device performance and patient health and wellbeing. Multi-functional diamond-like carbon (DLC) coatings doped with bioactive elements of silver (Ag) and copper (Cu) offer great potential to impart the next generation of load-bearing medical implants with improved wear resistance and strong potency against microbial infections. This article provides a critical review of the state-of-the-art in Ag and Cu doped DLC coatings, beginning with an overview of the current applications of DLC coatings in implant technology followed by a detailed discussion of Ag/Cu doped DLC coatings with particular focus on the relationship between their mechanical and antimicrobial performance. Finally, it ends with a discussion on the potential long-term impact of developing a truly multifunctional ultra-hard wearing bioactive DLC coating to extend the lifetime of total joint implants. Wireless power and data transmission (WPDT) solutions for medical implants are highly desired. However, achieving a high-power transmission efficiency and data rate simultaneously over an inductive link remains a significant challenge. This paper presents an innovative WPDT circuit that incorporates additional MOSFETs with an inductor in a Class-E power amplifier (PA), achieving amplitude-shift keying (ASK) modulation to address this issue. Firstly, the efficiency of the inductive power transmission link and Class-E PA was analyzed, providing design insights. Then, leveraging the insights, the proposed circuit was designed in such a way that it could effectively switch between two load networks to maintain high transfer efficiency for ASK modulation. Based on the load networks, the relationship between introducing the inductor’s value and the data modulation index (MI) was derived to help achieve the desired high-power transmission efficiency. Additionally, the design and calculation of the proposed circuit are also presented. Finally, the proposed circuit was validated through simulations and experiments, demonstrating a power delivery to a load of 84.1 mW with a power transmission efficiency of 70.8% at a data rate and carrier frequency of 3 Mbps and 16 MHz, respectively. Furthermore, the bit error rate (BER) is less than 10−6 with an MI of 10%.

BACKGROUND: The bulk metallic glass (BMG), Pd79Ag3.5P6Si9.5Ge2, has a high fracture toughness and has been found to accommodate post-yield stress, unlike most other BMG. Moreover, due to its greater noble gas composition it has a intrinsic corrosion resistance, ideal for dental and orthopedic implants. OBJECTIVE: This present study aimed to evaluate the in vivo application of Pd79Ag3.5P6Si9.5Ge2 in a large translational sheep model to assess its efficacy to be utilized as an endosteal device. METHODS: Twelve implants in the form of cylindrical rods (3 mm in diameter) were produced through rapid quenching. Each sheep (n = 12) received one osteotomy in the mandibular region using rotary instrumentation, which was filled with Pd79Ag3.5P6Si9.5Ge2. After 6- and 24-weeks the animals were euthanized, and samples collected en bloc to conduct histomorphometric analysis. The degree of osseointegration were assessed through bone-to-implant contact (BIC). RESULTS: All samples revealed favorable BIC along with with fibrous connective tissue layers at both 6- and 24-weeks. Bone along with interfacial remodeling was observed in proximity with the metallic glass surface at 6 weeks with higher degrees of bone organization being observed at the later healing time, 24 weeks. CONCLUSIONS: The synthesized BMG, given its unique combination of toughness and strength, revealed potential to serve as an alternative to commonly used Ti alloys. [ABSTRACT FROM AUTHOR]

Medical implants are essential tools for treating chronic illnesses, restoring physical function, and improving the quality of life for millions of patients worldwide. However, implant failures due to infection, mechanical wear, corrosion, and tissue rejection continue to be a major challenge. Nanocomposites, composed of nanoparticles or nanofillers dispersed in a matrix material, have shown promising results in enhancing implant performance. This paper provides an overview of the current state of research on the use of nanocomposites for medical implants. We discuss the types of nanocomposites being developed, including polymer-, metal-, and ceramic-based materials, and their advantages/disadvantages for medical implant applications. Strategies for improving implant performance using nanocomposites, such as improving biocompatibility and mechanical properties and reducing wear and corrosion, are also examined. Challenges to the widespread use of nanocomposites in medical implants are discussed, such as biocompatibility, toxicity, long-term stability, standardisation, and quality control. Finally, we discuss future directions for research, including the use of advanced fabrication techniques and the development of novel nanocomposite materials. The use of nanocomposites