Abstract

Background: The performance of 'high end' paediatric resuscitation is a rare event in emergency medicine. A recent study demonstrated that paediatric intubation occurs less than once for every 1000 paediatric ED attendances.1 Clinical exposure to resuscitative procedures is very low, with attendant risks of skill deterioration.2 Due to low frequency, high stakes and high stress, paediatric resuscitation scenarios are at high risk for medication errors, which may be significant.3 Various resources are available for emergency clinicians to determine medication dosing in emergency situations. Despite this errors still occur. In order to address this issue, our paediatric resuscitation committee has recently developed a new paediatric resuscitation dosing resource, which provides medication doses and instructions on dilution. Objective(s): To provide an overview of the development and initial evaluation of a weight-based paediatric resuscitation dosing manual. Method(s): Cross-sectional survey of end-users of the dosing manual. Result(s): To be presented. Data collection currently ongoing. Interim results suggest that the manual is rated highly by paediatric medical and nursing staff. Major themes include increased confidence in resuscitation situations, improved performance in simulated paediatric resuscitation, and less risk of medication error Conclusion(s): To be presented. We feel that the new paediatric resuscitation dosing manual is a useful resource for clinical settings which may be faced with a critically ill child.

Abstract

The mature preinvasive chorionic girdles of horse, mule, donkey and extraspecies donkey-in-horse conceptuses, and the very young endometrial cups on d 37 of gestation in mares carrying horse, mule and transferred donkey-in-horse conceptuses, were compared histologically and ultrastructurally to determine possible mechanisms underlying failure of endometrial cup development in the donkey-in-horse model of equine abortion. The progenitor chorionic girdle from the failing donkey-in-house pregnancy was similar in size to the normal donkey chorionic girdle but the trophoblast cells within the former were smaller, less organised and showed definite signs of degeneration and pyknosis. In the 37 d endometrial cups both in the horse and mule pregnancies, the recently invaded, differentiated and enlarging endometrial cup cells had penetrated deeply into the endometrial stroma and were becoming tightly packed between the persisting endometrial glands. In the donkey-in-horse pregnancy, on the other hand, relatively few donkey chorionic girdle cells had begun the invasion process and the majority of these, having penetrated and dislodged the horse luminal epithelium, did not penetrate the basement membrane beneath. Very few cells had reached the endometrial stroma and these had already attracted considerable numbers of lymphocytes to the area. It is concluded that unknown factors in the horse uterus affect adversely all phases of the development, attachment and invasion of donkey chorionic girdle cells, thereby leading to very little or no endometrial cup development and equine chorionic gonadotropin secretion in the extraspecific donkey-in-horse pregnancy created by embryo transfer.

Abstract

Differential Strömgren ${uvby}$observations from the Four College Automated Photoelectric Telescope (FCAPT) are presented for the CP stars HR 149 and HR 7911 and the mCP stars HD 32966 and HD 171782. The Mercury-Manganese star HR 149 is found to be constant while HR 7911 with a period of 5.9617 days probably has a component which is a magnetic CP star. Improved periods were derived for HD 32966 and HD 171782 of 3.0927 and 4.4674 days, respectively. The former is a rather large amplitude mCP star. Both have at least one light curve different in shape from the others observed.

49. A phase I study of Triapine in combination with doxorubicin in patients with advanced solid tumors.

Abstract

Purpose: To assess the maximum-tolerated dose (MTD), dose-limiting toxicity (DLT), pharmacokinetics and antitumor activity of Triapine administered in combination with doxorubicin.Study Design: Patients were treated with doxorubicin intravenously (IV) on day 1 and Triapine IV on days 1-4 of a 21-day cycle. The starting dose (level 1) was doxorubicin 60 mg/m(2) and Triapine 25 mg/m(2). PK analysis was performed at various time-points before and after treatment.Results: Twenty patients received a total of 49 courses of treatment on study. At dose level 2 (doxorubicin 60 mg/m(2), Triapine 45 mg/m(2)), two patients experienced DLTs (febrile neutropenia, grade 4 thrombocytopenia). An additional three patients were enrolled at dose level 1 without initial toxicity. Enrollment then resumed at dose level 2a with a decreased dose of doxorubicin (45 mg/m(2)) with Triapine 45 mg/m(2). The two patients enrolled on this level had two DLTs (diarrhea, CVA). Enrollment was planned to resume at dose level 1; however, the sixth patient enrolled to this cohort developed grade 5 heart failure (ejection fraction 20%, pretreatment EF 62%) after the second course. Thus, doxorubicin and Triapine were reduced to 45 and 25 mg/m(2), respectively (level 1a), prior to resuming enrollment at dose level 1, the MTD. The main drug-related toxicity was myelosuppression. Non-hematologic toxicities included mild-to-moderate fatigue, grade 3 diarrhea and grade 4 CVA. There was one treatment-related death due to heart failure. While no objective responses were observed, subjective evidence of clinical activity was observed in patients with refractory melanoma and prostate cancer.Conclusions: Pretreated patients with advanced malignancies can tolerate the combination of Triapine and doxorubicin at doses that achieve subjective clinical benefit with the main treatment-related toxicities being myelosuppression and fatigue. The MTD was determined to be doxorubicin 60 mg/m(2) on day 1 and Triapine 25 mg/m(2) on days 1-4 of a 21-day cycle. [ABSTRACT FROM AUTHOR]
