Background: Lactic acidosis (LA) frequently occurs after heart transplantation (HTx). It is hypothesized to be related to inotropic support or metabolic derangements from chronic heart failure. As such, restoring hemodynamic stability with mechanical circulatory support before HTx should mitigate this problem. Our aim was to evaluate the incidence and outcomes of LA after HTx., Methods: We evaluated HTx recipients January 2000 to May 2011. Post-HTx outcomes included graft dysfunction, length of intensive care unit stay, length of hospital stay, inotropic support, and survival., Results: Of 143 eligible patients, 98.6% had LA, 67% severe, after HTx. Data were analyzed based on the severity of LA. Time to peak lactate, intensive care unit stay, length of hospital stay, peak glucose, inotropic dose, graft dysfunction, and survival after HTx were similar between groups. Statistically significant differences included pretransplant support (25.6% mechanical circulatory support in nonsevere vs. 44.9% severe LA), hospitalization at the time of HTx (37.2% vs. 21.4%), glucose at the time of peak lactate (182.88 ± 69.80 vs. 221.31 ± 56.91), ischemic time (187.4 ± 63.1 vs. 215.5 ± 68.1), and duration of inotrope., Conclusion: Severe LA is common after HTx, though it appears to be transient and benign. Mechanical circulatory support after HTx does not prevent LA. High lactate levels are associated with longer ischemic times, longer duration of inotrope, and correspond with higher glucose levels. The underlying mechanism is yet to be satisfactorily elucidated.

Objective: To evaluate the association between BMI: kg/m(2) and mortality among Hispanic adults., Design and Methods: Eight years (1997-2004) of National Health Interview Survey data linked to public-use mortality follow-up data through 2006 were acquired. Using Cox proportional hazards regression, separate models for two attained age strata (18 to <60 years, ≥60 years) adjusting for sex, smoking, and physical activity with over 38,000 analyzable respondents were fit., Results: Among those aged ≥60 years, underweight (BMI ≤ 18.5) associated with elevated mortality (hazard ratio [HR] = 2.19; 95% confidence interval [CI], 1.38-3.46), whereas overweight (BMI of 25 to <30) and obesity grade 1 (BMI of 30 to <35) associated with reduced mortality (HRs = 0.79; 95% CI, 0.65-0.95 and 0.71; 95% CI, 0.56-0.91), respectively. There were no significant associations between BMI and mortality among the 18 to <60 years attained age strata or among never smokers for either age strata., Conclusions: Overweight and obesity are not obviously associated with elevated mortality among Hispanic adults., (Copyright © 2013 The Obesity Society.)

In this study, wheat, Triticum aestivum L. (em Thell), genotypes containing the Dnx, Dn7, Dn6, and Dn4 genes for resistance to the Russian wheat aphid, Diuraphis noxia (Kurdjumov) (Hemiptera: Aphididae), along with Dn0, a susceptible control, were assessed to determine the categories of D. noxia biotype 2 (RWA2) resistance in each genotype and RWA2 feeding behaviors on Dnx and Dn0 plants by using the electronic penetration graph technique. At 14 d postinfestation, Dn0 plants exhibited intense chlorosis and leaf rolling, and all test genotypes expressed some degree of chlorosis and leaf rolling, except Dn7, which was not damaged. Both Dn7 and Dnx expressed antibiosis effects, significantly reducing the numbers of aphids on plants and the intrinsic rate of aphid increase. Dn6 plants seemed to contain tolerance, exhibiting tolerance index measurements for leaf and root dry weight and plant height that were significantly lower than those of the susceptible Dn0 plants. Principal component analyses indicated that antibiosis and leaf rolling data explained 80% of the variance among genotypes. Electronic penetration graph analysis demonstrated contrasting results between RWA1 and RWA2 phloem sieve element phase feeding events, but results indicated that Dnx resistance factors are present in the sieve element cells or phloem sap. Plants containing Dnx exhibit antibiosis resistance to D. noxia RWA2 similar to that in plants containing the Secale cereale L. (rye)-based Dn7 gene without the negative baking quality traits associated with Dn7.

Objective: To examine the agreement between direct arterial blood pressure measurements obtained from 2 arteries and indirect blood pressure measurements obtained with an oscillometric blood pressure monitor (OBPM) during normotension and phenylephrine-induced hypertension in dogs., Animals: 16 male Beagles., Procedures: In anesthetized dogs, arterial catheters were placed in the lingual and dorsal pedal arteries for measurement of arterial blood pressure. A blood pressure cuff was placed on either the dog's fore- or hind limb and connected to an OBPM. Systolic, diastolic, and mean arterial blood pressures (SAP, DAP, and MAP, respectively) were recorded from both arteries and the OBPM every 5 minutes for 30 minutes (baseline), during a 30-minute period in which dogs received a phenylephrine infusion IV to induce hypertension, and for 30 minutes after discontinuation of the infusion. Mean differences in blood pressure values and confidence intervals were calculated to compare the indirect and direct measurement techniques., Results: In dogs, oscillometry underestimated SAP during normotension, and the difference between oscillometric and direct measurements increased during hypertension. Oscillometry underestimated DAP, but the difference between oscillometric and direct measurements decreased during hypertension. There was close agreement among techniques for MAP determinations. Biases between direct measurements and OPBM blood pressure values measured from dogs' forelimbs or hind limbs were not significantly different., Conclusions and Clinical Relevance: In normotensive dogs, oscillometric measurements of MAP and SAP agreed more closely with direct arterial pressure measurements than oscillometric estimates of DAP. Oscillometric measurement of MAP was accurate during both normotension and hypertension in dogs.
