Background: Frontonasal dysplasia type-2(FND2), a rare phenotypically variable and heterogeneous developmental anomaly resulting from mutation of the ALX4 gene, is primarily characterized by malformation of the skull and facial skeleton. This study was designed to showcase a clinical, imaging, and genetic analysis of FND2 in a consanguineous family of Bangladeshi origin. Methodology: Clinical imaging and whole genome sequencing of mother, father and patient was done by using Nextera DNA flex library preparation kit (Illumina, USA) using Novaseq 6000 next generation sequencer to find out ALX4 mutation which causes FND2 in patient. Result: We report the clinical as well as molecular findings in an 8-year-old girl with FND2. The child presented with various characteristic features of skull and facial anomalies associated with FND 2 along with numerous other features many of which have not been described in previous literature. The parents also showed some key clinical, radiological, and genetic features of FND 2. The whole genome sequencing (WGS) revealed homozygosity for a 793C-T transition in the ALX4 gene, which resulted in premature termination at codon 265 (p.Arg265Ter). Both of her parents were heterozygous carriers of ALX4 mutation. Conclusions: This is the first report that associates clinical, imaging, and genomics analyses in a Bangladeshi patient for better understanding the disease FND2. These results will facilitate diagnosis and genetic counseling of the future FND2 patients.

Plastics utilized in packaging have a significant impact on the environment, leading to considerable concerns regarding human and environmental well-being. Researchers globally are working to counteract these incidents by integrating biopolymers like starch, cellulose, chitosan, etc., into the packaging sector because of their nontoxic nature, biodegradability, and eco-friendly properties. This study aims to extract cellulose from jackfruit (Artocarpus heterophyllus) peel by combining bleaching and alkaline treatment (17.5% w/v NaOH) and utilizes the extracted cellulosic compounds to produce a practical biodegradable film. The constructed film can be an alternative to synthetic films currently used in industries, minimize environmental harm caused by plastics, and offer a waste management option for jackfruit peels. The study extracted 28.04% cellulose from jackfruit peel wastes, and it was subsequently utilized to develop a biocompatible composite film containing polyvinyl alcohol (PVA) and extracted cellulose. The percentage of cellulose being used in PVA is 0%, 20%, 50%, and 80% compared to pure PVA film. Mechanical properties (tensile atrength, elastic modulus, tensile energy absorption, and strain) as well as thermogravimetric analysis (TGA/DTA), Fourier transform infrared (FTIR), water absorption, and soil burial test were done to define the material and functional properties of 0%, 20%, 50%, and 80% cellulose-PVA composite film. Among four films, 20% of the jackfruit-extracted cellulose-reinforced PVA film has shown better results compared to others. It has shown maximum thermal stability at 368.2°C. Conversely, the 50% cellulose-reinforced PVA film has maximum contraction at 57.4°C with a value of 130.6 μm compared to other percentages in terms of thermomechanical analysis. It also shows the maximum water absorption percentage. It is evident from this study that a cellulosic component generated from jackfruit peels can be used with PVA to make biodegradable packaging films.

