Abstract

We announce the complete genome sequences of 14 Serratia bacteriophages isolated from wastewater treatment plants. These phages define two previously undescribed types which we call the Carrot-like phage cluster (phages Carrot, BigDog, LittleDog, Niamh, Opt-148, Opt-169, PhooPhighters, Rovert, Serratianator, Stoker, Swain, and Ulliraptor) and Tlacuache-like phage cluster (Tlacuache and Opt-155).

Abstract

BACKGROUND: The COVID-19 pandemic has caused unprecedented health care challenges mandating surgical service reconfiguration. Within our hospital, emergency and elective streams were separated and self-contained Protected Elective Surgical Units were developed to mitigate against infection-related morbidity. Aims of this study were to determine the risk of COVID-19 transmission and mortality and whether the development of Protected Elective Surgical Units can result in significant reduction in risk. METHODS: A retrospective observational study of consecutive patients from 18 specialties undergoing elective or emergency surgery under general, spinal, or epidural anaesthetic over a 12-month study period was undertaken. Primary outcome measures were 30-day postoperative COVID-19 transmission rate and mortality. Secondary adjusted analyses were performed to ascertain hospital and Protected Elective Surgical Unit transmission rates. RESULTS: Between 15 March 2020 and 14 March 2021, 9,925 patients underwent surgery: 6,464 (65.1%) elective, 5,116 (51.5%) female, and median age 57 (39-70). A total of 69.5% of all procedures were performed in Protected Elective Surgical Units. Overall, 30-day postoperative COVID-19 transmission was 2.8% (3.4% emergency vs 1.2% elective P < .001). Protected Elective Surgical Unit postoperative transmission was significantly lower than non-Protected Elective Surgical Unit (0.42% vs 3.2% P < .001), with an adjusted likely in-hospital Protected Elective Surgical Unit transmission of 0.04%. The 30-day all-cause mortality was 1.7% and was 14.6% in COVID-19-positive patients. COVID-19 infection, age > 70, male sex, American Society of Anesthesiologists grade > 2, and emergency surgery were all independently associated with mortality. Abstract

Demonstrate how to construct a low fidelity laparoscopic salpingectomy model which can be used in the EMIGS LaparoBowl. The model was constructed using supplies easily available online. The model is used within the EMIGS LaparoBowl. The model is used at our institution for residency simulation training. A low fidelity laparoscopic salpingectomy model was constructed for use in the EMIGS LaparoBowl. This low fidelity model for laparoscopic salpingectomy is a high yield task trainer that has been an excellent clinical tool for resident education at our institution. Adequate training in laparoscopic surgery, such as salpingectomy, is vital to the future success of OBGYN residents. Abstract

Background. The use of the bone-seeking properties of bisphosphonates (BPs) to target the delivery of therapeutic drugs is a promising approach for the treatment of bone metastases. Currently, the most advanced example of this approach is a gemcitabine-ibandronate conjugate (GEM-IB), where the bone-targeting BP ibandronate (IB) is covalently linked to the antineoplastic agent gemcitabine (GEM) via a spacer phosphate group. In the present study, we describe the development of a new analytical platform to evaluate the metabolism and pharmacokinetics of GEM-IB in mice and dogs and the results of proof-of-concept studies assessing the pharmacokinetics of GEM-IB in dogs and mice. Methods. We validated analytical platforms to analyze GEM-IB and five of its major metabolites IB, gemcitabine-5′-phosphate (GEMMP), gemcitabine (GEM), 2′,2′-difluoro-2′-deoxyuridine-5′-phosphate (dFdUMP), and 2′,2′-difluoro-2′-deoxyuridine (dFdU) and performed proof-of-concept pharmacokinetic studies in mice (5 mg/kg i.p.) and dogs (5 mg/kg i.v.). Results. Intra- and inter-run accuracy and imprecision (3 days) of the assays met the (FDA) acceptance criteria. The proof-of-concept plasma pharmacokinetic studies in mice showed AUCs of 1278, 10,652, 405, 38, 1063, 3389, and 38 h·ng/mL for GEM-IB, IB, GEMMP, dFdU-MP, GEM, and dFdU, respectively. In dog plasma, AUCs of 295, 5725, 83, 11, 1625, and 6569 h·ng/mL were observed for GEM-IB, IB, GEMMP, dFdUMP, GEM, and dFdU. Conclusions. Pharmacokinetic studies in dogs and mice showed that GEM-IB is rapidly converted to IB and GEM; dFdU is formed (from GEM) with a delay. The rapid disappearance of GEM-IB from circulation could be explained by a combination of metabolism and rapid distribution to tissue/bone.

