Abstract

We identify a pseudolocal conserved charge in the Fredkin and Motzkin quantum spin chains and explore its consequences for the hydrodynamics of systems with Fredkin- or Motzkin-type kinetic constraints. We use this quantity to formulate an exact upper bound ${\cal O}(L^{-5/2})$ on the gap of the Fredkin and Motzkin spin chains. Our results establish that transport in kinetically constrained dynamical systems with Fredkin or Motzkin constraints is subdiffusive, with dynamical exponent $z \geq 5/2$., Comment: Improved presentation

5. Slow crossover from superdiffusion to diffusion in isotropic spin chains

Abstract

Finite-temperature spin transport in integrable isotropic spin chains (i.e., spin chains with continuous nonabelian symmetries) is known to be superdiffusive, with anomalous transport properties displaying remarkable robustness to isotropic integrability-breaking perturbations. Using a discrete-time classical model, we numerically study the crossover to conventional diffusion resulting from both noisy and Floquet isotropic perturbations of strength $\lambda$. We identify an anomalously-long crossover time scale $t_\star \sim \lambda^{-\alpha}$ with $\alpha \approx 6$ in both cases. We discuss our results in terms of a kinetic theory of transport that characterizes the lifetimes of large solitons responsible for superdiffusion.

Abstract

The Earth BioGenome Project (EBP) is an audacious endeavor to obtain whole-genome sequences of representatives from all eukaryotic species on Earth. In addition to the project's technical and organizational challenges, it also faces complicated ethical, legal, and social issues. This paper, from members of the EBP's Ethical, Legal, and Social Issues (ELSI) Committee, catalogs these ELSI concerns arising from EBP. These include legal issues, such as sample collection and permitting; the applicability of international treaties, such as the Convention on Biological Diversity and the Nagoya Protocol; intellectual property; sample accessioning; and biosecurity and ethical issues, such as sampling from the territories of Indigenous peoples and local communities, the protection of endangered species, and cross-border collections, among several others. We also comment on the intersection of digital sequence information and data rights. More broadly, this list of ethical, legal, and social issues for large-scale genomic sequencing projects may be useful in the consideration of ethical frameworks for future projects. While we do not-and cannot-provide simple, overarching solutions for all the issues raised here, we conclude our perspective by beginning to chart a path forward for EBP's work.

Abstract

BackgroundHeterozygous mutations in caspase recruitment domain family member 14 gene (CARD14) have been shown to be associated with psoriasis and familial pityriasis rubra pilaris (PRP). Many subjects with CARD14 mutations display features of both disorders, which can result in diagnostic uncertainty. In addition, these eruptions are often recalcitrant to conventional psoriasis therapies such as methotrexate, oral retinoids, and tumor necrosis factor-α inhibitors.ObjectiveWe sought to describe the clinical characteristics, family history, and response to therapy in subjects with papulosquamous eruptions due to mutations in CARD14.MethodsSubjects were referred for genetic testing as part of a registry of subjects with inherited disorders of keratinization. DNA was isolated from blood or saliva, and multiplex targeted sequencing or whole exome sequencing was performed. Clinical histories of subjects with CARD14 mutations were reviewed.ResultsWe identified 15 kindreds with CARD14-associated papulosquamous eruption (CAPE). Characteristic features of CAPE include early age of onset; prominent involvement of the cheeks, chin, and ears; family history of psoriasis or PRP; minimal response to conventional topical and systemic psoriasis therapies; and improvement with ustekinumab.LimitationsRelatively small sample size.ConclusionsMany subjects with CARD14 mutations display characteristics of both psoriasis and PRP. We propose the term CARD14-associated papulosquamous eruption to describe this spectrum of disease. Subjects with clinical features suggestive of CAPE should undergo CARD14 sequencing and may benefit from treatment with ustekinumab.

Abstract

Background Wounds continue to be of global concern, amid a lack of widespread adoption of evidence-based practice (EBP). Negative Pressure Wound Therapy (NPWT) is supported by a wealth of evidence and has been widely adopted to manage a broad range of wound indications. This case series demonstrates the safe and effective use of single use NPWT (sNPWT) in the management of non-healing surgical wounds, alongside optimised wound care strategies and interventions. Aim To present how adopting sNPWT to manage non-healing surgical wounds, alongside a holistic approach to patient care can improve the clinical outcomes. Management and outcomes Five case studies are presented from hospital and community healthcare settings. All patients had non-healing surgical wounds, and all received treatment with sNPWT; length of therapy days varied between 19 and 91 days. Following treatment with NPWT, three wounds healed, one reached near full approximation of wound edges, and one was managed palliatively with marked improvement in quality-of-life outcomes. Conclusion In all five cases, the non-healing surgical wounds, either healed or improved substantially when treated with an evidence-based approach incorporating sNPWT alongside optimised wound care strategies and interventions. Implications for clinical practice Holistic patient-centered care in conjunction with sNPWT may improve outcomes for patients experiencing non-healing surgical wounds. [ABSTRACT FROM AUTHOR]