in medical implants has the potential to improve patient outcomes and advance healthcare, but continued research and development will be required to overcome the challenges associated with their use. The deployment of structures that enable localized release of bioactive molecules can result in more efficacious treatment of disease and better integration of implantable bionic devices. The strategic design of a biopolymeric coating can be used to engineer the optimal release profile depending on the task at hand. As illustrative examples, here advances in delivery of drugs from bone, brain, ocular, and cardiovascular implants are reviewed. These areas are focused to highlight that both hard and soft tissue implants can benefit from controlled localized delivery. The composition of biopolymers used to achieve appropriate delivery to the selected tissue types, and their corresponding outcomes are brought to the fore. Using biomaterials to create new technologies like sensors, electrodes, prosthetics, bioelectrodes, skin substitutes, and drug delivery systems is known as biotechnology. It is crucial for medical procedures like surgery, dentistry, prosthetics, biosensors, electrophoresis, bioelectricity, implantation, and many other fields of human endeavor. Mesoporous bioactive glasses (MBGs) are the main bioactive materials used for bone regeneration due to their large surface area and high pore content, which can increase bioactivity and facilitate new bone formation. Their large surface area and high pore volume result in higher surface chemical reactivity as compared to nonmesoporous bioactive glasses, hence they have a higher chemical reactivity. Cells were aligned on the surface of an implant in some other investigation when topographical characteristics were produced by electro-hydrodynamic printing with hydroxyapatite, and permanent small silica spheres are commonly used in biomedical applications for cell labeling or medication administration. Because of the inclusion of porosities in MBG matrixes, as well as their large surface area, the deposition of hydroxyl carbonate apatite is considerably accelerated. MBGs can be cultured in the laboratory with a variety of methods, depending on how they will be employed in medical therapy. Melt-quenching therapy, spray pyrolysis method, sol-gel manufacturing technique, spray drying process, and modified Stber method are some of these tactics. Bacterial resistance to antibiotics and host defense systems is primarily due to bacterial biofilm formation in antibiotic therapy. In the present study, two complexes, bis (biphenyl acetate) bipyridine Cu (II) (1) and bis (biphenyl acetate) bipyridine Zn (II) (2), were tested for their ability to prevent biofilm formation. The minimum inhibitory concentration and minimum bactericidal concentration of complexes 1 and 2 were 46.87 ± 1.822 and 93.75 ± 1.345 and 47.87 ± 1.345 and 94.85 ± 1.466 μg/mL, respectively. The significant activity of both complexes was attributed to the damage caused at the membrane level and was confirmed using an imaging technique. The biofilm inhibitory potential levels of complexes 1 and 2 were 95% and 71%, respectively, while the biofilm eradication potential levels were 95% and 35%, respectively, for both complexes. Both the complexes showed good interactions with the E. coli DNA. Thus, complexes 1 and 2 are good antibiofilm agents that exert their bactericidal actions possibly by disrupting the bacterial membrane and interacting with the bacterial DNA, which can act as a powerful agent to restrain the development of bacterial biofilm on therapeutic implants. In recent times, wireless power transfer systems have been identified as a reliable option to supply power to medical implants. Up to now, Wireless Power Transfer Systems (WPTS) have only been used to charge batteries of low-power medical implants. However, for medical implants requiring a relatively higher power, such as a ventricular assist device, which is an implanted blood pump in the patient's abdominal cavity, an external power supply has been used. When WPTS is used for medical implants, it increases the number of required power converter stages and hardware complexity along with the volume, which tends to reduce the overall efficiency. In addition, the existence of uncertainties in WPTS-based medical implants, such as load and mutual inductance variations, can lead to system instability or poor performance. The focus of this paper is to design a WPTS to supply power to the pump motor directly through its inverter based on the requirements of the motor drive system (MDS) without resorting to an additional DC-to-DC converter stage. To this end, the constraints that the drive system imposes upon WPTS have been identified. In addition, to make a reliable closed-loop operation, a µ-synthesis robust controller is designed to make sure the system maintains its stability and performance with respect to the system's existing uncertainties. A