There is little information on this phenomenon among patients with relapsing-remitting multiple sclerosis (RRMS), one of the most common causes of disability and loss of autonomy in young adults. The aim of this study was to assess state hopelessness and its associated factors in early-stage RRMS.Methods: A multicenter, non-interventional study was conducted. Adult patients with a diagnosis of RRMS, a disease duration ≤ 3 years, and an Expanded Disability Status Scale (EDSS) score of 0– 5.5 were included. The State-Trait Hopelessness Scale (STHS) was used to measure patients´ hopelessness. A battery of patient-reported and clinician-rated measurements was used to assess clinical status. A multivariate logistic regression analysis was conducted to determine the association between patients’ characteristics and state hopelessness.Results: A total of 189 patients were included. Mean age (standard deviation-SD) was 36.1 (9.4) years and 71.4% were female. Median disease duration (interquartile range-IQR) was 1.4 (0.7, 2.1) years. Symptom severity and disability were low with a median EDSS (IQR) score of 1.0 (0, 2.0). A proportion of 65.6% (n=124) of patients reported moderate-to-severe hopelessness. Hopelessness was associated with older age (p=0.035), depressive symptoms (p=< 0.001), a threatening illness perception (p=0.001), and psychological and cognitive barriers to workplace performance (p=0.029) in the multivariate analysis after adjustment for confounders.Conclusion: Hopelessness was a common phenomenon in early-stage RRMS, even in a population with low physical disability. Identifying factors associated with hopelessness may be critical for implementing preventive strategies helping patients to adapt to the new situation and cope with the disease in the long term.Keywords: relapsing-remitting multiple sclerosis, hopelessness, depressive symptoms, workplace difficulties, suicide

No significantly different clinical or functional characteristics were observed between the phenotypes. A positive correlation was observed between SI eosinophilia and both emergency visits in the last 12 months (p = 0.041; r = 0.214) and FeNO values (p = 0.000; r = 0.368). Blood eosinophilia correlated with SI eosinophilia (p = 0.001; r = 0.362) and was the best predictor of bronchial eosinophilia, followed by FeNO, and total blood IgE (area under the receiver operating characteristic curve (AUC-ROC) 72%, 65%, and 53%, respectively), although precision was only fair.Conclusion: In consultations for severe asthma, the most frequent phenotype was eosinophilic. Peripheral blood eosinophilia is a reliable marker for discriminating between different bronchial inflammatory phenotypes, is useful in enabling doctors to select a suitable biologic treatment and so prevent asthma exacerbation, and is a better predictor of bronchial eosinophilia than FeNO and IgE values.Keywords: asthma, sputum induction, phenotype, eosinophilia

8. Anelastic response of the Earth's crust underneath the Canary Islands revealed from ocean tide loading observations

Abstract

We report on the analysis of M2 ocean tide loading (OTL) kinematic GPS vertical displacement and tidal gravity measurements using 26 GPS and four gravimetric sites across the Canary Islands archipelago. In this region, the standard deviation among recent ocean tide models is lower than 0.4 cm in amplitude and 0.3° in phase, which are suitably accurate for displacement modelling. However, for gravity we need to model regional ocean tides to achieve enough accuracy in the loading calculations. Particularly, this study improves the predicted OTL gravity variations when global ocean models are replaced with the regional model CIAM2 which assimilates local tide gauge data. These small ocean tide model errors allow us to use the differences between observed and predicted OTL values to study the elastic and anelastic properties of the solid Earth around the Canary Islands. In the prediction of OTL, we first used the recent elastic STW105 and S362ANI seismic models, obtaining average observed minus predicted residuals of 1.2-1.3 mm for vertical displacement and 3 nm s-2 for gravity. After the STW105 and S362ANI models were adjusted for anelasticity, by considering a constant quality factor Q at periods ranging from 1 s to 12.42 hr, the average misfit between observations and predicted OTL values reduced to 0.7-0.8 mm for vertical displacement and to 1 nm s-2 for gravity. However, the average vertical displacement misfit is made up from site misfits less than 0.5 mm in western islands but for the easternmost islands of Lanzarote and Fuerteventura, they still reach up to nearly 2 mm at some sites, which still exceeds the uncertainty in the GPS observations. It is hypothesized that mantle upwelling underneath the Canary Islands, creating spatial variations in the elastic properties, causes the large residuals observed in the eastern islands. 9. Gravity changes at Cumbre Vieja, La Palma (Canary Islands), associated with the 2021 eruption

