5. Re-conceptualising student resilience through the liminal space of university, from a non-traditional Applied Health and Social Care graduate perspective

Abstract

This thesis explores the unique recollections of non-traditional Applied Health and Social Care (n-tAHSC) graduates' experiences as n-tAHSC students in the liminal space of university. The literature reveals that non-traditional (n-t) students face multiple challenges in UK Higher Education (HE), such as limited knowledge of HE, balancing paid work and study and other responsibilities. These stressors are increased when n-t students study AHSC programmes owing to the interdisciplinary hazards, which include ambiguity around career trajectory, lack of discipline-specific codes of conduct and lack of preparation for stressful people facing roles. Yet these students still achieve successful outcomes. A potential facilitating cause of this success is resilience, identified as both a buffer against the stressors that instigate poor mental health and a catalyst for positive mental health and success in HE. To this end, this thesis conceptualises the articulations and practices (conditions, causes and contingencies) of resilience from the unique perspectives of successful non-traditional Applied Health and Social Care (Sn-tAHSC) graduates, a population that has been under-researched across n-t student resilience literature until now. Using a constructivist approach a sample of 20 successful Sn-tAHSC graduates were interviewed via online video call using Microsoft Teams. Using both a Grounded Theory approach and a Bourdieusian lens to interpret and analyse new understandings from the opinions and views of successful Sn-tAHSC graduates, across domains of agency, habitus, field and capital. The outcomes of the study show that n-tAHSC students articulate resilience differently than their 'traditional' student counterparts. Building on a concept of bouncing forward the n-tAHSC student commences university with a repertoire of conditions and strategies for resilience, learned from overcoming previous unique challenges and adversities in their former habitus. These students believe resilience to be further bolstered in the liminal space of university (field) through growth mindsets, problem-focussed coping and social models of resilience where they develop self-efficacy, through sharing and exchanging capital for resilience through formal and informal social interactions with peers, staff and services. This thesis makes an original contribution to the evidence by reconceptualising resilience and creating a unique toolkit for n-tAHSC student resilience. Revealing distinct facilitating conditions, causes and contingencies for resilience where unique 'bouncing forward' resilience strategies enable n-tAHSC students to not only survive the challenges of HE, but flourish into successful AHSC graduates.

9. The immune microenvironment in young patients with triple negative breast cancer

Abstract

Introduction: Triple negative breast cancers (TNBC) are high grade tumours, with poorer disease specific survival. Following diagnosis of metastatic disease, survival is typically short and novel treatment strategies are required. The immune system is increasingly recognised to play an important role in TNBC biology; the tumour microenvironment (TME) is central to immune evasion. I explored the interaction between the TME, immunogenicity and prognosis in a cohort of young women with TNBC. Methods: The POSH study is a prospective observational study of almost 3000 patients aged 40 years or younger with a first diagnosis of invasive breast cancer. Associations between clinicopathological features and molecular markers (using tissue microarrays) and prognosis were assessed in those with TNBC. These factors were integrated into a prognostic score using ROC analysis. Correlates of immune activity were identified and digital pathology analyses utilised. Murine breast fibroblasts were differentiated into the myofibroblastic phenotype in vitro, with qtPCR, western blot, cell count, immunofluorescence and metabolism assays performed. The effect of myofibroblast co injection on tumour growth using the 4T1 model in vivo was studied. Finally, AZD0156, a novel cancer associated fibroblast (CAF) inhibitor, was assessed for efficacy and compared to immunotherapy in vivo. Results: Tumour infiltrating lymphocyte (TIL) density was highly prognostic and was significantly higher in those with a germline BRCA mutation. Bioinformatics of publically available datasets revealed that extracellular matrix pathways were upregulated in lymphocyte deplete TNBC cases. An inverse correlation between smooth muscle actin (SMA) and TIL was found. Murine breast fibroblasts were transformed in vitro using TGF β; myofibroblasts augmented tumour growth in a TNBC murine model. An ATM inhibitor (AZD0156) inhibited the myofibroblastic phenotype and was effective at reducing tumour growth using the CAF rich 4T1 model in vivo. This effect was not mediated via CD8 infiltration but there was a reduction in CAF density. Conclusion: The TME is an important factor in immune evasion in TNBC. CAF inhibition may be an effective treatment strategy and should be investigated in combination with other agents.

Abstract

This research explored women's lived experiences of societal enforcement of motherhood through mechanisms of surveillance and judgement. Using a descriptive qualitative phenomenological design, 24 Australian women were interviewed about their lived experiences as women within society. Thematic analysis revealed five themes: women's enforcement of hegemonic gender; the enforcement of motherhood in public and private spaces; labelling to control women; the gendered nature of social control; and the impacts of enforcement of motherhood. Overall, this study's findings draw attention to how the surveillance, and subsequent judgement, of women within society plays an important role in maintaining the unrealistic expectations placed on women to become mothers and adhere to the ideology of good motherhood. Competition, surveillance and judgement are central to keeping women subordinate and distracting them from the ways in which structures of hegemonic gender function to oppress them within society. If freed from the constraints of hegemonic gender, mothering could be a site of equality where power is shared between parents rather than a site for continued oppression. [ABSTRACT FROM AUTHOR]

