Summary: Although the male epididymal fat pad is an effective site for islet transplantation, females lack this tissue. Here, we present a protocol to assess the parametrial fat pad (PFP) adjacent to the uterine horn in females as an alternative site for islet transplantation. We describe steps for islet isolation from the pancreas, counting, transplantation into PFP, and monitoring for engraftment. Transplantation into PFP is minimally invasive, time efficient, and supports long-term engraftment of syngeneic islets and rejection of allogeneic islets.For complete details on the use and execution of this protocol, please refer to Zhang et al. Tigray war broke out on November 4, 2020, and lasted until November 3, 2022. The war has caused a significant loss of human life and a catastrophic economic and humanitarian crisis. The war affected the food and water supplies to farmers to care their livestock and this led to animal death, malnutrition, and suffering. In addition, a significant number of animals have been subjected to flee the region and killed during the warfare. The veterinary sector is significantly damaged and animals became a victim. Veterinarians and animal health workers have fled the region because of the war, and this adds an extra burden to the sector. Although the impact of this war on animal life, welfare and overall, on the livestock infrastructure is significant, no study has been conducted so far. We analyzed the level of damage to the veterinary sector and number of animal loss following the war. Our analytical study showed the war has claimed a total loss of 2,487,047 cattle, 1,690,096 sheep, 3,803,860 goats, 610,976 donkey, 4,280,815 poultry, and 231,985 beehives. This caused an estimated total financial loss of 53.56 billion Ethiopian birr (∼1.01 billion USD). We also analyzed the destruction level of the veterinary infrastructures in percentage, and the associated financial loss due to facility damage and animal disappearances from the region. A complete destruction (100 %) of veterinary facilities was reported in 10 districts and this accounts to an estimated financial loss of 68.59 million Ethiopian birrs (1.3 million USD). In conclusion, Tigray war has caused an immense impact to animal welfare and veterinary sector and a collaborative effort between governmental and nongovernmental organizations, and professional bodies is required to restore the sector. This study also highlighted how the war jeopardize animal right and wellbeing. Thus, we believe this study will be an input for national and international policy makers working on international convention for animal protection and rights.

