7. Sleep Management: The Use of Continuous Physiological Monitoring to Support Patient Self-Care

Abstract

Circadian misalignment (CM) is a sleep disruption where an individual’s circadian rhythm and actual sleep-wake cycle are unsynchronized. CM is frequently misdiagnosed and difficult to treat but can adversely impact short-term and long-term health outcomes. CM often interacts bidirectionally with chronic conditions affecting cognitive and physical functionality. Continuous physiological monitoring (CPM) of the hormone melatonin would support real-time, patient-driven interventions to address CM. However, patient-driven self-care tasks involve complex information processing and decision-making. Effective CPM use requires situation awareness regarding current circadian health and projected future status, sufficient background knowledge to identify circadian intervention alternatives, and selection of appropriate alternatives given their current state. Melatonin data has characteristics distinct from biomarkers previously subject to CPM, introducing further challenges around data presentation. Task analysis efforts can identify the cognitive processes involved in five different tasks associated with a continuous melatonin monitor to identify necessary informational elements to include in interface design.

8. Parents and Teachers: Partners in Whole-Person Formation.

Abstract

Education may promote the development of students' whole-person identity formation--the spiritual, psychological, social, emotional, and intellectual. This paper considers how identify formation and self-esteem are related, how parents form the foundation for identity, and how teachers can contribute to further growth. Erikson's theory of psychological development and Lickona's theory of identity formation are used as the basis for discussion. Part 1 of the paper focuses on practices of home and school that foster the foundational elements of security, autonomy, initiative, and industry. Part 2 identifies how characteristics of the home environment nurture positive identity formation through effective communication, consistency, accountability, and a value-clarifying atmosphere. Part 3 describes mechanisms of school behavior that are related to the foundational issues of self-esteem and achievement, especially through holding high, but realistic, expectations for students. Part 4 deals with how the school environment can support positive identity formation through establishing systems that support success, using appropriate direction of student progress, setting realistic goals based on developmental readiness, teaching children how to manage time, moving toward increasing student independence, equipping children with skills needed to achieve expectations, holding children accountable for responsible school practices, and providing opportunities to apply their learning. 12. Long-term outcomes in patients treated with tissue-sparing posterior cervical fusion to revise a 1-level pseudarthrosis following ACDF.

Abstract

• This observational study assessed long-term radiographic outcomes in 45 subjects treated with a tissue-sparing PCF technique to revise a 1-level pseudarthrosis following ACDF. • Assessed a median 39 months after revision, investigators identified solid fusion in 91 % of cases. An independent imaging lab measured segmental range of motion less than 2° in 93 % of cases and observed contiguous bridging bone across the segment in 80 % of cases. • This tissue-sparing technique required a median 49 min to complete with an estimated 10ccs of blood lost. Subjects were discharged after a median 1 night in the hospital. • There were no instances of hospital re-admission nor subsequent surgical intervention for persistent pseudarthrosis. The overall rate of adverse events was 2.2%. Observational Study Symptomatic pseudarthrosis is one long-term complication in patients treated with anterior discectomy and fusion (ACDF). When revising a pseudarthrosis, a surgeon must decide to intervene posteriorly and/or anteriorly. Open posterior cervical fusion (PCF) is attractive for high rates of arthrodesis, however this technique introduces risks of added complications resulting from extensive soft tissue dissection. The purpose of this study was to assess long-term outcomes in patients undergoing tissue-sparing PCF with facet instrumentation to treat a single level pseudarthrosis. Forty-five subjects were recruited from six participating sites. All subjects had a history of ACDF that was subsequently revised with tissue-sparing PCF to treat symptomatic pseudarthrosis at one level. Long-term radiographic assessments included flexion and extension X-ray and multi-planar CT. Subjects additionally completed a patient satisfaction questionnaire. Radiographs were assessed by investigators and an independent core imaging lab to diagnose implant integrity and arthrodesis at the revised levels. The revision procedure required a median 49 min to complete with an estimated blood loss of 10 cc. Subjects were discharged a median 1 day following treatment. There were no instances of hospital re-admission nor subsequent surgical interventions. Study follow-up assessments were performed a median 39 months from revision. Surgeons diagnosed complete fusion in 91 % of cases. The core imaging lab identified bridging bone across the revised segment in 80 % of cases. Range of motion was < 2° in 93 % of cases. Seventy-four percent of subjects reported being satisfied with their outcomes. This study summarizes long-term radiographic outcomes in a cohort of patients receiving tissue-sparing PCF for the treatment of pseudarthrosis. Assessed years after revision, patients achieved rates of arthrodesis similar to open PCF without the soft tissue dissection responsible for perioperative morbidity and long-term soft tissue pain. 16. Impact of Illustrated Postoperative Instructions on Knowledge and Retention During a Cleft Lip and Palate Surgical Mission