Abstract

To better understand the discrepancy between women's motherhood aspirations and outcomes, this study explored the impact of socio‐cultural structures of gender. Taking a qualitative phenomenological approach, interviews were conducted with 24 Victorian women aged between 25 and 45 years. Three themes resulted from data analysis, reflecting the meaning women gave to their lived experiences: Motherhood is central to womanhood; motherhood ambivalence; and the tension of navigating motherhood and career aspirations. Most women aspired to be valued as women who could balance motherhood with a rewarding career. However, hegemonic gender, particularly in the workplace, continues to ensure that these roles remain incompatible, forcing women to anticipate career pauses, changes and/or absences. Overall, these themes demonstrated how socio‐cultural structures of hegemonic gender continue to shape women's lived experiences of gender inequality and constrain their ability to achieve their aspirations for their future lives. Abstract

Policy can be used and experienced as a tool for social inclusion or exclusion; it can empower or disenfranchise. Women’s reproductive decision‐making and health is impacted by policy, and women’s experiences of diverse and intersecting marginalised social locations can influence their experiences of policy. This research aimed to explore how intersectionality is considered within Victorian state government policies that influence and impact women’s reproductive decision-making. A systematic search of Victorian (Australia) government policy instruments was undertaken, identifying twenty policy instruments. Policies were analysed using an intersectional policy analysis framework using a two‐stage process involving deductive coding into the domains of the framework, followed by inductive thematic analysis within and across domains. Findings reveal inconsistencies within and across policies in how they consider intersecting social relations of power in the representation of problems, women’s positionings, policy impacts, and policy solutions. These gaps could exclude and marginalise individuals and groups and contribute to systemic inequities in women’s reproductive decision-making and the outcomes of those decisions, particularly among already marginalised groups. The lack of women’s voices in policy further excludes and marginalises those impacted by the policy and limits the representation of all women in policy. Policy development needs to meaningfully involve women with diverse and intersecting marginalised social locations, and critical reflexivity of all stakeholders, to ensure policies can better account for the experiences of, and impacts upon, women who are marginalised and effect change to promote social inclusion and equity in women’s reproductive decision‐making.

Abstract

INTRODUCTION: It has been recognized that increasing body mass index (BMI) is associated with improved outcome from immune checkpoint inhibitors (ICIs) in patients with various malignancies including non-small cell lung cancer (NSCLC). However, it is unclear whether baseline BMI may influence outcomes from first-line chemoimmunotherapy combinations. METHODS: In this international multicenter study, we evaluated the association between baseline BMI, progression-free survival (PFS) and overall survival (OS) in a cohort of patients with stage IV NSCLC consecutively treated with first-line chemoimmunotherapy combinations. BMI was categorized according to WHO criteria. Results: Among the 853 included patients, 5.3% were underweight; 46.4% were of normal weight; 33.8% were overweight; and 14.5% were obese. Overweight and obese patients were more likely aged ≥70 years (p=0.00085), never smokers (p<0.0001), with better baseline Eastern Cooperative Oncology Group-Performance Status (p=0.0127), and had lower prevalence of central nervous system (p=0.0002) and liver metastases (p=0.0395). Univariable analyses showed a significant difference in the median OS across underweight (15.5 months), normal weight (14.6 months), overweight (20.9 months), and obese (16.8 months) patients (log-rank: p=0.045, log rank test for trend: p=0.131), while no difference was found with respect to the median PFS (log-rank for trend: p=0.510). Neither OS nor PFS was significantly associated with baseline BMI on multivariable analysis. CONCLUSIONS: In contrast to what was observed in the context of chemotherapy-free ICI-based regimens, baseline BMI does not affect clinical outcomes from chemoimmunotherapy combinations in patients with advanced NSCLC.

50. Self-management programmes for adult patients with bronchiectasis: a systematic review and realist synthesis

Abstract

Purpose: Self-management for bronchiectasis patients has been identified as an important component that could potentially empower patients to manage their conditions and improve quality of life. Evidence was reviewed to investigate what self-management programmes work, why and in what circumstances. Methods: A comprehensive database search was conducted on seven databases for evidence published up to June 2020. Self-management programmes were examined in each study and had to include at least two of the following selfmanagement interventions: education, exercise, adherence to treatment, symptom monitoring, airway clearance techniques (ACT) and action plans. A realist logic of analysis was applied to produce context-mechanism-outcome configurations to provide an explanatory account of which self-management intervention programmes works, for whom and in what circumstances. Results: Seven eligible articles (n = 273) contributed to the development of three context-mechanism-outcome configurations articulated for three self-management programmes that included education and action planning, education and ACT, and education, exercise and ACT. Provision of resources, knowledge, patient empowerment, self-regulation and social facilitation can improve self-efficacy, healthrelated quality of life and exercise capacity in different contexts. Conclusions: The findings of this review has identified potentially important underlying processes that contribute to the short-term increase of clinical outcomes in different contexts.