Background: The surgical management of symptomatic adult Chiari malformation type 1 (CM1) with or without syringomyelia (SM) continues to be a dilemma considering the outcomes. Objectives: The study aimed to compare the clinical outcomes between stealth cranioplasty (SC) and one of the most commonly practiced standard procedures, posterior fossa decompression with duraplasty (PFDD). Clinical outcomes between SC and another procedure posterior fossa decompression (PFD) were also compared. Methods: This comparative cross-sectional study was carried out on 37 males and 16 females symptomatic adult CM1 patients, ranging from 18 to 47 years of age from June 2019 to May 2021. Clinical outcomes were assessed, compared, and analyzed in terms of changes in clinical symptoms and signs, chicago chiari outcome scale (CCOS) score, and occurrence of complications. Results: Of the 53 patients, 23, 19 and 11 underwent SC, PFDD, and PFD, respectively. There were no significant post-operative changes in symptoms and signs among groups except changes in limb weakness between SC and PFDD (P=0.004). Considering average CCOS score, SC performed better only than PFDD (P=0.003), while category-wise SC was better than both PFDD (P=0.004) and PFD (P=0.010). Considering complications, the PFDD group had a significantly higher rate of complications than the SC group (P=0.001), while there was no significant difference in the rate of complications between the PFD and SC groups. This case report describes a case of avulsed left maxillary central incisor tooth which was replanted in the dental office. The tooth was gently rinsed of any debris and placed in normal saline during the examination and preparation of the replantation socket. The tooth was replanted and functionally splinted. The following week the tooth was opened for pulp extirpation and placement of calcium hydroxide. Two weeks later, the root canal was filled with gutta-percha and zinc oxide eugenol sealer, and the access cavity was restored with a bonded composite restoration; the splint was removed. Objective: To evaluate the clinical and radiological outcome of MTA in nonvital teeth with open apices. Methods: Twenty-five non-vital teeth with open apices were treated with MTA apexification procedure. Standard endodontic procedures were followed and an apical plug of at least 5 mm was created by using MTA after a calcium hydroxide intra-canal dressing for at least 1 week. Final obturation was done after 24 hours by vertical compaction technique using gutta percha. Patients were recalled at 3, 6, 9 and 12 months interval. Clinical outcome was evaluated by assessing pain, tenderness, mobility and sinus tract. Blind to the treatment record, two examiners assessed the pre-treatment and post-treatment radiographs. Each radiograph was scored with the Periapical Index (PAI) and the size of the apical lesion was measured. The presence of an apical bridge over MTA was also noted. Results: Clinically 92% success rate was found whereas radiologically absolute success rate was 84%. Before treatment the mean PAI was 3.6 and mean size of the lesion was 3.24 mm. But, after 12 months follow up, the mean PAI was 1.36 and the mean lesion size was 0.68 mm. An apical barrier over MTA was distinguishable in 5 cases. Conclusion: Apexification using MTA can be considered as a predictable treatment option than calcium hydroxide apexification. DOI: http://dx.doi.org/10.3329/updcj.v3i2.17994 Update Dent. Reattachment of a fractured fragment to the remaining tooth is challenging but one of the best treatment protocol in regards to aesthetics, function as well as patients acceptance. This case report presents a 20 years old male with an oblique complicated crown fracture of maxillary left central incisor tooth. The procedure used to repair the fracture regarding this case included flap surgery with endodontic treatment. The root canal was filled with a root canal sealer and gutta-percha. After root canal obturation, fragments were reattached with duel cure composite resin using a glass fibre post. After 12 months evaluation, clinical and radiographic examinations showed a stable re-attachment, good aesthetics and healthy periodontium. Background: The greatest threats to developing teeth are dental caries and traumatic injury. If pulpal exposure occurs, then a pulpotomy procedure aims to preserve pulp vitality to allow for normal root development. Historically, calcium hydroxide has been the material of choice for pulpotomy procedures. Recently, an alternative material called mineral trioxide aggregate (MTA) has demonstrated the ability to induce hard-tissue formation in pulpal tissue. Objectives: This prospective study was conducted to observe the clinical and radiological findings of pulpotomies done with Mineral Trioxide Aggregate (MTA) &Calcium Hydroxide{Ca(OH)2} and to evaluate success rate of MTA in maintaining pulpal health in teeth with carious lesion. Materials and Methods: In this clinical trial study, 40 primary molars were treated by a conventional pulpotomy technique. They all fulfill the inclusion criteria and gave consent regarding this study. All selected teeth were evenly divided into 2 test groups. In group 1, the MTA pulpotomy (experimental) was performed, whereas in group 2, the conventional Ca(OH)2 pulpotomy (control) was done. The teeth of both