This study was therefore designed to evaluate the frequency of AHM and associated risk factors in patients attending a national referral hospital in Asmara, Eritrea.Methods: A total of 335 respondents (females: 222 (66.3%) participated in this cross-sectional study. Adherence to Medication was evaluated using a modified version of the Medication Adherence Report Scale (MARS). Additional data were abstracted from patients’ medical records and a WHO STEPwise questionnaire.Results: The mean (±SD) age of the study participants was 59.65 (± 12.20) years (females: 56.82 (± 9.95) vs males: 57.17 (± 9.60) years, p-value=0.001). Similarly, the median (IQR) for diastolic blood pressure (DBP) and systolic blood pressure (SBP) were 85.00 mmHg (IQR: 80.00– 90.00 mmHg) and 145.00 mmHg (IQR: 130.00– 160.00 mmHg), respectively. Overall, 246 (73.4%) and 244 (72.8%) participants had poor knowledge and poor adherence to AHM, respectively. In the multivariate analysis, factors associated with increased odds of poor adherence to AHM included sex (females: adjusted odds ratio (aOR): 4.95; 95% CI: 1.52– 16.11, p value=0.008); employment status (Ref: employed/NGO) (self-employed: aOR: 1.95; 95% CI: 0.45– 8.48, p-value=0.373) (housewife: aOR: 0.13; 95% CI: 0.04– 0.413, p-value=0.001) (unemployed: aOR: 1.38; 95% CI:0.32– 5.98; p-value=0.670); lack of attendance of health talk at the clinic (aOR: 2.33; 95% CI: 1.17– 4.63; p-value=0.016); high cost of transportation (yes: aOR: 6.87; 95% CI: 3.25– 14.52; p-value< 0.001); knowledge (poor: aOR: 6.23; 95% CI: 2.36– 18.05, p-value< 0.001) and hypertension stage (Stage 3: aOR: 5.55; 95% CI, 1.44– 21.37, p-value=0.013). Low level of knowledge regarding hypertension-related complications or associated risk factors was also noted.Conclusion: The high level of poor adherence to anti-hypertension medications and low level of knowledge on hypertension should raise concern. Vaccination is considered the most appropriate way to control visceral leishmaniasis (VL). With this background, the r-LdODC protein as well as its derived HLA-DRB1-restricted synthetic peptides (P1: RLMPSAHAI, P2: LLDQYQIHL, P3: GLYHSFNCI, P4: AVLEVLSAL, and P5: RLPASPAAL) were validated in BALB/c mice against visceral leishmaniasis. The study was initiated by immunization of the r-LdODC protein as well as its derived peptides cocktail with adjuvants (r-CD2 and MPL-A) in different mice groups, separately. Splenocytes isolated from the challenged and differentially immunized mice group exhibited significantly higher IFN-γ secretion, which was evidenced by the increase in the expression profile of intracellular CD4+IFN-γ T cells. However, the IL-10 secretion did not show a significant increase against the protein and peptide cocktail. Subsequently, the study confirmed the ability of peptides as immunoprophylactic agents, as the IE-I/AD-I molecule overexpressed on monocytes and macrophages of the challenged mice group. The parasitic load in macrophages of the protein and peptides cocktail immunized mice groups, and T cell proliferation rate, further established immunoprophylactic efficacy of the r-LdODC protein and peptide cocktail. This study suggests that the r-LdODC protein, as well as its derived HLA-DRB1-restricted synthetic peptides, have immunoprophylactic potential and can activate other immune cells’ functions towards protection against visceral leishmaniasis. However, a detailed study in a humanized mice model can explore its potential as a vaccine candidate.

Abstract Parasites comprise diverse and complex organisms, which substantially impact human and animal health. Most parasites have complex life-cycles, and by virtue of co-evolution have developed multifaceted, often life-cycle stage-specific relationships with the immune system of their hosts. The complexity in the biology of many parasites often limits our knowledge of parasite-specific immune responses, to in vitro studies only. The relatively recent development of methods to stably manipulate the genetic make-up of many parasites has allowed a better understanding of host-parasite interactions, particularly in vivo. In this regard, the use of transgenic parasites can facilitate the study of immunomodulatory mechanisms under in vivo conditions. Therefore, in this review, we specifically highlighted the current developments in the use of transgenic parasites to unravel the host’s immune response to different life-cycle stages of some key parasite species such as Leishmania, Schistosoma, Toxoplasma, Plasmodium and Trypanosome and to some degree, the use of transgenic nematode parasites is also briefly discussed.

Topics include damage assessment of educational resources and materials by administrative zone, examples of individual-level interventions with war-affected children, and summary of total victims who had an affiliation with the education sector in the region.

