Abstract

BackgroundFew contemporary data exist evaluating care patterns and outcomes in heart failure (HF) across the spectrum of kidney function.ObjectivesThis study sought to characterize differences in quality of care and outcomes in patients hospitalized for HF by degree of kidney dysfunction.MethodsGuideline-directed medical therapies were evaluated among patients hospitalized with HF at 418 sites in the GWTG-HF (Get With The Guidelines-Heart Failure) registry from 2014 to 2019 by discharge CKD-EPI (Chronic Kidney Disease Epidemiology Collaboration)-derived estimated glomerular filtration rate (eGFR). We additionally evaluated the risk-adjusted association of admission eGFR with in-hospital mortality.ResultsAmong 365,494 hospitalizations (age 72 ± 15 years, left ventricular ejection fraction [EF]: 43 ± 17%), median discharge eGFR was 51 ml/min/1.73 m2 (interquartile range: 34 to 72 ml/min/1.73 m2), 234,332 (64%) had eGFR

Abstract

The strong performance of the banking industry in 1995 is likely to continue in 1996. However, growth will be harder to achieve in the new year because of structural challenges confronting bankers. Revenue is one area of growth that is expected to present some difficulties as banks contend with dwindling excess liquidity, tightening deposit spreads and increasing credit costs. Bankers can meet such structural challenges by implementing measures that are geared toward continual efficiency improvements, effective capital management, identification of niche markets that offer a competitive edge and the allocation of resources to these markets. Through appropriate competitive responses, banks will be able to achieve superior returns despite the challenges they face.

Abstract

Banks cannot afford to become complacent despite the industry's current high level of profitability. Notwithstanding their strong capital ratios, increased loan volume, improved credit quality and record earnings, banks must prepare themselves for the challenges presented by the rapid pace of technological innovations, shifting customer preferences, and fiercer competition from other banks and non-banks that are well-positioned to meet such changes. Industry consolidation is one of the best strategies for coping with these challenges. Through acquisitions and mergers, banks can cut the costs of providing financial services by doing away with redundant functions and exploiting economies of scale found in many banking operations. Consolidation also allows banks to combine their resources so that they will be able to afford the huge costs of transforming themselves into the kind of organizations that customers will expect.

Abstract

A 12 percent improvement in bank profit performance is predicted over 1986, when median net income per share rose 10 percent for 170 leading US banks followed by Keefe, Bruyette & Woods Inc. Money center banks should see a 10 percent advance, but this projection increases to 12 percent when applied to operating earnings. Keefe Bank Index data shows that 24 money center and major regional banking companies are not suffering sectorial profitability declines affecting other industries undergoing deregulation. Margin stability, reduced loan loss provision, and cost containment efforts should influence the potential of the banking industry to produce another solid earnings gain in 1987.

During 1984, the dollar profits of the 25 largest U.S. bank holding companies increased an average of five percent compared to 13 percent in 1982 and 1983. Profits were adversely affected by foreign loan exposure of multinational banks, low inflation, and narrow spreads. The quality of assets generally deteriorated in 1984 due to the agricultural and energy sectors of the domestic economy and loans to troubled debtor nations. Expense controls, healthier balance sheets, and momentum gained in the last quarter of 1984 are expected to contribute to improved bank earnings in 1985.

Abstract

This report is the second of two reports on retail level purchasing practices. In an effort to reduce inventories and increase the use of commercial distribution systems, DoD implemented best commercial inventory initiatives for the purchase and distribution of military supply requirements. DoD expects the initiatives to transform its methods of procuring materiel into advanced private sector business practices. Among other benefits, the transformation should provide retail level organizations lower pricing as a result of leveraged buying power. The initiatives included a Defense Logistics Agency (DLA) electronic catalog pilot program ordering system (April 1997) and a DoD electronic mall (January 1998). The electronic catalog was incorporated into the electronic mall in January 1998 to provide virtual one-stop shopping over the Internet. Defense Reform Initiative Directive No. 43, "Defense- wide Electronic Commerce," May 20, 1998, established the Joint Electronic Commerce Program Office (JECPO). JECPO, under the direction and oversight%of the DoD Chief Information Officer, serves as the primary entity to integrate electronic commerce in the DoD business cycle and to manage the DoD electronic commerce program. Orders for the electronic catalog products from April 1997 through January 20, 1999, were about $553,000, and electronic mall sales since inception, including electronic catalog sales, through March 1999 were approximately $40.7 million.

Introduction. This report is the first in a series of reports on retail level purchasing practices. DLA initiatives included an electronic catalog ordering system (April 1997) and a blanket purchase agreement for office supplies (October 1998) issued against a General Services Administration Federal supply schedule. Retail organizations procuring material through those initiatives paid DLA a cost recovery rate (percentage of the price of materiel ordered) for administering the initiatives. The FY 1999 rate for the electronic catalog and blanket purchase agreement for office supplies was 7 percent and 4.8 percent, respectively. Orders for the electronic catalog products from April 1997 through January 20,1999, were $553,000, and FY 1999 sales for office supplies are projected to be 2.9 million. Objective. The overall audit objective was to determine whether retail level organizations in DoD were using the most economical and efficient source of supply when purchasing commercial brand name items through centralized Federal procurement programs. The specific objective of this report was to evaluate the financial impact of the electronic catalog and office supplies initiatives on retail organizations for materiel ordered through those initiatives. We also included a review of the management control program as it applied to the specific audit objective.

Abstract

The audit was requested by the Assistant Deputy Under Secretary of Defense (Materiel and Distribution Management). This report is the first in a series of reports on dual management of commercially available items. The program, one of several Defense Logistics Agency (DLA) initiatives to expand sales of commercial items, involves developing an electronic catalog from which customers may browse, select, and order commercially available items. The DLA program will be tested in phases, with four customers and seven vendors participating. Initial testing of the electronic catalog, one customer and one vendor, began on April 15, 1997. If successful, the catalog is planned to be rapidly expanded into a major buying tool for DLA customers. The planned completion date for the pilot program is September 1997. The audit objectives were to determine the extent of products available through non-Defense Federal organizations for which DoD also operates central procurement programs and to evaluate whether the DoD programs are providing services without added benefit to DoD. The objective of this report was to evaluate the electronic catalog to determine the extent of commercially available items in the pilot program that were also available from the General Services Administration. Subsequent reports will determine the extent of specific commodities available through non-Defense Federal organizations for which DoD also operates centralized procurement programs. We also reviewed the adequacy of management control programs as they applied to the stated objectives.

Abstract

Profit pressures, increased competition, and changes in the banking business draw attention to the management of operating expenses. Some banks have responded by restructuring their organizations, controlling operating expenses, and curtailing growth rates. Improvement also occurs in the relationship of expenses to revenues and of income to head count. The growing trend of expense control is illustrated by the example of Chemical Bank New York Corp, which expects pre-tax savings of $150 million in 1988. These savings are the result of eliminating staff, the closing of domestic and overseas branches, and reductions in consumer and middle-market projects. The future sale of assets without strategic importance is expected to further enhance the effects of restructuring at Chemical Bank New York Corp.