groups were finally restored by Glass Ionomer Cement (GIC). The children were recalled for clinical and radiographic evaluations after 3, 6, and 12 months. Result: The follow-up evaluations revealed failure due to pain and swelling detected at 6 and 12 months postoperative evaluations in only 3 teeth treated with Ca(OH)2 and one treated with MTA. The remaining 36 teeth appeared to be clinically and radiographically successful 12 months postoperatively. Calcific metamorphosis was a radiographic finding in 4 teeth treated with MTA and 2 teeth treated with Ca(OH)2. Conclusion: Based on this clinical and radiographical evaluation study of 3, 6, and 12 months follow-up, MTA could be used as a safe material for pulpotomy in cariously and mechanically exposed primary molars and seems to be a suitable alternative to calcium hydroxide. Palato-gingival groove is a developmental anomaly often affecting the maxillary lateral incisor. The aim of this presentation is to describe the clinical management of a maxillary lateral incisor tooth with a palato-gingival groove with perio-endo lesion having immature apex. Despite complex anatomy, this case was managed by using Mineral Trioxide Aggregate plug technique with as a non-surgical endodontic treatment. Background: The presence of a single root canal in mandibular first premolar cannot be assumed always. The variability in canal morpholohy of root canal includes the number & shape of the canal as well as pathways of the canals. Methods: In this study, 100 mandibular first premolar teeth were evaluated by clearing technique. Collected teeth were cleaned and merged with 5.25% NaOCl for 48 hours. Then the teeth were decalcified with 5% nitric acid for 72 hours followed by dehydrated sequentially with 80-100% alcohol. After dehydration, Indian Ink was injected into the canal through the previously prepared access cavity. Finally the teeth were made transperent by 98% methylsalicylate and examined. Results: Out of 100 mandibular first premolar teeth, 89 were single rooted, 10 with double rooted and only 1 was triple. On evaluation of canal configuration according to Weine classification, mandibular first premolars had 64% type I, 5% type II, 22% type III and 9% type IV. Apical delta was found incase of 8% mandibular first premolar tooth. Conclusion: Based on this study, mandibular first premolar teeth of Bangladeshi population have multiple roots and canals and variable canal configurations. Abstract Background Beck Depression Inventory (BDI‐II) is a widely used valid instrument to assess the severity of depression in clinical and normal settings. To meet the necessity of a standard scale for measuring depression among above 265 million Bangla speaking population around the world, this scale was translated and validated. Methods Two translations of BDI‐II into Bangla were prepared, and then, two back translations were done by medical and language experts in parallel. Thereafter, sentence revision followed by pretest on 20 respondents was done to finalize the Bangla version of BDI‐II (BDI‐II BV). Afterward, a cross‐sectional, comparative, and descriptive study was conducted to validate the scale by purposive sampling technique consisting of 111 persons (both clinical and normal) in three tertiary‐level hospitals in Bangladesh. Everyone was given to fill up BDI‐II BV at first. Then, they were given to fill up BDI‐II BV (n = 49), Bangla version of Depression Anxiety Stress Scales 21‐item (DASS21‐BV, n = 47) and BDI‐II (n = 25) 3–7 days later. The diagnosis of depressive disorder was made according to DSM‐5. Correlation study and factor analysis were completed. Results The mean age was 28.83(±8.70) years. The male–female ratio was 1:0.82. Correlation of scores for BDI‐II BV with the DASS21‐BV depression subscale was .920; BDI‐II BV with BDI‐II was .985 (Cronbach's α .993; t test not significant) and BDI‐II BV applied first and the second time was .960 (Cronbach's α .979; z test not significant). The Black Bengal goat (BBG) is a dwarf sized heritage goat ( Capra hircus) breed from Bangladesh , and is well known for its high fertility, excellent meat and skin quality. Here we present the first whole genome sequence and genome-wide distributed single nucleotide polymorphisms (SNPs) of the BBG. A total of 833,469,900 raw reads consisting of 125,020,485,000 bases were obtained by sequencing one male BBG sample. The reads were aligned to the San Clemente and the Yunnan black goat genome which resulted in 98.65% (properly paired, 94.81%) and 98.50% (properly paired, 97.10%) of the reads aligning, respectively. Notably, the estimated sequencing coverages were 48.22X and 44.28X compared to published San Clemente and the Yunnan black goat genomes respectively. On the other hand, a total of 9,497,875 high quality SNPs (Q ≥ 20) along with 1,023,359 indels, and 8,746,849 high quality SNPs along with 842,706 indels were identified in BBG against the San Clemente and Yunnan black goat genomes respectively. The dataset is publicly available from NCBI BioSample (SAMN10391846), Sequence Read Archive (SRR8182317, SRR8549413 and SRR8549904), with BioProject ID PRJNA504436 . These data might be useful genomic resources in conducting genome wide association studies, identification of quantitative trait loci (QTLs) and functional genomic analysis of the Black Bengal goat.

