Abstract

The deposition process of the IrMn$_3$/Co$_{70}$Fe$_{30}$ bilayer of antiferromagnet/ferromagnet (AF/FM) type was modified by introducing a nitrogen additive in argon plasma during the magnetron sputtering of the Co$_{70}$Fe$_{30}$ layer. This slight modification significantly enhanced the exchange bias energy of the AF/FM bilayer for an IrMn layer thickness ranging from 20{\AA} to 50{\AA} and reduced the coercivity of the FM layer. Calculations indicate that the boost in exchange bias energy and the reduction in coercivity can be attributed to the increased anisotropy energy of the antiferromagnet, resulting in more effective pinning of the ferromagnet by antiferromagnetic grains. The increase in anisotropy is caused by the diffusion of nitrogen from the FM into the AF layer, as established by X-ray diffraction, Neutron Reflectometry, and X-ray Magnetic Circular Dichroism. Our research allows to improve magnetic characteristics of exchange-coupled FM/AF structures through minor modifications in the sputtering process and/or save up to 20% of the costly IrMn$_3$ target by reducing the thickness of the AF layer.

2. Comparing Teaching with Tactile Guidance with Video Modeling with Self-Evaluative Video Feedback to Train Medical Skills

Abstract

Modern medical training consists largely of lecture-based instruction and in vivo or video modeling of specific skills. Other instructional methods, such as teaching with acoustical guidance (TAGteach), have rarely been evaluated. In this study, we compared teaching with tactile guidance, or tactile TAGteach in which a vibratory stimulus is delivered to indicate a correct response, with video modeling and self-evaluative video feedback to teach four participants two medical skills: simple interrupted suture and endotracheal intubation. The results showed that both instructional methods improved performance. However, three participants met the mastery criterion in the tactile TAGteach condition first, although this instructional method required more time to train the skills. We discuss the implications of these findings for training skills to medical practitioners.

Abstract

BACKGROUND: Poor sleep quality is associated with higher mortality and lower quality of life in patients with chronic kidney disease-associated pruritus (CKD-aP). Difelikefalin reduces itch in patients with CKD-aP undergoing haemodialysis (HD). This post hoc analysis of the Phase 3 difelikefalin studies (Study 3105 and the pooled dataset from KALM-1 and KALM-2) evaluated whether itch reduction in individuals with CKD-aP improved sleep quality. METHODS: Itch intensity was assessed in patients undergoing HD who had moderate-to-severe CKD-aP treated with intravenous difelikefalin (0.5 µg/kg, three times weekly) (N = 222, Study 3105; N = 426, KALM-1 and -2) or placebo (N = 425, KALM-1 and -2) for 12 weeks, using the Worst Itch Intensity Numerical Rating Scale (WI-NRS). Sleep quality was assessed using the sleep disability question of the 5-D Itch Scale (5-D SDQ) in all studies and, in Study 3105, with the Sleep Quality Numeric Rating Scale (SQ-NRS). Abstract

CRISPR-Cas are adaptive immune systems in bacteria and archaea that utilize CRISPR RNA-guided surveillance complexes to target complementary RNA or DNA for destruction1-5. Target RNA cleavage at regular intervals is characteristic of type III effector complexes6-8. Here, we determine the structures of the Synechocystis type III-Dv complex, an apparent evolutionary intermediate from multi-protein to single-protein type III effectors9,10, in pre- and post-cleavage states. The structures show how multi-subunit fusion proteins in the effector are tethered together in an unusual arrangement to assemble into an active and programmable RNA endonuclease and how the effector utilizes a distinct mechanism for target RNA seeding from other type III effectors. Using structural, biochemical, and quantum/classical molecular dynamics simulation, we study the structure and dynamics of the three catalytic sites, where a 2-OH of the ribose on the target RNA acts as a nucleophile for in line self-cleavage of the upstream scissile phosphate. Strikingly, the arrangement at the catalytic residues of most type III complexes resembles the active site of ribozymes, including the hammerhead, pistol, and Varkud satellite ribozymes. Our work provides detailed molecular insight into the mechanisms of RNA targeting and cleavage by an important intermediate in the evolution of type III effector complexes.

