Abstract

Increased awareness by industry of greenhouse gas (GHG) emissions is forcing companies to examine electrical technologies as alternatives to fossil fuel‐driven thermal ones. Spray drying of milk is recognized as a high‐energy process and is of particular importance to New Zealand. It requires inlet air temperatures higher than most existing electrically‐driven heat‐pumps can reliably achieve. Already some companies internationally are considering freezing and shipping milk concentrates rather than drying which begs the question as to whether the GHG emissions of frozen concentrate outweigh the GHG avoided by not drying. This research note assesses the relative GHG emissions and indicates a GHG advantage for frozen concentrate shipping. 7. A critical analysis of national policies, systems, and structures of patient empowerment in England and Greece

It may also influence patient and public involvement, patient experience, preference, and adherence.Objective: The objective of this study was to compare PE within national policies, systems, and structures in England and Greece for achieving integrated people-centered health services.Methods: We performed a critical search and review of policy and legislation papers in English and Greek languages. This included 1) general health policy and systems papers, 2) PE, patient and/or public involvement or patients’ rights policy and legislation (1990–2015), and 3) comparative or discussion papers for England and/or Greece.Results: A total of 102 papers on PE policies, systems, and structures were identified initially; 80 papers were included, in which 46 were policy, legislative, and discussion papers about England, 21 were policy, legislation, and discussion papers about Greece, and 13 were comparative or discussion papers including both the countries. In England, National Health Service policies emphasized patient-centered services, involvement, and empowerment, with recent focus on patients’ rights; while in Greece, they emphasized patients’ rights and quality of services, with recent mentions on empowerment. The health ombudsman is a very important organization across countries; however, it may be more powerful in Greece, because of the nonexistence of local mediating bodies. Micro-structures at trusts/hospitals are comparable, but legislation gives more power to the local structures in Greece.Conclusion: PE policies and systems have been developed and expressed differently in these countries. However, PE similarities, comparable dimensions and mechanisms, were identified. For both the countries, comparative research and these findings could be beneficial in building connections and relationships, contributing to wider European and international developments on PE, involvement, and patients’ rights and further impact on patient preferences and adherence. 14. Listening to those on the frontline: service users' experiences of London tuberculosis services

Prevalence varies considerably between and within boroughs. Overall, research suggests inadequate control of London's TB transmission; TB has become a health care priority for all London Primary Care Trusts. Service users' experiences and satisfaction with care provision have not been explored adequately previously.Methods: A qualitative research design, using semi-structured face-to-face interviews was used. Ten service users, purposively selected in key risk groups across London, were interviewed. All interviews were digitally recorded with users' permission, transcribed verbatim, and analyzed thematically.Results: Participants were treated in local hospitals for 6–12 months. Treatment was administered by TB nurses to inpatients and outpatients receiving directly observed therapy in consultation with medical staff and home visits for complex cases. Two participants did not realize the importance of compliance. Overall, they were satisfied with many TB services' aspects, communication, and service organization. Early access, low suspicion index amongst some GPs, and restricted referral routes were identified as service barriers. Other improvement areas were information provision on drug side effects, diet, nutritional status, and a few health professionals' attitudes. The effects on people varied enormously from minimal impact to psychological shock; TB also affected social and personal aspects of their life. With regard to further support facilities, some positive views on managed accommodation by TB-aware professionals for those with accommodation problems were identified.Conclusion: This first in-depth study of TB service users' experiences across London offers valuable insights into service users' experiences, providing information and recommendations for a strategic framework for TB service organization and delivery. Overall, further research is needed; TB services – local, national, and international – need to be more closely aligned with service users' complex needs.Keywords: qualitative study, interviews, TB, service organization, services improvement

