Abstract

Background: Adequate intake of essential fatty acids (EFAs) is required for optimal development of the central nervous system and visual acuity in infants. Little information exists regarding the EFA status of vulnerable populations living in Southern regions. Objective: We examined the adequacy of EFA status in Cuban breast-feeding mothers and their infants. Design: Blood and breast-milk samples were obtained from a cross-sectional sample of mothers and term infants in Havana at 2 mo postpartum. We determined the fatty acid profiles of total lipids in breast milk, plasma, and erythrocytes and assessed infant visual acuity by using Teller acuity cards. Results: Of the 56 mothers and infants examined, none showed biochemical signs of poor EFA status. Compared with values reported in the literature, mothers had an adequate EFA profile in their breast milk, plasma, and erythrocytes. The docosahexaenoic acid (DHA) concentration in breast milk was 0.43 [+ or -] 0.26% of total fatty acids. It appeared that these breast-fed infants had an adequate dietary supply of DHA, as reflected by the mean plasma and erythrocyte DHA concentrations (2.82 [+ or -] 0.84% and 7.41 [+ or -] 1.16% of total fatty acids, respectively). Infant visual acuity testing showed a mean of 2.00 + 0.68 cycles/degree, which is within the normal range of mean binocular acuities for 2-mo-old term infants. The data did not show any relation between EFA concentrations and visual acuity. Conclusion: The results suggest that n-3 fatty acid deficiency and potential related deficits in early visual neural development are rare, if they exist at all, in breast-feeding women and their infants in Havana. 31. Preferential Looking - Clinical Lessons

Abstract

Preferential looking-based tests of acuity have been available for over a decade. The authors discuss their use in clinical practice, particularly in three groups in whom acuity could not be quantified by traditional means: normal infants, and young children who are either mentally retarded, or who have visual disorders. Preferential looking (PL) testing has increased our understanding of the natural history of visual pathway disorders and has revealed certain patterns of acuity development. Early acuity development may be normal, delayed, or stationary, while in later infancy and childhood the following abnormal patterns have been identified: asymptotic, parallel, catch-up, or regressing. While this information has introduced a degree of complexity hitherto unknown which, if misunderstood, can lead to test misinterpretation, it offers the clinician invaluable information to improve patient care, and may also offer clues to the fundamental mechanisms of visual development.

41. Grating Acuity Tests Should Not Be Used for Social Service Purposes in Preliterate Children

Abstract

In an Editorial published in the August 1994 issue of the Archives, Dr Kushner1 expressed concern that because recently introduced tests underestimate the severity of the deficit, clinicians are being misled and the needs of the visually impaired child are not being met. Our response focuses on preferential looking-based tests (acuity card procedures [ACPs], including Teller Acuity Cards), although certain comments are relevant also to electrophysiological tests.One factor is the "low sensitivity [of ACP] for detecting subnormal visual acuity"1 due to the wide normal range in infancy. Consequently, compared with letter-based tests such as the HOTV test (Good-Lite Inc, Chicago, Ill), ACP is a "functional" technique.2 Two points militate against this view. 43. The diagnostic accuracy and feasibility of paediatric visual field tests: A systematic review.

Abstract

Purpose: Alternative non-standard paediatric visual field (VF) tests have been developed to address the challenges associated with standard approaches. However, diagnostic accuracy of these new VF tests has not yet been rigorously evaluated. This systematic review aims to explore diagnostic accuracy and feasibility of non-standard VF tests in paediatric patients., Methods: The following databases were searched for English language studies comparing a non-standard paediatric VF test to standard methods, such as standard automated perimetry (SAP), manual kinetic perimetry (MKP) and confrontation testing (CT): EMBASE, PubMed, Ovid MEDLINE, Web of Science, Scopus, VisionCite, Cochrane Library, ClinicalTrials.gov, African Index Medicus, LILACS, Trip and grey literature databases. Studies included were of children ≤18 years old with suspected or known VF defects (n > 3). Case reports, case series, editorials and letters were excluded. This review follows the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Risk of bias was assessed using the Quality Assessment of Diagnostic Accuracy Studies (QUADAS-2) tool., Results: Thirty studies (n = 2074 children, age range: 2 months to 18 years) published between 1990 and 2023 met the inclusion/exclusion criteria. Twenty index tests were reported, sorted into four categories of methods: behavioural/observational (5), electrophysiological (2), modifications of standard perimetry (11) and eye tracking (2). Risk of bias, based on the QUADAS-2 results, was unclear or high for most studies. Sensitivities of 10 studies (behavioural/observational [2]; electrophysiological [2]; modifications of standard perimetry [2] and eye tracking [4]) were 60%-100%, 75%, 80%-94% and 70%-100%, and specificities were 98%-100%, 86%-88%, 68%-100% and 50%-100%, respectively., Conclusions: Alternative non-standard paediatric VF tests, and behavioural/observational methods in particular, show potential for adequate diagnostic accuracy and feasibility, although limited to certain conditions and ages. Studies with complete reporting and low risk of bias are needed to determine the diagnostic accuracy and feasibility of non-standard paediatric VF tests., (© 2025 College of Optometrists.)