10. Blood Markers Show Neural Consequences of LongCOVID-19.

Abstract

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) persists throughout the world with over 65 million registered cases of survivors with post-COVID-19 sequelae, also known as LongCOVID-19 (LongC). LongC survivors exhibit various symptoms that span multiple organ systems, including the nervous system. To search for neurological markers of LongC, we investigated the soluble biomolecules present in the plasma and the proteins associated with plasma neuronal-enriched extracellular vesicles (nEVs) in 33 LongC patients with neurological impairment (nLongC), 12 COVID-19 survivors without any LongC symptoms (Cov), and 28 pre-COVID-19 healthy controls (HC). COVID-19 positive participants were infected between 2020 and 2022, not hospitalized, and were vaccinated or unvaccinated before infection. IL-1β was significantly increased in both nLongC and Cov and IL-8 was elevated in only nLongC. Both brain-derived neurotrophic factor and cortisol were significantly elevated in nLongC and Cov compared to HC. nEVs from people with nLongC had significantly elevated protein markers of neuronal dysfunction, including amyloid beta 42, pTau181 and TDP-43. This study shows chronic peripheral inflammation with increased stress after COVID-19 infection. Additionally, differentially expressed nEV neurodegenerative proteins were identified in people recovering from COVID-19 regardless of persistent symptoms.

11. Clinicians’ perspectives of immersive tools in clinical mental health settings: a systematic scoping review

Abstract

Abstract Background Virtual Reality in mental health treatment has potential to address a wide spectrum of psychological and neurocognitive disorders. Despite the proven benefits, integration into clinical practice faces significant challenges. There is a critical need for research into clinicians’ perceptions of virtual reality due to the gap between rapid technological advancements and their adoption in mental health services. Method A scoping review was conducted to comprehensively understand clinicians’ perspectives on the application of immersive virtual reality technologies within mental health settings. 4 data bases were searched, from inception, with the search areas of clinicians’, technology, perspectives and mental health. The scoping review followed the PRISMA-ScR checklist. All results were thematically analysed to identify and categorise themes with a focus on qualitative analyses of clinicians’ experiences and perceptions of VR applications in therapeutic contexts. Results 17 articles were selected, encompassing a range of mental health settings. The findings indicate that the integration of VR in clinical environments is heavily influenced by clinicians’ knowledge and experience, with unfamiliarity often leading to scepticism. Positive attitudes towards VR, bolstered by direct experience and training, were found to drive acceptance, as clinicians’ acknowledged its potential to complement traditional therapies. However, there are still gaps in understanding VR’s therapeutic applications, particularly concerning its impact on human interaction and its suitability for specific patient groups. Balancing VR’s clinical benefits with ethical and safety concerns is crucial, especially when working with vulnerable populations. Furthermore, structural and administrative support is essential to overcoming the financial and logistical challenges of VR implementation, ensuring its safe and effective integration into mental health care. Conclusion While VR holds significant potential for enhancing mental health care, its successful integration into clinical practice necessitates addressing existing gaps in knowledge, training, and structural support. By carefully balancing its clinical benefits with ethical, practical, and safety considerations, VR can be effectively utilised as a valuable tool in mental health treatment, providing innovative solutions while ensuring that patient care remains paramount.

12. Virtual Exchange in Teacher Preparation: Identifying and Disrupting Dangerous Narratives about the Middle East and North Africa Region

Abstract

This study investigated the impact of virtual exchange on disrupting dangerous narratives about the Middle East and North Africa (MENA) region among pre-service elementary teachers in the Midwestern region of the United States. The virtual exchange program provided direct interaction with teacher candidates in Morocco. Analysis of pre- and post-exchange memos revealed a significant shift from negative narratives to positive, generalized perceptions of the MENA region. While the shift towards positive generalizations represents progress, it also emphasizes the need for nuanced understanding and ongoing engagement. After participating in the exchange, pre-service teachers also communicated a strong desire to teach about the world and its people. This study highlights the possibilities of virtual exchange as a tool for preparing future educators who have been influenced by post-9/11 educational and media narratives. It advocates for the integration of virtual exchange as a mainstream approach to foster teaching for global understanding among teacher candidates.

