Background and aims: The association between adiposity measures and dyslipidemia has seldom been assessed in a multipopulational setting. Methods and results: 27 populations from Europe, Australia, New Zealand and Canada (WHO MONICA project) using health surveys conducted between 1990 and 1997 in adults aged 35-64 years (n = 40,480). Dyslipidemia was defined as the total/HDL cholesterol ratio >6 (men) and >5 (women).Overall prevalence of dyslipidemia was 25% in men and 23% in women. Logistic regression showed that dyslipidemia was strongly associated with body mass index (BMI) in men and with waist circumference (WC) in women, after adjusting for region, age and smoking. Among normal-weight men and women (BMI84.8 cm) in normal-weight men, menopause in women and regular smoking further defined subgroups at increased risk. Conclusion: standard categories of BMI and WC, or their combinations, do not lead to optimal risk stratification for dyslipidemia in middle-age adults. Sex-specific adaptations are necessary, in particular by taking into account abdominal obesity in normal-weight men, post-menopausal age in women and regular smoking in both sexes

- Abstract
Objectives: We piloted a computerised cognitive training battery in a group of participants with Parkinson's disease without dementia to investigate the relevance of the training to daily life and the feasibility and the acceptability of the tasks. Previous studies of CT have had limited success in the benefits of training, extending to improvements in everyday function. By taking a pragmatic approach and targeting training to the cognitive skills affected by Parkinson's disease (planning, attention, and recollection), whilst using tasks that emulated real-life scenarios, we sought to understand whether participants perceived the training to be effective and to identify the elements of the training that elicited beneficial effects. Methods: Four participants completed a cognitive training session comprising three distinct tasks 5 days a week over two weeks. Participants completed baseline questionnaires examining health-related quality of life, everyday cognition, and apathy before the training period, after the last session, and two weeks after the last session. An interview was held after participants had completed the training. Results: The findings indicated that participants felt the training was acceptable, enhanced their awareness, and encouraged them to monitor their thinking abilities. The group interview indicated that the training was feasible; participants felt the tasks had potential to improve everyday performance, but more supporting information should be provided to facilitate this transfer. Responses to the questionnaires reflected these findings, indicating improvement for some participants' cognition and quality of life. Objective measures supported the subjective reports; there were improvements in some but not all domains. Performance on the planning and recollection tasks improved over the training period, and the evidence for improvement on the attention task was mixed. - Abstract
Background and aims: The association between adiposity measures and dyslipidemia has seldom been assessed in a multipopulational setting. Methods and results: 27 populations from Europe, Australia, New Zealand and Canada (WHO MONICA project) using health surveys conducted between 1990 and 1997 in adults aged 35-64 years (n = 40,480). Dyslipidemia was defined as the total/HDL cholesterol ratio >6 (men) and >5 (women).Overall prevalence of dyslipidemia was 25% in men and 23% in women. Logistic regression showed that dyslipidemia was strongly associated with body mass index (BMI) in men and with waist circumference (WC) in women, after adjusting for region, age and smoking. Among normal-weight men and women (BMI<25 kg/m2), an increase in the odds for being dyslipidemic was observed between lowest and highest WC quartiles (OR = 3.6, p < 0.001). Among obese men (BMI ≥ 30), the corresponding increase was smaller (OR = 1.2, p = 0.036). A similar weakening was observed among women. Classification tree analysis was performed to assign subjects into classes of risk for dyslipidemia. BMI thresholds (25.4 and 29.2 kg/m2) in men and WC thresholds (81.7 and 92.6 cm) in women came out at first stages. High WC (>84.8 cm) in normal-weight men, menopause in women and regular smoking further defined subgroups at increased risk. Conclusion: standard categories of BMI and WC, or their combinations, do not lead to optimal risk stratification for dyslipidemia in middle-age adults. Sex-specific adaptations are necessary, in particular by taking into account abdominal obesity in normal-weight men, post-menopausal age in women and regular smoking in both sexes

Not all patients who undergo surgery for colitis (colon inflammation) can be definitively diagnosed with either ulcerative colitis or Crohn's disease. The incidence of this indeterminate colitis ranges from 9 to 20 percent. A report is presented of the long-term outcome for these patients. There were 53 patients identified; this represented 7.9 percent of patients who underwent colectomy for inflammatory bowel disease at one hospital between 1960 and 1983. There were 46 patients available for follow-up, which ranged from 2.5 years to 28 years (the average time was 10 years). These patients were divided into groups based on information available at the time of surgery: group 1, probable Crohn's disease (19 patients); group 2, probable ulcerative colitis (11 patients); and group 3, indeterminate colitis (16 patients). During follow-up, the probable diagnosis changed in only five patients. One Crohn's disease patient was determined to actually have ulcerative colitis. The four other patients were originally in group 3; three were reclassified with ulcerative colitis and one with Crohn's disease. Indeterminate colitis appears to behave more like ulcerative colitis than Crohn's disease. None of the patients ultimately remaining in group 3 (indeterminate colitis) underwent later small bowel surgery. These findings indicate that patients with indeterminate colitis may undergo the same restorative procedures as patients with ulcerative colitis, including reestablishment of continuity of the intestine (ileoanal pouch anastomosis). - Abstract
A semi-quantitative assessment is made of the animals observed in archived videotapes taken from the research submersible Jago, during diamond mining and exploratory surveys off the mouth of the Orange River on the west coast of southern Africa (28°15′S, 29°11′S) in November 1996. The seabed environment is described and nekton associations with substratum features are identified. The area is characterized by heterogeneity to its physical and biological struture. The variety of observed nekton is low, and communities are dominated by goby Sufflogobius bibarbatus, juvenile hake Merluccius spp. and cuttlefish Sepia spp. (on soft substrata), as well as false jacopever Sebastes capensis and kingklip Genypterus capensis (on rocky substrata).

The rapid development of a saccular aneurysm during successfully treated bacterial endocarditis, in an ascending aorta previously affected by syphilis which had been treated with adequate chemotherapy 15 years previously, is described. Although bacterial endocarditis does occasionally complicate syphilitic aortic valve disease, the literature does not contain any report of resulting aneurysm development. Five cases of herniation of the heart are described, of varying aetiology and presentation. In one, the pericardial deficiency is congenital. Two are traumatic—one with severe disruption of the rib cage, and one without. Two are post-operative—one early, and one late. Their aetiology, diagnosis and treatment are discussed. If the possibility of herniation is considered, the diagnosis may often be made from a plain chest radiograph.