26. Why do ABCC5 KO mice show a lean phenotype?

Abstract

ABCC5 is an ATP-dependent membrane transporter that has been shown to transport glutamate conjugates in mammalian systems. Global loss of ABCC5 from mice causes a lean phenotype with enhanced enteroendocrine GLP-1 secretion. Here we develop a protocol for the live imaging of GLP-1 secretion in vitro using a model L-cell line. ABCC5 KO gut histology analysis shows no difference in gut villi length or surface area between WT and KO mice. Despite numerous studies showing enhanced brown adipose tissue (BAT) and white adipose tissue (WAT) thermogenic capacity in the presence of elevated GLP-1 levels, data presented here do not show increased non-shivering thermogenesis occurring in our KO animals. Analysis of fat pads from ABCC5 KO mice show that both their BAT and epididymal WAT depots have significantly lower mass than WT depots. Western blot and H&E; histological analysis suggests that inguinal WAT depots of ABCC5 KO mice may show a lack of browning. We theorise that the adipose physiology described is due to changes in glutamatergic signalling in the sympathetic nervous system innervating BAT and WAT. Finally, we conclude that the lean phenotype of ABCC5 KO mice cannot be attributed to an increased thermogenic capacity of BAT and WAT.

Abstract

The case of a seven-year-old girl illustrates that proteins produced by the bacterium that causes pertussis can cross the blood-brain barrier and enter the brain. Pertussis is the medical term for whooping cough. She developed pertussis and a condition called encephalopathy, which is characterized by brain dysfunction. Analysis of cerebrospinal fluid samples revealed that she had antibodies in her brain to pertussis toxin and filamentous hemagglutinin, which are both produced by the bacterium. These proteins probably passed directly into her brain, where they stimulated an immune response., A 7-year-old unimmunized girl with pertussis presented with respiratory failure and electroencephalographic evidence of an encephalopathy. The cerebrospinal fluid (CSF)/serum ratio of antibodies to pertussis toxin and filamentous hemagglutinin were lland nine-fold higher than the CSF/serum ratio of total immunoglobulin G. The CSF/serum ratio of albumin was normal. These findings indicate production of antibodies in the central nervous system to Bordetella pertussis antigens and imply, therefore, that the pertussis encephalopathy in this girl was associated with the entry of pertussis antigens into the central nervous system. 30. Are we ready for Education 4.0 within ASEAN higher education institutions? Thriving for knowledge, industry and humanity in a dynamic higher education ecosystem?

Abstract

Purpose The purpose of this paper is to explore the perception, readiness and change involved in the implementation of Education 4.0 within the region of Association of Southeast Asian Nations (ASEAN) among policymakers, enablers (lecturers) and receivers (students), within globalisation, referred here as the Fourth Industrial Revolution (4IR). Design/methodology/approach This study used a mixed method research design using quantitative data from a Likert scale of 1–5, involving: (1) Not Ready; (2) Ready; (3) No Sure; (4) Quite Ready and (5) Extremely Ready. Open-ended questions formed the qualitative approach taken by the researchers to uncover the richness of the respondents' perceptions of Education 4.0. The test items reliability index of 0.744 drew quantitative data from the perspective of the educational policymakers, enablers and receivers to reveal their collective definition of each construct (knowledge, industry and humanity). Findings This study has exposed the importance of knowing and capturing the interrelated components of an educational ecosystem that exists in higher education (HE) within the ASEAN region. The personal readiness of respondents towards Education 4.0 is very high; yet concern was raised about the financial and managerial readiness of institutions across the region. Originality/value This study highlighted the dynamic nature of the HE ecosystem and the connectivity between the elements of Education 4.0 – knowledge, industry and humanity within the ASEAN region.

A scintigraphic method facilitates the examination of the influence of age on pharyngeal and oral transit and clearance of bolus during swallowing. It is indicated that there is a significant increase in the times of pharyngeal clearance, pharyngeal transit and oral transit as a result of ageing. There is also an enhancement in the time of glottis' exposure to the swallowed bolus.

To date, there is no consensus on what good mentoring looks like and whether current models are fit for purpose for Black and Minoritised Ethnicity AHPs.Methods: Scoping review to understand mentoring among Black and Minoritised Ethnicity (BME) AHPs through searches in four electronic databases using pre-defined criteria.Results: From 1440 studies screened, 2 studies were included in this review that researched mentoring for BME AHPs in a health and/or social care setting. Themes that emerged from the literature were related to access and amount of mentoring for BME AHPs, psychosocial mentoring may influence cross-race mentorship outcomes and organisational issues related to workforce pressures and management support. There is uncertainty around the importance of same race relationships in mentoring but there is some evidence that within cross-race mentoring this could influence outcomes.Conclusion: It is unknown if mentoring enables career success in BME AHPs. There is emerging evidence on the importance of psychosocial mentoring within cross-race relationships. 40. Training attention in children with acquired brain injury: a study protocol of a randomised controlled trial of the TALI attention training programme