13. 'You Have to Work Ten Times Harder': First-in-Family Students, Employability and Capital Development

Abstract

Since the 1990s, UK government policy has sought to increase access to higher education, with a plan to improve social mobility. However, enhancing the employability prospects for all has proven difficult to achieve through widening participation alone. This research explores this paradox, via the experiences of first-in-family undergraduates as they prepare to enter the UK graduate labour market. The concept of capital development is applied to understand the structural disadvantages experienced by students who lack a familial university background. Twenty-five interviews with first-in-family students at a Russell Group university in the UK were analysed thematically, with the application of the Graduate Capital Model. Findings reveal the high value attached by these students to human capital and the barriers they face in accruing social and cultural capital. This research illustrates how students who lack such capital face numerous obstacles in developing the strong career identities necessary to transition to graduate employment. Whilst the Graduate Capital Model gives valuable insight into the experiences of these students, the role of economic capital in shaping prospects is also recognised. Recommendations are proposed as to how universities, careers services and employers might act in support of first-in-family students' graduate transitions.

14. Children's Participation in Child Welfare Decision Making: Recognising Dichotomies, Conceptualising Critically Informed Solutions.

Abstract

This article suggests that there exists a dichotomy within social work related to listening to the voice of the child, contributing to a diminution of social works practical, ethical and legal obligation to enact Article12 of the United Nations Convention on the Rights of the Child (UNCRC). The article gives an in-depth exploration of this phenomenon and offers insights as to why this dichotomy exists, arguing that if social work is to realise its obligation to listen to the voice of the child, it must find solutions that accommodate the professions responsibilities to listen to children. One such solution is offered within the paper that may assist social work resolve its problematic relationship with Article12. An adapted model of Honneth's recognition theory is offered as a unique theoretical and practical method to help social work accommodate its duty to safeguard children whilst simultaneously listening to their wishes and feelings at individual, State and societal level. The theory can be used to critique current practice, offering social workers a template to build participatory practices, based on the equal recognition of children's citizenship and agency. Honneth's theory can be used to evaluate how well new participatory practices satisfy the criteria of love, rights and solidarity. By accepting Honneth's theory of recognition, social work can begin to make progress addressing the practice dichotomy concerning participation aspiration and reality. The article offers an innovative and unique contribution to the literature on children's participation by adapting Honneth's recognition theory and applying it exclusively to the participation rights of children known to social services, offering novel ways social workers can enable this group of children to lay claim to their right to participate.

15. Experiences of Adoption Disruption: Parents' Perspectives

Abstract

Context: Adoption can provide a lifetime of benefits for children. Some adoptions however, experience severe challenges resulting in disruption, with children leaving home prematurely. Method: This qualitative study in Northern Ireland used interviews with parents from thirteen families whose adoptions had disrupted, to explore their perspective on the experience. Findings: Key findings focused on issues relating to firstly, the adopted child; secondly, adoption services; and finally, the parents and other family members. The impact of early adverse experiences on the children (developmental trauma) played out through behaviours, often violent, that their parents found extremely challenging. These increased as the children aged and had serious effects on family life. The adoptive parents thought they could have been better prepared through the adoption process to face challenging behaviours and more appropriately supported to prevent disruption. When their adopted child was admitted to state care, the parents typically felt initial relief but also guilt and loss. After the child had left home, they generally wished for more involvement with him or her despite the difficulties experienced. Discussion: This study confirmed previous findings about the extent of trauma experienced by some adoptive children, and the challenges that this may present to the adoptive parents. It highlighted how the manifestation of the trauma experienced by the child may lead to adoptive parents themselves experiencing primary or secondary trauma or compassion fatigue (defined in the Discussion section below) or a combination of all three. The preparation of adoptive parents should include greater awareness of possible challenges, and how to cope with these. The development of trauma-informed approaches to practice and service delivery is required to support families with adopted children more effectively. Co-production models for service development may assist in addressing the types of issues identified in post-adoption support services.

Abstract The contribution of chronic kidney disease (CKD) towards the risk of developing cardiovascular disease (CVD) is magnified with co-existing type 1 or type 2 diabetes. Lipids are a modifiable risk factor and good lipid management offers improved outcomes for people with diabetic kidney disease (DKD). The primary purpose of this guideline, written by the Association of British Clinical Diabetologists (ABCD) and UK Kidney Association (UKKA) working group, is to provide practical recommendations on lipid management for members of the multidisciplinary team involved in the care of adults with DKD.

