1. Know Diabetes by Heart: Education about the Linkage between Cardiovascular Disease and Diabetes

Abstract

Objective: The objective of this study was to investigate how a 1-hour education session that highlights American Heart Association/American Diabetes Association's Know Diabetes by Heart (KDBH) messaging changed the knowledge and behavioural intention of participants. Design: Single group, quasi-experimental study evaluating the impact of the KDBH education intervention on participants' knowledge and intentions to engage in risk-lowering behaviours related to the link between diabetes and cardiovascular disease. Setting: A free, 1-hour, synchronously delivered education session was offered through an online platform and facilitated by Cooperative Extension agents. Methods: A pre- and post-Qualtrics survey was used to assess changes in knowledge and intended behaviours. Slides were provided by the American Diabetes Association and tailored for clarification and evaluation purposes. Results: The study included 259 adults with an average age of 54 years (standard deviation [SD] = 16.67) and 72.97% being 46 years or older. When participants were asked if having type 2 diabetes put them at risk for development of hypertension, unhealthy cholesterol levels, heart attack, or stroke, 84.84% answered correctly at baseline. After session completion, the percentage correct increased to 92.2% (p = 0.002). Individual's knowledge of cardiovascular disease as the leading cause of death for people with type 2 diabetes was assessed; 66.80% answered correctly pre-intervention. After completing the KDBH programme, 95.37% answered correctly (p = 0.001). In addition, more than half of those with type 2 diabetes reported intended to adopt several behaviours related to reducing the risk of cardiovascular disease at the completion of the programme. Conclusions: The pilot study indicates successful information transfer in a 1-hour education session focused on chronic disease risks associated with T2DM. Results support the use of synchronous online platforms for diabetes and heart disease information transfer. Future prospective studies measuring sustained knowledge, behaviour adoption and incidence of heart-disease before and after education are needed.

Abstract

Vaccination rates against COVID-19 are lower in rural areas across the country, including SC. This paper highlights the work conducted by Clemson Cooperative Extension Service in SC to increase rural access to COVID-19 testing and vaccine services. Extension Agents engaged in clinical and community partnerships to coordinate COVID-19 vaccine and testing clinics in rural and underserved communities. Agents established 204 test clinics and 220 vaccination clinics. The total number of people tested and vaccinated was 9,331 and 2,037, respectively. The success of this project shows Extension agents can be instrumental in bridging the gap between clinical resources and rural communities. [ABSTRACT FROM AUTHOR]

Abstract

Objective: The goals of this study were to determine whether completion of a community-based diabetes self-management support (DSMS) program delivered through a university Cooperative Extension network increased Patient Activation Measure (PAM) scores and to examine predictors of improvement in PAM score in individuals participating in the DSMS. Methods: The Health Extension for Diabetes (HED) is a 4-month program delivered via a paraprofessional extension agent in partnership with an established diabetes self-management education and support program. The study population included 148 adults (median age 69 years; interquartile range 60–74 years) with diabetes recruited from local community organizations. Data for the analysis were collected before and after participation in the intervention as part of a longitudinal study, using the PAM and Self-Efficacy for Diabetes instruments. Descriptive statistics were gathered, and hypothesis tests and simple and multivariable regression analyses were conducted. Results: The mean PAM score increased by 6.58 points, with a 5-point change considered clinically significant. From pre- to post-intervention, PAM scores significantly decreased for 23 participants, decreased for 6, did not change for 14, increased for 21, and significantly increased for 84. Higher pre-intervention PAM scores, younger age, greater educational attainment, and higher baseline self-efficacy scores were associated with increased post-intervention PAM scores when not controlling for potential covariates. Age was no longer associated with higher PAM scores after controlling for covariates. Conclusion: Community-based DSMS interventions can be effective in generating positive change in individuals' activation. HED provides a feasible and accessible DSMS option that addresses key diabetes self-management components while effectively improving individuals' activation. It is recommended that people living with diabetes attend a DSMS program such as HED to increase their ability to effectively self-manage various components of their chronic condition. Abstract

Background: Nusinersen is an antisense oligonucleotide drug that modulates pre–messenger RNA splicing of the survival motor neuron 2 (SMN2) gene. It has been developed for the treatment of spinal muscular atrophy (SMA). Methods: We conducted a multicenter, double-blind, sham-controlled, phase 3 trial of nusinersen in 126 children with SMA who had symptom onset after 6 months of age. The children were randomly assigned, in a 2:1 ratio, to undergo intrathecal administration of nusinersen at a dose of 12 mg (nusinersen group) or a sham procedure (control group) on days 1, 29, 85, and 274. The primary end point was the least-squares mean change from baseline in the Hammersmith Functional Motor Scale–Expanded (HFMSE) score at 15 months of treatment; HFMSE scores range from 0 to 66, with higher scores indicating better motor function. Secondary end points included the percentage of children with a clinically meaningful increase from baseline in the HFMSE score (≥3 points), an outcome that indicates improvement in at least two motor skills. Results: In the prespecified interim analysis, there was a least-squares mean increase from baseline to month 15 in the HFMSE score in the nusinersen group (by 4.0 points) and a least-squares mean decrease in the control group (by –1.9 points), with a significant between-group difference favoring nusinersen (least-squares mean difference in change, 5.9 points; 95% confidence interval, 3.7 to 8.1; P Conclusions: Among children with later-onset SMA, those who received nusinersen had significant and clinically meaningful improvement in motor function as compared with those in the control group. 13. Analysis of Highly-Enriched Uranium Spectra Obtained with Lanthanum Bromide Scintillation Detection Systems

Abstract

Cerium-doped lanthanum bromide scintillation detection systems have recently been examined as an alternative to conventional detection systems, namely high-purity germanium, cadmium zinc telluride, and thallium-doped sodium iodide systems for various reasons including portability, sizing, and efficacy. As a non-destructive assay technique, these detectors quantify gamma rays from various samples to measure and identify specific radioisotopes. In nuclear facilities specializing in uranium, these detectors are mainly utilized to detect characteristic low-energy gamma rays of uranium-235, specifically, 143 keV, 163 keV, 186 keV, 202 keV and 205 keV gamma rays. Accurately distinguishing closely-spaced gamma rays in spectral data is a common challenge in the non-destructive assay field that is typically tackled through programs that analyze gamma-ray photopeaks. Unfortunately, standardized programs that cater to conventional detectors do not produce accurate results for highly-enriched uranium samples examined by lanthanum bromide detectors, as lanthanum bromide crystals have their own intrinsic background radiation and resolution. Thus, the development of a program that could anticipate and accurately analyze spectra of sources containing uranium-235 from lanthanum bromide detection systems was conducted and is presented in this thesis. In the spectral analysis program formed, uranium-235 source spectra are calibrated and subtractions of an intrinsic background spectrum, high-energy gamma-ray Compton continua, and a scatter-in Compton continuum from the 186 keV gamma ray proceeds. The program then fits Gaussian functions to each characteristic photopeak to determine its area, width, and centroid. The peak information retrieved from this program will ultimately be used with a separate differential attenuation program to quantify the amount of uranium-235. The spectral analyzation program developed can characterize the mass of uranium-235 in a sample to 10.33 percent error using a lanthanum bromide detection system without the use of a differential attenuation program. It should be noted that the primary purpose of determining uranium-235 mass with the spectra analyzation program alone was to investigate which uranium-235 photopeaks were susceptible to systematic errors and to guide future development in background subtraction methodologies. Overall, the software created in this research is successful in quickly characterizing lanthanum bromide spectra and is suggested for use in future non-destructive assay application