Abstract

The 2021 eruption at the Cumbre Vieja rift (La Palma, Canary Islands) lasted 85 days from September 19th to December 13th, becoming the longest historical eruption in this island. During this period, an elongated fissure eruption built a volcanic cone reaching 1,121 m a.s.l. Lava emerged from six major craters and up to nine minor vents with alternating activity, according to temporal variations in the eruptive process.We conducted several gravity surveys before, during and after this eruption that provided us with valuable spatial-temporal information to study this volcanic event. The obtained Bouguer anomaly map revealed a model of the shallow crustal structures before the eruption, which allowed to study the framework where the volcanic process developed. The analysis of the observed space-time gravity changes suggested a model of the magmatic feeding system, which draws the spatio-temporal path of magma ascent from the crust-mantle boundary to the surface. In addition, we also investigated the differences among the several gravity surveys to infer the possible mass changes that took place during the volcanic process. The interpretation of these gravity changes associated to the 2021 eruption at the Cumbre Vieja rift is supported by the analysis of the spatio-temporal distribution of the earthquake hypocenters preceding the eruption and the ground displacement calculated using GNSS data.

13. Re-induction With Intravenous Ustekinumab in Patients With Crohn's Disease and a Loss of Response to This Therapy

Abstract

BACKGROUND: A significant percentage of patients treated with ustekinumab may lose response. Our aim was to evaluate the short-term efficacy and safety of intravenous re-induction with ustekinumab in patients with Crohn's disease who have lost the response to the treatment. METHODS: This is a retrospective, observational, multicenter study. Treatment efficacy was measured at week 8 and 16; clinical remission was defined when the Harvey-Bradshaw Index was =4 points, and clinical response was defined as a decrease of =3 points in the index compared with the baseline. Adverse events and treatment decisions after re-induction were also collected. RESULTS: Fifty-three patients from 13 centers were included. Forty-nine percent had previously failed to respond to 2 biological treatments, and 24.5% had failed to respond to 3. The average exposure time to ustekinumab before re-induction was 17.7 ± 12.8 months. In 56.6% of patients, the administration interval had been shortened to every 4 to 6 weeks before re-induction. At week 8 and 16 after re-induction, 49.0% (n = 26) and 43.3% (n = 23), respectively, were in remission, whereas 64.1% (n = 34) and 52.8% (n = 28) had a clinical response. Patients who achieved remission at week 16 had lower C-reactive protein levels than those who did not respond (2.8 ± 1.6 vs 12.5 ± 9.5 mg/dL; P = 0.001). No serious adverse events related to re-induction were observed. CONCLUSION: Intravenous re-induction with ustekinumab is an effective and safe strategy that recovers the response in approximately half of the patients with refractory Crohn's disease who experience a loss of response. Re-induction can be attempted before switching out of the therapy class.

Abstract

Background: The impact of biologics on the risk of postoperative complications (PC) in inflammatory bowel disease (IBD) is still an ongoing debate. This lack of evidence is more relevant for ustekinumab and vedolizumab. Aims: To evaluate the impact of biologics on the risk of PC. Methods: A retrospective study was performed in 37 centres. Patients treated with biologics within 12 weeks before surgery were considered “exposed”. The impact of the exposure on the risk of 30-day PC and the risk of infections was assessed by logistic regression and propensity score-matched analysis. Results: A total of 1535 surgeries were performed on 1370 patients. Of them, 711 surgeries were conducted in the exposed cohort (584 anti-TNF, 58 vedolizumab and 69 ustekinumab). In the multivariate analysis, male gender (OR: 1.5, 95% CI: 1.2–2.0), urgent surgery (OR: 1.6, 95% CI: 1.2–2.2), laparotomy approach (OR: 1.5, 95% CI: 1.1–1.9) and severe anaemia (OR: 1.8, 95% CI: 1.3–2.6) had higher risk of PC, while academic hospitals had significantly lower risk. Exposure to biologics (either anti-TNF, vedolizumab or ustekinumab) did not increase the risk of PC (OR: 1.2, 95% CI: 0.97–1.58), although it could be a risk factor for postoperative infections (OR 1.5, 95% CI: 1.03–2.27). Conclusions: Preoperative administration of biologics does not seem to be a risk factor for overall PC, although it may be so for postoperative infections.

