Abstract

COVID study groups - PORTUGAL: Portuguese Laboratory Network for the Diagnosis of COVID-19 and Public Health Department of the Health Administrative Regions, Physicians that provided data and samples from suspected cases and SARS-CoV-2 genetic characterization. INSA laboratory team for the diagnosis of SARS-CoV-2. Algarve Biomedical Center and Unilabs. We compared 19,207 cases of SARS-CoV-2 variant B.1.1.7/S gene target failure (SGTF), 436 B.1.351 and 352 P.1 to non-variant cases reported by seven European countries. COVID-19 cases with these variants had significantly higher adjusted odds ratios for hospitalisation (B.1.1.7/SGTF: 1.7, 95% confidence interval (CI): 1.0-2.9; B.1.351: 3.6, 95% CI: 2.1-6.2; P.1: 2.6, 95% CI: 1.4-4.8) and B.1.1.7/SGTF and P.1 cases also for intensive care admission (B.1.1.7/SGTF: 2.3, 95% CI: 1.4-3.5; P.1: 2.2, 95% CI: 1.7-2.8). 23. Rapid diagnosis of seasonal Influenza virus and cohorting of hospitalised patients on a 'flu ward'. A prospective analysis of outcomes.

Abstract

Background: The influenza season of 2017/2018 was burdensome in comparison to previous years. In patient management of seasonal influenza patients is poorly described.Aim: To assess the impact of managing influenza patients on a dedicated influenza ward on antimicrobial use and duration, and length of stay (LOS).Methods: A prospective cohort study from Jan 1st to Feb 28th 2018. Patients diagnosed with influenza in the Emergency Department (ED) were cohorted under infectious disease (ID) or a general internal medicine (GIM) firms on a 35 bed influenza ward. At times of maximum capacity some patients were managed on other wards by other firms 'non flu ward'.Findings: 91 patients were admitted to the influenza ward from ED (64 ID, 27 GIM), 38 had influenza A. Patients managed by ID were more likely to be switched to oral antibiotics sooner median 3 vs 5 days p=.049. Antibiotic duration was shorter for patients managed by the ID firm median 7 vs 9 days p=.016. LOS was shorter for patients managed by the ID firm on the flu ward vs 'non flu ward', median 5 vs 9 days p=.007. No significant difference was seen between ID and GIM LOS on the flu ward, median 5 vs 7 days p=0.30.Conclusion: Influenza patients managed by an infectious disease service on an influenza ward had reduced length of intravenous (IV) and total antimicrobial use compared to a GIM service and had reduced LOS compared to the standard of care, 'non flu ward' influenza patients. Abstract

Many practical clinical questions regarding the management of human immunodeficiency virus (HIV)- associated neurocognitive disorder (HAND) remain unanswered. We sought to identify and develop practical answers to key clinical questions in HAND management. Sixty-six specialists from 30 countries provided input into the program, which was overseen by a steering committee. Fourteen questions were rated as being of greatest clinical importance. Answers were drafted by an expert group based on a comprehensive literature review. Sixty-three experts convened to determine consensus and level of evidence for the answers. Consensus was reached on all answers. For instance, good practice suggests that all HIV patients should be screened for HAND early in disease using standardized tools. Follow-up frequency depends on whether HAND is already present or whether clinical data suggest risk for developing HAND. Worsening neurocognitive impairment may trigger consideration of antiretroviral modification when other causes have been excluded. The Mind Exchange program provides practical guidance in the diagnosis, monitoring, and treatment of HAND.

31. Long-term protection against carriage of hepatitis B virus after infant vaccination

Abstract

BACKGROUND: Carriage of hepatitis B virus (HBV) is a major risk factor for liver cirrhosis and hepatocellular carcinoma. Infant vaccination has been effective in preventing horizontal transmission during early childhood. It is unknown whether protection is maintained into early adulthood. METHODS: In 1984, early childhood vaccination was introduced in 2 rural Gambian villages. In 2003, serological assessment of 81.5% of 1,350 eligible participants 1-24 years old was done, to determine vaccine efficacy against infection and carriage. RESULTS: Overall vaccine efficacy against infection and carriage was 83.4% (95% confidence interval [CI], 79.8%-86.6%) and 96.5% (85% CI, 93.9%-98.9%), respectively. Vaccine efficacy against infection was similar when restricted to primary responders (85.3%), but a significant effect of peak antibody concentration was found. Both vaccine efficacy and levels of hepatitis B surface antibody (anti-HBs) decreased with age, resulting in a vaccine efficacy against infection and carriage among 20-24-year-old participants of 70.9% (95% CI, 60.4%-80.5%) and 91.1% (95% CI, 75.8%-100%), respectively. Fifteen years after vaccination, fewer than half of the vaccinees had detectable anti-HBs. The prevalence of carriage in the unvaccinated population was similar to the prevalence 20 years earlier. CONCLUSIONS: HBV vaccination early during life can provide long-lasting protection against carriage, despite decreasing antibody levels. The role played by subclinical boosting and the necessity of a booster need to be evaluated.

Abstract

A 7-year-old shar-pei was presented because of a recurrent dermatologic condition. Skin biopsies revealed an idiopathic (primary) cutaneous mucinosis that initially responded to corticosteroids. The condition reappeared 2 years later and subsequent biopsies revealed a mast cell tumor in some of the skin sites previously diagnosed with mucinosis.

Abstract

A Malamute was examined for acute abdominal pain and collapse. Radiographs of the abdomen showed a pneumoabdomen, and a lucent mass in the region of the liver. Abdominocentesis yielded an inflammatory exudate with bacteria compatible with a Bacillus or a Clostridium sp. A quadrate lobe torsion was found at postmortem.