Abstract

The evolving development landscape of biotherapeutics and their growing complexity from simple antibodies into bi- and multi-specific molecules necessitates sophisticated discovery and engineering platforms. This review focuses on mammalian display technology as a potential solution to the pressing challenges in biotherapeutic development. We provide a comparative analysis with established methodologies, highlighting key aspects of mammalian display technology, including genetic engineering, construction of display libraries, and its pivotal role in hit selection and/or developability engineering. The review delves into the mechanisms underpinning developability-driven selection via mammalian display and their broader implications. Applications beyond antibody discovery are also explored, alongside advancements towards function-first screening technologies, precision genome engineering and AI/ML-enhanced libraries, situating them in the context of mammalian display. Overall, the review provides a comprehensive overview of the current mammalian display technology landscape, underscores the expansive potential of the technology for biotherapeutic development, addresses the critical challenges for the full realisation of this potential, and examines advances in related disciplines that might impact the future application of mammalian display technologies.

Abstract

SUMMARY: A variety of milling conditions can impact the physical quality (pellet durability index (PDI) and feed form (FF) composition) of broiler diets. Likewise, FF composition has been observed to affect broiler prehension and feed consumption patterns. Numerical differences in nutrient composition between pellets and fines have also been reported. Therefore, feeding diets with poor physical quality may lead to variations in nutrient intake among broilers. Reductions in the physical quality of feed could be partially related to frictional die wear. However, no research has indirectly evaluated the effects of die wear (i.e., production time) on the physical quality of finished feed from commercial feed mills. Therefore, the objective of this experiment was to quantify the effects of production time, die condition (new or reconditioned), and feed mill on PDI and FF composition of finisher 1 broiler diets from 4 commercial feed mills over a 12-wk production period. Production time (P > 0.05) did not affect dietary PDI and FF composition during each production period. Die condition and feed mill interacted (P < 0.05) to affect FF composition at one feed mill with the new die producing a higher and lower concentration of pellets and crumbles, respectively, than the reconditioned die. However, feed mill (P < 0.05) affected all measures. Thus, the physical quality of broiler diets is not affected by production time but die condition may influence FF composition. However, this effect was feed mill dependent. Overall, feed mill was most responsible for the physical quality of broiler diets.

Abstract

Abstract CRISPR-Cas are adaptive immune systems in bacteria and archaea that utilize CRISPR RNA-guided surveillance complexes to target complementary RNA or DNA for destruction1–5. Target RNA cleavage at regular intervals is characteristic of type III effector complexes6–8. Here, we determine the structures of the Synechocystis type III-Dv complex, an apparent evolutionary intermediate from multi-protein to single-protein type III effectors9,10, in pre- and post-cleavage states. The structures show how multi-subunit fusion proteins in the effector are tethered together in an unusual arrangement to assemble into an active and programmable RNA endonuclease and how the effector utilizes a distinct mechanism for target RNA seeding from other type III effectors. Using structural, biochemical, and quantum/classical molecular dynamics simulation, we study the structure and dynamics of the three catalytic sites, where a 2′-OH of the ribose on the target RNA acts as a nucleophile for in line self-cleavage of the upstream scissile phosphate. Strikingly, the arrangement at the catalytic residues of most type III complexes resembles the active site of ribozymes, including the hammerhead, pistol, and Varkud satellite ribozymes. Our work provides detailed molecular insight into the mechanisms of RNA targeting and cleavage by an important intermediate in the evolution of type III effector complexes.

