Abstract

The ever increasing demands placed upon machine performance have resulted in the need for more comprehensive particle accelerator modeling. Computer simulations are key to the success of particle accelerators. Many aspects of particle accelerators rely on computer modeling at some point, sometimes requiring complex simulation tools and massively parallel supercomputing. Examples include the modeling of beams at extreme intensities and densities (toward the quantum degeneracy limit), and with ultra-fine control (down to the level of individual particles). In the future, adaptively tuned models might also be relied upon to provide beam measurements beyond the resolution of existing diagnostics. Much time and effort has been put into creating accelerator software tools, some of which are highly successful. However, there are also shortcomings such as the general inability of existing software to be easily modified to meet changing simulation needs. In this paper possible mitigating strategies are discussed for issues faced by the accelerator community as it endeavors to produce better and more comprehensive modeling tools. This includes lack of coordination between code developers, lack of standards to make codes portable and/or reusable, lack of documentation, among others., Comment: 21 pages, 1 figure. Abstract

This design report describes the construction plans for the world's first multi-pass SRF ERL. It is a 4-pass recirculating linac that recovers the beam's energy by 4 additional, decelerating passes. All beams are returned for deceleration in a single beam pipe with a large-momentum-aperture permanent magnet FFAG optics. Cornell University has been pioneering a new class of accelerators, Energy Recovery Linacs (ERLs), with a new characteristic set of beam parameters. Technology has been prototyped that is essential for any high brightness electron ERL. This includes a DC electron source and an SRF injector Linac with world-record current and normalized brightness in a bunch train, a high-current linac cryomodule, and a high-power beam stop, and several diagnostics tools for high-current and high-brightness beams. All these are now being used to construct a novel one-cryomodule ERL in Cornell's Wilson Lab. Brookhaven National Laboratory (BNL) has designed a multi-turn ERL for eRHIC, where beam is transported more than 20 times around the 4km long RHIC tunnel. The number of transport lines is minimized by using two arcs with strongly-focusing permanent magnets that can control many beams of different energies. A collaboration between BNL and Cornell has been formed to investigate this multi-turn eRHIC ERL design by building a 4-turn, one-cryomodule ERL at Cornell. It also has a return loop built with strongly focusing permanent magnets and is meant to accelerate 40mA beam to 150MeV. This high-brightness beam will have applications beyond accelerator research, in industry, in nuclear physics, and in X-ray science.

Abstract

Background Conflicts of interest (COI) are considered ubiquitous in many healthcare arrangements (1) but there is disagreement on how COI should be defined, whether non-financial conflicts deserve attention, and the relationship between COI and harm. We conducted a study of Australian healthcare professionals and students to gain a better understanding of the way that COI are understood in practice. In this paper we outline an empirically derived taxonomy of the understanding of, and attitudes towards, conflicts of interest. Methods We carried out 25 semi-structured interviews with clinicians working in a number of fields across Australia and held six focus group discussions with medical students in New South Wales. Interviewees and focus groups followed similar question routes investigating participants’ understanding of COI and views of management. All data were compared and analysed using a matrix of pre-determined questions. Results There were, broadly, two views of COI: that COI were potentially harmful and morally compromising, and another that saw COI as less serious and easily managed through existing structures. Definitions of COI varied widely and were both financial and non-financial. Causes of COI were, variously, systemic, individual, and/or relational. Some participants associated COI with moral wrongdoing, and a variety of potential harms were identified. Views on how COI should be managed were similarly varied. Conclusions We found considerable heterogeneity in how COI are understood in practice. This has implications for management systems that are currently in place, and we suggest a more sophisticated system for considering and mitigating conflicts of interest.

Abstract

With the discussion paper from the ongoing Review of Pharmacy Remuneration and Regulation still open for public input, there is an opportunity to reflect on how the services currently delivered by community-based pharmacies in Australia can be improved. Some of the questions up for discussion concern the ways in which pharmacists and patients navigate pharmacists’ dual roles as retailers and dispensers. In the post below, bioethicists Wendy Lipworth, Christopher Mayes and Ian Kerridge discuss what is at stake when therapeutic and commercial boundaries are blurred, and deliver a warning to other health professionals who might consider the addition of merchandising to their professional portfolios.

Abstract

Over the past hundred years, industrial agriculture and the globalised food system have produced cheaper, longer lasting and more diverse food items. We can now enjoy tropical fruits in winter, purchase whole chickens at the price of a cup of coffee, and eat fresh bread long after it was baked. Once celebrated as the benevolent results of food science and ingenuity of farmers, these cheap and safe foods are dismissed by critics as the tainted fruits of “Big Food” – the culinary version of Big Tobacco and Big Oil. Food is no longer simply a matter of taste or convenience. Our food choices have become ethical and political issues. An innocuous but central strategy in these debates is the food label. In recent years there has been an explosion of ethico-political food labels to address concerns such as slavery, nutrition, environmental degradation, fair trade and animal cruelty. These disparate concerns are unified by their connection to the amorphous culprit “Big Food”. The idea is that by knowing what is in our food and how it was produced, we will reject unethical food corporations, buy from ethical producers and thereby promote justice. But is this necessarily so?

