16. Multi-fluid modelling of dry convection

Abstract

Convection schemes are a large source of error in existing climate and weather prediction models, likely because most schemes are not scale aware and because net mass transport by convection and convective memory are usually ignored. A relatively new approach called the multi-fluid method may be able to address this issue by modelling the convective updraft as a separate fluid from the rest of the atmosphere, allowing convective plumes to be modelled prognostically at any resolution. In this thesis, we investigate the properties of multi-fluid systems and create a 2-fluid model for dry convection where the fluids are defined by the sign of the vertical velocity. We derive and analyse 20 numerical schemes for transferring mass between fluids (entrainment and detrainment). Two of these schemes, which transfer the mass and fluid properties implicitly, are accurate, bounded, conserve momentum and do not increase the total energy for any timestep. We also extend the stability analysis of Thuburn et al. (2019), who showed that a Kelvin-Helmholtz-like instability exists in the 2-fluid incompressible equations with a shared pressure. By using a pressure perturbation for each fluid proportional to fluid convergence, we show that the instability is significantly suppressed. Moreover, using transfer terms between fluids which are also proportional to convergence, the instability can be removed. We also analyse a high resolution dry convection test case and show that the convergence-based pressure and transfer assumptions are accurate. By using the transfer scheme and fluid pressure anomaly proportional to velocity divergence, the 2-fluid scheme was able to transport heat more effectively than the single-fluid scheme for multi-column grey-zone resolutions. Some errors remain for the multi-column test cases because parameters were tuned for the singlecolumn set-up, for which the 2-fluid scheme is able to accurately model the dry rising bubble test case.

Abstract

Convection schemes are a large source of error in global weather and climate models, and modern resolutions are often too fine to parameterise convection but are still too coarse to fully resolve it. Recently, numerical solutions of multi-fluid equations have been proposed for a more flexible and consistent treatment of sub-grid scale convection, including net mass transport by convection and non-equilibrium dynamics. The technique involves splitting the atmosphere into multiple fluids. For example, the atmosphere could be divided into buoyant updrafts and stable regions. The fluids interact through a common pressure, drag and mass transfers (entrainment and detrainment). Little is known about the numerical properties of mass transfer terms between the fluids. We derive mass transfer terms which relabel the fluids and derive numerical properties of the transfer schemes, including boundedness, momentum conservation and energy conservation. Numerical solutions of the multi-fluid Euler equations using a C-grid are presented using stable and unstable treatments of the transfers on a well-resolved two-fluid dry convection test case. We find two schemes which are conservative, stable and bounded for large timesteps, and maintain their numerical properties on staggered grids., Comment: 22 pages, 5 figures, 2 tables

Abstract

Purpose The aim of this Intensive Care Medicine Rapid Practice Guideline (ICMâRPG) was to formulate evidenceâbased guidance for the use of dexmedetomidine for sedation in invasively mechanically ventilated adults in the intensive care unit (ICU). Methods We adhered to the methodology for trustworthy clinical practice guidelines, including use of the Grading of Recommendations Assessment, Development, and Evaluation approach to assess the certainty of evidence, and the Evidence-to-Decision framework to generate recommendations. The guideline panel comprised 28 international panelists, including content experts, ICU clinicians, methodologists, and patient representatives. Through teleconferences and webâbased discussions, the panel provided input on the balance and magnitude of the desirable and undesirable effects, the certainty of evidence, patients' values and preferences, costs and resources, feasibility, acceptability, and research priorities. Results The ICMâRPG panel issued one weak recommendation (suggestion) based on overall moderate certainty of evidence: 'In invasively mechanically ventilated adult ICU patients, we suggest using dexmedetomidine over other sedative agents, if the desirable effects including a reduction in delirium are valued over the undesirable effects including an increase in hypotension and bradycardia'. Abstract

BACKGROUND: In the ARTESiA trial (Apixaban for the Reduction of Thromboembolism in Patients With Device-Detected Subclinical Atrial Fibrillation), apixaban, compared with aspirin, reduced stroke or systemic embolism in patients with device-detected subclinical atrial fibrillation (SCAF). Clinical guidelines recommend considering SCAF episode duration when deciding whether to prescribe oral anticoagulation for this population. METHODS: We performed a retrospective cohort study in ARTESiA. Using Cox regression adjusted for CHA2DS2-VASc score and treatment allocation (apixaban or aspirin), we assessed frequency of SCAF episodes and duration of the longest SCAF episode in the 6 months before randomization as predictors of stroke risk and of apixaban treatment effect. RESULTS: Among 3986 patients with complete baseline SCAF data, 703 (17.6%) had no SCAF episode ≥6 minutes in the 6 months before enrollment. Among 3283 patients (82.4%) with ≥1 episode of SCAF ≥6 minutes in the 6 months before enrollment, 2542 (77.4%) had up to 5 episodes, and 741 (22.6%) had ≥6 episodes. The longest episode lasted <1 hour in 1030 patients (31.4%), 1 to <6 hours in 1421 patients (43.3%), and >6 hours in 832 patients (25.3%). Higher baseline SCAF frequency was not associated with increased risk of stroke or systemic embolism: 1.1% for 1 to 5 episodes versus 1.2%/patient-year for ≥6 episodes (adjusted hazard ratio, 0.89 [95% CI, 0.59–1.34]). In an exploratory analysis, patients with previous SCAF but no episode ≥6 minutes in the 6 months before enrollment had a lower risk of stroke or systemic embolism than patients with at least one episode during that period (0.5% versus 1.1%/patient-year; adjusted hazard ratio, 0.48 [95% CI, 0.27–0.85]). The frequency of SCAF did not modify the reduction in stroke or systemic embolism with apixaban (P interaction=0.1). The duration of the longest SCAF episode in the 6 months before enrollment was not associated with the risk of stroke or systemic embolism during follow-up (<1 hour: 1.0%/patient-year [reference]; 1–6 hours: 1.2%/patient-year [adjusted hazard ratio, 1.27 (95% CI, 0.85–1.90)]; >6 hours: 1.0%/patient-year [adjusted hazard ratio, 1.02 (95% CI, 0.63–1.66)]). SCAF duration did not modify the reduction in stroke or systemic embolism with apixaban (P trend=0.1). CONCLUSIONS: In ARTESiA, baseline SCAF frequency and longest episode duration were not associated with risk of stroke or systemic embolism and did not modify the effect of apixaban on reduction of stroke or systemic embolism.