Abstract

The gut microbiota-brain axis has been associated with the pathogenesis of numerous disorders, but the mechanism(s) underlying these links are generally poorly understood. Accumulating evidence indicates the involvement of gut microbe-derived metabolites. Circulating levels of the gut microbe/host co-metabolite p-cresol sulfate (pCS) correlate with cerebrovascular event risk in individuals with chronic kidney disease (CKD), but whether this relationship is mechanistic is unclear. We hypothesized that pCS would impair the function of the blood–brain barrier (BBB), the primary brain vasculature interface. We report that pCS exposure impairs BBB integrity in human cells in vitro and both acutely (≤6 hours) and chronically (28 days) in mice, enhancing tracer extravasation, disrupting barrier-regulating tight junction components and ultimately exerting a suppressive effect upon whole-brain transcriptomic activity. In vitro and in vivo mechanistic studies showed that pCS activated epidermal growth factor receptor (EGFR) signaling, sequentially activating the intracellular signaling proteins annexin A1 and STAT3 to induce mobilization of matrix metalloproteinase MMP-2/9 and disruption to the integrity of the BBB. This effect was confirmed as specific to the EGFR through the use of both pharmacological and RNA interference approaches. Confirming the translational relevance of this work, exposure of the cerebromicrovascular endothelia to serum from hemodialysis patients in vitro led to a significant increase in paracellular permeability, with the magnitude of permeabilization closely correlating with serum pCS, but not most other uremic toxin, content. Notably, this damaging effect of hemodialysis patient serum was prevented by pharmacological blockade of the EGFR. Our results define a pathway linking the co-metabolite pCS with BBB damage and suggest that targeting the EGFR may mitigate against cerebrovascular damage in CKD. This work further provides mechanistic evidence indicating the role of gut microbe-derived metabolites in human disease.

Abstract

Summary: Background: Bacille Calmette-Guérin (BCG) vaccination has off-target (non-specific) effects that are associated with protection against unrelated infections and decreased all-cause mortality in infants. We aimed to determine whether BCG vaccination prevents febrile and respiratory infections in adults. Methods: This randomised controlled phase 3 trial was done in 36 healthcare centres in Australia, Brazil, the Netherlands, Spain, and the United Kingdom. Healthcare workers were randomised to receive BCG-Denmark (single 0.1 ml intradermal injection) or no BCG in a 1:1 ratio using a web-based procedure, stratified by stage, site, age, and presence of co-morbidity. The difference in occurrence of febrile or respiratory illness were measured over 12 months (prespecified secondary outcome) using the intention-to-treat (ITT) population. This trial is registered with ClinicalTrials.gov, NCT04327206. Findings: Between March 30, 2020, and April 1, 2021, 6828 healthcare workers were randomised to BCG-Denmark (n = 3417) or control (n = 3411; no intervention or placebo) groups. The 12-month adjusted estimated risk of ≥1 episode of febrile or respiratory illness was 66.8% in the BCG group (95% CI 65.3%–68.2%), compared with 63.4% in the control group (95% CI 61.8%–65.0%), a difference of +3.4 percentage points (95% CI +1.3% to +5.5%; p 0.002). The adjusted estimated risk of a severe episode (defined as being incapacitated for ≥3 consecutive days or hospitalised) was 19.4% in the BCG group (95% CI 18.0%–20.7%), compared with 18.8% in the control group (95% CI 17.4%–20.2%) a difference of +0.6 percentage points (95% CI −1.3% to +2.5%; p 0.6). Both groups had a similar number of episodes of illness, pneumonia, and hospitalisation. There were three deaths, all in the control group. There were no safety concerns following BCG vaccination. Interpretation: In contrast to the beneficial off-target effects reported following neonatal BCG in infants, a small increased risk of symptomatic febrile or respiratory illness was observed in the 12 months following BCG vaccination in adults. There was no evidence of a difference in the risk of severe disease. Abstract

Abstract Disease outbreaks can drastically disturb the environment of surviving animals, but the behavioural, ecological, and epidemiological consequences of disease-driven disturbance are poorly understood. Here, we show that an outbreak of High Pathogenicity Avian Influenza Virus (HPAIV) coincided with unprecedented short-term behavioural changes in Northern gannets (Morus bassanus). Breeding gannets show characteristically strong fidelity to their nest sites and foraging areas (2015–2019; n = 120), but during the 2022 HPAIV outbreak, GPS-tagged gannets instigated long-distance movements beyond well-documented previous ranges and the first ever recorded visits of GPS-tagged adults to other gannet breeding colonies. Our findings suggest that the HPAIV outbreak triggered changes in space use patterns of exposed individuals that amplified the epidemiological connectivity among colonies and may generate super-spreader events that accelerate disease transmission across the metapopulation. Such self-propagating transmission from and towards high density animal aggregations may explain the unexpectedly rapid pan-European spread of HPAIV in the gannet.