Th1 and Th2 cytokines determine the outcome of Leishmania major infection and immune protection depends mainly on memory T cells induced during vaccination. This largely hinges on the nature and type of memory T cells produced. In this study, transgenic Leishmania major strains expressing membrane-associated ovalbumin (mOVA) and soluble ovalbumin (sOVA) were used as a model to study whether fully differentiated Th1/Th2 and Th17 cells can recall immune memory and tolerate pathogen manipulation. Naïve OT-II T cells were polarised in vitro into Th1/Th2 cells, and these cells were transferred adoptively into recipient mice. Following the transferral of the memory cells, the recipient mice were challenged with OVA transgenic Leishmania major and a wild-type parasite was used a control. The in vitro-polarised T helper cells continued to produce the same cytokine signatures after being challenged by both forms of OVA-expressing Leishmania major parasites in vivo. This suggests that antigen-experienced cells remain the same or unaltered in the face of OVA-transgenic Leishmania major. Such ability of these antigen-experienced cells to remain resilient to manipulation by the parasite signifies that vaccines might be able to produce immune memory responses and defend against parasitic immune manipulation in order to protect the host from infection. Chickpea (Cicer arietinum L.) is the world’s third most vital food legume after beans and peas in production level. Yet, its productivity in the last decade has been declined, and it has been contended that the usual native soil rhizobial populations are insufficient/ineffective in N2-fixation. Rhizobium inoculation of the seed may substitute costly N-fertilizers and provide a useful way of achieving sustainable production. Hence, to supply an adequate rhizobial population in the rhizosphere, seed inoculation of chickpea with an effective and importunate rhizobial strain is essential in soils having no/feeble bacterial existence and has revealed optimistic effect on nodule number and mass, growth, yield, and its attributes over uninoculated ones. Its effect has been influenced by N content and P-deficiency of soil, rhizobium strain, variety, T°, pH, salinity, and moisture stress. Phosphorus (P) demand is high in chickpeas, and P deficiency also has a negative effect on chickpea production success. Several research results revealed significant effects of P rate (30–200 kg P2O5 ha−1) on nodule number, mass, and rating plant−1; LAI, RGR, DM, plant height, and branches plant−1; pods and grains plant−1, grain and biomass yields, 100-grain weight, and HI compared to the control. P rates response has been affected by moisture level, pH, available P and N, and variety. Particularly, joint use of P rate and rhizobium inoculation on chickpea has been stated to improve nodulation, growth, and yield and soil fertility. Various studies on the integrated use of P rate and rhizobium inoculation under varying situations showed enhanced nodulation, growth, and yield over the P rate or rhizobium inoculation alone. This might be attributed to adequate P supply and improved utilization with the provision of suitable N2-fixing bacteria for enhanced nodulation and adequate N supply.

Abstract Objective This study was performed with the objective of identifying the proportion of emerging and endemic livestock diseases using cross sectional survey. Result A total of 285 clinically diseased animals were presented to a veterinary clinic and diagnosed tentatively based on history, clinical sign, and simple laboratory diagnostics and from the study, actinomycosis (15.83%), mastitis (15%), tick infestation (10%), respiratory diseases (9.16%) and gastro intestinal parasitism (9.16%) were confirmed with higher proportion in large animals. Pasteurollosis (38, 31%), contagious ecthyma (12, 10%), tick infestation (9, 0%), mite infestation (9, 10%), sheep and goat pox (9, 10%), and gastrointestinal parasitism (9, 17%) were frequently encountered diseases in sheep and goat respectively. In equids, back sore, epizootic lymphangitis and lameness accounted a proportion of 22.95, 21.31, and 13.11% respectively. In conclusion, result of the present study showed that the proportion of livestock disease is high, and it affects the socioeconomic status of the local community in the study area as a result of mortality and production loss.

This study was conducted during 2018/19 under drip irrigation in the dry season to examine the effect of irrigation and N levels on yield, economic performance, and incidence of blossom end rot (BER) on tomato. A 3 × 4 factorial design with subdivided plots was implemented. Three irrigation levels (50%, 75%, and 100% ETc) were randomly assigned in the main plots and four N levels (0, 46, 92, and 138 kg ha−1) to the subplots. Climate data were imported into AquaCrop model climate dataset for determining irrigation water amount and irrigation scheduling. Irrigation scheduling was determined using the FAO AquaCrop model. Data were subjected to analysis of variance (ANOVA) using GenStat software. There was significant interaction effect of irrigation and N levels on yield, yield parameters, and BER incidence on tomato. Highest fruit diameter and fruit length were attained from the combined application of 75% ETc and 138 kg N ha−1. Besides, maximum fruits per plant and marketable yield were obtained under combined use of 100% ETc with 138 kg N ha−1 and 75% ETc with 92 kg N ha−1, respectively, whereas lowest yield performance was recorded when 50% ETc is coupled with 0 kg N ha−1. However, highest (21.91%) and lowest (7.03%) BER incidence was found under the combined use of 50% ETc and 0 kg N ha−1 100% ETc and 92 kg N ha−1, respectively. The economic analysis revealed that application of 46 kg N ha−1 was economically feasible irrespective of the irrigation water levels.

