7. An examination of relationships between road accidents and traffic flow

Abstract

In this thesis it is suggested that the cost-effectiveness of road safety expenditure on low cost engineering remedial works could be improved because the currently adopted methods for assessing expenditure priorities do not necessarily identify thosa sites at which the greatest potential for accident reduction exists. An alternative method for the generation of more cost-effective programmes of works is proposed and justified. This method adopts the rationale of identifying those sites at which accidents are occurring in higher numbers than would otherwise be expected for such sites with equivalent traffic volumes and locations. The justification for the method involves detailed statistical analyses of over 10,000 accidents occurring in Lothian Region for the years 1979-1982 which demonstrate that there are significant relationships between accidents and traffic volumes and location details (eg junction type, form of junction control, adjacent roadside development and carriageway type). On this basis, models for accident occurrence have been determined. The analyses show that the temporal distribution conforms with a Poisson process and that the spatial distribution is negative binomial. It is shown - for both links and junctions - that whilst there are significant differences between the models for different accident types, they do not, in aggregate, produce significantly better models for all accidents than simple all accident models. In addition, the importance of regression-to-mean has been established as an effect which should be accounted for not just at the monitoring stage of completed schemes but as an integral part of the initial site selection process. Finally, it is demonstrated that the proposed method, which is called Potential Accident Reduction (PAR), may provide an improvernent of cost-effectiveness of road safety expenditure of up to 25% over the currently adopted methods.

Abstract

Charcot-Marie-Tooth disease type 2D (CMT2D) is an autosomal-dominant axonal peripheral neuropathy characterized by impaired motor and sensory function in the distal extremities. Mutations in the glycyl-tRNA synthetase (GARS) gene cause CMT2D. GARS is a member of the ubiquitously expressed aminoacyl-tRNA synthetase (ARS) family and is responsible for charging tRNA with glycine. To date, 13 GARS mutations have been identified in patients with CMT disease. While functional studies have revealed loss-of-function characteristics, only four GARS mutations have been rigorously studied. Here, we report the functional evaluation of nine CMT-associated GARS mutations in tRNA charging, yeast complementation, and subcellular localization assays. Our results demonstrate that impaired function is a common characteristic of CMT-associated GARS mutations. Additionally, one mutation previously associated with CMT disease (p.Ser581Leu) does not demonstrate impaired function, was identified in the general population, and failed to segregate with disease in two newly identified families with CMT disease. Thus, we propose that this variant is not a disease-causing mutation. Together, our data indicate that impaired function is a key component of GARS-mediated CMT disease and emphasize the need for careful genetic and functional evaluation before implicating a variant in disease onset.

Abstract

Background: SPATA7 mutations have been associated with different autosomal recessive retinal degeneration phenotypes. Long-term follow-up has not been described in detail. Materials and methods: A Hispanic patient with SPATA7 mutations was evaluated serially over a 12-year period with kinetic and static chromatic perimetry, optical coherence tomography (OCT), and fundus autofluorescence (AF) imaging. Electroretinography (ERG) was performed at the initial visit. Results: The patient was homozygous for a mutation in SPATA7 (p.V458fs). At age 9, the ERG showed an abnormally reduced but preserved rod b-wave and no detectable cone signals. There were two islands of vision: a midperipheral island with greater cone than rod dysfunction and a central island with normal cone but no rod function. Serial measures of rod and cone vision and co-localized retinal structure showed that the midperipheral island slowly became undetectable. By age 21, only the central island and its cone function remained, but it had become more abnormal in structure and function. Conclusion: The disease resulting from SPATA7 mutations in this patient initially presented as a cone-rod dystrophy (CRD), but changed over time into a phenotype more reminiscent of late-stage retinitis pigmentosa (RP). The differential diagnosis for both CRD and RP should include this rare molecular cause of autosomal retinal degeneration. An evolving phenotype complicates not only clinical diagnosis and patient counselling but also future strategies aimed at treating specific retinal regions.