Abstract

Abstract Peripheral T cell lymphomas are typically aggressive with a poor prognosis. Unlike other hematologic malignancies, the lack of target antigens to discriminate healthy from malignant cells limits the efficacy of immunotherapeutic approaches. The T cell receptor expresses one of two highly homologous chains [T cell receptor β-chain constant (TRBC) domains 1 and 2] in a mutually exclusive manner, making it a promising target. Here we demonstrate specificity redirection by rational design using structure-guided computational biology to generate a TRBC2-specific antibody (KFN), complementing the antibody previously described by our laboratory with unique TRBC1 specificity (Jovi-1) in targeting broader spectrum of T cell malignancies clonally expressing either of the two chains. This permits generation of paired reagents (chimeric antigen receptor-T cells) specific for TRBC1 and TRBC2, with preclinical evidence to support their efficacy in T cell malignancies.

Abstract

Abstract This trial assessed the feasibility and acceptability of Kidney BEAM, a physical activity and emotional well-being self-management digital health intervention (DHI) for people with chronic kidney disease (CKD), which offers live and on-demand physical activity sessions, educational blogs and videos, and peer support. In this mixed-methods, multicentre randomised waitlist-controlled internal pilot, adults with established CKD were recruited from five NHS hospitals and randomised 1:1 to Kidney BEAM or waitlist control. Feasibility outcomes were based upon a priori progression criteria. Acceptability was primarily explored via individual semi-structured interviews (n = 15). Of 763 individuals screened, n = 519 (68%, 95% CI 65 to 71%) were eligible. Of those eligible, n = 303 (58%, 95% CI 54–63%) did not respond to an invitation to participate by the end of the pilot period. Of the 216 responders, 50 (23%, 95% CI 18–29%) consented. Of the 42 randomised, n = 22 (10 (45%) male; 49 ± 16 years; 14 (64%) White British) were allocated to Kidney BEAM and n = 20 (12 (55%) male; 56 ± 11 years; 15 (68%) White British) to the waitlist control group. Overall, n = 15 (30%, 95% CI 18–45%) withdrew during the pilot phase. Participants completed a median of 14 (IQR 5–21) sessions. At baseline, 90–100% of outcome data (patient reported outcome measures and a remotely conducted physical function test) were completed and 62–83% completed at 12 weeks follow-up. Interview data revealed that remote trial procedures were acceptable. Participants’ reported that Kidney BEAM increased their opportunity and motivation to be physically active, however, lack of time remained an ongoing barrier to engagement with the DHI. An randomised controlled trial of Kidney BEAM is feasible and acceptable, with adaptations to increase recruitment, retention and engagement. Trial registration NCT04872933. Abstract

Abstract Southern elephant seals (SES) experience a ‘catastrophic molt’, a costly event characterized by the renewal of both hair and epidermis that requires high peripheral vascular circulation. Molting animals are therefore constrained by high metabolic heat loss and are thought to fast and remain on land. To examine the ability of individuals to balance the energetic constraints of molting on land we investigate the stomach temperature and movement patterns of molting female SES. We find that 79% of females swam and 61% ingested water or prey items, despite the cost of cold-water exposure while molting. This behavior was related to periods of warm and low wind conditions, and females that dived and ingested more often, lost less body mass. We conclude that the paradigm of fasting during the molt in this species, and the fitness consequences of this behavior should be reconsidered, especially in the context of a changing climate.

36. The Impact of Mental Computation on Children's Mathematical Communication, Problem Solving, Reasoning, and Algebraic Thinking

Abstract

Moving from arithmetic to algebraic thinking at early grades is foundational in the study of number patterns and number relationships. This qualitative study investigates mental computational activity in a third grade classroom's and its relationship to algebraic thinking and reasoning. The data sources include classroom observations, field notes, students' verbal and written communications, and interviews. The study occurs in two phases; phase one includes establishing roles, rules, and expectations regarding how to talk about mathematical ideas; and phase two involves creating a classroom community that encourages participation, active listening, students' voices, and multiple perspectives. The findings of the study suggest that students' verbal communication enhances their problem-solving, reasoning, and communication. In addition, the findings suggest that creating learning opportunities for all students to do sophisticated mathematics requires competent and caring teachers who know their students' backgrounds, who understand the subject, and have strong pedagogical knowledge.