Abstract Background Communicable diseases are the leading causes of illness, deaths, and disability in sub-Saharan Africa. To address these threats, countries within the World Health Organization (WHO) African region adopted a regional strategy called Integrated Disease Surveillance and Response (IDSR). This strategy calls for streamlining resources, tools, and approaches to better detect and respond to the region's priority communicable disease. The purpose of this study was to analyze the incremental costs of establishing and subsequently operating activities for detection and response to the priority diseases under the IDSR. Methods We collected cost data for IDSR activities at central, regional, district, and primary health care center levels from Burkina Faso, Eritrea, and Mali, countries where IDSR is being fully implemented. These cost data included personnel, transportation items, office consumable goods, media campaigns, laboratory and response materials and supplies, and annual depreciation of buildings, equipment, and vehicles. Results Over the period studied (2002–2005), the average cost to implement the IDSR program in Eritrea was $0.16 per capita, $0.04 in Burkina Faso and $0.02 in Mali. In each country, the mean annual cost of IDSR was dependent on the health structure level, ranging from $35,899 to $69,920 at the region level, $10,790 to $13,941 at the district level, and $1,181 to $1,240 at the primary health care center level. The proportions spent on each IDSR activity varied due to demand for special items (e.g., equipment, supplies, drugs and vaccines), service availability, distance, and the epidemiological profile of the country. Conclusion This study demonstrates that the IDSR strategy can be considered a low cost public health system although the benefits have yet to be quantified. These data can also be used in future studies of the cost-effectiveness of IDSR.

Abstract Background Malaria is a huge public health problem in Africa that is responsible for more than one million deaths annually. In line with the Roll Back Malaria initiative and the Abuja Declaration, Eritrea and other African countries have intensified their fight against malaria. This study examines the impact of Eritrea's Roll Back Malaria Programme: 2000–2004 and the effects and possible interactions between the public health interventions in use. Methods This study employed cross-sectional survey to collect data from households, community and health facilities on coverage and usage of Insecticide-Treated Nets (ITNs), Indoor Residual Spraying (IRS), larvicidal activities and malaria case management. Comparative data was obtained from a similar survey carried out in 2001. Data from the Health Management Information System (HMIS) and reports of the annual assessments by the National Malaria Control Programme was used to assess impact. Time series model (ARIMA) was used to assess association. Results In the period 2000–2004, approximately 874,000 ITNs were distributed and 13,109 health workers and community health agents were trained on malaria case management. In 2004, approximately 81% households owned at least one net, of which 73% were ITNs and 58.6% of children 0–5 years slept under a net. The proportion of malaria cases managed by community health agents rose from 50% in 1999 to 78% in 2004. IRS coverage increased with the combined amount of DDT and Malathion used rising from 6,444 kg, in 2000 to 43,491 kg, in 2004, increasing the population protected from 117,017 to 259,420. Drug resistance necessitated regimen change to chloroquine plus sulfadoxine-pyrimethamine. During the period, there was a steep decline in malaria morbidity and case fatality by 84% and 40% respectively. Malaria morbidity was strongly correlated to the numbers of ITNs distributed (β = -0.125, p < 0.005) and the amount (kg) of DDT and Malathion used for IRS (β = -2.352, p < 0.05). The correlation between malaria case fatality and ITNs, IRS, population protected and annual rainfall was not statistically significant. Conclusion Eritrea has within 5 years attained key Roll Back Malaria targets. ITNs and IRS contributed most to reducing malaria morbidity.