Abstract

Objective To determine the impact of illustrated postoperative instructions on patient-caregiver knowledge and retention.Design Prospective study with all participants receiving an educational intervention.Setting Pediatric plastic surgical missions in Guatemala City, Guatemala, between 2019 and 2020.Participants A total of 63 majority-indigenous Guatemalan caregivers of patients receiving cleft lip and/or palate surgery.Intervention Illustrated culturally appropriate postoperative care instructions were iteratively developed and given to caregivers who were surveyed on illustration-based and text-based information at preoperative, postoperative, and four-week follow-up time points.Main Outcome Measure Postoperative care knowledge of illustration-based versus text-based information as determined by the ability to answer 11 illustration- and 8 text-based all-or-nothing questions, as well as retention of knowledge as determined by the same survey given at four weeks follow-up.Results Scores for illustration-based and text-based information both significantly increased after caregivers received the postoperative instructions (+13.30 ± 3.78 % SE, + 11.26 ± 4.81 % SE; P< .05). At follow-up, scores were unchanged for illustration-based (−3.42 ± 4.49 % SE, P> .05), but significantly lower for text-based information (−28.46 ± 6.09 % SE, P< .01). Retention of text-based information at follow-up correlated positively with education level and Spanish literacy, but not for illustration-based.Conclusions In the setting of language and cultural barriers on a surgical mission, understanding of illustration-based and text-based information both increased after verbal explanation of illustrated postoperative instructions. Illustration-based information was more likely to be retained by patient caregivers after four weeks than text-based information, the latter of which correlated with increased education and literacy.

Abstract

Becker muscular dystrophy (BMD) is a genetic neuromuscular disease of growing importance caused by in‐frame, partial loss‐of‐function mutations in the dystrophin (DMD) gene. BMD presents with reduced severity compared with Duchenne muscular dystrophy (DMD), the allelic disorder of complete dystrophin deficiency. Significant therapeutic advancements have been made in DMD, including four FDA‐approved drugs. BMD, however, is understudied and underserved—there are no drugs and few clinical trials. Discordance in therapeutic efforts is due in part to lack of a BMD mouse model which would enable greater understanding of disease and de‐risk potential therapeutics before first‐in‐human trials. Importantly, a BMD mouse model is becoming increasingly critical as emerging DMD dystrophin restoration therapies aim to convert a DMD genotype into a BMD phenotype. We use CRISPR/Cas9 technology to generate bmx(Becker muscular dystrophy, X‐linked) mice, which express an in‐frame ~40 000 bpdeletion of exons 45–47 in the murine Dmdgene, reproducing the most common BMD patient mutation. Here, we characterize muscle pathogenesis using molecular and histological techniques and then test skeletal muscle and cardiac function using muscle function assays and echocardiography. Overall, bmxmice present with significant muscle weakness and heart dysfunction versus wild‐type (WT) mice, despite a substantial improvement in pathology over dystrophin‐null mdx52mice. bmxmice show impaired motor function in grip strength (−39%, P< 0.0001), wire hang (P= 0.0025), and in vivoas well as ex vivoforce assays. In aged bmx, echocardiography reveals decreased heart function through reduced fractional shortening (−25%, P= 0.0036). Additionally, muscle‐specific serum CK is increased >60‐fold (P< 0.0001), indicating increased muscle damage. Histologically, bmxmuscles display increased myofibre size variability (minimal Feret's diameter: P= 0.0017) and centrally located nuclei indicating degeneration/regeneration (P< 0.0001). bmxmuscles also display dystrophic pathology; however, levels of the following parameters are moderate in comparison with mdx52: inflammatory/necrotic foci (P< 0.0001), collagen deposition (+1.4‐fold, P= 0.0217), and sarcolemmal damage measured by intracellular IgM (P= 0.0878). Like BMD patients, bmxmuscles show reduced dystrophin protein levels (~20–50% of WT), whereas Dmdtranscript levels are unchanged. At the molecular level, bmxmuscles express increased levels of inflammatory genes, inflammatory miRNAs and fibrosis genes. The bmxmouse recapitulates BMD disease phenotypes with histological, molecular and functional deficits. Importantly, it can inform both BMD pathology and DMD dystrophin restoration therapies. This novel model will enable further characterization of BMD disease progression, identification of biomarkers, identification of therapeutic targets and new preclinical drug studies aimed at developing therapies for BMD patients.