Abstract

This report is the outcome of a consensus-building project to agree on best practices for environmental and nutritional Life Cycle Assessment (nLCA) methodology, and identify future research needs. The project involved 30 nutritional and environmental LCA researchers from 18 countries. It focused on the assessment of food items (as opposed to meals or diets).Best practice recommendations were developed to address the intended purpose of an LCA study and related modeling approach, choice of an appropriate functional unit, assessment of nutritional value, and reporting nLCA results. An nLCA study should report the quantities of as many essential nutrients as possible and aim to provide information on the nutritional quality and/or health impacts in addition to nutrient quantities. Outstanding issues requiring further research attention include: defining a minimum number of nutrients to be considered in an nLCA study; treatment of nutrients to limit; use of nutrient indexes; further development of Impact Assessment methods; representation of nutritional changes that may occur during subsequent distribution and food preparation in cradle-to-gate nLCA studies; and communication of data uncertainty and variability. More data are required for different regions (particularly developing countries); for the processing, distribution, retail, and consumption life cycle stages; and for food loss and waste. Finally, there is a need to extend nLCA methodology for the assessment of meals and diets, to consider further how to account for the multi-functionality of food in a sustainability framework, and to set nLCA studies within the context of environmental limits.These results provide a robust basis for improving nLCA methodology and applying it to identify solutions that minimize the trade-offs between nourishing populations and safeguarding the environment.