Abstract

Patients with multiple sclerosis (MS) suffer with age an early immunosenescence process, which influence the treatment response and increase the risk of infections. We explored whether lipid-specific oligoclonal IgM bands (LS-OCMB) associated with highly inflammatory MS modify the immunological profile induced by age in MS. This cross-sectional study included 263 MS patients who were classified according to the presence (M+, n=72) and absence (M-, n=191) of LS-OCMB. CSF cellular subsets and molecules implicated in immunosenescence were explored. In M- patients, aging induced remarkable decreases in absolute CSF counts of CD4+ and CD8+ T lymphocytes, including Th1 and Th17 cells, and of B cells, including those secreting TNF-alpha. It also increased serum anti-CMV IgG antibody titers (indicative of immunosenescence) and CSF CHI3L1 levels (related to astrocyte activation). In contrast, M+ patients showed an age-associated increase of TIM-3 (a biomarker of T cell exhaustion) and increased values of CHI3L1, independently of age. Finally, in both groups, age induced an increase in CSF levels of PD-L1 (an inductor of T cell tolerance) and activin A (part of the senescence-associated secretome and related to inflammaging). These changes were independent of the disease duration. Finally, this resulted in augmented disability. In summary, all MS patients experience with age a modest induction of T-cell tolerance and an activation of the innate immunity, resulting in increased disability. Additionally, M- patients show clear decreases in CSF lymphocyte numbers, which could increase the risk of infections. Thus, age and immunological status are important for tailoring effective therapies in MS.

Abstract

This paper deals with the first geochemical data from an unexplored sector of the Cordon de Inacaliri Volcanic Complex (Central Andes, Chile). The site is located at ∼5,150-5,200 m a.s.l., inside the Pabelloncito graben where, at about 9 km NW of the studied area, the only currently working geothermal power plant of South America, named Cerro Pabellon, occurs. Diffuse soil CO2 and soil temperature measurements were carried out to unravel the structural control on the rising fluids and estimate the total CO2 output, the heat flow rate and the heat flux, aimed at assessing a preliminary evaluation of the geothermal potential of the area. The study area is characterized by a pervasive hydrothermal mineralogical alteration, CO2 flux values of up to ∼4,400 g m-2 d-1 and soil temperatures up to the boiling point of water at that altitude. All these features are likely related to an endogenous source. Spatial distribution of both soil CO2 flux and temperature depict an ENE-striking lineament, whose intersection with the NW-striking Pabelloncito graben forms a favourable structural setting for the discharge of hydrothermal fluids. The total CO2 output emission of the studied area (∼0.0179 km2) was ∼0.53 t d-1, with an associated discharge of steam of 6.45 t d-1 (CO2/H2O ratio = 0.08). An electric capacity potential of 1.08 MWe km-2 was computed from the heat flow rate and heat flux values. Our results suggest that this part of the Pabelloncito graben is an interesting geothermal prospect and a good candidate for further exploration studies.

Abstract

The role of immunosuppression among coronavirus disease 2019 (COVID-19) patients has not been elucidated and management may be challenging. This observational study included confirmed COVID-19 patients. The primary endpoint was the development of moderate-severe acute respiratory distress syndrome (ARDS). Time to moderate-severe ARDS, the need for mechanical or noninvasive ventilation (MV/NIV), death, and a composite of death or MV/NIV were secondary endpoints. Of 138 patients included, 27 (19.6%) were immunosuppressed (IS) and 95 (68.8%) were male, with a median (IQR) age of 68 (54-78) years. A significantly lower proportion of IS patients (25.9%) compared to non-IS patients (52.3%) developed moderate-severe ARDS, in both unadjusted (0.32; 95% CI, 0.13-0.83; p = .017) and adjusted (aOR, 0.25; 95% CI, 0.08-0.80; p = .019) analyses. After stratifying by pathologies, only IS patients with autoimmune diseases remained significant (aOR 0.25; 95% CI, 0.07-0.98; p = .046). Nonsignificant trends toward a longer time to moderate or severe ARDS, a lower need for MV/NIV, and a lower risk of death or MV/NIV were detected among IS. In our cohort of COVID-19 patients, nonsevere immunosuppression was associated with a lower risk of moderate-severe ARDS, especially among AD. This suggests a potential protective effect from a hypothesized hyper-inflammatory response.

En la actualidad la tarificación de los servicios de transmisión debe ser de tal manera que financie el sistema de transmisión incentivando económicamente a éste a realizar futuras inversiones de expansión y permitiendo al bloque generador desarrollarse en un ambiente de competencia. En nuestro país al igual que otros se ha llevado a cabo la implementación del esquema de tarificación marginal acorde a costos marginales implícitos en los precios nodales del sistema sumando además a éste, un cargo adicional que complemente la totalidad para la remuneración a la empresa de transmisión. El presente trabajo propone la investigación y la aplicabilidad al caso ecuatoriano (anillo de 230 KV), de los denominados FACTORES DE PARTICIPACIÓN MARGINAL Y MEDIA como metodologías de asignación del cargo complementario, realizando el análisis comparativo tanto cualitativo como cuantitativo entre las metodologías propuestas (factores de participación) y la utilizada actualmente en nuestro país denominada Postage Stamp o Precio Estampilla.