Abstract

Nutritional strategies to realize the genetic potential of modern pigs without compromising welfare while reducing environmental impact are critical for success in the swine industry today. By harnessing the potential of 25-OH-D3, it is possible to fine-tune mineral utilization, thereby allowing for the reduction of excessive calcium (Ca) and phosphorus (P), while promoting bone development and muscle deposition in finisher pigs [1]. A study was conducted using 60 male pigs [initial body weight (BW) = 29.9 kg ± 0.5 kg] to evaluate the effect of reducing dietary Ca and P and supplementing with 25-OH-D3and triterpenoid, on growth, bone and blood characteristics. Pigs entered the experimental room at 10 wk of age and were randomly allocated to one of six pens in groups of 10. Pigs had a 2-wk adaptation where they were offered a grower diet (NE:12.3MJ, CP:17.5%) before receiving their respective treatments from 12 wk (NE:11.5MJ, CP:16.19%). Three experimental diets were evaluated for 7 wk; Positive control: 0.7% total Ca, 0.36% total P, vit D3at 1,500 IU/kg feed (PC), negative control: PC diet + 30% reduced Ca and 10% reduced P (NC), and NC diet with 1,500 IU D3, in calcidiol form (25-OH-D3, HyD) and triterpenoid (NC+). Pigs had ad libitum access to feed which was recorded individually via automated electronic feeders. At slaughter, blood was collected, and body and carcass composition scans were taken from pigs using dual-energy x-ray absorptiometry (DXA) and metacarpals were collected. Data were analyzed in JMP17.0 using one-way ANOVA, including treatment as a main effect and 10-wk weight as a covariate, with pig as the experimental unit. Table 1 shows the results. Pigs offered NC+ tended to have a heavier slaughter weight compared with NC, with PC being intermediate (P= 0.08). There were no differences in feed-intake, but NC+ increased average daily gain compared with NC (P= 0.03), and NC+ tended to be more efficient (P= 0.08). The DXA-scan demonstrated that bone mineral density and content in NC+ pigs were comparable with PC and NC. Additionally, reducing Ca and P in NC resulted in reduced ash, Ca and P in the metacarpals. Interestingly, NC+ had similar bone P content to PC, indicating that NC+ allows for reduced Ca and P, without compromising bone composition. As expected, NC+ had greater 25-OH-D3blood concentrations (P< 0.001). Although there was no difference in blood for Ca, P concentrations were greater in NC+ compared with other groups (P= 0.007), suggesting that NC+ allowed for greater P circulating concentrations, despite being reduced in the diet. In conclusion, supplementing a reduced Ca and P diet with calcidiol and triterpenoid can enhance weight gain and feed-conversion as well as improving blood 25-OH-D3and bone P concentrations. The use of this solution shows that diets can be formulated to be more sustainable without compromising performance or bone-health.