Abstract

Food systems have become increasingly efficient and technologically advanced in providing food products to meet the needs of the world’s growing population. However, providing healthy diets within environmental limits remains a key sustainability issue as food systems continue to over use increasingly scarce natural resources while making a major contribution to environmental impacts such as climate change, water scarcity and biodiversity loss. At the same time, hundreds of millions of people suffer from chronic hunger or adult obesity, and healthy diets continue to be more expensive than energy- and nutrient-sufficient diets. Many stakeholders are interested in the question of how to assess the environmental impacts of healthy diets, and in exploring solutions for minimizing trade-offs between nourishing populations and safeguarding the environment. Life cycle assessment (LCA) studies have an important role in contributing to solutions because they evaluate the environmental impacts of different practices, products and systems, and they facilitate the exploration of benefits and trade-offs across multiple types of impacts. LCA studies of food items increasingly address nutritional as well as environmental aspects, and this requires consideration of additional methodological aspects. The Food and Agriculture Organization of the United Nations (FAO) initiated a project to identify opportunities for further developing environmental and nutritional LCA methodology and building consensus about best practice, and to propose future research needs. The project involved 30 environmental and nutritional LCA researchers from 18 countries and ran between May and November 2021. The project focused on assessing food items as opposed to raw materials, meals and/or diets, and it is a step towards more comprehensive meal and dietary LCA studies. A key issue concerns the intended purpose of an LCA study. This requires some consideration of the reason for the study, the intended application and the audience. Foods may be consumed for a variety of reasons including for their nutritional value, for enjoyment, and/or as a means of taking part in – or contributing to – socio-cultural functions. This report defines a nutritional LCA (nLCA) study as an LCA study where the provision of nutrient(s) is considered as either the main function or one of the main functions of a food item. nLCA studies should be undertaken by multidisciplinary teams involving nutritional and health scientists as well as environmental scientists. At the outset, the goal and scope of an nLCA study should be carefully defined following the recommendations in this report, including: • Undertake an nLCA when nutrients are and/or nutrition is relevant to the decision-maker and decision context (Chapter 3). • Clearly identify the target audience and the target population for a study because different populations have different nutritional requirements, and this may influence the assessment of nutritional value (Chapter 5). • Report the quantities of as many essential nutrients as possible (Chapters 5 and 6). • Aim to provide information on the nutritional quality and/or health impacts in addition to nutrient quantities (Chapter 5, Chapter 6, Section 7.5.4). • The system boundaries should include all stages of the product life cycle that affect nutritional value. The final processing, storage and/or preparation of food items may have a notable effect on their nutritional value, and this should be taken into consideration (including the potential fortification of foods) (Chapter 5). • Choose a modelling perspective (typically attributional or consequential) based on the relevance to the decision situation (Chapter 4). The functional unit in an nLCA study can be defined in many ways (Chapter 6). These include: a quantity of one or more nutrients, a nutrient density value (calculated using a nutrient index), a quality-corrected quantity of nutrient(s), or another nutritional property (such as energy content). Alternatively, the functional unit could be a serving size. In all cases it should be relevant to the target population and its nutritional requirements. If possible, nutrients not included in the functional unit should be listed and discussed in the LCA report. Figure 5 and Figure 6 provide decision trees for guiding the choice of a functional unit in an nLCA study. When assessing nutrition, consideration should be given to: accounting for nutritional value as well as nutrient quantities (for example, using nutrient indices), separate treatment of nutrients to encourage (e.g. calcium) and of nutrients to limit (e.g. sodium), and assessment of non-nutrients that contribute to nutrition (e.g. dietary fibre) (Chapter 5, Chapter 6). In the impact assessment, research on the potential human health impacts of food items is at an early stage (Section 7.5.4). This report recommends using a nutrition impact category to account for the benefits or impacts of nutrition on human health (Section 8.1.1). Other particularly relevant impact categories to consider in an nLCA of food items include (Chapter 7): climate change, water use, land use, eutrophication (related to fertilizer use), ecotoxicity (related to pesticide use), and other human health impacts (in particular, fine particulates from agricultural activities and indoor use of some stoves, pesticide exposure and residues in food, chemical migration into food from contact materials, persistent organic pollutants and metals that bioconcentrate in foods). Antibiotic resistance is an emerging issue that requires consideration in the context of an LCA of food items (Section 7.7). Additional insights may be gained from integrating elements of the ecosystem services approach into an LCA (Section 7.6). The project led to numerous recommendations about reporting nLCA results. To increase the usefulness of nLCA studies of food items for future meal and dietary studies, the results should be reported using a mass- or volume-based reference flow as well as a nutritional functional unit. The results should identify whether the approach is nutrient-based or whether it also includes nutritional aspects. And the reported result should discuss the limitations of the adopted approach, for example, not accounting for interactions with other food items in a meal or diet (i.e. food matrix and meal effects) (Chapter 5). Some additional outstanding issues were identified in the project that require further attention (Chapter 8): • definition of a minimum number of nutrients to be considered in an nLCA study, and whether this should be based on food groups (Chapter 6); • treatment of nutrients to limit alongside, or separately from, encouraged nutrients (Chapter 6); • use of nutrient indices to assess nutrition (Chapter 6); • nutrition impact category methodology (Section 7.5.4), and further development of impact assessment methods for other impact categories (Chapter 7); • how to represent nutritional changes that could occur during food storage, distribution and preparation if the system boundary for an nLCA study is set at a life cycle stage prior to consumption (Chapter 4); • guidelines for use of an nLCA in different applications, including use of attributional and consequential modelling perspectives (Chapter 4); • representation of data uncertainty and variability in nLCA studies (Chapter 4); and •representative data for different regions (particularly developing countries) for the processing, distribution, retail, and consumption life cycle stages, and for food loss and waste (Chapter 4). Finally, there is a need to extend nLCA methodology for the assessment of meals and diets, to consider further how to account for the multi-functionality of food in a sustainability framework, and to set nLCA studies within the context of environmental limits. In summary, the constructive consensus-building process described in this report led to the identification of key outstanding issues and recommendations for the environmental and nutritional LCA of food items. The results provide a robust basis for future research to improve nLCA methodology and apply it to identify solutions that minimize the trade-offs between nourishing populations and safeguarding the environment

26. The relative contribution of training intensity and duration to daily measures of training load in professional rugby league and union

Abstract

This study examined the relative contribution of exercise duration and intensity to team-sport athlete’s training load. Male, professional rugby league (n = 10) and union (n = 22) players were monitored over 6- and 52-week training periods, respectively. Whole-session (load) and per-minute (intensity) metrics were monitored (league: session rating of perceived exertion training load [sRPE-TL], individualised training impulse, total distance, BodyLoad™; union: sRPE-TL, total distance, high-speed running distance, PlayerLoad™). Separate principal component analyses were conducted on the load and intensity measures to consolidate raw data into principal components (PC, k = 4). The first load PC captured 70% and 74% of the total variance in the rugby league and rugby union datasets, respectively.. Multiple linear regression subsequently revealed that session duration explained 73% and 57% of the variance in first load PC, respectively, while the four intensity PCs explained an additional 24% and 34%, respectively. Across two professional rugby training programmes, the majority of the variability in training load measures was explained by session duration (~60–70%), while a smaller proportion was explained by session intensity (~30%). When modelling the training load, training intensity and duration should be disaggregated to better account for their between-session variability.