Abstract

Cette synthèse donne un aperçu des principaux résultats et analyses des études conduites en France et au Maghreb entre 2012 et 2014 sur les services culturels récréatifs et éducatifs que procurent les zones humides méditerranéennes. L'Observatoire des zones humides méditerranéennes (OZHM), géré par la Tour du Valat dans le cadre de l'initiative méditerranéenne de Ramsar (MedWet), en a coordonné le travail. En partenariat avec l'Institut Agronomique Méditerranéen de Montpellier (IAMM), la synthèse des neuf sites étudiés a été réalisée en 2015, ce qui a permis une première analyse régionale et sous-régionale. Ce travail a aussi pour vocation d'établir un premier état de référence qualitatif utile pour les suivis ultérieurs. Ce travail n'aurait pas pu être réalisé sans la participation active des gestionnaires et associations des neufs sites étudiés et des institutions publiques qui ont facilité le travail: la Direction générale des Forêts en Algérie et en Tunisie et le Haut Commissariat des Eaux et Forêts et de la Lutte contre la Désertification au Maroc.

Abstract

In the game of Cops and Robbers, one of the most useful results is that an isometric path in a graph can be guarded by one cop. In this paper, we introduce the concept of wide shadow on a graph, and use it to provide a short proof of the characterization of 1-guardable graphs. As an application, we show that 3 cops can capture a robber in any planar graph with the added restriction that at most two cops can move simultaneously, proving a conjecture of Yang and strenghtening a classical result by Aigner and Fromme. Abstract

Changes in blood natural killer (NK) cells, important players of the immune innate system, have been described in multiple sclerosis (MS). We studied percentages and total cell counts of different effector and regulatory NK cells in cerebrospinal fluid (CSF) of MS patients and other neurological diseases to gain clearer knowledge of the role of these cells in neuroinflammation. NK cell subsets were assessed by flow cytometry in CSF of 85 consecutive MS patients (33 with active disease and 52 with stable MS), 16 with other inflammatory diseases of the central nervous system (IND) and 17 with non‐inflammatory neurological diseases (NIND). MS patients showed a decrease in percentages of different CSF NK subpopulations compared to the NIND group. However, absolute cell counts showed a significant increase of all NK subsets in MS and IND patients, revealing that the decrease in percentages does not reflect a real reduction of these immune cells. Remarkably, MS patients showed a significant increase of regulatory/effector (CD56bright/CD56dim) NK ratio compared to IND and NIND groups. In addition, MS activity associated with an expansion of NK T cells. These data show that NK cell subsets do not increase uniformly in all inflammatory neurological disease and suggest strongly that regulatory CD56bright and NK T cells may arise in CSF of MS patients as an attempt to counteract the CNS immune activation characteristic of the disease.

Abstract

The occurrence of smectite-illite and smectite-chlorite minerals series was studied along a thick clay cap (~300 m) drilled in the Cerro Pabellón geothermal field (northern Andes, Chile). X-ray diffraction (XRD) and scanning electronic microscopy (SEM) were used to characterize the alteration mineralogy and clay mineral assemblages and their changes with depth. Cerro Pabellón is a high-enthalpy blind geothermal system, with a reservoir zone from ~500 m to 2000 m depth, with temperatures of 200-250°C. Three main hydrothermal alteration zones were identified: (1) argillic; (2) sub-propylitic, and (3) propylitic, with variable amounts of smectite, illite-smectite, chlorite-smectite, mixed-layer chlorite-corrensite, illite and chlorite appearing in the groundmass and filling amygdales and veinlets. Chemical and XRD data of smectites, I-S and illites show, with some exceptions, a progressive illitization with depth. The evolution of I-S with depth, shows a sigmoidal variation in the percentage of illite layers, with the conversion of smectite to R1 I-S at ~180-185°C. These temperatures are greater than those reported for other similar geothermal fields and might indicate, at least in part, the efficiency of the clay cap in terms of restricting the circulation of hydrothermal fluids in low-permeability rocks. Our results highlight the importance of a better understanding of clay-mineral evolution in active geothermal systems, not only as a direct (or indirect) way to control temperature evolution, but also as a control on permeability/porosity efficiency of the clay cap.