A 18-year-old boy came with the complaints of mild pain during mastication on the lower right posterior tooth for two weeks. He gave the history of severe pain on offending tooth one month before, which subsided within a few days after taking medication from a local pharmacy. On clinical examination, a deep occlusal caries was present on the lower right first molar tooth. The tooth was tender to the vertical percussion but not associated with any swelling or discharge. The vitality test revealed that the tooth was non-vital.

A 22-year-old male reported to the outpatient department with the complaints of occasional pain and tingling sensation on his maxillary left posterior teeth and mandibular anterior teeth for 3 to 4 months with slight mobility of mandibular anterior teeth. He had no significant medical history as well as no history of trauma and familial history of similar disease. He also stated that he went to Dhaka Dental College where he received clinical and radiological investigations and diagnosed as a case of external tooth resorption. In the Dhaka Dental College, upper left 1st and 2nd premolar, mandibular right central and lateral incisor teeth were extracted.

Aim and Objective: In this article, we describe a novel technique of reconstruction of posterior fossa by cranioplasty with use of preshaped titanium mesh following posterior fossa decompression (PFD) for Chiari malformation type I (CMI) with syringomyelia (SM) in symptomatic adults. Materials and Methods: Eleven patients underwent limited PFD and expansive cranioplasty with preshaped titanium mesh, what we term as “Stealth Cranioplasty” (SCP), following arachnoid preserving duraplasty (APD) and hexagonal tenting of the duraplasty with the cranioplasty (HTDC) for the management of symptomatic adult CMI with SM. All these patients had syringes extending from 3 to >10 vertebral levels. Results: Seven male and four female symptomatic CMI adult patients, between age ranges of 22 and 44 years (mean 29.45 years), presented with different neurological symptoms related to CMI and SM for 6–84 months (mean 37.09 months). All the patients underwent PFD, APD followed by SCP and HTDC and were followed up for 7–54 months (mean 35.90 months). Of 11 patients, 8 patients improved according to the Chicago Chiari Outcome Scale (CCOS) with score of 13–15 while 3 patients remained unchanged with CCOS of 12, and there was no worsening. There was no complication related to Chiari surgery in any of the patients. All the patients had good reestablishment of cisterna magna. Two patients had marked reduction of syrinx while eight patients had moderate-to-mild reduction and one patient had no change of syrinx. None of the patients needed redo surgery. Conclusion: SCP is an effective, fruitful, and cost-effective technique for the management of symptomatic adult CMI with SM. This technique has the advantages of preventing complications and recurrences in addition to the improvement of symptoms by addressing the basic pathology.

In the present study, the clinical outcome of mineral trioxide aggregate and Biodentine as repairing materials in the management of pulpal floor perforation was compared in vivo. Thirty teeth having pulpal floor perforation were enrolled in this study, out of which 15 perforations were repaired with mineral trioxide aggregate and the remaining 15 were subjected to Biodentine treatment. Following perforation repair, each tooth was then subjected to root canal therapy. Patients were recalled at 3, 6 and 12 month interval. A 33 year old male patient came to the outpatient Department complaining of the bad appearance of his upper left central incisor tooth. He had no history of trauma in that area but the tooth was discolored gradually (Figure 1A). Furthermore, the tooth was asymptomatic. There was no pain and discomfort during mastication. The patient had gastric irritation for the last 5 years and still using omeprazole irregularly. The clinical examination indicates that the left central incisor tooth was yellowish in color and the dentine was found to be exposed (Figure 1B). There was gross tooth tissue loss at the labial surface that includes the incisal edge of the tooth.

The purpose of the present study was to assess the giomer retention following application of one step self etch adhesive and one-step self-etch adhesive plus an extra hydrophobic adhesive layer in non-carious cervical lesion. Twenty four participants having bilateral cervical lesion at the maxillary premolars teeth were selected. Following cleaning of the cervical lesions, 24 left premolar teeth were restored with giomer by using the one-step self-etch system (Group A) and 24 right premolar teeth were restored with giomer by using one-step self-etching plus extra hydrophobic adhesive layer (Group B). Participants were recalled at 6 and 15 months for evaluating the clinical outcome. The results showed that at 15 months, 18 giomer restorations in Group A and 21 in Group B were intact inside the cavity and no significant difference was found between the two groups. It can be concluded that giomer restoration used with one-step self- etching plus additional adhesive layer slightly improved the retention of the material.
