Abstract

The NCCN Guidelines for Genetic/Familial High-Risk Assessment: Breast, Ovarian, and Pancreatic focus primarily on assessment of pathogenic/likely pathogenic (P/LP) variants associated with increased risk of breast, ovarian, pancreatic, and prostate cancer, including BRCA1, BRCA2, CDH1, PALB2, PTEN, and TP53, and recommended approaches to genetic counseling/testing and care strategies in individuals with these P/LP variants. These NCCN Guidelines Insights summarize important updates regarding: (1) a new section for transgender, nonbinary and gender diverse people who have a hereditary predisposition to cancer focused on risk reduction strategies for ovarian cancer, uterine cancer, prostate cancer, and breast cancer; and (2) testing criteria and management associated with TP53 P/LP variants and Li-Fraumeni syndrome.

Abstract

We estimated total mortality using catch curves based on relative return rates (RRs) of coded wire tagged lake trout (Salvelinus namaycush) in US waters of Lake Huron. RR was calculated as age-specific CPUE per million of fish stocked. Annual mortality for the late 1990s through early 2000s was estimated as 38% from the 1991-1995 year classes, with an effective age range of 5-10 years, and then was estimated as 24% for the post-2000 period from the 1996-2009 year classes. The two estimates from simple catch curve regressions based on average RR at age values were the same as from a mixed model with individual RR values from all stocking events. These two estimates were also comparable to the findings from statistical catch-at-age assessments with fundamentally different assumptions. Our approach is not constrained by the assumption that the expected recruitment is a constant over time and thus has the advantage to use multiple observations on each age from multiple cohorts. Our approach has broad applicability to aquatic ecosystems in which multiple mark-and-release events of fish stocking have been implemented. Nous avons estime la mortalite totale en utilisant des courbes de prises reposant sur les taux de retour relatifs (RR) de touladis (Salvelinus namaycush) dotes de micromarques codees, dans les eaux americaines du lac Huron. Le RR a ete calcule comme etant la CPUE selon l'age par million de poissons ensemences. La mortalite annuelle de la fin des annees 1990 au debut des annees 2000 a ete estimee a 38 % a partir des classes annuelles de 1991 a 1995, pour une fourchette d'ages reels de 5-10 ans, puis a 24 % pour la periode post-2000 a partir des classes annuelles de 1996 a 2009. Les deux valeurs estimees a partir de regressions simples de courbes de prises basees sur les valeurs de RR moyennes selon l'age sont les memes que celles issues d'un modele mixte integrant les RR de tous les episodes d'ensemencement. Ces deux valeurs estimees sont aussi semblables aux resultats devaluations statistiques des prises en fonction de l'age reposant sur des hypotheses fondamentalement differentes. Notre approche n'est pas restreinte par l'hypothese de la constance dans le temps du recrutement attendu et presente donc l'avantage de permettre d'utiliser de multiples observations pour chaque classe d'age de differentes cohortes. Abstract

The incidence of colorectal cancer (CRC) among individuals younger than age 50 (early-onset CRC [EOCRC]) has substantially increased, and yet the etiology and molecular mechanisms underlying this alarming rise remain unclear. We compared tumor-associated T-cell repertoires between EOCRC and average-onset CRC (AOCRC) to uncover potentially unique immune microenvironment-related features by age of onset. Our discovery cohort included 242 patients who underwent surgical resection at Cleveland Clinic from 2000 to 2020. EOCRC was defined as younger than age 50 years at diagnosis (N = 126) and AOCRC as 60 years of age or older (N = 116). T-cell receptor (TCR) abundance and clonality were measured by immunosequencing of tumors. Logistic regression models were used to evaluate the associations between TCR repertoire features and age of onset, adjusting for sex, race, tumor location, and stage. Findings were replicated in 152 EOCRC and 1984 AOCRC cases from the Molecular Epidemiology of Colorectal Cancer Study. EOCRC tumors had significantly higher TCR diversity compared with AOCRC tumors in the discovery cohort (odds ratio [OR] = 0.44, 95% confidence interval [CI] = 0.32 to 0.61, P  < .0001). This association was also observed in the replication cohort (OR = 0.74, 95% CI = 0.62 to 0.89, P  = .0013). No significant differences in TCR abundance were observed between EOCRC and AOCRC in either cohort. Higher TCR diversity, suggesting a more diverse intratumoral T-cell response, is more frequently observed in EOCRC than AOCRC. Further studies are warranted to investigate the role of T-cell diversity and the adaptive immune response more broadly in the etiology and outcomes of EOCRC. [ABSTRACT FROM AUTHOR]

Abstract

Biorefining is seen as a potential method to produce protein‐rich feed for monogastric farm animals from grassland, which does not compete with human nutrition. Therefore, a new biorefinery facility was constructed and tested in this experiment by using silages from grass clover mixture and red clover. After biorefining, press juice was stored for further use while press cake was re‐ensiled. Samples from silage, press juice, fresh press cake and re‐ensiled press cake were analysed for nutrient composition, fermentation parameters, amino acids (AA) and biogenic amines (BA) concentrations. Furthermore, digestibility of silage and re‐ensiled press cake was tested in wethers. We found that press juice contained about 205 g crude protein (CP), more than 180 g crude ash and more than 130 g volatile organic compounds (VOC, all values per kg dry matter [DM]). Furthermore, press juice had an unfavourable AA ratio for use as a monogastric feed (methionine and cysteine were reduced). Forage type hardly affected nutritive value of press juice. Re‐ensiling of press cake was successful as a sufficient decrease of pH and VOC concentration was observed. Press cake had 26–36 g/kg DM lower CP content and 0.77–1.12 MJ/kg DM lower metabolisable energy content than silage with greater differences in red clover than in grass clover mixture. Press juice can be used as feed in monogastric animals, but its use is limited due to its low CP content and unfavourable AA profile. Abstract