27. Sports injury prevention is complex: We need to invest in better processes, not singular solutions

Abstract

In recent years an understanding has developed that sports injuries are the emergent outcomes of complex, dynamic systems. Thus, the importance of local contextual factors on injury outcomes is increasingly being acknowledged. These realisations place injury prevention research at a crossroads. Currently, injury prevention researchers develop universally applicable injury prevention solutions, but the adoption of these solutions in practice is low. This occurs because implementation contexts are both unique and dynamic in nature, and as a result singular, static solutions are often incompatible. In contrast, practitioners address injury prevention through iterative cycles of trial and error, aiming to optimise the injury prevention process within their own unique contexts. The purpose of this critical review is to draw attention to the misalignment between research and practice-based approaches to injury prevention. In light of this, we propose alternative research approaches that acknowledge the process-driven nature of injury prevention in practice. We propose that a core focus of sport injury prevention research should be to provide practitioners with useful and relevant information to support their decision-making around their localised injury prevention practice. Through this approach injury prevention research ceases to be about what works, and begins to engage with understanding what works in what contexts and why?

28. The 2018 Mw 7.5 Highlands Earthquake in Papua New Guinea: Implications for Structural Style in an Active Fold and Thrust Belt

Abstract

The Papuan Fold and Thrust Belt (PFTB) in Papua New Guinea is actively forming within a complex tectonic setting at the boundary of the obliquely converging Australian and Pacific plates. The tectonic setting and inaccessibility of the PFTB make it one of the least well‐understood fold and thrust belts on Earth. On February 25, 2018, a Mw 7.5 earthquake occurred within the PFTB, triggering an aftershock sequence which included five events ≥Mw 6. In this study, we combine seismological, GPS and remote sensing observations to investigate the spatiotemporal distribution of crustal deformation during these events. All earthquakes ≥Mw 6 were related to reverse offset on northeast‐dipping fault planes and five out of the six, including the mainshock, were associated with midcrustal focal depths (∼15–30 km). During the sequence, the PFTB underwent up to 1.2 m of uplift and ground deformation occurred over 7,500 km2. Combining these observations with our geological knowledge of this convergent margin highlights the primary control of the northern Australian passive margin on PFTB structural style. We propose that the earthquake sequence was related to tectonic inversion on a hidden extensional fault system beneath the PFTB and lateral variations along this fault zone had significant influence on the complex distribution of ground deformation. It follows that this fault system has had an important control on the evolution of variable structural styles within the PFTB. This study highlights the complexity that can characterize the evolution and structural style of fold and thrust belts.

29. Re-thinking athlete training loads: would you rather have one big rock or lots of little rocks dropped on your foot?

Abstract

Determination of athlete training loads is of great interest to sport practitioners and is widely used in the prescription and monitoring of physical conditioning programmes. Although a number of methods of load quantification are used, a common feature is that total load calculations are the product of exercise intensity and duration. We argue that these methods may be limited, however, as they do not account for non-linearities in the biological response to stress, with the end result being that they fail to fully account for the load imposed by high-intensity or interval-based training sessions. We end with a call for sport scientists to develop novel method of training load quantification to better deal with this issue.

36. The association between breastfeeding duration and adiposity using waist-to-hip ratio and mid-upper arm circumference in South African children aged between one and five years.