Abstract

It’s estimated over 5 million children have been born worldwide as a result of assisted reproductive technology treatments. Assisted reproductive technology, an umbrella term that includes in vitro fertilisation (IVF), is a highly profitable global industry, and fertility clinics are increasingly regarded as an attractive investment option. In 2014, two major IVF clinics - Virtus and Monash IVF — floated on the stock exchange. Excited financial analysts observed at the time that: people will pay almost anything to have a baby. Over the past 12 months, there have been numerous critical media analyses of the IVF industry in Australia, including Monday night’s ABC Four Corners program, The Baby Business. The episode suggested IVF doctors are recommending treatments that are expensive, unsafe and likely to be futile. The following morning the Fertility Society of Australia rejected these assertions, saying: Four Corners presented no evidence to support these claims. One of the claims made in the program was that IVF doctors have a financial incentive to treat women with the more invasive practice of IVF. The program suggested this financial incentive conflicts with the doctor’s duty of care towards the patient. Four Corners highlighted the conflicted nature of commercialised IVF, where some IVF doctors are more concerned about their own interests (making money for themselves or their clinics) than they are about their patients. Not surprisingly, the Fertility Society of Australia strongly denied such conflicts of interest exist. It argued that the profession is both highly ethical and highly regulated.

26. Big Food with a regional flavour: how Australia’s food lobby works

Abstract

Criticism of the food industry has itself become a niche industry. But the tendency to embrace a US-centric conception of how the industry works risks masking local variants and inhibiting a targeted response in other countries. Since the 2001 book Fast Food Nation, a spate of books, films and documentaries on the American food industry have helped to shape the popular idea of “Big Food”. The food industry is depicted as a highly organised set of multinational food and beverage lobbyists peddling the global diet of sugary drinks and highly processed, energy-dense salty foods – akin to tobacco industry lobbyist Nick Naylor in the 2005 film Thank You For Smoking. But although it is highly globalised, the food industry is far from homogeneous. Big Food in Australia is not the same as the industry in the United States, where much of the popular media has come from. Still, that doesn’t mean Australian food and beverage lobbying is benign. Responding to the threat posed by the food industry to public health locally requires a clear understanding of food industry tactics in the context of Australia’s political and lobbying culture.

27. Because we can, does it mean we should? The ethics of GM foods

Abstract

Food is cultural, social and deeply personal, so it’s no surprise that modifications to the way food is produced, distributed and consumed often lead to ethical debates. Developments in the genetic modification (GM) of foods and crops has resulted in a raft of controversies. Ethics can help here. While science determines whether we can safely modify the genetic makeup of certain organisms, ethics asks whether we should. Ethics tries to move beyond factual statements about what is, to evaluative statements about the way we should act towards ourselves, each other and the environment we inhabit. But things are not always so clear-cut. Three areas of ethics can help frame some of the concerns with GM food and crops: virtue, moral status and consequences.

28. Joe Hockey’s medical research fund is nothing more than a distraction

Abstract

Health is a basic requirement for an individual to lead a good life. Without health you have nothing; when we are sick, it’s difficult to work, to care for others, to participate in the things we enjoy. We seek treatment so we can get back to our normal lives. Because health is so important to our wellbeing, there is widespread agreement— including among ethicists —that a fair and accessible healthcare system is something that we should pursue. And although the Australian healthcare system is far from perfect, it has provided universal access to healthcare for almost 40 years. A universal healthcare system – one that is open to everyone, whether or not they can afford to pay – is a basic feature of a good and just society. Tony Abbott and Joe Hockey want us to panic about a "budget emergency", including the idea that our current health system is unsustainable. Rising healthcare costs pose a challenge to governments everywhere. But this is not a new problem, and will not bring about economic or social catastrophe any time soon. This amplified threat is being used to justify measures that are now well known: introducing co-payments for GP fees; disestablishing Medicare Locals; transferring health agencies to the department of health with reduced funding; and stripping $80bn in funding from the states, particularly in health and education. This will not only force the states to increase their own goods and services taxes, but reduce the public services they can afford to provide. It will end universal access to health care, make Medicare a mere "safety net", overwhelm hospitals, and increase the inequities that are increasingly a feature of Australian society.