Abstract

To boost profitability in pig production, it is important to achieve increased lean growth rates and maximize efficiency, especially in grower-finisher pigs where feed intakes are high. Molecular research, especially the study of gene expression, is a proven tool for understanding the precise mechanisms behind muscle tissue growth, including a recent focus on pathways such as the mammalian target of rapamycin (mTOR). In the current work, gene expression analyses were used to study the mode of action of the combination of calcidiol (25-OH-D3, HyD) and triterpenoid, which has shown significant growth and efficiency improvements in grower-finisher pigs (McCormack et al., 2024). Three experimental diets were evaluated for 7 wk; Positive control: 0.7% total Ca, 0.36% total P, vit D3at 1500IU/kg feed (PC), negative control: PC diet + 30% reduced Ca and 10% reduced P (NC), and NC diet with 1,500IU D3in calcidiol form (25-OH-D3, HyD) and triterpenoid (NC+). At 19 wk of age, 16 animals per treatment were sacrificed and tissue samples were taken from the longissimus muscle and preserved in RNA later solution. The RNA was extracted, quality control checked, and mRNA sequenced using the Illumina platform with 150 base pair paired-end sequencing and 30M read depth. The sequenced data were aligned to the Sus scrofa reference genome (Sscrofa11.1) using Kallisto. Data were normalized and analyzed for differentially expressed genes (DEGs) between treatments using DeSeq2. Genes were assigned to biological pathways using Biofractal’s customisation of the Reactome pathway database. For statistical analysis, RNA integrity number and treatment were included in the model. In the PC compared with the NC, there were 71 DEGs using a false discovery rate (FDR) of < 0.10. Altered pathways included the inhibition of cellular stress pathways and increased cell turnover in the muscle of PC compared with NC pigs. In the NC+ compared with the NC, there were 155 DEGs (FDR < 0.10). The most significantly upregulated gene in the NC+ treatment was adenylate cyclase (FDR < 0.001), an upstream regulator of several pathways related to energy metabolism and muscle protein synthesis. This mechanism was confirmed in the pathway analysis, which found that the NC+ treatment activated PKA-mediated phosphorylation of CREB, a pathway that inhibits muscle atrophy. Furthermore, the NC+ treatment activated several pathways related to muscle growth, including the mammalian target of rapamycin complex 1 (mTORC1). In this study the addition of calcidiol (25-OH-D3, HyD) and triterpenoid to the diet was found to specifically affect molecular pathways in the muscle of grower-finisher pigs involved in both the inhibition of muscle breakdown as well as activating muscle growth.

Abstract

IMPORTANCE: Motor vehicle crashes (MVCs) are an important public health concern. Recent trends suggest that introducing rideshare services has decreased the incidence of MVCs. However, detailed analyses linking rideshare volume, convictions for impaired driving, and nonfatal MVC traumas remain inconclusive. OBJECTIVE: To determine if there is an association between rideshare use and MVC traumas and convictions for impaired driving in Houston, Texas. DESIGN, SETTING, AND PARTICIPANTS: This multicenter cohort study was conducted between January 2007 and November 2019 with hospital data from the Red Duke Trauma Institute within the Memorial Hermann Hospital–Texas Medical Center and Ben Taub General Hospital. Rideshare data from Uber and Google covered trips taken within Houston, Texas, from February 2014 (the date of deployment of Uber to Houston) to December 2018. Impaired driving convictions included all indictments made by the Harris County, Texas, District Attorney’s office from January 2007 to December 2018. All adults with MVC traumas evaluated at both centers in the study population (individuals >16 years with a mechanism of injury classified under “motor vehicle collision”) were included. Impaired driving incidents were included only if the final legal outcome was conviction. MAIN OUTCOMES AND MEASURES: The primary study outcomes were the incident rate ratios for hourly MVC traumas and daily impaired driving convictions. RESULTS: A total of 23 491 MVC traumas (involving patients with a mean [SD] age of 37.9 [17.8] years and 14 603 male individuals [62.1%]), 93 742 impaired driving convictions, and more than 24 million Uber rides were analyzed. Following the introduction of Uber in February 2014, MVC traumas decreased by 23.8% (from a mean [SD] of 0.26 [0.04] to 0.21 [0.06] trauma incidents per hour) during peak trauma periods (Friday and Saturday nights). The incident rate ratio of MVC traumas following Uber deployment was 0.33 (95% CI, 0.17-0.67) per 1000 indexed rides (P = .002). Furthermore, rideshare use was associated with a significant, geographically linked reduction in impaired driving convictions between January 2014 to December 2019 (incidence rate ratio, 0.76 [95% CI, 0.73-0.78]; P &lt; .001). CONCLUSIONS AND RELEVANCE: In this study, introducing rideshare services in the Houston metropolitan area was associated with significant reductions in MVC traumas and impaired driving convictions. Increased use of rideshares may be an effective means of reducing impaired driving and decreasing rate of MVC traumas.