37. 'It Changes Me from Nothing to Something': Identifying Educative-Psychic Violence in a Public Diplomacy Program for Nonelite Youths

Abstract

Since 2004, the English Access Microscholarship Program, a U.S. public diplomacy initiative, has impacted at least 150,000 nonelite youths. U.S. Department of State employees created the program in response to suicide bombings committed by Moroccan youths at international sites in Casablanca. The program later expanded throughout the Middle East and then across the world, eventually operating in more than 85 countries for students aged 13-20. In this paper, I examine images promoted by the program associated with the mission for students to develop an "appreciation for U.S. culture and democratic values through cultural enhancement activities" using critical concepts such as "educative-psychic violence" and Ngugi wa Thiong'o's description of education as a "cultural bomb." Troubling content promoted by the program features students depicting themselves as lacking dignity and worth paired with portrayals of gaining dignity and worth through their participation in the program. Additionally, the program's "cultural enhancement activities" intended to promote "appreciation for U.S. culture and democratic values" often occur in communities deeply impacted by the U.S.-led War on Terror and amid dramatic economic and military power differentials. Finally, my analysis encourages academic engagement with policy makers on the use of education within public diplomacy initiatives.

Abstract

Background Pre-existing cardiovascular disease (CVD) or cardiovascular risk factors have been associated with an increased risk of complications following hospitalisation with COVID-19, but their impact on the rate of recovery following discharge is not known.Objectives To determine whether the rate of patient-perceived recovery following hospitalisation with COVID-19 was affected by the presence of CVD or cardiovascular risk factors.Methods In a multicentre prospective cohort study, patients were recruited following discharge from the hospital with COVID-19 undertaking two comprehensive assessments at 5 months and 12 months. Patients were stratified by the presence of either CVD or cardiovascular risk factors prior to hospitalisation with COVID-19 and compared with controls with neither. Full recovery was determined by the response to a patient-perceived evaluation of full recovery from COVID-19 in the context of physical, physiological and cognitive determinants of health.Results From a total population of 2545 patients (38.8% women), 472 (18.5%) and 1355 (53.2%) had CVD or cardiovascular risk factors, respectively. Compared with controls (n=718), patients with CVD and cardiovascular risk factors were older and more likely to have had severe COVID-19. Full recovery was significantly lower at 12 months in patients with CVD (adjusted OR (aOR) 0.62, 95% CI 0.43 to 0.89) and cardiovascular risk factors (aOR 0.66, 95% CI 0.50 to 0.86).Conclusion Patients with CVD or cardiovascular risk factors had a delayed recovery at 12 months following hospitalisation with COVID-19. Targeted interventions to reduce the impact of COVID-19 in patients with cardiovascular disease remain an unmet need.Trail registration number ISRCTN10980107.

Abstract

Summary: Background: The COVID-19 pandemic led to the rapid development and deployment of several highly effective vaccines against SARS-CoV-2. Recent studies suggest that these vaccines may also have off-target effects on the immune system. We sought to determine and compare the off-target effects of the adenovirus vector ChAdOx1-S (Oxford-AstraZeneca) and modified mRNA BNT162b2 (Pfizer-BioNTech) vaccines on immune responses to unrelated pathogens. Methods: Prospective sub-study within the BRACE trial. Blood samples were collected from 284 healthcare workers before and 28 days after ChAdOx1-S or BNT162b2 vaccination. SARS-CoV-2-specific antibodies were measured using ELISA, and whole blood cytokine responses to specific (SARS-CoV-2) and unrelated pathogen stimulation were measured by multiplex bead array. Findings: Both vaccines induced robust SARS-CoV-2 specific antibody and cytokine responses. ChAdOx1-S vaccination increased cytokine responses to heat-killed (HK) Candida albicans and HK Staphylococcus aureus and decreased cytokine responses to HK Escherichia coli and BCG. BNT162b2 vaccination decreased cytokine response to HK E. coli and had variable effects on cytokine responses to BCG and resiquimod (R848). After the second vaccine dose, BNT162b2 recipients had greater specific and off-target cytokine responses than ChAdOx1-S recipients. Interpretation: ChAdOx1-S and BNT162b2 vaccines alter cytokine responses to unrelated pathogens, indicative of potential off-target effects. The specific and off-target effects of these vaccines differ in their magnitude and breadth. The clinical relevance of these findings is uncertain and needs further study. Innovation is essential for future power systems to be safe and secure, clean and sustainable, affordable and equitable, and reliable and resilient, according to a recent National Academies report. But state regulatory reforms are needed to encourage adoption of new technologies to support evolution of the nation’s power systems.1Berkeley Lab's report, The Role of Innovation in the Electric Utility Sector, provides consumer, labor, utility, third-party provider, and clean technology consultant perspectives on this theme. To achieve state targets for clean energy and greenhouse gas emissions, some state regulatory utility commissions are exploring new approaches to spur innovation:-For utilities, regulatory and marketing flexibility, increased funding for demonstration projects, and performance-based ratemaking including multi-year rate plans-For third parties, ways to provide utility customers with innovative products and services directlyAmong the questions the report addresses:1. Abstract