number of experimental results are provided to verify the effectiveness of the adopted WPTS design approach and the corresponding closed-loop controller for WPTS. Furthermore, the experimental findings for the maximum efficiency tracking (MET) approach (to minimize WPTS coil losses) and constant DC link voltage control approach are shown and compared. According to experimental results and system efficiency analysis, the former appears to perform better. We aimed to describe the outcome of totally implantable venous-access port (TIVAP)-related infections due to Gram-negative aerobic bacilli (Pseudomonas aeruginosa and other Pseudomonas spp., Acinetobacter spp., and Stenotrophomonas maltophilia), or GNAB, and assess the safety of conservative treatment. We conducted a retrospective study in a French teaching hospital, from January 2016 to December 2020, including adult patients treated for TIVAP-related infection due to GNAB. Success of conservative treatment was defined as a functional TIVAP 3 months after infection with no recurrence. We performed a bivariate analysis and analyzed causes for treatment failure. We included 68 patients (53 TIVAP-related bloodstream infections, 11 TIVAP-related infections, and 4 probable TIVAP-related infections) due to GNAB, mostly P. aeruginosa (50/68, 74%). TIVAP removal was initially decided for 49/68 patients (72%). Among the 19/68 (28%) patients with conservative treatment (all for infections caused by P. aeruginosa), 5/19 (26%) had successful treatment, 7/19 (37%) experienced failure (without sepsis or septic shock), 6/19 (32%) died within 3 months without TIVAP removal and no signs of infection recurrence, and 1 patient had TIVAP removal as it was no longer required. TIVAP-related infections caused by GNAB frequently require TIVAP removal. Conservative treatment can be performed in selected patients with a non-complicated infection caused by P. aeruginosa, who can benefit from the continuation of antineoplastic chemotherapy or palliative care. Recently, film materials based on the combination of noble metals have showed promising results for surface modification of medical implants, allowing both to improve biocompatibility and to acquire the increased antibacterial effect. An important challenge here is to combine the developed coating morphology, which is favorable for biological response, with a high protective function, which, on the contrary, requires a compact coating microstructure. In this work, we aimed to solve this problem with respect to the TiNi implant material. We have tested two types of compact thin sublayers: Iridium (Ir'), formed by metal-organic chemical vapor deposition (MOCVD), and gold (Au), formed by physical vapor deposition (PVD). Subsequently these sublayers were coated with a developed-columnar-iridium (Ir) by MOCVD. Features of the microstructure, chemical and phase composition of all these film materials were studied using powder X-ray diffraction (XRD), scanning electron microscopy (SEM) and X-ray photoelectron spectroscopy (XPS). The changes in the characteristics of TiNi martensitic transformation due to MOCVD experiments were also studied by differential scanning calorimetry (DSC). The biocompatibility of Ir'/TiNi, Au/TiNi, Ir/Ir'/TiNi, Ir/Au/TiNi samples was assessed by cytoxicity testing (Man-1 cells) and measuring of nickel content in the biological extracts. The application of both sublayers effectively reduces the release of nickel, which was previously shown for Ir/TiNi samples. This prevents the toxic effect. Coatings are essential for biomedical applications antifouling and antimicrobial properties, supporting cell adhesion and tissue integration and particularly interesting in this field are nanogel (nGel)‐based coatings. Since biomaterials differ in physiochemical properties, specific nGel‐coating strategies need to be developed for every distinct material, leading to complex coating strategies. Hence, the solution lies in adopting a universal strategy to apply the same nGel coating with the same function on a wide range of implant surfaces. To this end, a universal nGel‐based coating approach provides the same coating using a single method on implant materials including stiff polymer materials, metals, ceramics, glass, and elastomers. The coating formation is achieved by electrostatic interactions between oxygen plasma–activated surfaces and positively charged nGels using a spray‐deposition method. Fluorescent labels are introduced into the nGels as a model for post‐modification capabilities to increase the functionality of the coating. The coating is highly stable under in vitro physiological conditions with the retention of its function on different clinically relevant materials. Meanwhile, the in vivo study indicates that the nGel coating on a polyvinylidene fluoride hernia mesh is stable and biocompatible, therefore, making the coating and the coating strategy, a highly impactful approach for future clinical developments.