Most pancreatic islets are destroyed immediately after intraportal transplantation by an instant blood-mediated inflammatory reaction (IBMIR) generated through activation of coagulation, complement, and proinflammatory pathways. Thus, effective mitigation of IBMIR may be contingent on the combined use of agents targeting these pathways for modulation. CD47 and thrombomodulin (TM) are two molecules with distinct functions in regulating coagulation and proinflammatory responses. We previously reported that the islet surface can be modified with biotin for transient display of novel forms of these two molecules chimeric with streptavidin (SA), that is, thrombomodulin chimeric with SA (SA-TM) and CD47 chimeric with SA (SA-CD47), as single agents with improved engraftment following intraportal transplantation. This study aimed to test whether islets can be coengineered with SA-TM and SA-CD47 molecules as a combinatorial approach to improve engraftment by inhibiting IBMIR. Mouse islets were effectively coengineered with both molecules without a detectable negative impact on their viability and metabolic function. Coengineered islets were refractory to destruction by IBMIR ex vivo and showed enhanced engraftment and sustained function in a marginal mass syngeneic intraportal transplantation model. Improved engraftment correlated with a reduction in intragraft innate immune infiltrates, particularly neutrophils and M1 macrophages. Moreover, transcripts for various intragraft procoagulatory and proinflammatory agents, including tissue factor, HMGB1 (high-mobility group box-1), IL-1β, IL-6, TNF-α, IFN-γ, and MIP-1α, were significantly reduced in coengineered islets. These data demonstrate that the transient codisplay of SA-TM and SA-CD47 proteins on the islet surface is a facile and effective platform to modulate procoagulatory and inflammatory responses with implications for both autologous and allogeneic islet transplantation., (Copyright © 2024 by The American Association of Immunologists, Inc.)

Background: Eritrea has no data on type 1 diabetes incidence in children and youth; therefore, a study was undertaken to determine this in persons aged <25 years., Methods: Data were collected on new type 1 diabetes diagnoses during 2019, from district, provincial and national hospitals. Type 1 diabetes was diagnosed according to standard WHO criteria. No secondary ascertainment source was available. 95% confidence intervals were computed based on approximation to the Poisson distribution, and age and gender effects were analysed with Poisson regression., Results: There were 532 new cases of type 1 diabetes. Mean ± standard deviation (range) age of diagnosis was 16.2 ± 5.7 (1.5-24.9) years, and peak age group was 15-19 years (n = 200, 37.6%), with mode at 18 years. Incidence <15 years was 11.5/100,000 individuals [9.9-13.2], with the highest incidence in the 10-14 years group (19.0/100,000 [15.5-23.1]). Incidence then peaked in the 15-19 years age group (50.2/100,000 [43.5-57.7]) and remained high in the 20-24 years group (46.2/100,000 [39.0-54.3]). There was a male:female ratio of 1.37 (p = 0.001). Two hundred and thirty-eight (44.7%) presented in diabetic ketoacidosis., Conclusion: Type 1 diabetes incidence in Eritrea is moderate <15 years, and high 15-24 years. The 15-19 and 20-24 years rates appear to be the highest published to date. Given the study was only for one year, further confirmatory prospective information will clarify the situation and document trends. Background: Depression is consistently associated with increased risk of Human Immunodeficiency Virus infection and poor antiretroviral treatment adherence. Though many factors have been reported as determinant factors of depression, site-specific evidence is needed to identify factors associated with depression among adults on antiretroviral treatment., Methods: An institution based cross-sectional study was carried out from March to May 2015 among 411 adults HIV/AIDS patients on ART clinic follow-up. Participants were selected using systematic random sampling techniques. Data were collected using chart review and interviewer- administered techniques. Both bivariable and multivariable logistic regressions were used to compute the statistical test associations by SPSS version-20. Variables with p value < 0.05 were considered as statistically significant., Results: Four hundred eleven patients with a mean age ± Standard Deviation of 36.1±9.2 years and with a total response rate of 97.6% were enrolled in the study. The prevalence of depression was 14.6% (95% CI, 10.90-18.2). Factors independently associated with depression were nonadherence to ART, eating two meals per day or less, having side effect of ART medication, being in the WHO Stage II or above of HIV/AIDS, and living alone with AOR (95% CI) of 3.3 (1.436, 7.759), 2.8 (1.382, 5.794), 4.7 (1.317, 16.514), 2.8 (0.142, 0.786), and 2.4 (1.097, 5.429), respectively., Conclusion: Though the magnitude of depression was found relatively low, it was commonly observed as a mental health problem among adult patients with HIV/AIDS on ART. Programs on counseling and close follow-up of adherence to ART, drug side effects, and nutrition should be strengthened. Health facilities should link adult patients with HIV/AIDS who live alone to governmental and nongovernmental social supporter organizations.

