10. Molecular and functional characterisation of the chemopreventative effects of dietary polyphenols in intestinal cancer

Abstract

It is estimated that over half of colorectal cancer (CRC) cases in the UK are preventable through lifestyle changes. Perhaps unsurprisingly, bowel cancer is strongly linked to dietary choices. Diets that are high in fat and low in fibre are associated with increased risk of cancer while diets rich in fruit, vegetables and fibre have a reduced risk. Several studies have investigated the effect of certain dietary components in CRC initiation and development. Previous work, in humans and animals, has demonstrated that the polyphenols found in black raspberries (BRBs) have chemopreventative and therapeutic effects. However, the exact mechanism for these effects remain unknown. As CRC can originate from an intestinal stem cell (ISC) it is possible that the chemopreventative role is due to the impact BRBs have on the normal and/or malignant ISCs. This thesis aimed to investigate the chemopreventative effect of BRBS on normal intestinal tissue and on the initial and later stages of intestinal tumourigenesis, in the context of ISC dynamics and activated Wnt signalling. This was achieved using a Cre-loxP based approach to conditionally delete Apc (the negative regulator of the Wnt signalling pathway) within different compartments of the adult murine intestinal epithelium and also utilised the 3D intestinal organoid system. Exposure to a BRB enriched diet is reported here to be well tolerated in mice and have no major detrimental effects on normal intestinal homeostasis and health. Feeding of BRB diet 2 weeks prior to Apc gene ablation is shown to partially attenuate the ‘crypt-progenitor’ phenotype typical of acute Apc loss. In this context of activated Wnt signalling, BRBs altered ISC dynamics in vivo and reduced the self-renewal capacity of Apc deficient cells ex vivo. Additionally, long-term feeding of BRB diet was shown to significantly improve survival of mice which developed macroscopic stem-cell derived Wnt-driven adenomas. Together, these data are the first evidence that BRBs play a role in CRC chemoprevention by protectively regulating the ISC compartment. These findings further support the use of BRB intervention in cancer prevention in the context of Wnt-driven tumourigenesis.

14. Real-World Effectiveness of Intravenous and Oral Antibiotic Stepdown Strategies for Gram-Negative Complicated Urinary Tract Infection With Bacteremia.

Abstract

Background Robust data are lacking regarding the optimal route, duration, and antibiotic choice for gram-negative bloodstream infection from a complicated urinary tract infection source (GN-BSI/cUTI). Methods In this multicenter observational cohort study, we simulated a 4-arm registry trial using a causal inference method to compare effectiveness of the following regimens for GN-BSI/cUTI: complete course of an intravenous β-lactam (IVBL) or oral stepdown therapy within 7 days using fluoroquinolones (FQs), trimethoprim-sulfamethoxazole (TMP-SMX), or high-bioavailability β-lactams (HBBLs). Adults treated between January 2016 and December 2022 for Escherichia coli or Klebsiella species GN-BSI/cUTI were included. Propensity weighting was used to balance characteristics between groups. The 60-day recurrence was compared using a multinomial Cox proportional hazards model with probability of treatment weighting. Results Of 2571 patients screened, 759 (30%) were included. Characteristics were similar between groups. Compared with IVBLs, we did not observe a difference in effectiveness for FQs (adjusted hazard ratio, 1.09 [95% confidence interval,.49–2.43]) or TMP-SMX (1.44 [.54–3.87]), and the effectiveness of TMP-SMX/FQ appeared to be optimal at durations of >10 days. HBBLs were associated with nearly 4-fold higher risk of recurrence (adjusted hazard ratio, 3.83 [95% confidence interval, 1.76–8.33]), which was not mitigated by longer treatment durations. Most HBBLs (67%) were not optimally dosed for bacteremia. Results were robust to multiple sensitivity analyses. Conclusions These real-world data suggest that oral stepdown therapy with FQs or TMP-SMX have similar effectiveness as IVBLs. HBBLs were associated with higher recurrence rates, but dosing was suboptimal. Further data are needed to define optimal dosing and duration to mitigate treatment failures. [ABSTRACT FROM AUTHOR]

Background Fluoroquinolones (FQs) are effective for oral step-down therapy for gram-negative bloodstream infections but are associated with unfavorable toxic effects. Robust data are lacking for trimethoprim-sulfamethoxazole (TMP-SMX) and high-bioavailability β-lactams (HBBLs). Methods In this multicenter observational cohort study, we simulated a 3-arm registry trial using causal inference methods to compare the effectiveness of FQs, TMP-SMX, or HBBLs for gram-negative bloodstream infections oral step-down therapy. The study included adults treated between January 2016 and December 2022 for uncomplicated Escherichia coli or Klebsiella species bacteremia of urinary tract origin who were who were transitioned to an oral regimen after ≤4 days of effective intravenous antibiotics. Propensity weighting was used to balance characteristics between groups. 60-day recurrence was compared using a multinomial Cox proportional hazards model with probability of treatment weighting. Results Of 2571 patients screened, 648 (25%) were included. Their median age (interquartile range) was 67 (45–78) years, and only 103 (16%) were male. Characteristics were well balanced between groups. Compared with FQs, TMP-SMX had similar effectiveness (adjusted hazard ratio, 0.91 [95% confidence interval,.30–2.78]), and HBBLs had a higher risk of recurrence (2.19 [.95–5.01]), although this difference was not statistically significant. Most HBBLs (70%) were not optimally dosed for bacteremia. A total antibiotic duration ≤8 days was associated with a higher recurrence rate in select patients with risk factors for failure. Conclusions FQs and TMP-SMX had similar effectiveness in this real-world data set. HBBLs were associated with higher recurrence rates but suboptimal dosing may have contributed. 21. Parents'/Carers' Perceptions and Experiences of Growing, Preparing and Eating Their Own Fruit and Vegetables as Part of the 'Field to Fork' Project

Abstract

This paper reports research into a project to encourage KS1 and KS2 pupils to eat more healthily by supporting their families to grow their own fruit and vegetables at home. Participants were recruited through a Primary School Trust comprising four primary schools in the North West of England. They were given practical support to enable them to grow fruit and vegetables. The findings of pre-programme focus groups with parents/carers suggested families tended not to eat the daily government-recommended five portions of fruit and vegetables; cost, time and knowledge being the main inhibitors. With resources, knowledge and support most participants were enabled to grow their own fruit and vegetables leading to increased consumption. Availability to and familiarity with fresh fruit and vegetables by tending and growing them first-hand was shown to increase their consumption.
