In the vast majority of cases total knee arthroplasty is efficient for pain relief and improving the joint function. However, there is still a subset of patients who are not satisfied with their replacement surgery. Reasons cited include persistent pain, restricted movement, an unnatural feeling of the knee and complaints of instability. Additionally, active patients sometimes report limitations in performing high-level activities. Traditional designs of total knee prostheses cannot fully replicate the physiological kinematics of the knee joint. Medial pivot systems aim to reproduce the physiological biomechanics of the knee joint. Medially, they resemble, similar to a natural and healthy knee, a ball fitting into a highly congruent socket, ensuring stability in all planes and allowing for a constant ligament tension on the medial side. Externally, these implants are less congruent, permitting the natural rotational movement of the knee joint. The combination of natural, uncompromised knee mobility and high stability is intended to reproduce a physiological biomechanic of knee joint. This is the basis for good knee function in daily activities and sports. In Germany, these treatment concepts are used relatively infrequently compared to other countries, although their use has increased significantly.