Abstract

INTRODUCTION: Childhood inattention has been linked with poor academic outcomes, and increased lifetime social, occupational and psychiatric morbidity. Children with an acquired brain injury (ABI) are particularly susceptible to attention deficits and may benefit from interventions aimed at enhancing attention. The primary objective of this study is to evaluate the short-term efficacy of the TALI Train programme, compared with a placebo, on the outcome of attention in children with ABI. METHODS AND ANALYSIS: The study is a parallel, double-blind, randomised controlled trial. Participants will consist of 80 children with a diagnosis of ABI aged 4-9 years 11 months. Participants will be randomly allocated to either (1) TALI Train (intervention group), an adaptive game-based attention training programme, or (2) a non-adaptive placebo programme (control group). Both programmes are delivered on a touchscreen tablet, and children complete five 20 min sessions per week for a 5-week period at home. Assessment of selective, sustained and executive attention (primary outcomes), and behavioural attention, working memory, social skills and mathematics ability (secondary outcomes) will occur at baseline, post-training, and at 3-month and 6-month follow-up to assess immediate and long-term efficacy of TALI Train compared with placebo. Assessments will be completed at the Royal Children's Hospital in Melbourne, Australia. All assessments and analyses will be undertaken by researchers blinded to group membership. Latent growth curve modelling will be employed to examine primary and secondary outcomes. ETHICS AND DISSEMINATION: Ethics approval has been obtained from the Royal Children's Hospital Human Research Ethics Committee (HREC) (38132) and the Monash University HREC (17446). Results will be disseminated through peer-reviewed journals, conference presentations, media outlets, the internet and various community/stakeholder activities. 46. Excision of a discontinuous-frequency interference signal with harmonic structure

Abstract

We consider the problem of excising a sinusoidal signal which is piecewise constant in frequency with random transition times, and which is duplicated at fixed frequency offsets. This signal interferes with a signal of interest. To remove this interfering signal, the most likely interfering frequencies at each point in time are estimated using a pair of sliding estimation windows obtained using compressed likelihood techniques, which account for all shifted versions of the interfering signals. This two-window approach avoids line broadening. The exact transition times are then estimated using median filtering, a sliding two-window scan of changes in frequency, and fine-tuning by comparison with the original signal. After identifying frequency transition times and the local interfering frequency, the interference is excised from the band of interest.

Abstract

We consider a the problem of coherently combining signals having several sinusoidal components as measured via multiple sensors, in which each sensor has its own transfer function. It is desirable to combine the sensor outputs to improve the signal-to-noise ratio, but simply co-adding the signals could result in signal cancellation, due to phase changes in the sensors. We propose here a combining architecture which blindly adjusts the phases of the signals to maximize signal output. Different combining filters are considered: an allpass filter, and FIR filters designed according to a maximum SNR and minimum mean-squared error constraint. The allpass filters are trained both via steepest ascent and simplex optimization. The allpass combining filters provide excellent SNR improvement, while preserving all the frequency components.

Contains fulltext : 168023.pdf (publisher's version ) (Closed access), Alveolar capillary dysplasia with misalignment of pulmonary veins (ACDMPV) is a lethal lung developmental disorder caused by heterozygous point mutations or genomic deletion copy-number variants (CNVs) of FOXF1 or its upstream enhancer involving fetal lung-expressed long noncoding RNA genes LINC01081 and LINC01082. Using custom-designed array comparative genomic hybridization, Sanger sequencing, whole exome sequencing (WES), and bioinformatic analyses, we studied 22 new unrelated families (20 postnatal and two prenatal) with clinically diagnosed ACDMPV. We describe novel deletion CNVs at the FOXF1 locus in 13 unrelated ACDMPV patients. Together with the previously reported cases, all 31 genomic deletions in 16q24.1, pathogenic for ACDMPV, for which parental origin was determined, arose de novo with 30 of them occurring on the maternally inherited chromosome 16, strongly implicating genomic imprinting of the FOXF1 locus in human lungs. Surprisingly, we have also identified four ACDMPV families with the pathogenic variants in the FOXF1 locus that arose on paternal chromosome 16. Interestingly, a combination of the severe cardiac defects, including hypoplastic left heart, and single umbilical artery were observed only in children with deletion CNVs involving FOXF1 and its upstream enhancer. Our data demonstrate that genomic imprinting at 16q24.1 plays an important role in variable ACDMPV manifestation likely through long-range regulation of FOXF1 expression, and may be also responsible for key phenotypic features of maternal uniparental disomy 16. Moreover, in one family, WES revealed a de novo missense variant in ESRP1, potentially implicating FGF signaling in the etiology of ACDMPV.