Magnesium biodegradable alloys are a promising material for self-dissolving surgical implants. Magnesium is known to be sensitive to electrochemical corrosion due to the galvanic effect between the matrix and particles of secondary phases and inclusions. Another important factor is the pH level. The behavior of certain chemical reactions depends on the pH level, so one can assume that the pH level of a corrosive medium at the material surface is a factor determining what chemical reactions can occur there. Finally, there is evidence that variability of the crystallographic orientation of the grains may be a cause of anisotropy of corrosion properties. The purpose of this work is to reveal the influence of the electrode potential of the microstructural elements, the crystallographic orientation of the grains, and the pH level of the near-surface volume of the corrosion solution on the corrosion process. In the study, sections of 2×1.5 mm were marked on the ZX10 alloy samples, for which maps of the distribution of crystallographic orientations and chemical composition were drawn. To assess the influence of the electrode potential of the particles, the authors carried out a Kelvin probe mapping in the 90×90 µm area. Next, corrosion tests were carried out with video filming of the surface on the marked area. To determine the pH level influence, the solution circulation in the cell was varied. Upon completion of the tests, corrosion products and corrosion damage were examined in detail. According to the results, the pH level in the liquid near-surface micro-volumes has a greater influence than the electrode potential of the particles as it provokes the formation of corrosion products of a different composition, which leads to passivation of the surface areas around the particles. The authors identified two different types of filiform corrosion. For filiform corrosion, a correlation between the corrosion direction and the crystallographic orientation of the grains was established.

In recent years, antibacterial coatings have become an important approach in the global fight against bacterial pathogens. Developments in materials science, chemistry, and biochemistry have led to a plethora of materials and chemical compounds that have the potential to create antibacterial coatings. However, insufficient attention has been paid to the analysis of the techniques and technologies used to apply these coatings. Among the various inorganic coating techniques, atomic layer deposition (ALD) is worthy of note. It enables the successful synthesis of high-purity inorganic nanocoatings on surfaces of complex shape and topography, while also providing precise control over their thickness and composition. ALD has various industrial applications, but its practical application in medicine is still limited. In recent years, a considerable number of papers have been published on the proposed use of thin films and coatings produced via ALD in medicine, notably those with antibacterial properties. The aim of this paper is to carefully evaluate and analyze the relevant literature on this topic. Simple oxide coatings, including TiO2, ZnO, Fe2O3, MgO, and ZrO2, were examined, as well as coatings containing metal nanoparticles such as Ag, Cu, Pt, and Au, and mixed systems such as TiO2-ZnO, TiO2-ZrO2, ZnO-Al2O3, TiO2-Ag, and ZnO-Ag. Through comparative analysis, we have been able to draw conclusions on the effectiveness of various antibacterial coatings of different compositions, including key characteristics such as thickness, morphology, and crystal structure. The use of ALD in the development of antibacterial coatings for various applications was analyzed. Furthermore, assumptions were made about the most promising areas of development. The final section provides a comparison of different coatings, as well as the advantages, disadvantages, and prospects of using ALD for the industrial production of antibacterial coatings.