Abstract

According to WHO estimates, in 2022, over 37 million children aged five and younger were overweight1. Emerging evidence is linking excessive weight gain before the age of 3 years with later cardiometabolic risks2. Breastfeeding has been reported to be protective against early obesity in children between 2 and 6 years of age3. Additionally, breastfeeding infants reduces rapid weight gain at the end of infancy4. Waist-to-hip ratio (WHR) and mid-upper-arm circumference (MUAC) are cost-effective methods of measuring adiposity distribution. Studies employing these methods in children, particularly in South Africa (SA) are lacking. This study investigates the association between the duration of breastfeeding and WHR and MUAC among a small South African cohort. A cross-sectional study consisting of 262 Black African children aged 1-to-5 years of age was conducted. Children were recruited from local SA clinics via the convenience sampling technique during routine immunisation clinic visits. Data were analysed using R (R-core team 2022) & SPSSVS28 (IBM 2021). The London Metropolitan University sub-ethics committee, the Durban University of Technology (SA University) and the KwaZulu-Natal Department of Health granted ethics for the study. The relevant authorities granted gatekeeper permission from all the facilities before the commencement of data collection. Participants gave informed consent in the local language and were made aware that they could withdraw at any time without any consequences. The waist and hip circumference in centimetres were measured using anthropometric tape, WHT was then calculated by dividing the waist circumference by the Hip circumference. The MUAC was measured using a MUAC tape. The measurements were taken by trained personnel Duration of breastfeeding (weeks) was used as the outcome variable and the WHR and (MUAC) were used as predictor variables. Pearson's correlation test was used to measure the relationship between the predictor variables. A p-value of <0.05 was considered significant. Of the total 262 children that participated in our study, 50.5% were male while 49.5% were female. The average age of the participants was three years. Our data (both WHR & MUAC) was normally distributed according to the generalised linear model. The minimum breastfeeding duration was zero weeks while the maximum duration was 104 weeks (24 months). The mean waist-to-hip ratio was 0.9143, while the mean MUAC was 16.13cm. According to the Pearson correlation test, breastfeeding duration is negatively associated with both WHR (r = −0.18, p< 0.768) and MUAC (r = 0.17 p< 0.790), but this association is not statistically significant. Increasing breastfeeding duration was weakly associated with a decrease in both WHR and MUAC. In our study, although there is a trend in the correlation between breastfeeding duration, WHR and MUAC, the relationship between these predictors is not significant. Abstract

The use of ultra-short pulse wideband ultrasonic transducers in Non-Destructive-Testing (NDT) has been investigated both theoretically and experimentally. It is demonstrated that the resolution of pulse-echo NDT is affected by diffraction effects which also complicate the interpretation of echo signals. These diffraction effects are interpreted in terms of the plane- and edge-wave model of *transducer fields. Improverents can be obtained by the use of non-uniformly excited transducers of two basic types: the first, the plane-wave-only (PWO) source; is more strongly excited at its centre than towards the rim, where the excitation is gradually reduced to zero in order to remove the edge wave. The second type, an edge-wave-only (EWO) source, is more strongly excited at its rim than in the centre, thereby effectively removing the plane wave. Computer modelling of pressure waveforms in the field of PWO and EWO sources has been carried out using an extension to the impulse response method. Experimental point-pressure waveform measurements in the field of a prototype EWO transducer, made using a miniature ultrasonic probei are in reasonable agreement with the calculated results. Detailed calculations are made of the transmit-receive mode (pulse-echo) responses arising from solid targets of various size in a flu- id medium interrogated by uniformly and non-uniformly excited sources. The theoretically predicted results are in good agreement with experimentally measured results obtained using a conventional transducer and an equivalent prototype EWO transducer. The effects of target size, field position and material on both the amplitude and shape of the echo responses are investigated. The structure of the responses is explained in terms of the plane and edge waves radiated by the source. Implications for the use of techniques to both size (Distance, Gain, Size curves) and characterise (ultrasonic spectroscopy) defects are examined. The applications of new, nonuniformly excited transducers in high-resolution NDT and ultrasonic imaging are evaluated.