Abstract

Absence of dystrophin results in muscular weakness, chronic inflammation and cardiomyopathy in Duchenne muscular dystrophy (DMD). Pharmacological corticosteroids are the DMD standard of care; however, they have harsh side effects and unclear molecular benefits. It is uncertain whether signaling by physiological corticosteroids and their receptors plays a modifying role in the natural etiology of DMD. Here, we knocked out the glucocorticoid receptor (GR, encoded by Nr3c1) specifically in myofibers and cardiomyocytes within wild-type and mdx52 mice to dissect its role in muscular dystrophy. Double-knockout mice showed significantly worse phenotypes than mdx52 littermate controls in measures of grip strength, hang time, inflammatory pathology and gene expression. In the heart, GR deletion acted additively with dystrophin loss to exacerbate cardiomyopathy, resulting in enlarged hearts, pathological gene expression and systolic dysfunction, consistent with imbalanced mineralocorticoid signaling. The results show that physiological GR functions provide a protective role during muscular dystrophy, directly contrasting its degenerative role in other disease states. These data provide new insights into corticosteroids in disease pathophysiology and establish a new model to investigate cell-autonomous roles of nuclear receptors and mechanisms of pharmacological corticosteroids.

49. Speech Outcomes Following Operative Management of Velopharyngeal Dysfunction (VPD) in Non-Syndromic Post-Palatoplasty Cleft Palate Patients

Abstract

Objective Approximately 30% of patients with a history of repaired cleft palate (CP) go on to suffer from velopharyngeal dysfunction (VPD). This study discusses the operative management of VPD and postoperative speech outcomes in a cohort of CP patients.Setting An academic tertiary pediatric care center.Methods Retrospective cohort study.Patients Patients with history of repaired CP (Veau I-IV) who underwent operative management of VPD between January 1st, 2010 and December 31st, 2020. Operative modalities were posterior pharyngeal flap (PPF), sphincter pharyngoplasty (SPP), Furlow palate re-repair, and buccal myomucosal flap palate lengthening (PL).Outcome measures The primary outcome measure is postoperative speech improvement evaluated by the Pittsburgh Weighted Speech Scale (PWSS).Results 97 patients met inclusion criteria. 38 patients with previous straight-line primary palatoplasty underwent Furlow re-repair; these patients were significantly younger (7.62 vs 11.14, P< .001) and were more likely to have severe VPD per PWSS (OR 4.28, P< .01, 95% CI 1.46-12.56) when compared to VPD patients with previous Furlow repair. 21.1% of these patients required an additional non-revisional VPD procedure. The remaining patients underwent a non-revision procedure (26 PPF, 22 SPP, 11 PL); all experienced significant (P< .001 on paired t-test) reductions in PWSS total and subgroup VPD severity scores without difference in improvement between operation types. SPP was statistically associated with all-cause complication (OR 2.79, 95% CI 1.03-7.59, P< .05) and hyponasality (OR 3.27, 95% CI 1.112-9.630, P< .05).Conclusion Furlow re-repair reduced need for additional VPD operations. Speech outcomes between non-revisional operations are comparable, but increased complications were seen in SPP.