29. The Harm of Bioethics: A Critique of Singer and Callahan on Obesity

Abstract

Debate concerning the social impact of obesity has been ongoing since at least the 1980s. Bioethicists, however, have been relatively silent. If obesity is addressed it tends to be in the context of resource allocation or clinical procedures such as bariatric surgery. However, prominent bioethicists Peter Singer and Dan Callahan have recently entered the obesity debate to argue that obesity is not simply a clinical or personal issue but an ethical issue with social and political consequences. This article critically examines two problematic aspects of Singer and Callahan's respective approaches. First, there is an uncritical assumption that individuals are autonomous agents responsible for health-related effects associated with food choices. In their view, individuals are obese because they choose certain foods or refrain from physical activity. However, this view alone does not justify intervention. Both Singer and Callahan recognize that individuals are free to make foolish choices so long as they do not harm others. It is at this point that the second problematic aspect arises. To interfere legitimately in the liberty of individuals, they invoke the harm principle. I contend, however, that in making this move both Singer and Callahan rely on superficial readings of public health research to amplify the harm caused by obese individuals and ignore pertinent epidemiological research on the social determinants of obesity. I argue that the mobilization of the harm principle and corresponding focus on individual behaviours without careful consideration of the empirical research is itself a form of harm that needs to be taken seriously. 30. Paediatric renal transplantation in Northern Ireland (1984-1998)

Abstract

Over the last 20 years a comprehensive paediatric nephrology service has been developed in Northern Ireland, based in the academic medical unit at the Royal Belfast Hospital for Sick Children (RBHSC). In the 15 years 1984-1998 a total of 77 renal transplants have taken place in patients aged 18 years and under. Initially transplants were only considered in children over five years of age but in the past eight years children as young as two years have successfully received kidneys. Aggressive nutritional support combined with peritoneal dialysis has enabled survival to a size when transplantation is feasible. The 5 year graft survival was 64%, with two children dying following transplantation. The complexity of managing this age group is reflected by the fact that a total of 10 transplants (13%) failed in the first 30 days. These figures compare favourably with statistics reported by similar paediatric centres from across the United Kingdom and Republic of Ireland, and with local results in adult patients. This demonstrates that a successful end stage renal replacement programme for children is achievable in a relatively small population, which is geographically isolated.

31. Slurs aside, let’s talk about the ethics of public health measures

Abstract

Predictable positions followed the recent announcement of an increase in tobacco tax by 12.5% a year for four years. Public health advocates praised the tax, labelling those questioning it as “tobacco industry apologists”. Libertarians, on the other hand, decried it as further evidence of the “nanny state” and The Australian’s Adam Creighton went as far as to compare the measure to Nazis. Apart from incensing readers, the noise from these well-worn positions drowns out significant public health concerns. The uncritical acceptance from public health and knee-jerk rejection from libertarians leaves little room to ask whether increasing the tobacco tax is unquestionably good. It’s time for a new conversation.

Public health has made enormous contributions to global health through policies and interventions. However, public health measures have also increased human suffering through policies of forcedsterilization, mandatory screening of vulnerable populations, detention of people with infectious diseases, and the quarantine and social isolation of “sick” people or communities. Until recently, the values and ethics that public health is based on have been assumed to be “good” and have not been subject to critical analysis. In the last decade, however, bioethicshas shifted its attention from the clinic to the public sphere, and many of the ethical issues raised by public health have become the focus of both philosophical and empirical study. Bioethicsin public health addresses the tension between local and global health needs; the role of the community in decision making; the duties, obligations, and limitations of the state in relation to the provision of health care; the targeting of vulnerable populations by interventions aimed at preventing disease; and the nature and importance of ethical principles and rights in health care. This bibliography provides a collection of resources that address the ethical theories, theoretical frameworks, key concepts, and practices of public health.

34. The Violence of Care: An Analysis of Foucault’s Pastor

Michel Foucault begins his 1977-1978 lecture series–Security, Territory, Population–by stating that he will investigate the vague concept of biopower introduced in The Will to Knowledge and Society Must Be Defended. 1 Famously, or perhaps frustratingly, Foucault never explicitly develops the concept of biopower in Security, Territory, Population or in subsequent work. Rather much of Security, Territory, Population is devoted to a lengthy analysis of the shepherd 2 in Hebrew and Christian contexts as the historical root of pastoral power. This is not to suggest that Foucault’s focus on the shepherd and governmental practice in Security, Territory, Populationis a turn away from the problems of biopower introduced in The Will to Knowledge and Society Must Be Defended. Rather, I will argue with Roberto Esposito that pastoral power is “the first genealogical incunabulum of biopower.” 3 That pastoral power provides the first example of life itself being seized and governed. Thus Foucault’s focus on the pastor in Security, Territory, Populationwill be read as an investigation into the genealogical root of biopower. This paper will address Foucault’s analysis of the Hebrew and Christian pastor and argue that Foucault’s analysis of pastoral power in Security, Territory, Population neglects an important characteristic of the shepherd/pastor figure: violence