With the rising demand for implantable orthopaedic medical devices and the dominance of device-associated infections, extensive research into the development of novel materials has been prompted. Among these, new-generation titanium alloys with biocompatible elements and improved stiffness levels have received much attention. Furthermore, the development of titanium-based materials that can impart antibacterial function has demonstrated promising results, where gallium has exhibited superior antimicrobial action. This has been evidenced by the addition of gallium to various biomaterials including titanium alloys. Therefore, this paper aims to review the antibacterial activity of gallium when incorporated into biomedical materials, with a focus on titanium-based alloys. First, discussion into the development of new-generation Ti alloys that possess biocompatible elements and reduced Young’s moduli is presented. This includes a brief review of the influence of alloying elements, processing techniques and the resulting biocompatibilities of the materials found in the literature. The antibacterial effect of gallium added to various materials, including bioglasses, liquid metals, and bioceramics, is then reviewed and discussed. Finally, a key focus is given to the incorporation of gallium into titanium systems for which the inherent mechanical, biocompatible, and antibacterial effects are reviewed and discussed in more detail, leading to suggestions and directions for further research in this area.

The widespread occurrence of bacterial infections and their increased resistance to antibiotics has led to the development of antimicrobial coatings for multiple medical implants. Owing to their desirable properties, gold nanoparticles (AuNPs) have been developed as antibacterial agents. This systematic investigation sought to analyze the antibacterial effects of implant material surfaces modified with AuNPs. The data from 27 relevant studies were summed up. The included articles were collected from September 2011 to September 2021. According to the retrieved literature, we found that medical implants modified by AuNPs have good antibacterial effects against gram-positive and gram-negative bacteria, and the antibacterial effects would be improved by near-infrared (NIR) radiation. The study is aimed to virtually miniaturize medical implants produced of the biocompatible Ti with improved mechanical performance. The results on the simulation-driven design of medical implants fabricated of nanostructured commercially pure Ti with significantly enhanced mechanical properties are presented. The microstructure of initially coarse-grained Ti has been refined to ultrafine grain size by severe plastic deformation. The ultrafine-grained (UFG) Ti exhibits remarkably high static and cyclic strength, allowing to design new dental and surgical implants with miniaturized geometry. The possibilities to reduce the implant dimensions via virtual fatigue tests for the digital twins of two particular medical devices (a dental implant and a maxillofacial surgery plate) are explored with the help of finite element modeling. Additionally, the effect of variation in loading direction and the fixation methods for the tested implants are studied in order to investigate the sensitivity of the fatigue test results to the testing conditions. Modern healthcare is transforming from hospital-centric to individual-centric systems. Emerging implantable and wearable medical (IWM) devices are integral parts of enabling affordable and accessible healthcare. Early disease diagnosis and preventive measures are possible by continuously monitoring clinically significant physiological parameters. However, most IWM devices are battery-operated, requiring replacement, which interrupts the proper functioning of these devices. For the continuous operation of medical devices for an extended period of time, supplying uninterrupted energy is crucial. A sustainable and health-compatible energy supply will ensure the high-performance real-time functioning of IWM devices and prolong their lifetime. Therefore, harvesting energy from the human body and ambient environment is necessary for enduring precision healthcare and maximizing user comfort. Energy harvesters convert energy from various sources into an equivalent electrical form. This paper presents a state-of-the-art comprehensive review of energy harvesting techniques focusing on medical applications. Various energy harvesting approaches, working principles, and the current state are discussed. In addition, the advantages and limitations of different methods are analyzed and existing challenges and prospects for improvement are outlined. This article presents the development of a power loss emulation (PLE) system device to study and find ways of mitigating skin tissue heating effects in transcutaneous energy transmission systems (TETS) for existing and next generation left ventricular