Abstract

This correspondence is concerned with a meeting of the Appalachian National Park Association for the purpose of election of officers in 1901. The association was formed in 1899 for the purpose of promoting the idea of a national park in the eastern U.S. Although housed in Asheville, North Carolina, the organization was a multi-state effort, attracting representatives from six southern states. One of the highlights of the group’s activities was a convention held in 1902 to which 1,500 people attended. The association lobbied Congress for the creation of a park, but with limited success. The association disbanded in 1905.

Abstract

The Appalachian National Park Association was formed in 1899 for the purpose of promoting the idea of a national park in the eastern U.S. In this 14-page communication written within weeks of its organization, the association adopted the magazine, “Southern Pictures and Pencillings,” as the official organ of the association. The publication was produced by A. H. McQuilkin, a Chicago-based editor who moved to Asheville, North Carolina in the mid-1890s and became involved with the association.

Abstract

In this series of letters, various authors and newspapers request information on Appalachian National Park movement for the purpose of writing an article. In promoting a national park in the southern Appalachians, Chase Ambler wrote dozens of editorials, including this one titled “Status of the Appalachian Forest Reserve Movement.” These editorials were submitted to newspapers and magazines throughout the east coast. After the editorials were published, the Appalachian National Park Association often reprinted and circulated them as part of their promotional campaign.

Abstract

Various organizations and individuals expressed support of an Appalachian National Park and a bill before Congress authorizing the purchase of four million acres for that purpose. A movement for a national park in the Appalachians was spearheaded by Appalachian National Park Association, formed in 1899. The association raised public awareness of the issue, but disbanded before the formation of the Great Smoky Mountains National Park.

Abstract

Aim: HLA Ab or DSA pre/post transplantation (Tx) has been associated with poor renal graft outcomes. It is unclear which HLA sensitized patient will or will not experience rejection or graft loss. The purpose of our study was to identify recipients (recips) at greatest risk for post Tx rejection and/or graft loss. Methods: We evaluated 277 renal-Tx recips that had sufficient pre/post HLA Ab data. Antibody mediated rejections (AMR) were biopsy confirmed and treated with Thymoglobin, Pheresis and Rituxin while cellular rejections were treated with steroids. All donor-recips were ABO compatible and transplanted following a negative donor-specific FCXM using historical and pre-Tx recip sera. Recips were risk-ranked according to whether they had or did not have HLA and/or DSA post-Tx. A subgroup of patients with persistent HLA or DSA Ab (presenting at least two or more times) was identified. HLA Ab and/or DSA were identified using SAB (One Lambda, Inc.) and a Luminex based solid phase assay platform. Mean Fluorescence Intensity (MFI) of 1,000 was considered positive. Results: Of 277 renal Tx recips, 92 presented with no HLA or DSA post-Tx while 185 recips had positive Ab post-Tx. Of the 185 recips with Ab post-Tx, 31% (58/185) presented with DSA. The frequency of AMR for no Ab vs positive Ab post-Tx was significantly different 8% (7/92) vs 21% (39/185), p<0.01 as was the three year graft survival of 92% (82/92) vs 87% (164/185), p<0.02 for the same patients. There were 58 patients with positive post-Tx Ab that were DSA positive, however, only 67% (39/58) of these displayed persistent Ab and only 54% (21/39) of these lost their grafts. Moreover, 50 recips that presented with a pre-Tx positive DSA would not have been considered for a donor crossmatch using the virtual crossmatch. Conclusions: These data suggest that not all post-Tx Abs that are DSA positive result in AMR or graft loss. Recips with persistent DSA appear to be at a higher immunological risk for AMR and/or graft loss. [Copyright &y& Elsevier]

Abstract

These data suggest the presence of virtual crossmatch positive, low titer donor specific anti-human leukocyte antigen antibody (DSA) positive sera does not guarantee that real-time crossmatches will be positive. DSA that are present at an undiluted concentration but absent at a 1:8 or 1:16 dilution may not have to be considered unacceptable. This would allow potential recipients wider access to donor organs by reducing the number of listed unacceptable antigens. It also calls into question the use of virtual crossmatching, which may be disenfranchising minorities. Finally, evaluation of antibody concentration by dilution or titer may be clinically more useful than mean fluorescence intensity.

Abstract

We evaluated patient sera for flow PRA, FCXM, and end-point donor-antigen titer, and we correlated the results with graft survival. You cannot accurately predict a positive or negative FCXM result-not even when the sera have donor-specific antigens-unless you actually perform a crossmatch. Using fluorescence intensity as a surrogate for antibody concentration does not correlate quantitatively with the occurrence of a positive or negative crossmatch. Therefore, it is imperative to give each recipient a chance at being offered a donor organ by performance of a real-time crossmatch and not rely on a virtual evaluation.

Abstract

In an effort to deal with the critical nursing shortage, new graduate nurses are entering highly specialized areas that once were available only to experienced nurses. A critical challenge exists for the new graduate when faced with the application of intensive theoretical knowledge in practice, and implementation of high-technology interventions, to meet the needs of patients and families. Innovative strategies are required to prepare the new graduate for this high-acuity practice environment and lessen the impact of reality shock. This article describes the implementation of medical-surgical nurse extern and student nurse aide programs that expanded to critical care. Program outcomes included positive feedback from participants about increased clinical experience, RN role perception, and comfort in the clinical practice setting. Program components are detailed, outcomes discussed, and implications for practice presented.