28. Social anxiety : personal narratives on journeys to recovery

Abstract

Introduction: Social anxiety is a common experience. For some it is a debilitating, chronic difficulty, which becomes problematic during childhood and can have devastating effects. Talking therapies are useful for many, particularly in reducing anxiety. However, many people never access treatment and for those that do, therapies do not tend to improve their quality of life, particularly their social worlds. Do we need to look beyond clinical recovery measures when assessing therapeutic outcomes? There has been no research that has explored personal recovery in social anxiety. This thesis seeks to understand whether people with problematic social anxiety experience personal recovery and if so, how. Methods: A participatory action research (PAR) approach was used to develop the project. People with problematic social anxiety advised on study design, data collection, analysis and dissemination of the findings. 8 narratives of living with problematic social anxiety were collected to explore how people negotiate social anxiety and what this can tell us about personal recovery. A narrative analysis was then carried out, drawing upon Frank’s (1995, 2012) dialogical narratives analysis of illness stories and Adame and Hornstein’s (2006) typology of emotional distress narratives. Findings: The participants’ stories of living with problematic social anxiety highlighted the variety of ways that people make sense of this difficulty. The types of stories told were reminiscent of Frank’s (1995, 2010) illness narratives, as people told stories of restitution, chaos and quest. People drew upon traditional, counter and alternative mental health narratives to negotiate social anxiety, reminding us of the multiple ways people can find to overcome emotional distress. Discussion: The PAR study showed how people struggling with a mental health difficulty can be at the centre of research which strives to better understand their struggles and improve talking therapies. The study reminds us that the “social” aspects of social anxiety need to be better acknowledged within therapies so that we do not only focus on reducing anxiety but help people improve their relationships and quality of life.

Abstract

BACKGROUND: Previous studies, performed between 2009-2019, in the Netherlands observed an until now still unexplained increased risk for pneumonia among residents living close to goat farms. Since data were collected in the provinces Noord-Brabant and Limburg (NB-L), an area with relatively high air pollution levels and proximity to large industrial areas in Europe, the question remains whether the results are generalizable to other regions. In this study, a different region, covering the provinces Utrecht, Gelderland, and Overijssel (UGO) with a similar density of goat farms, was included to assess whether the association between goat farm proximity and pneumonia is consistently observed across the Netherlands.METHODS: Data for this study were derived from the Electronic Health Records (EHR) of 21 rural general practices (GPs) in UGO, for 2014-2017. Multi-level analyses were used to compare annual pneumonia prevalence between UGO and data derived from rural reference practices ('control area'). Random-effects meta-analysis (per GP practice) and kernel analyses were performed to study associations of pneumonia with the distance between goat farms and patients' home addresses.RESULTS: GP diagnoses of pneumonia occurred 40% more often in UGO compared to the control area. Meta-analysis showed an association at a distance of less than 500m (~70% more pneumonia compared to >500m) and 1000m (~20% more pneumonia compared to >1000m). The kernel-analysis for three of the four individual years showed an increased risk up to a distance of one or two kilometers (2-36% more pneumonia; 10-50 avoidable cases per 100,000 inhabitants per year).CONCLUSIONS: The positive association between living in the proximity of goat farms and pneumonia in UGO is similar to the previously found association in NB-L. Therefore, we concluded that the observed associations are relevant for regions with goat farms in the entire country.

Abstract

Objective: Virtual care (VC) is an accepted modality of care delivery, and shared decision‐making (SDM) benefits patients with rheumatologic and chronic conditions (RCCs). Unfortunately, research suggests reduced quality of SDM during VC. This study explores the benefits and shortcomings of SDM regarding RCCs during VC with suggestions for optimally using VC during SDM. Methods: Following Stiggelbout's framework for SDM, we conducted focus groups of patients with RCCs and providers to understand their experiences with SDM during VC, probing for facilitating and challenging factors. We conducted content analysis of the transcripts, defining themes, and inductively reasoned to identify relationships among themes. We summarized the facilitators, barriers, and opportunities for improving SDM during VC that participants proposed. Results: Virtual SDM shares several similarities with in‐person practice, as both draw upon trusting patient‐provider relationships, following the same general steps, and relying on effective communication. VC presents solutions for known barriers to in‐person SDM, expanding time for making decisions and access to care. Technology and virtual health systems introduce new barriers to SDM, and participants list opportunities for overcoming these concerns. Conclusion: VC is a tool that can enhance and even support superior SDM compared with in‐person visits when implemented successfully, a condition requiring the development of nuanced skills to correctly identify when and how to best use VC for SDM as well as technology and health care structures that integrate SDM into VC. Abstract

Atomic oxygen erosion of polymers in low Earth orbit (LEO) poses a serious threat to spacecraft performance and durability. Forty thin film polymer and pyrolytic graphite samples, collectively called the PEACE (Polymer Erosion and Contamination Experiment) Polymers, were exposed to the LEO space environment on the exterior of the ISS for nearly four years as part of the Materials International Space Station Experiment 1 & 2 (MISSE 1 & 2) mission. The purpose of the MISSE 2 PEACE Polymers experiment was to determine the atomic oxygen (AO) erosion yield (E(sub y), volume loss per incident oxygen atom) of a wide variety of polymers exposed to the LEO space environment. The Ey values were determined based on mass loss measurements. Because many polymeric materials are hygroscopic, the pre-flight and post-flight mass measurements were obtained using dehydrated samples. To maximize the accuracy of the mass measurements, obtaining dehydration data for each of the polymers was desired to ensure that the samples were fully dehydrated before weighing. A comparison of dehydration and rehydration data showed that rehydration data mirrors dehydration data, and is easier and more reliable to obtain. Tests were also conducted to see if multiple samples could be dehydrated and weighed sequentially. Rehydration curves of 43 polymers and pyrolytic graphite were obtained. This information was used to determine the best pre-flight, and post-flight, mass measurement procedures for the MISSE 2 PEACE Polymers experiment, and for subsequent NASA Glenn Research Center MISSE polymer flight experiments.