The human DNA repair enzyme MUTYH excises mispaired adenine residues in oxidized DNA. Homozygous MUTYH mutations underlie the autosomal, recessive cancer syndrome MUTYH-associated polyposis. We report a MUTYH variant, p.C306W (c.918C>G), with a tryptophan residue in place of native cysteine, that ligates the [4Fe4S] cluster in a patient with colonic polyposis and family history of early age colon cancer. In bacterial MutY, the [4Fe4S] cluster is redox active, allowing rapid localization to target lesions by long-range, DNA-mediated signalling. In the current study, using DNA electrochemistry, we determine that wild-type MUTYH is similarly redox-active, but MUTYH C306W undergoes rapid oxidative degradation of its cluster to [3Fe4S]+, with loss of redox signalling. In MUTYH C306W, oxidative cluster degradation leads to decreased DNA binding and enzyme function. This study confirms redox activity in eukaryotic DNA repair proteins and establishes MUTYH C306W as a pathogenic variant, highlighting the essential role of redox signalling by the [4Fe4S] cluster.

Abstract

ObjectivesCombinations of gemcitabine, 5-fluorouracil (5-FU), and platinum have demonstrated improved outcomes compared with singlet chemotherapy in pancreatic and biliary cancers. This phase II study examined efficacy and safety of a novel schedule of cisplatin, fixed-dose-rate gemcitabine and infusional 5-FU.Materials and methodsPatients with metastatic adenocarcinoma of the pancreas or biliary tract, previously untreated or having received 1 cytotoxic regimen for advanced disease, were treated with gemcitabine 1000 mg/m intravenously (IV) over 100 minutes, cisplatin 35 mg/m IV over 30 minutes, and 5-FU 2400 mg/m IV over 48 hours on day 1 of a 14-day cycle. Patients were treated until disease progression or for 12 cycles. After 12 cycles, patients with stable or responding disease could continue gemcitabine and 5-FU. The primary endpoint was objective response.ResultsThirty-nine patients were treated: 8 with biliary cancer (all untreated) and 31 with pancreatic cancer (17 untreated, 14 previously treated). Best response in 25 untreated patients was partial response in 40%, stable disease in 40%, and progressive disease in 20%. In 14 previously treated pancreatic patients, best response was partial response in 7%, stable disease in 50%, and progressive disease in 43%. Median overall survival in untreated patients was 10.3 versus 4.9 months in previously treated patients. Adverse events were primarily uncomplicated hematologic toxicity, ≥grade 3 neutropenia (54%), anemia (21%), and thrombocytopenia (13%).ConclusionBiweekly cisplatin, fixed-dose-rate gemcitabine, and infusional 5-FU demonstrated a high response rate and were well tolerated, encouraging further investigation of this regimen in metastatic pancreatic and biliary cancers.

Abstract

Genome-wide association studies (GWAS) have identified more than 200 common genetic variants independently associated with colorectal cancer (CRC) risk, but the causal variants and target genes are mostly unknown. We sought to fine-map all known CRC risk loci using GWAS data from 100,204 cases and 154,587 controls of East Asian and European ancestry. Our stepwise conditional analyses revealed 238 independent association signals of CRC risk, each with a set of credible causal variants (CCVs), of which 28 signals had a single CCV. Our cis-eQTL/mQTL and colocalization analyses using colorectal tissue-specific transcriptome and methylome data separately from 1299 and 321 individuals, along with functional genomic investigation, uncovered 136 putative CRC susceptibility genes, including 56 genes not previously reported. Analyses of single-cell RNA-seq data from colorectal tissues revealed 17 putative CRC susceptibility genes with distinct expression patterns in specific cell types. Analyses of whole exome sequencing data provided additional support for several target genes identified in this study as CRC susceptibility genes. Enrichment analyses of the 136 genes uncover pathways not previously linked to CRC risk. Our study substantially expanded association signals for CRC and provided additional insight into the biological mechanisms underlying CRC development.

Abstract

BackgroundDNA damage is an established mediator of carcinogenesis, although genome-wide association studies (GWAS) have identified few significant loci. This cross-cancer site, pooled analysis was performed to increase the power to detect common variants of DNA repair genes associated with cancer susceptibility.MethodsWe conducted a cross-cancer analysis of 60,297 single nucleotide polymorphisms, at 229 DNA repair gene regions, using data from the NCI Genetic Associations and Mechanisms in Oncology (GAME-ON) Network. Our analysis included data from 32 GWAS and 48,734 controls and 51,537 cases across five cancer sites (breast, colon, lung, ovary, and prostate). Because of the unavailability of individual data, data were analyzed at the aggregate level. Meta-analysis was performed using the Association analysis for SubSETs (ASSET) software. To test for genetic associations that might escape individual variant testing due to small effect sizes, pathway analysis of eight DNA repair pathways was performed using hierarchical modeling.ResultsWe identified three susceptibility DNA repair genes, RAD51B (P < 5.09 × 10(-6)), MSH5 (P < 5.09 × 10(-6)), and BRCA2 (P = 5.70 × 10(-6)). Hierarchical modeling identified several pleiotropic associations with cancer risk in the base excision repair, nucleotide excision repair, mismatch repair, and homologous recombination pathways.ConclusionsOnly three susceptibility loci were identified, which had all been previously reported. In contrast, hierarchical modeling identified several pleiotropic cancer risk associations in key DNA repair pathways.ImpactResults suggest that many common variants in DNA repair genes are likely associated with cancer susceptibility through small effect sizes that do not meet stringent significance testing criteria.