Abstract Cilia are hairlike protrusions that project from the surface of eukaryotic cells and play key roles in cell signaling and motility. Ciliary motility is regulated by the conserved nexin-dynein regulatory complex (N-DRC), which links adjacent doublet microtubules and regulates and coordinates the activity of outer doublet complexes. Despite its critical role in cilia motility, the assembly and molecular basis of the regulatory mechanism are poorly understood. Here, using cryo-electron microscopy in conjunction with biochemical cross-linking and integrative modeling, we localize 12 DRC subunits in the N-DRC structure of Tetrahymena thermophila. We also find that the CCDC96/113 complex is in close contact with the DRC9/10 in the linker region. In addition, we reveal that the N-DRC is associated with a network of coiled-coil proteins that most likely mediates N-DRC regulatory activity.

49. Time to Dust Off Your Passport: A Roadmap to Enhance Your Path to the EM Workforce

Abstract

Learning Objectives: The goal of this project was to promote early professional and clinical development of residents by creating a well-organized, visually-appealing roadmap of co-curricular requirements to augment resident training and provide a well-rounded residency experience.Background: With the rapid growth of EM as a specialty and an increasing number of accredited residency programs, EM has become increasingly more competitive. EM physician workforce projections are daunting, particularly to current and future trainees. The prospect of securing a desirable and fulfilling job after residency is becoming exceedingly more uncertain, even with fellowship training. While there are evolving larger-scale initiatives to address this, an onus falls on individual trainees and their residency programs to improve resident skillset and marketability.Curricular Design: Core faculty from various EM subspecialties proposed unique learning experiences within their respective areas of expertise to enhance resident training. Determined by educational benefit, feasibility, and potential impact, the co-curricular requirements were compiled into the Resident Citizenship Passport (Image 1), a visual roadmap organized by subspecialty and class year. This provides EM residents with opportunities to augment their skills as well-rounded EM physicians and educators. It includes elements of experiential learning through concrete learning experiences and encourages learners to reflect on their experiences to create new educational content. Social cognitive learning methods facilitate close interaction with faculty members and staff, leading to early mentorship and timely professional development. A slight modification of requirements was made for senior residents as this was implemented amidst their training. Progress is actively tracked through a google sheet, which residents can continually view.Impact/Effectiveness: While still in a pilot phase, this innovation has been easily implemented and well-received by the residents, as it lays out the requirements in a centralized, organized, and visually-appealing graphic. This has also unexpectedly incentivized residents to fulfill requirements by inspiring an element of competition amongst one another.

50. Resident Clinical Exposure Variability at Graduation

Abstract

Learning Objectives: To quantify individual differences in resident clinical exposure during training at a 3-year academic emergency medicine residency.Background: Experiential learning theory suggests that clinical exposures during residency are critical to developing expertise. Research in other specialties has shown significant individual differences in resident clinical exposures during training, but this has not been recently evaluated in emergency medicine (EM).Objective: To quantify individual differences in resident clinical exposure during training at a 3-year academic emergency medicine residency.Methods: We performed a retrospective review of electronic health records from 2013-2021 at our main clinical site (of four) to quantify the number and type of clinical encounters seen by each resident. Visits were attributed to the first assigned resident. We included data from residents who completed all three years of residency consecutively. We categorized primary patient chief complaints according to the 20 domains of the ABEM Model of Clinical Practice following a published consensus method with EM faculty. We calculated and reported descriptive statistics.Results: We collected data from 70 residents. Means and ranges of exposures in the top 10 most commonly identified domains are displayed in Figure 1.Conclusions: We found variability in resident clinical exposures at our primary training site. Residencies may benefit from examining resident clinical exposures to identify opportunities for individual resident improvements.