Purpose: To assess Eritrea's progress towards elimination of trachoma as a public health problem, we reviewed and compiled current knowledge on the distribution and burden of trachoma in Eritrea, then undertook further population-based surveys where indicated, with support from the Global Trachoma Mapping Project (GTMP)., Methods: For the systematic review, undertaken in March 2014, we searched (1) PubMed, using the terms ((blind* or trachoma or trichiasis) AND Eritrea); (2) the online database of rapid assessments of avoidable blindness; (3) our own grey literature collections; and (4) the Global Atlas of Trachoma database. In June and July 2014, we conducted nine population-based prevalence surveys, for each of which 30 villages were systematically selected with probability proportional to population size; in each village, 30 households were systematically selected. All consenting residents of selected households aged ≥1 year were examined by GTMP-certified graders for signs of trachoma. Data on household-level access to water and sanitation were also collected., Results: One previous rapid assessment of avoidable blindness, three peer-reviewed publications, and two grey literature reports detailing sets of trachoma prevalence surveys conducted in 2006 and 2011, respectively, were located. Post-intervention impact surveys were needed in seven evaluation units (EUs, framed at sub-Zoba-level: population range 40,000-120,000) of Debub and Northern Red Sea, while baseline surveys were needed in two EUs of Anseba. Four of the seven impact survey EUs and both baseline survey EUs returned trachomatous inflammation-follicular prevalences in 1-9-year-olds of ≥5%; six of the seven impact survey EUs and one of the two baseline survey EUs returned trichiasis prevalences in ≥15-year-olds of ≥0.2%. The prevalence of access to water and sanitation varied widely between EUs., Conclusion: Interventions are still required in Eritrea to eliminate trachoma as a public health problem. Data from these surveys will guide the Ministry of Health to undertake programme planning using a sound evidence base.

Background: The geographic distribution and burden of dengue is increasing globally. This study aims to evaluate dengue outbreaks and to substantiate the need for strengthened surveillance, reporting and control in Eritrea., Methods: Data from two cross-sectional dengue epidemic investigations in 2005 and 2010 were analyzed. Samples were tested for dengue virus-specific IgM and IgG antibodies using capture enzyme-linked immunosorbent assays. Dengue vectors' breeding attributes were characterized and epidemic risk indices determined. National routine surveillance weekly reports from 2005 to the second quarter of 2015 were analyzed for spatiotemporal trends., Results: Dengue outbreaks increased in Eritrea from 2005 to 2015 with clinical presentation varying markedly among patients. The house and container indices for Aedes aegypti were 40 and 39.6 % respectively, with containers having A. aeqypti varying significantly ( P  < 0.04). Serum from 33.3 % ( n  = 15) and 88 % ( n  = 26) of clinical dengue cases in Aroget sub-Zoba (district) of Gash Barka Zoba (region) contained anti-DENV IgM antibody in 2005 and 2006, respectively. The national surveillance data from 2005 to 2015 indicate an overall spatiotemporal increase of dengue fever., Conclusions: The increase in dengue outbreaks has been confirmed in Eritrea and necessitates strengthening of surveillance and health worker and laboratory capacity, as well as targeted vector control interventions.