assist devices (LVADs). Skin thermal profile measurements were made using the PLE system prototype and also separately with a TETS in a porcine model. Subsequent data analysis and separate computer modelling studies permit understanding of the contribution of tissue blood perfusion towards cooling of the subcutaneous tissue around the electromagnetic coupling area. A 2-channel PLE system prototype and a 2-channel TETS prototype were implemented for this study. The heating effects resulting from power transmission inefficiency were investigated under varying conditions of power delivery levels for an implanted device. In the part of the study using the PLE setup, the implanted heating element was placed subcutaneously 6–8 mm below the body surface of in vivo porcine model skin. Two operating modes of transmission coupling power losses were emulated: (a) conventional continuous transmission, and (b) using our proposed pulsed transmission waveform protocols. Experimental skin tissue thermal profiles were studied for various levels of LVAD power. The heating coefficient was estimated from the porcine model measurements (an in vivo living model and a euthanised cadaver model without blood circulation at the end of the experiment). An in silico model to support data interpretation provided reliable experimental and numerical methods for effective wireless transdermal LVAD energization advanced solutions. In the separate second part of the study conducted with a separate set of pigs, a two-channel inductively coupled RF driving system implemented wireless power transfer (WPT) to a resistive LVAD model (50 Ω) to explore continuous versus pulsed RF transmission modes. The RF-transmission pulse duration ranged from 30 ms to 480 ms, and the idle time (no-transmission) from 5 s to 120 s. The results revealed that blood perfusion plays an important cooling role in reducing thermal tissue damage from TETS applications. In addition, the results analysis of the in vivo, cadaver (R1Sp2) model, and in silico studies confirmed that the tissue heating effect was significantly lower in the living model versus the cadaver model due to the presence of blood perfusion cooling effects. As biodegradable materials, Fe–Mn alloys have a lot of promise, particularly because they can be employed as metallic implants with excellent mechanical properties. Besides allowing for patient customisation, powder-bed fusion of these alloys could help overcome their main drawback, i.e., slow degradation inside the human body, by increasing the component surface with inbuilt structural porosity. The quality of additive-manufactured products depends on their temperature history, making knowledge of the heat-transfer characteristics of the powder-bed fusion process very important. While accurate determinations of temperature gradients and the melt-pool sizes still represent a considerable challenge for all materials, this is particularly true for Fe–Mn alloys, where research is currently limited to a handful of pioneering works, and experimental determinations of the melt-pool contours prove extremely difficult and unreliable. To explore the origins of measurement inconsistency, melt-pool compositions of Fe–Mn specimens were analysed in the scope of this research. Concentric patterns of high- and low-Mn content practically indistinguishable from the melt-pool boundary on the macroscale were revealed within the melt-pool. A microscopic analysis of elemental content distribution was performed and the concentric patterns were attributed to the pronounced segregation of the alloy in conjunction with convective currents. A novel, calibration-free 3D finite-element model of heat transfer during laser powder-bed fusion is proposed to overcome these experimental difficulties and validated against the experimental melt-pool measurements. The advent of semiconductor technology in the mid-20th century created unprecedented opportunities to develop a new generation of small-scale wireless medical sensing devices that can support remote monitoring of patients' vital signs. The first radio pills were developed as early as the 1950’s using only a few transistors. These swallowable capsules could sense and wirelessly transmit vital parameters from inside the human body. Since then we have witnessed the rapid progress of medical devices driven by the evolution of semiconductor technology, from single-transistor oscillators to complex mixed-signal multi-channel and multi-modal systems. This paper retraces the evolution of biotelemetry devices from their very early inception to the smart miniaturized systems of modern days, focusing on semiconductor-enabled sensing methods and circuits developed over the last six decades. The paper also includes the author's perspective on current and future trends in the development of CMOS-based biotelemeters, focusing on concepts of implant modularity, miniaturization and hybrid energy harvesting solutions.
