4. New Age navigation: innovative information interfaces for electronic journals.

Abstract

While it is typical for electronic journals to offer conventional search features similar to those provided by electronic databases, a select number of e-journals have also made available higher-level access options as well. In this article, we review several novel technologies and implementations that creatively exploit the inherent potential of the digital environment to further facilitate use of e-collections. 5. Plasma exchange treatment of peripheral neuropathy associated with plasma cell dyscrasia.

Abstract

Plasma exchange was used to treat 10 patients with polyneuropathy and a monoclonal antibody (plasma cell dyscrasia). Six patients had improvement of the neuropathy, while three patients had stabilisation of the neuropathy during plasma exchange. The patients who improved maintained a 64% or greater decrease in the monoclonal antibody between exchanges. Patients with axonal polyneuropathy as well as patients with demyelinating polyneuropathy recovered some neurologic function. With cessation of plasma exchanges, the monoclonal antibody titre approached pre-treatment levels and the neuropathy progressed. Plasma exchange was effective in rapidly lowering the level of monoclonal antibody and allowing for some recovery of neurologic function. [ABSTRACT FROM AUTHOR]

11. Boronate Titanium Alkylidene Reagents for Diversity-Based Synthesis of Benzofurans

Abstract

Novel titanium benzylidenes (Schrock carbenes) bearing an arylboronate group are generated from thioacetals with low valent titanium species, Cp2Ti[P(OEt)3]2, and alkylidenate Merrifield resin-bound esters to give enol ethers. Treatment with 1% TFA gives 2-substituted (benzo[b]furan-5-yl)boronates, and solid-phase Suzuki cross-coupling gives 2,5-disubstituted benzofurans. Steps in the syntheses of thioacetal substrates include selective lithiation-boronation, hydrolysis of a MOM group without affecting a boronate ester, and cross-coupling with bis(pinacolato)diboron.

12. Photopheresis therapy of cutaneous T-cell lymphoma: the Yale-New Haven Hospital experience

Abstract

Over the past two years, the photopheresis treatment program at Yale-New Haven Hospital has treated 32 patients with cutaneous T-cell lymphoma. There were 19 erythrodermic patients who had photopheresis as their first systemic therapy. Five of these cleared 75 percent of their skin and the majority of erythrodermics achieved an improved quality of life. Those with best responses were treated earlier in the course of their disease and had more normal proportions of CD4 and CD8 subsets when compared to patients with poor responses. In addition, patients with tumor stage disease and those patients in relapse after intensive radio- and chemotherapy were treated with photopheresis. These results demonstrate that photopheresis has a role in the management of cutaneous T-cell lymphoma. Images FIG. 3

Abstract

A woman who emigrated to the United States from the Dominican Republic developed the first signs of cutaneous T cell lymphoma during the last trimester of her pregnancy. This patient, found to have a positive reaction against human T-lymphotropic (leukemia-lymphoma) virus type I (HTLV-I), was followed up prospectively from the appearance of the initial skin lesion to the development of high-count helper T cell leukemia. Antibodies reactive with the core protein of HTLV-I were also identified in her husband and mother but not in her 2-year-old daughter. Examination of the patient's course provides clues about the latency period and transmission of HTLV-I and highlights similarities between HTLV-I-positive and HTLV-I-negative cutaneous T cell lymphoma.
