Objectives: To determine the frequency and predictors of outcome of gastrointestinal complications (GIC) in critically ill children. Methods: This descriptive study was prospectively conducted in The Pediatric Intensive Care Unit (PICU), After obtaining approval from the Institutional Ethical Committee and informed consent from the parents, all children (aged one month to 14 years), of either gender, admitted to the Pediatric Intensive Care Unit (PICU) during the study period were included. The frequency of the defined GIC: vomiting, high gastric residue volume (GRV), diarrhea, constipation, and gastrointestinal bleed were recorded daily for the first week of the PICU stay. Results: 41% of hospitalised individuals developed GIC within 48 hours. 47.4% of GIC patients had high GRV, including 39.7% constipation, 23.1% vomiting, and 17.9% abdominal distension. 41% of patients had two GI problems, whereas 27% had one. 14% had more than three problems. GIC patients stayed longer (8 days) than those without GIC (4 days). Children with mechanical breathing, sedatives and relaxants, multiorgan dysfunction syndrome (MODS), and inotropes had more gastrointestinal problems. In this paper, we have performed machine translation using modified Transformer model on TED Talk Dataset and from this we have selected four language pairs (English-Portuguese, Russian-English, French-Portuguese, Turkish-English) for testing. We have evaluated and analyzed the machine translation model on critical criteria's such as dataset size, batch size and training time over all four language pairs and have used BLEU scoring to generalize the results. We have searched for general trends and have analyzed the impacts of these variations on the scoring of the translation upon variation of different parameters, this analysis has been performed on each language pairs as well as have been compared with the results of other language pairs. From the results we found that with increase in dataset size, batch size and training time the BLEU score increases. The English - Portuguese pair achieves highest BLEU score of 23.2 from all language pairs and we get lowest BLEU score of 19.1 from the French - Portuguese pair.

Intrachoroidal cavitation is a morphological entity usually described in high myopic eyes. It is usually seen in peripapillary location, though macular location has also been reported. It has been associated with optic disc tilting and posterior staphyloma. We report a case of a 27 year old female who presented to us with recent onset scotoma in her visual field. On examination she had a fundal coloboma with intercalary membrane detachment with intrachoroidal cavitation at the edge of the coloboma. We hypothetise that this intrachoroidal cavitation is a secondary phenomenon and is responsible for recent onset scotoma in her visual field.

We are reporting four cases of acquired double orifice mitral valve (DOMV) of rheumatic etiology (rare) presenting as significant mitral stenosis (MS) treated successfully by percutaneous intervention. All four patients are young (3 males, 1 female) who presented with dyspnea of Newyork heart association (NYHA) class II with mean duration of 1.3 years. Typical clinical findings of MS were present in all. Echocardiography confirmed the diagnosis of moderate to severe MS of rheumatic etiology with double orifice of mitral valve of Trowitzsch incomplete bridge variety [1], giving appearance of two equal (like a pair of spectacles in one patient) or unequal size orifices (in three patients) in parasternal short axis view. Color Doppler examination revealed separate jets originating from each orifice, determined severity of the lesion and evaluated the eligibility for balloon mitral valvuloplasty (BMV). BMV was carried out under transeosophagial echo (TEE) guidance (for perfect septal puncture and crossing the separate orifices) using Inoue balloon of appropriate size (in accordance with patient’s height) with intention to break central fibrous strand converting into a single orifice. All four patients underwent successful BMV (clinical & ECHO criteria) after dilating the separate orifices (except case 4) but the fibrous strand could not be broken. Hence, this study shows that good results can be obtained in acquired double orifice mitral valve with significant MS with BMV even without breaking the fibrous strand. Considering rarity of isolated DOMV cases and rheumatic etiology being very rare, this report of four rheumatic acquired DOMV cases with successful BMV done with good result is significant.

WIMAX (World Wide Interoperability for Microwave Access) is used to provide a wireless solution in the metropolitan area networks. WIMAX network is capable of wide range coverage, high data rates, secured transmission and mobility supported at vehicular speed. Mobile WIMAX is one of the best concepts for system designed in fixed wireless access to provide good performance and cost effective solution. In this paper, performance of the WIMAX network is analyzed in terms of throughput, end to end delay, and jitter. Simulation is carried out using QUALNET simulator. In this paper AODV, DYMO and ZRP routing protocols are applied to the created mobility scenario with variable transmission range. AODV and DYMO shows low jitter and low end to end delay. It is concluded that AODV and DYMO gives optimized result below 40dbm transmission range with respect to throughput and ZRP shows high throughput above 40dbm transmission range.

Sigma theory is commonly used to predict the conditions that result in half of the entering particles being retained within a continuous-flow centrifuge. In this research, a 2D computational fluid dynamics (CFD) model of flow through a tubular bowl centrifuge (Sharples, 5 cm radius) was developed with Fluent software. A volume-of-fluid (VOF) approach was used for most of the simulations to track the motion of the liquid and gas phases, because the location of the gas/liquid interface depends on the centrifuge operating conditions. The CFD results indicate that the assumptions of plug flow and rigid body motion, which are fundamental assumptions of Sigma theory, were not valid for the conditions tested (700 < Rez < 3500 and 1000 < speed (rpm) < 3000). The average interfacial axial velocities (Viz) were 6−18 times greater than the plug-flow velocity (Vpf), and the average interfacial swirl velocities (Viθ) were 0.12−0.41 of that corresponding to rigid body motion (Vrbm).