Abstract

Effective intraoperative tumor margin assessment is needed to reduce re-excision rates in breast-conserving surgery (BCS). Mapping the attenuation coefficient in optical coherence tomography (OCT) throughout a sample to create an image (attenuation imaging) is one promising approach. For the first time, three-dimensional OCT attenuation imaging of human breast tissue microarchitecture using a wide-field (up to ~45 × 45 × 3.5 mm) imaging system is demonstrated. Representative results from three mastectomy and one BCS specimen (from 31 specimens) are presented with co-registered postoperative histology. Attenuation imaging is shown to provide substantially improved contrast over OCT, delineating nuanced features within tumors (including necrosis and variations in tumor cell density and growth patterns) and benign features (such as sclerosing adenosis). Additionally, quantitative micro-elastography (QME) images presented alongside OCT and attenuation images show that these techniques provide complementary contrast, suggesting that multimodal imaging could increase tissue identification accuracy and potentially improve tumor margin assessment.

Abstract

Diversity and inclusion in the workplace optimise performance through the input of a range of perspectives and approaches that drive innovation and invention. However, gender inequity is prevalent throughout society and females remain underrepresented in geoscience careers. This study provides the current status of gender equity in geosciences throughout Australasia within the context of broader gender equity policy, frameworks and initiatives and suggests additional solutions and opportunities to improve gender equity and the retention of women in the geoscience workforce. At an individual institutional level in academia, females make up between 23 %-52 % of the total geoscience departmental or school staff in Australia, 26 %-39 % of the total staff in New Zealand, 29 % of total staff at the University of Papua New Guinea and 18 % at the University of the South Pacific. Significant gender imbalance exists at more senior levels, with disproportionately more males than females, a pattern typical of many Science Technology Engineering and Maths (STEM) disciplines. Gender inequity is prevalent within the general membership, committee roles and in award recipients of Australasian geoscience professional associations. Within the Geological Society of Australia and Geoscience Society of New Zealand, only 4 % (n D 47) and 18 % (n D 161), respectively of past award recipients for national and general awards were female. All past awards considered in this study that are named in honour of a person were named in honour of a man (n D 9). In recent years, women-focused networks have begun to play an invaluable role to support the retention and promotion of women in geosciences and provide a supportive mentoring environment to discuss challenges and share advice. The improved visibility of women in the geoscientific community is an ongoing issue that can in part be addressed through the development of public databases of women geoscientists. Abstract

The nutritional double burden of disease refers to the phenomenon of undernutrition, wasting, stunting, micronutrient deficiency coinciding with overweight, obesity, and diet-related non-communicable diseases, within individuals, households and populations throughout the lifecycle. This study aimed to determine whether there were differences in hemoglobin levels between anthropometric categories and socio-economic factors among women aged 15 to 49 years old in South Africa. Data were obtained from the Demographic and Health Survey (DHS) South Africa survey 2016. There were 2690 women between 15 and 49 years old included in the sample. Variables selected for analysis included height and weight, hemoglobin (adjusted for altitude), wealth index, access to improved water and sanitation. Variables were tested for normality using Q-Q plots. Missing data was removed. Frequencies and percentages were reported for categorical data. Non-parametric continuous variables were reported as medians and interquartile ranges. As data were not normally distributed, analysis was conducted using the Kruskall-Wallis test and Mann-Whitney U test. The type I error rate was set to p <.05. Where it was found that a significant difference exists, post hoc Dunn tests were performed to determine the location of the differences. Anemia was prevalent among 28.9% of the sample and 63.5% were either overweight or obese. Hemoglobin levels were significantly different between normal weight women and women with a body mass index in the obese class I and obese class II respectively (Kruskall-Wallis = 27.014; df = 5; p =.000; n = 2690). There were significant differences in hemoglobin levels between women with access to improved sanitation and those without access (Mann-Whitney U test p =.017), but hemoglobin levels were similar between women with access to improved water and those without (Mann-Whitney U test p =.175). Poorer women had significantly different hemoglobin levels to the wealthiest women in the sample (Kruskall-Wallis = 29.568; df = 4; p =.000). The nutritional double burden of disease is prevalent in South Africa among women of childbearing age. A wealth disparity exists among South African women in terms of hemoglobin levels.
